My fiancé was transferred to Citadel after a hospital stay. The place has not been updated since the 70's. It is not inviting and reeks of urine. The staff are not helpful. My fiancé had to constantly remind them of his gluten and dairy allergy. They would say they would check on it and not come back. I'm trying to get him home or transferred to a new facility. read more

My husband was in this facility a few weeks and they were awesome with him. All staff is friendly and helpful. Yes, some times it gets hectic with all the patients they take care of, but they were still great. My husband could not stop raving about how excellent the food is as well. I almost think he prefers their cooking to mine. read more

Nice facility! I came as a guest to a family. The entrance in the lobby was very inviting, and appropriately decorated for the season. The nurses station was neat and open. I went to speak with a staff member there and Mark was more than willing to help me, twice in fact. I noticed that the meal that was served to the patient, looked tasty and appealing, and something that I would be willing to eat myself, complemented with a dessert. Again, this is my initial impression and having been here this being my second time, seems like a friendly, clean and a nice atmosphere in which to do rehab at! My dad spent a week at Citadel Post Acute recovering from heart surgery. The staff was positive and extremely attentive to my dad's care. They paid attention to small details that made him feel more comfortable, such as a second cup of coffee. The therapists worked with him and kept his program upbeat and positive. My dad is home now but stated that he had excellent care at Citadel. As the family member, I was kept informed of my dad's progress. The case manager, head nurse, and the entire team kept me up to date and addressed my concerns. When my dad was discharged, i felt confident they he was ready to continue his care at home. read more

Absolutely do NOT recommend! My dad was transferred from the hospital to Citadel after he had a mini stoke (and back-related left leg weakness/pain). He was there LESS THAN 12 hours (probably about 8.5 hrs, so 1am) and he needed help to the restroom. He pushed the call button for the Nurse several times and NOBODY came. The (shared) room was tiny and his bed was probably 10-15 steps from his bed. He was tired of waiting and decided to make the trip himself however, I think almost as soon as he stood from the bed, he fell and hit his head! We (my mom and I) still don't know how he got up from the floor. We (my mom and I) assume somebody helped him up. However NOBODY called our house to notify us of his fall! We had to hear second hand from my Aunt (Dad's sister, who lives in Tennessee). My Aunt had called my Dad at the facility around 10am and my Dad mentioned falling in the night and then my Aunt called me to ask if I knew he had fallen, which I did not prior to my Aunt calling. The case manager was busy (and that's fine she's one woman and has to prioritize her cases by level of urgency) and the Director of Nursing was not in her office (possibly at lunch, also fine) so there was nobody immediately available to discuss transferring him to a different facility. This was around 1pm. My dad fell asleep a few hours later and when the Nurse Practitioner came in to assess him, he would not wake up. They called the EMTs and decided to send him to the hospital which is where I am writing this review from. Because of the fall he has a possible concussion. read more

Hello there my mom was just discharged from Citadel Post Acute. She finally received the care and staff she deserved. This facility earned my mothers trust in helping her understand that she should not be living on her own and helped her get the physical therapy and occupational therapy she needs. With Covid going on the phone calls were slow coming in from staff however I know they were busy and they were taking care of their patients. My mother had been in a facility prior which was Alta Mesa which gave me the runaround that she was in a safe environment and she was an "unsafe discharge and not to worry that she would not be going home with her own decision." With that being said, it was a matter of time she would return needing medical help. Citadel Post Acute convinced her that it was a safe place to stay and that they were taking care of her and she earned their trust. I cannot emphasize that enough, that , that is so important with elder people who do live on their own. Jackie the discharge coordinator and nursing staff were great! In am pleased with the staff and I would recommend this skilled nursing facility for your love ones. read more

If I could give this place -0 I would. My brother unfortunately got placed in this shit hole. Went to visit him and he was screaming out in pain, IV pump is beeping, and no one was coming to see what was the matter. All the staff was up at the nursing station having a good time for themselves. I'm also in the medical field so when I looked over my brother who has a feeding tube I could see the bag was empty and the line was clogged. Fortunately a separate team we had hire to help him came in time to help us. They removed 240 mls of food from his stomach! That had been sitting in there for who knows how long. Unfortunately my brother is terminally Ill. But if someone is crying out in pain you come you don't ignore them your getting payed to do your job so do it! WOULD NEVER RECOMMEND THIS PLACE TO ANYONE!!!! They just ignore and have NO communication with the family. I have lots more stories to tell this wasn't the first or last time. read more

