You have to be on top of things, make communication happen and visit your family members daily in the morning, noon and evening to make sure care is meeting your standards. Especially since there are not enough staff. Normal staffing is 18 to 20 patients per nurse. Staff management and caseworker communication is imperative. read more

This is rather slow in coming, but believe me when I say that it has taken me a year to calm down enough to write this without using "bad" words!! When I was discharged from the hospital, I did not have the "PROMISED" option to select my rehab facility. I have to say, that the best thing about Citadel Post Acute, was the morning oatmeal!! As a newly diagnosed diabetic, I received NO guidance on diet, nor were there any menu choices suitable for a patient with diabetes!! Pasta & bread at every meal?? One whole week with NO salad options?? PT, OT workers were caring & encouraging & they did get me up, standing & walking again. There were two night nurses who actually seemed to give a darn, but the rest of the "caregiving" staff did not seem to give much of a darn about patient care. And don't even get me started on the "medical care" - I contracted an eye infection that the doctor (?) did not look at until after they had been treating it with saline drops (no improvement) for over a week!! And in the 6 weeks that I was there, they stuck me with 4 different room-mates, & apparently because I was not a "complainer", each one was worse than the one before! One even tried to smoke in the bathroom while I was 6 feet away on OXYGEN!! Stay away!!! read more

This is a clean place with some of the best CNAs around. Miles and Rooster are the best here! Along with Eric the wound care nurse. I would personally like to THANK them for everything they did for my step mom while she was here. There was only one time that I was disappointed with this place. I witnessed a nurse berating a patient here. Saying foul things and that's not right. I actually spoke to a other nurse who was apologizing for something she had nothing to do with. I understand this job may be thankless and very hard at times but these patients depend upon you all. Take a break if you need it, EVEN IF YOU CAN'T FIND TIME, YOU NEED TO. This place was pretty good in the beginning,except Medicare was supposed to pay for 20 days with my mom there. The first week they were trying to get her out of there. They also put her in independent living when a family member kept insisting that she could not live by yourself. So she went there for one day and hospice came in and said no way she could not live there by herself so hospice moved her into a long-term care. And because my mom Signed a lease they charge for two weeks. Medicare paid for the first seven days so they overcharged my mom. We tried to call them and they will not return our call. I can't believe that they would do this to her especially since a family member kept telling them that she cannot take care of herself. She was left there with no groceries and no way to get groceries. They put her in a three-story building. When she asked to be downstairs. My mom is very unstable on her feet. I just thank God. She's a good place now, but we'll never go back there. Very disappointed. read more

We chose this facility for my elderly mother who needed a few weeks of mobility therapy. The hospital said that this is a 5-star facility, so we were excited at first...until my mother was checked in. Long story short, the nursing staff are all rude and incompetent, except for Sam. He is the only nurse that will give you a clear answer. The other nurses will answer your questions with "I don't know" or "so-and-so handles that". And that's IF they even acknowledge you! My mother would pull the emergency cord and the staff wouldn't come for at least 30 minutes. She didn't get any physical therapy for the 1st week she was there until I showed up and complained. Then she only got 2 PT sessions during her 3-week stay. Her roommate vomited on the floor and the staff didn't come to help her clean up until 90 minutes later, bagged up her clothes and left it in the bathroom for 2 days before throwing her belongings away instead of washing them like they're supposed to. DON'T SEND YOUR LOVED ONE HERE UNLESS YOU HATE THEIR GUTS AND WANT THEM TO SUFFER. read more

I really have mixed feelings about this facility after my mom needed to stay here. There are some good employees there, but unfortunately the bad ones outweigh the good ones. My mom has begged me never to send her back here if it comes up again. If a person is unhappy with their job, they probably should quit and not take their frustrations out on the patients. I completely understand they are understaffed and some patients' needs outweigh others, but this is not an excuse to be rude to the patients and take it out on n the patients. This seems to be the case here. I had hoped the positive employees I encountered would help with my mom's overall stay, and I thank the ones that were kind to her during her stay. The hospital gave us a printed list of skilled nursing facilities with ratings between 1-5 stars. Shame on them! Turns out the first one on the list was Citadel Post Acute with a 5 star rating. WHAT A JOKE! Whoever rated this awful place should be forced to stay here. I followed my mom from hospital to Citadel in my own vehicle arriving immediately after her arrival. I entered the facility through the front lobby. The younger man at the check in desk informed me they had Covid Patients. I replied " it would've been nice to know this before prior to my mom and I coming to facility. " Well, wouldn't you know. Both my mom and I caught Covid. Neither one of us had Covid before. I proceeded from lobby to my mother's tiny room which she shared with another patient. The hallway smelled of feces. Patients were rolling around at will. The beds in my mom's room were set up like an " L ". Her roommate's feet were down near my mom's head. My mom was up against the wall. Her roommate' s television was in my mom's left ear. Awful arrangement! Immediately after the check in process and attempting