    My mother is currently in this nursing center for 6 weeks and this is what she receives for her meals. Is there any nutritional value to these? And because of Covid, they will not allow me to bring food to her. And she is not on any food restrictions.

    Some nurses do not I repeat do not administer medication correctly. They put my mom in the hospital TWICE! They almost killed her.

    This is rather slow in coming, but believe me when I say that it has taken me a year to calm down…read moreenough to write this without using "bad" words!! When I was discharged from the hospital, I did not have the "PROMISED" option to select my rehab facility. I have to say, that the best thing about Citadel Post Acute, was the morning oatmeal!! As a newly diagnosed diabetic, I received NO guidance on diet, nor were there any menu choices suitable for a patient with diabetes!! Pasta & bread at every meal?? One whole week with NO salad options?? PT, OT workers were caring & encouraging & they did get me up, standing & walking again. There were two night nurses who actually seemed to give a darn, but the rest of the "caregiving" staff did not seem to give much of a darn about patient care. And don't even get me started on the "medical care" - I contracted an eye infection that the doctor (?) did not look at until after they had been treating it with saline drops (no improvement) for over a week!! And in the 6 weeks that I was there, they stuck me with 4 different room-mates, & apparently because I was not a "complainer", each one was worse than the one before! One even tried to smoke in the bathroom while I was 6 feet away on OXYGEN!! Stay away!!!

I'd heard so many horror stories about SNF and reading the reviews online made it even scarier. I'm so happy that I chose Good Samaritan! I can't say enough about the teams that assisted me in my rehab from a broken hip. Two RN's, JoAnne and her team as well as Julie and her team made my recovery easier. The PT team, after one correction, were top notch! Here's something you don't hear too often about SNF's -- the food was awesome, that is, when Tammy was working in the kitchen. Otherwise it was dismal. The dietician was wonderful to order special food for my diet. Rosie, the Social Director, made me feel like I was with family; she is the best! All in all, the experience was good and I highly recommend Good Samaritan Society-Mesa. The only reason I couldn't go five stars is because of two incidents: Upon arrival from hospital my intake nurse was great and assured me that they would get my meds right. She was able to provide a pain med four hours after arrival according to dr's instructions. She left instructions for the night nurse to do the same every four four hours. Instead the night nurse totally ignored the dr's instructions and the ones left for her by the intake nurse. She wouldn't even come in to my room but sent word with a LPN that they didn't have the meds the doctor prescribed. (Which wasn't true). I was not offered any substitute, not even Tylenol! So for eight hours, I lay in pain, crying. The second reason I can't give five stars is because the executive team tried to dismiss me without pain meds, which I understand is not lawful in AZ. They had a temporary doctor on duty who could not or would not prescribe them. What? Yes, it's true. It was midday on Friday and I was told I would have to see my primary care physician to have him prescribe the proper pain meds. On a Friday??? (The last time I tried to get an appt with my primary care physician I had to book four weeks out.) I had remaining pain meds at this SNF that its' regular doctor prescribed and after a meeting with the nursing supervisor, we finally worked it out that those pain meds were mine and could go home with me. (My dr's office says that is the AZ State Law) It was enough to get me through the weekend. The pain meds were not strong Opiods, in fact, they just took the edge off the pain. I am allergic to Codeine and its derivatives so that eliminates all the oxy-contins, etc. Would these two events have not happened I would give Good Samaritan another star.

    My grandmother stayed at this facility for approximately two weeks. She is unable to walk or lift…read moreanything on her own, let alone unpack a suitcase. When I dropped off some of her belongings, everything was packed neatly in one suitcase: her nightgowns, slippers, bibles, her toiletries, and a few familiar comfort items from home. A few days later, I received a distressed call from her saying that her things had been thrown away and she had nothing left. Given her memory challenges, I called the facility for clarification. I was met with a dismissive and unprofessional response from a male staff member who said, "She has her things, and it's a lot of them," in a tone so indifferent it felt like I was being brushed off for even asking. I trusted the facility to tell me the truth and to help make sense of what my grandmother was experiencing. Instead, they used her memory issues, and my own understanding of them, as a shield to avoid accountability. When she was transferred to her new Adult Family Home, we finally opened the suitcase she left with. It was virtually empty. No clothes, no toiletries, none of the items I packed for her comfort. The new facility even reached out to us to ask why she had arrived with almost nothing. This wasn't just a mistake, it was a violation of trust. She only wore outfits given to her by the facility he entire time she was there, meaning there was no reason for anyone to have gone through her belongings at all. So where did everything, especially her clothes and perfumes, go? She could not have misplaced them as she physically cannot unpack or move her own suitcase. It seems as though someone at this facility went through her things, and instead of taking accountability, they dismissed both her and me. This is morally wrong. This is taking advantage of an elderly woman with memory issues and the trust of a family trying to believe the best of people. Do better--for the sake of the families who come to you vulnerable, and for the sake of the elders who rely on you for their basic dignity.

I come to the facility to visit my best friend and I see patients outside on wheelchairs cold with no shirt or blanket that grieved my spirit. My friend is having trouble walking. Sometimes they take one to two hours to come back I had to come from my house to this facility because she was ready to fall off her bed as they were going to turn her over. And it took an hour and a half for someone to show up in her room to help her this facility does not answer their phones !!!! Terrible terrible no love for their patients ! STAY AWAY!!!!!!!!!!!!!'

Disgusting! ALL of these SNF's are horrible in AZ. After 10 1/2 long months of being gone from home, fighting for his life from Covid, he died. The first evening my mom was there I walked in and she was laying on a plastic mattress with no sheets on the FLOOR!!! I demanded they raise the bed, put some rails on it and put sheets on it. They said they couldn't find the sheets to fit it! I told them to bring in a different bed then that had sheets that fit. The following day the in charge nurse told my father, in front of my mother that this was the best it was going to get. She would not be getting any better. At this point my mom couldn't speak, couldn't get up and walk, but knew what you were saying. It was the first time since her stroke and brain surgery that she cried. She knew what this nurse was saying. I was furious. I got my mom up, had her stand while I held on to her and we would just hug and sway back and forth to get weight on her legs and get her muscles working again. (Isn't this what a rehab hospital should be doing???). Within 3 days, my mom was able to walk to the restroom and use it. Now she didn't have to lay in her waste for HOURS waiting for someone to come change her. Within a week, they had a hard time keeping her down. She began talking again. Although still very confused with memories, she was improving greatly. That nurse tried to make me feel stupid like I had no idea and because my mom had had a seizure, she was now going to live an infantile existence for the rest of her life. If it was up to them, she would. They were doing absolutely ZERO memory care with her and occasional pt. I was horrified at what was going on. I had to throw fits to get her bathed. I was constantly being told they were understaffed or the 'shower girl' didn't show up. Today, almost a year later, my mom has some memory issues and dementia, but she is home and functioning pretty darn well! Especially for someone who was never going to get better! The picture is of my mom the other day, riding her bike. I've thought of taking it into the nurse and telling her don't you dare EVER tell a patient or their loved one this is the best it gets and she won't get any better.