If I could I would give zero stars. My mother was in hospice dying I walked outside to get some air and warm up in the 84° sun when I walked back in I had a slight increase in temperature from standing in the sun to warm up. I have been in and out of the front door five or six times today. My temperature never got above 98°. Standing in the sun to warm up with a jacket on it reached 100. I was forbidden to ever return to the facility as my mother lies in bed dying alone. Not to also mention my daughter was standing by the reception when April and Jesus said I don't care if her mother is dying in here her daughters never coming back again. Both Jesus and April think they run they run the place have absolutely no care and are rude to family members in person and on phone. read more

This facility should get a -5 rating. A family member had transferred into their care. Administrator Terry Speth was called a dozen times due to theft, neglect & questions on lack of proper patient care... over the three week period that my family had to deal with this place never once did Terry Speth call us back... nor did any charge nurse or floor nurse for that matter. Due to the lack of urgency and Theft, Medicare has been informed and Adult Protective Services is investigating. BE VERY CAREFUL if you or your family member is sent to this facility. read more

DO NOT send a loved one here. My dad was here for 14 days and on his 14th day he was positive for covid. Nobody returns your calls. I have called dozens of times in the 2 weeks and if the front desk even answers they say they will leave a note. I have guardianship over my dad due to his stroke and not once did they call me to discuss his care plan. They put him in a broken bed even though he is a high fall risk. They only gave him one shower even though I constantly left messages. His room phone was broken so it was extremely hard to reach him. One of the times he fell they sent him to the hospital for x-rays and I found out because the hospital called. When I called citadel they said he was in his room and everything was fine. They spent 14 days neglecting my dad, not returning my calls and today they sent him to another facility with out my permission because they gave him covid. DO NOT send ANYONE you care about to this place. The high ratings must be from a long time ago or from friends of the workers that gave fake reviews. read more

My husband fell at home and fractured his stoke affected leg. After hospital he was sent for skilled nursing to get PT for pivot transfer only. Ended up being there 3 weeks, real therapy started week and half later. Staff's favorite words are "I'll be right back". He was there 3 weeks and had one bed bath and one shower and I had to get the DoN involved. He was unable to stand or walk. Wanted to try and get to toilet with help, they said no. He had to go in bed. he told them he was done, "I'll be right back". He called me at home, ask me to call nurse, because they wouldn't come to his call. He had been laying in mess 45 minutes. His oxygen line fell on floor, they picked it up and gave it back to him. He stated it was wet, she said no it wasn't. Come to find out it was laying in urine on the floor as the bag had leaked. Line is to be thrown away any time it hits the floor. Probably only thing that saved him on that one was he got pneumonia while in there and was on a strong antibiotic We had 2 pages of complaints and we heard there were other people doing the same. If you have no choice but to go there, someone needs to stay with that person read more

If I could give a negative review, I would. This is a horrible facility and I would never recommend it to anyone, especially an elderly person who is confused, ill and in need of quality care. Let me start off by saying I understand policies have changed some due to COVID; however, I do not believe not involving the family in treatment plans of a patient is an acceptable practice in any situation. I could write a very lengthy review detailing every aspect of my dad's care and the contact the family had with this facility. I've decided to be as brief as I can. The visiting policy is terrible. You must continue to leave messages with the activity person in order to schedule a visit for the next day. Visits were held either in a visiting area in the building for 20 minutes and only if there was space. Otherwise, you were allowed to visit in an outside patio area in June in the Arizona heat. This does not seem like a very good option for a 90 year old man who is sick. In order to talk to staff, you must continue to leave messages, hoping someone will call you back. When staff did return phone calls, they were open to providing updates. But not including us in the treatment plan. The only staff member in this facility who treated my dad and the family with respect and kindness was the Speech Therapist. Once she was able to contact us, she continued to update us. The receptionist at this facility is the rudest, most unprofessional person I have ever had to deal with. She lacks any kind of compassion or empathy for a family going through a loved one's health crisis. I'll end by saying, after eight days of endlessly trying, I was finally able to take my mom to his room to visit him. Seeing him for the first time in his room, I knew he needed to get out of there. I was finally able to transfer my dad out of this facility. However, it was not to another rehab or skilled nursing facility, it was to a hospice inpatient facility, where he passed away four days later. Even if his passing was inevitable, not allowing his wife of over 40 years, which he continually asked for, to be with him during that week he was in their facility was cruel and unbelievably insensitive. And I'm sure after reading other reviews about this facility, there will be a reply from them with excuses. However, this was our experience with this facility. read more