to get my mom settled in I went to the front of facility to arrange for another skilled nursing facility. P.S. Building smells clean, nurse's station is located in the center which I find it helps efficiently with the staff being able to see down their halls. Ever since Cory Williams is the new Administrator there the staff retention is terrible and patient care is deplorable. The VA should look into the abuse of veterans there. And if you even dare to complain or bring it up to management you are written up or escorted out of the building. They spend more time hiding problems than really fixing anything there. Don't come work for this place. And if you are seeing patients there move them out. Your patients deserves better. Don't kiss ass in there just to get referrals. The discharge case managers accepts giftcards for referrals and hides it from management or management just turns a blind eye to it all. I've seen it personally and it's apalling. I hope the state does something about it soon. I've filed my complaints but I'm sure they will just destroy records or manipulate the charting. My wife was admitted yesterday to rehab her legs from a two week hospital stay. She is on oxygen 24-7. Thirty minutes after arrival, she called me in a panic as she had no oxygen. I called the front desk and they said they would take care of it. A little while later, she called again as her TV wouldn't work. They told her someone stole the remote. I headed there to see what's going on, and by the time I got there, they had found a remote. Today, while visiting, my wife said she hadn't received her medicine today. I told her wait awhile and see if they bring it. Around dinner time, she called and said no medicine and the nurses won't answer the call button. I call the front desk and she transfers me to the nursing station. Rings for 6 minutes and no answer. I call back and ask to speak to a supervisor. The only one on duty was the PT who had spoke to my wife. I ask her to look into the medicine issue and she said she would and would call me back. In the meantime, the nurse finally comes in my wife's room so she asks about her medicine. The nurse tells my wife she gave it to her at 7:30 am and it wasn't uncommon for Alzheimer's patients to forget. My wife doesn't have Alzheimer's and she takes close to 20 pulls in the am, and it's hard to forget several cups of pills. I drove there, and picked up my wife. I'd be cautious sending anyone there. The hospital picked it and told me it was five star rated by Medicare. I wouldn't even give it one star. I'm sure the owner will come on and add her comments, but after this many negative reviews, it's obvious she is only in it for the money and could care less about patients. I can't wait and see what they bill my insurance company for when they literally provided my wife no services. Funny thing is, every bad review on here, which is pretty much all of them, is exactly what we experienced in one day. read more

This is definitely going to be a review that if I could rate them zero I would. Citadel Post Acute Care is a horrible place to have a loved one stuck in. My wife was relegated to this establishment after a 3 week stay in Banner Desert. They suggested moving her to a Rehabilitation facility and gave me a list of several that I could choose from. I made three choices based on the ratings they provided me. One of the five-star choice s was Citadel. Which was the only one available that could take her in. That was our first mistake. I should have read some reviews before letting them take my wife there. They seemed nice the first days, but the level of care and service fell off quickly. My wife was there from 8/12/ 2023 until 9/27/2023. It became obvious that all they were interested in was the money they could generate from having rooms full. I'll start with the pathetic excuse for food that they serve because that is the least of the horrible items they present. Hopefully they allow my pictures of the food I took but suffice to say it was hardly edible and pathetically presented. The lowest level food quality products are definitely used to maximize profit I am sure. Let's move on to the level of care. My wife was there to get back to walking after a battle with edema and blood clots in her legs. The one and only positive thing I have to say is that the physical Therapy team seemed to know what they were doing for the most part. The most notable problems stemmed from my wife's inability to get out of bed on her own for most of her stay. So adult diapers were employed for most of her stay unless a nurse was available to help her out of bed and get to the bathroom. This is a list of some of the humiliation my wife was subjected to by the nursing staff and workers. One nurse told her to defecate in her diaper because he was too busy to help her to the bathroom right now. One time a nurse wouldn't help her to the bathroom. She was left overnight on more than occasion without being changed. She had asked for pain medications at nighttime and did not receive them until 6 am the next morning. This seemed to be an ongoing issue throughout her stay. The so-called nurse practitioner that oversaw her care and medication had no idea about my wife's ongoing issues and had her taking salt tablets that ended up making her condition worse. I was finally able to get her to her specialist and she was very upset with the medication they were making her take. Luckily, she was able to get that adjusted after several phone calls and that really helped the swelling start to go away. There was a period of 12 days my wife never received a shower and mostly only got showers twice a week. I have photos that show for three days she was left on urine-stained bed linens. One other time she was told they couldn't change her bed due to waiting for laundry to get done. One time she was told she could not have her pain medication because the facility was out of the prescription. I also have several photos of nurses sitting in the hallways playing on their phones while several call lights can be seen down the hallway. Response time for the call light was