The office staff of this doctor is delightful and accommodating. My frustration comes from the…read moredoctor himself. After having a fracture of my tibial plateau, I went to see Dr. Mai in Hanford. The exam was extremely minimal; no more than 10 minutes tops. He poked my leg and patted my knee. I told him it was extremely painful. I asked if there maybe should be an MRI or something to check out the soft tissue. He told me that he would wait for the fracture to heal first and added that the next few weeks would be hell, good luck, and left. I phoned the office several times in the next few weeks with various concerns. Mostly I was told to just be patient. So I did everything they told me to: Stay off the leg. That's it. Okay. Fast forward to the second and last appointment. He told me to take off the brace and did the poking and prodding thing again. I pointed out that there is still swelling and a great deal of pain. He said that it was normal, and said I was free and clear to use the leg again. Concerned about the continual swelling, I asked Dr. Mai if maybe an MRI would be appropriate, so we could see if there is further damage to the knee. He said no. Nearly verbatim: "If you were a younger person, this fracture might be tragic. They ride bicycles, hike, swim and all that. You are old. So it doesn't matter. Eventually, you will get arthritis, and will probably get your knee replaced." I stifled the urge to say something rude or poke him in the eye. I asked about physical therapy. "Oh no. Just use it as much as you can. Let pain be your guide," he said. Going home, my leg swelled up even further. My knee actually bent the wrong direction twice one day. I became more careful to avoid a repeat performance of this. Meanwhile, I searched for another doctor so I could get another opinion. My knee is very painful. My leg continues to be extremely swollen. My knee is pretty warm to the touch (warmer than the other side). I cannot use it to stand or sit. I need to grab something to help with that. It has now been nine weeks since my initial injury. I continue to go through intense pain. I continue to hope to find someone who might actually care to help me. I will say that the office staff, was caring and allowed me to get my records immediately when I traveled to their office. As all orthopedic doctors are more than an hour away, it's quite an ordeal to see anyone. Dr. Mai's records neglected to mention a complete medical history. I think (although what do I know. I am just a journalist and music/art teacher) that osteoporosis and a history of over thirty fractures, COPD and all would find their way onto a relevant history. He also said that he checked things that I do not recall him doing AT ALL.

The disorganized receptionist told him they didn't have his MRI and we had to go to the medical center to get it. The x-ray lady verified it had been sent and called them. After waiting on hold for 15 mins, someone finally came back on and said they found it. So we get back at 12:30 and wait for an hour and 15 mins and still wasn't called. We left for another appt. Disorganized, disrespectful. Don't unless you have to.

I went to Orthopaedic Associates a few months ago because of an ongoing hamstring injury and was…read moreseen by Amanda Pacillas, NP. During that appointment, Amanda told me she would get orders placed for both physical therapy and an MRI. I waited, but I never heard anything about either the physical therapy or the MRI. Neither happened. At the time, I let it go because I was hoping my hamstring would eventually get better on its own. Unfortunately, it didn't. It actually got worse. I recently called the office and asked about getting the MRI that I had originally been told would be ordered. Amanda's assistant told me I would have to come back into the office because too much time had passed and that insurance would need documentation that my condition had gotten worse. That didn't make sense to me. My question was simple: Why should I have to come back for another appointment to get an MRI that was supposed to have been ordered after my original appointment? I explained that I'm still having the same hamstring problem and that it has actually gotten worse. I asked whether this could be handled over the phone rather than requiring me to come back into the office. At that point, Amanda got on the phone herself and told me that it could not be done over the phone. Her tone immediately came across to me as flippant and dismissive. Rather than explaining why another office visit was necessary or addressing the fact that the MRI and physical therapy were supposed to have been ordered previously, she simply asked me, "Do you want to set up the appointment now or not?" I told her I would need to call back to schedule it. Frankly, her tone and attitude during the conversation made me question whether I even wanted to see her again. She responded, "Okay," and hung up. I understand that there can be insurance requirements and circumstances where a provider may determine that another examination is necessary. But medical orders can be handled by phone or telehealth in appropriate situations, so I don't think it was unreasonable to ask why I needed another in-person appointment--especially when the MRI was supposed to have been ordered during my original visit. If there was a specific insurance or clinical reason that required me to come back, I would have appreciated having that explained to me. What bothers me most is the lack of follow-through. I came in for a hamstring injury, was told I would receive orders for physical therapy and an MRI, and neither happened. Months later, when the injury had gotten worse and I called trying to get the MRI taken care of, I felt that my concerns about what happened the first time were dismissed rather than addressed. Amanda was actually very nice during my original appointment, which makes the experience afterward even more disappointing. Between the lack of follow-through and the way I was treated on the phone, I no longer have confidence that my injury and concerns would receive the attention they deserve if I returned to see her. Based on my experience, I would not recommend Amanda Pacillas. I hope the physicians and management at Orthopaedic Associates take concerns like this seriously. Patients should be able to expect that when they are told something will be ordered, it will actually be followed through on--and if something goes wrong, they should be treated respectfully when they call to get it resolved.