Where do I begin? My mother has been in this facility for a few weeks now. Even though the staffing ratios were poor, she waited for hours for meds, and the food was unrecognizable we kept her there because while on PT she was getting better. While waiting for her ALTCS application to be approved, the nurse case manager had my mother sign a paper to agree not to receive anymore PT since her Medicare benefits had exhausted. I was furious because she knew I was her healthcare proxy and POA! My mother's health has been declining since and Is at this moment being rushed to the ER! If it wasn't for my mother having a cell phone I would never have known nor would they have checked her blood work! I had to have my mother demand lab work to have her red blood cell count checked...she was on oxygen for days and the doctor just gave her an antihistamine!! She is now critically low on her red blood cell count! No one ever returns your calls or cares to give family members updates regardless if you speak to the head of nursing, the assistant, the nurse case manager, the social worker, front desk, NO ONE! Thank goodness my mother had her cell phone! She would call me crying at 1am because she was still waiting for her evening meds! Staff churn is so high that they repeatedly have one nurse to cover the entire facility both COVID and non-COVID sides. It took 2 weeks for the head of nursing to finally change the battery on my mother's defibrillator! She was without it for 2 weeks! My mother quickly learned how to change the battery herself because the staff never does. This device is supposed to keep revive her heart should it stop beating!! The neglect is so unbelievably horrific! I just learned today that the doctor told my mother, "your kids just don't seem to realize that your body is just old and giving up on you." Are you serious? Who tells an elderly woman that her body is giving up on her and to give up hope? No one will even tell me which hospital she is being taken to or get on the phone to tell me what is going on. I couldn't remove my mother from the facility because she contracted COVID while in the ED and there weren't any other options for her. This place needs to be shut down!!! I am reporting them to CMS. read more

My husband stayed at Citadel for 2 weeks after a being transferred from another facility. The CNAs were really great and caring. He was completely bedridden and had to rely on them for everything, including brief changes. They were always prompt and caring. Shout outs to Laura, Carly, and Alexis but they were all great. Also our case manager Jackie was awesome and and so incredibly helpful. I would have truly been lost without her direction. The only reasons I don't give it 5 stars is because they are not equipped to deal with mental health issues, (in my opinion). My husband is bipolar and having an extremely difficult time and he didn't get the psych consult we ask for from day one, literally to the nurse doing his intake, until 5 days before he was discharged. The psych evaluation was suppose to be put in by the social workers and they were a joke. They came into the room twice and didn't introduce themselves or explain what they wanted, just started asking questions like "what kind of music do you like" and it was only after my husband became difficult and upset did Michelle said she was the social worker and she put in the psych requests, it hadn't even been put in. But other than that I would recommend it for daily care and treatment. read more

This is the second time since January 2017 I have been admired to Citadel. I have nothing but good things to say it was just remodeled looked great. My call light is answered usually within 3 minutes. The nursing staff is amazing just to name a few Jen, Michelle, Sam and the CNA's are always Quick to help. We also have an amazing staff of occupational therapist and physical therapist. I would recommend Citadale to anyone who is looking for after hospital care. read more