horrible, sometimes hours between turning on the light and response. Especially at night when they would run one or two nurses. Usually at night late night I was told that they send one nurse home and leaves one nurse in charge of approximately 40 to 50 patients in my wife's hallway. On several occasions I overheard nurses talking in the hallway about other patients or how they didn't like this or that about working there and especially about what they get paid. Thereis much more to say and pictures to document but moving on to the financial side of things. When my wife was admitted I was told by the facility that I would need to make a copayment on my wife's stay because her insurance did not cover everything. Not knowing that was false I went along with this. I asked them about my wife's deductible because after three weeks in the hospital and an accident from earlier in the year I was pretty sure we had reached our maximum out of pocket for the year but was told I still owed $ 672 dollars per week at my cost. Turns out that was a lie or at least an ignorance of how the insurance worked. I ended up paying them $3680 out of my own pocket and since we are not wealthy people, I had to use a credit card to pay them. In a stroke of luck my wife's insurance company called to check on her progress and to let us know they were covering all costs at this point. I told her about what I was told by Citadel, and she was shocked that they had done that and that I was due a refund because they had already covered the cost. Citadel said that yes, the insurance company had paid the entire bill. Even though they knew this they continued to bill me as well. I was told by Citadel that they would process and refund my card. Never go here and have Therapy done after Surgery. Look elsewhere would be my suggestion. Could not get information on our 86 year old Mothers Progress from her Hip fracture. Contact numbers were incorrect. The place Stinks of Urine and messy Rude care people. We have no idea how much therapy she actually got. Then on the top of it all they gave us 1 day to figure out what to do with our Mother. When the Insurance coverage is over they just kick them out. BEWARE!! If I could give 0 I would! This company's behaviors have been so egregious I have no choice but to make sure no one's loved ones get involved. I wish I saw this review as I was searching. From the beginning the care was awful! He was transferred from the hospital needing to speak to me his POA. They #1 never called, #2, "never received" multiple messages left, and #3 never set him up with his needed devices required for his sleep listed on his chart that cause him not to breathe. I had consistent issues getting any phone calls and was told between the front desk and nurses that this is normal the nurses use this excuse. At some point they moved his room and AGAIN didn't set up his device until discharge!! He left weaker than I've ever had him leave a place. He has a brain tumor and was in a 5x gown (WAY oversized) his brief was completely soaked not even close to ready for discharge. No nurse was even available to help. They lost our bag of his stuff brought in then proceeded to fight us on it until we pushed and told them to pull the cameras to prove it was brought in. Then tried low balling what they would repay us significantly!! Then finally agreed in a GC but then that GC came with the code scratched off and when trying to contact the manager regarding this he initially ignored us then we pushed and he said it was "redeemed" conveniently the day AFTER we sent him the email where he could clear it up with Amazon then refused to contact us back! My grandmother spent time in this horrific facility and developed a awful bed sore that ended up sending her to ER & PCU care for sepsis/blood infection. Several different physicians state this was due to staff not turning her regularly and could have def been prevented. LPN "Stephanie" demonstrates lousy customer service & patient care. My sister in law was admitted to this facility in June and was there until July 31st. The staff never knows anything about anybody. They don't care about their patients & their families. My sister in law called me 3 times crying for help because she bled out & no one responded when she pushed the call button. I spoke to a lady in the Nurse's Managers office & told her I was upset because my sister in law should not have to call outside of the facility for help. Her response, "Well we got her help & that's all that matters"wtf!? They threw away her belongings knowing that I pick up her stuff to wash at home. The Citadel's operations model encourages their patients loved ones to participate in the healing process. Having full visitor access 24/7 and visiting my spouse in the facility 21 days in a row gave me a keen insight into the craft of post-acute and the healing process, unfortunately, not every patient responds to treatment and lives happily afterwards. Given the task of observing, treating, documenting, maintaining, and caring for an individual's every need is a demanding task, and EVERY staff member on the floor that I encountered met the challenge with zeal, compassion, PATIENCE, HIGH MORALE, and top tier professionalism. Yes, there were concerns at times, but they were always addressed despite the unpredictable waves of activity. At the end stage of post-acute treatment, the administration wisely assessed and arranged the next phase of my spouse's treatment to a specialty facility. My mom is currently at this facility and is trying to get transferred out. I have to call multiple times to get simple answers about her care. i was told she'd have bloodwork done on Monday, and when I called 20 times to get the results - due to my moms blood issues - they said the labs were never done. I am just disgusted with how this place is run. So far, the only great person i've spoken to is Kat, she was very helpful and polite. The nurses at the nurses station always say they'll call back or they'll leave a note. I did speak to the director of nursing and she was somewhat helpful. The nurses take a rude tone when they