I am always impressed when I meet with them at how they stay focused on patient care in all that they do. My long term hospice care wife was supposed to stay there for 5 days for her respite care. I dropped her off on Friday morning and spent about an hour an1/2 with the head nurse going over the care she needed. She has Epilepsy, Stroke, and had extensive brain, breast and liver cancer. She also has necrosis of the jaw which needed additional care with a waterpik and oral rinse twice daily. She stated she would pass her needs and my conversation on to the day and night staff. She assured me she would be taken very good care of. I got a call at 9:30 p.m. by a nurse stating Susan "slipped" out of bed an hit her head, but she was OK. Susan also e-mailed me about the lousy food. She can't talk due to her brain cancer and stroke. I decided Sunday morning she could not stay there any longer, so I picked her up at 12:30 p.m. They had her ready and we left. During the ride home she told me she hadn't eaten since dinner the night before, nor did they give her, her medications for her epilepsy and necrosis of the jaw. She also fell asleep on the way home. I immediately fed her and when we got home. I checked her hollow socket in her mouth from the nacrodis and it was full of bacteria. She stated they didn't use the waterpik on her. I then took her to the bathroom before I was going to give her a shower and put her to bed for a nap. It was at this time I discovered her urine soaked jeans. When I pulled her pants down to take them off along with her diaper I discovered her diaper was dripping wet with urine and Susan had stuffed about 10-15 kleenexes down the diaper to soak up the urine. She said she asked to go to the bathroom and eat breakfast that morning and no one did anything. As I got her ready for her shower I found bruises on the back of her head, cheek, arm and leg from "sliding" out of the bed. I gave her a shower then put her to bed where she slept for 3 hours. When she got up she needed to go to the bathroom. Before I could get her there, she had a sever case of diarrhea. Without asking if she had been constipated, they gave her stool softeners without telling me. Needles to say I had a real mess to clean up. Needless to say I'm glad I didn't leave her there until Wednesday. I also noticed when I went into her room, there were no styrofoam glasses of water anywhere around her bed, or night stand. She said they didn't give her any water in her room. I wouldn't recommend anyone use this facility for long or short term care. I thought Desert Cove was bad, but this place was far worse. read more

I found that the help was rude. they were slow to respond. they seemed to argue with each other and complain out loud in front of patients and families. trying to get through to anyone on the phone was a joke. waited mulitple times almost 30 minutes and finally hung up. asked for call back from nurses. never received calls back. wouldn't recommend this place! read more

If I could give zero stars I certainly would. I only allowed my mom to be a patient at The Citadel for just a few days. My mom cried when we walked in to visit her at 10:30am because she was sitting up in her wheelchair and no one had been by to check on her since breakfast. Her call light was not within her reach and she very much had to use the bathroom. We were scolded by her nursing assistant named GWEN for stopping her in the hallway because she was too busy! There are many other complaints but one of the worst happened the night before moms discharge. Mom wanted an ice chip to moisten her mouth so we gave her one. And right after we gave it to her her nurse PAMELA HARRIS walked into my mothers room with a small cup of a pill mashed up into pudding. I asked her to wait because my mom had ice in her mouth, my sister told PAMELA HARRIS mom had ice in her mouth but PAMELA HARRIS did not listen to us. She proceeded to spoon the pill into my moms mouth. My mom could barely speak in general but with all her might my mom uttered WAIT! But PAMELA HARRIS FORCED IT DOWN MY MOMS THROAT forcing my mom to swallow the ice chip along with the meds. My mom was having a lot of difficulties with choking and swallowing and PAMELA HARRIS forced my mom to do it. The next day my mom was coughing up blood. I assume because her throat was irritated. Also the next day, my mom ate breakfast and lunch, very little as usual because she also has no appetite but was not given a dinner tray. Her evening nurse was PAMELA HARRIS. My dad asked at the nurses station why she received no tray and heard PAMELA HARRIS tell a nursing assistant that she didn't get one because she had problems choking but someone ordered my mom a tray because she later received one. I fed it to her and she ate very well. Of all people PAMELA HARRIS picked up the almost empty tray. I asked her why my mother didn't receive one in the first place. She said she was told she had problems choking and I asked 'so is it The Citadel's policy starve people who have difficulty swallowing? As you can see my mom ate very well'. Needless to say I will NEVER EVER RECOMMEND The Citadel Care Center to anyone I care anything about! read more

For my school, we had to come here for our clinicals for our program. The actual CNAs (certified nurse assistants) were so rude to all the residents. They would force feed those who needed to be fed, the way that they talk to the residents were so rude, and everything. One of the residents said she got a bruise from the Hoyer lift (to help the residents to get into their wheelchair and back to bed when needed) because she has been hit by two of the CNAs and those CNAs were not even sorry. As students, we were not allowed to be left alone with the residents, unless we were just having normal conversations, and the CNAs were told that they were supposed to be with us and we were just assisting. They kept leaving us and would go MIA for the whole shift. We were supposed to follow the CNAs that we were assigned to but couldn't due to some of them not wanting students to follow them. I got the vibe that everybody who works here don't want to be there. Also, as nurse assistants, it is not in our scope of practice to adjust the oxygen tanks. Only time we were allowed to touch things related to oxygen tanks were just to adjust the part that goes into the nose to help the residents feel better. Nurses would be telling all the nursing assistant students to do everything that nurses should be doing or the things that are not part of the nursing assistant's scope of practice. They were so rude. read more