answer or when you call "too many times". HORRIBLE facility!!! My rating would actually be -5 if that was possible. If you care about your loved one, do NOT send them here. My husband was transferred to this facility for end of life care on 5/31 from St Joseph's hospital WITH his medical records, DNR orders and BOTH a medical and durable POA. This facility IGNORED his records, which clearly stated his drug allergies, and took it upon themselves to change his pain medication to one he was allergic to and then ignored his allergy symptoms. The staff are rude and uneducated and actively interfered when my POA agent tried to pick up personal belongings that he did not need at the nursing home. Upon his passing on 6/12 I was told that his personal effects, which included his full wallet and a laptop with charger and case, would be safe in their care until I could have them picked up. On 6/17 his personal effects were picked up only to discover that his complete laptop/case/charger and his wallet have been stolen. We have since discovered that his personal effects were not accurately cataloged from the moment he arrived at the facility. Due to his quick decline at the hospital I did not have time to research this facility. The CNA's show NO respect for patient rights and privacy. The facility itself is in the condition of a 1960's crack house. The Director is NEVER available to address problems. The FNP's know nothing and don't speak any english. RN's routinely ignore patients and skip medications, falsifying records. I had cash stolen from my fanny pack, reported to Mesa PD. When your insurance is about to end they give you 2 hours to find another place or get out. Social Workers are getting payola to place patients in their poor independent living area. No dignity here. My dad went to Citadel after he was in the hospital. After a week at Citadel, he was actually worse than he was when he arrived. We had him urgently sent back to the hospital for chest pain, seeing spots and feeling faint. The hospital admitted him for dangerously low blood pressure, dehydration and return of pneumonia. There are some good, caring people there, but it seems they are spread thin and there are a lot of others who are incompetent and should not be providing care. The room he was in was 6X6 if that, with patched walls, electric strip of plugs hangs over his feet, he was given pillows with no pillowcases and the sheets were stained. When he pushed the call button, he did not get a response for over 15 minutes and had to take himself to the bathroom. The food was cold and disgusting nothing a healing patient should be receiving. When I asked for something different for him to eat, I was told they have vending machines. The week we were there we did not receive physical therapy, a shower, or any help with basic needs. The facility itself is horrible and not a clean environment, as soon as you walk in you are hit with urine and cleaning products. During our time there I heard people begging for help, other families complaining of care and at one time I heard a patient ask for a shower due to accident and was told it was not her scheduled time for a shower. She begged and was crying, they said they would see what they could do, but I never saw anyone return to help her and I was across from her room for another 8 hours. Citadel may not be the prettiest building but that doesn't matter. It's the staff and the care a person gets is what matters most. My husband has been at Citadel for over 6 months in post acute and long term care. The people are caring and know what they are doing. I have full confidence in them. The nursing staff are amazing. The Director of nursing made sure my husband had everything he needed to get good care and be safe. He is 100% dependent on others. He requires to be fed, changed, dressed, etc. by the CNA's. They are amazing. They truly care about the patients. Abby, Jen and Kat are amazing Assistant Nursing Directors. The nurses are caring. They treat the family like family members. The kitchen food needs some work to provide balanced diet meals and pureed food for patients, but they are currently working on improving that. As a patient and family member, you feel heard when you have concerns. I can't forget to talk about the amazing Physical Therapy staff. Emily has done amazing work to improve my husband's muscle strength. Beth, Ashley and friends in Occupational therapy works miracles along with the PT staff working together as a team. The Isabel the speech therapist is great and pays close attention to my husband's ability to swallow. Case Management is so helpful to get my husband's needs met. My relative was in there for 2 weeks he was not shaven and his appetite was down and the nurse stated that he did not "feel good", so this is why he was not eating! I would recommend not using this facility with having to have NPs, CNAs, nurses give patients healthy grooming everyday, have them eat even though they do not say they want to this is what they are getting paid to do be a care giver! We should have been FaceTiming every day to see what was really being done or NOT Done due to COVID-19 you are not let in the facility and within one visit you would access the situation on grooming and eating etc as poor! I asked all these questions after reading the reviews and was assured that all my concerns would not happen and this came from an outside 3rd party! Now seeing the lack of care and the lack of communication on a daily basis about the caregiver inside of Citadel, Meza vs the loved ones outside and the ones that normally would be able to assess these items if allowed in! Glad for FaceTime however we took our loved one out and high recommend home care at this time of not being able to be allowed to visit you loved one. It was a big disappointment to have a nurse tell you that your loved one said he did not want to be shaved and not doing it Really that is why he was in there care to keep him groomed as he was every day when not sick and to go above and beyond in caring for a patients as you should expect, but are not getting even more so because of COVID-19.
