It is with great pleasure that write this review on behalf of my mother, who has been a resident at Care One at Wayne, since late Fall of 2024, she first resided in assisted living and now resides in Harmony, their Memory Care unit. My dear mother, recently celebrated her 90th birthday, on her special day, we had a very lovely, beautiful pink themed birthday party with a large pink rose themed cake, massive pink balloons, a large banner, Frank Sinatra music playing in the background in a private reserved area. She even was presented with a pink tiara and birthday slash! The majority of the other residents from her unit attended, plus some family members and staff, were all sang happy birthday to her and celebrated my Mom! Earlier in the day, my Mom and a dear girl friend of hers, we had a ladies lunch in a private dining room, where they set up a very elegant table for her birthday. The overall attention to detail, level of kindness and excitement for her birthday, really made her day! My Mom said to me, "The party was quite memorable and one the best weekends, she has had in a long time." Recently, on one of my many trips to her facility, my Mom shared with me, "This place feels like home, it's so homey here, everyone is very caring and it's so clean too." As the only daughter who lives quite far away, to hear and know that your loved one, in this case, my dear Mom, is taking care for and over all very happy at this stage in her life, is truly a gift. Also, I would like to add, whenever her friends and or my good friends and their spouses or parents come visit her, they always consistently remark to me, " How clean, bright decorative the facility is and how welcoming the staff is to them." In fact, they only have praise for this facility, each time they visit. Care One at Wayne, in my opinion, what makes this particular facility so caring (excuse the pun) is that the staff on-site, they all take tremendous pride, professionalism and up most care towards their residents. I am so grateful that my Mom is very content and well taken care of, this in itself is truly priceless, and the best gift of all! read more

Nothing but neglect from when I arrived from Chilton Hospital , told them I needed a shower with supervision as my legs were weak, gave me a shower chair and never came back. Shower was clogged and overflowed as I finally took the risk alone. Was told I needed iron since I had a GI Bleed and I heard Doc tell his assistant to order I receive it, she never gave order. My neighbors at Alex Hearle told me don't go here but I thought it was going to be ok, and closer for my kids. I should have listened! Finally checked myself out! Mattress is horrific, broke out in sores on my back. It should be called Care None. read more

Do NOT let a loved one go here. They WILL be neglected. My father spent up to 8 hours a day in dirty diapers. He had bed soars so bad he needs a wound nurse. read more

I agree with the reviews warning not to place an elderly relative here. My father was admitted after hip surgery and received next to no "care". On the morning after he was admitted I arrived at 10 am to find his food tray sitting out of his reach. I was told it was delivered at 8. The food was cold so he did not eat it. The call button was not within his reach and was placed on a shelf above his bed. I was advised that PT would be in shortly after 1 pm he was placed in a wheelchair and left sitting in his room. I went to check on when PT was coming and was not able to get an answer. I walked him around a bit to get him out of the room which appeared to be a former private room now being used by 2 patients. The area he were his bed was small and confined he could barely have a visitor. PT did not show up until after 3 pm by this time my father was exhausted and could not complete the evaluation. I was told he would be evaluated the next day or Monday. Every day in bed is detrimental especially with elderly. My family was there and did not see any PT performed with my Dad. On my next visit the food was not within his reach. I asked that he be assisted into a wheelchair because he asked to leave the room because it was depressing. I was told they were too busy and short staffed. That same day my father became cold and started shivering. I got him a blanket which wasn't enough and advised the nurse, who told me she would be there soon. My father was still shivering and complaining of cold I again went to the nurse and said he was not well and she finally came to check on him. They could not get a temperature because he was shivering so bad. They attributed it to dehydration and stated an IV. I was afraid I would lose him that night. I was unable to stay with him because he had a roommate so I arrived early the next morning and he was not doing well. I was told he was being transferred (luckily it was where I wanted him to go originally but they didn't have a bed at the time) My father still felt warm and was now coughing. When I brought up this concern, I was told the chest xray on Saturday was clear. The transfer was arranged and again it was a disaster. They ordered a wheelchair transport for a man who couldn't stand and when they came, they had no leg rest for the chair. When I got him out of this dump it was too late he had developed pneumonia. I lost my father and feel that the lack of care he received at CareOne was partially responsible. read more

Sent my dad here for rehab and he ended up with sepsis and died. And I've been trying to get the bill adjusted because he never should have been removed from Medicare but I have been unable to resolve the invoice. It's like I'm paying the people who killed my father. Do not recommend. read more

Care One has been nothing but amazing for my dad and to our whole family!! The type of care that he has received was more than we could have asked for. Every person that works there has cared so much about the well being of my dad while making sure we were all informed of anything we needed to know while he stayed there. The staff was so amazing at keeping my dad's spirits high everyday that the day he left the facility he was beyond emotional. P.S. my dad never cries. They walked with him every step of the way on his healing journey. He was so touched that still today he brings up Care One as one of the many reasons he is doing better. The staff, especially Indirra and Erica as well as countless others such as Menchu, Julius, Jillian, Katherine, Ryan, Barry, Karen, Shin, Mary, Chin, Leeah, Marlon, Vanessa, Yumel, Jenn, Jay, Serena, Dulce and especially Dr. Varghese have completely contributed to my dads over all care and reason he feels the confidence to keep getting better every single day. We are so grateful and beyond appreciative and blessed to have met an incredible team of people. Please forgive me if I missed anyone. You are all amazing in every single way. IF THERE WAS A ZERO STAR OPTION I WOULD NOT HAVE BOTHERED GIVING IT. My father is been admitted here after an open heart surgery, not sure about how they go about with PT, it's horrible for some reason they want to get rid of patients, they give wrong reports to insurance and tend to bypass their duties. It takes more than hour to respond to a call button which is the most important aspect as the facility seemed to be understaffed. They always look for complaining against the insurance company and I believed them at the very initial stage but the moment I saw the feedback they have shared with insurance as if they are kicking patients out of the facility. My father is still requires supervision while walking, he gets tired while walking or eating not sure on what perspective they think he is good to go for home care. Bunch of liars no proper doctors nothing, I would not recommend anyone to this place. They treated my father like garbage can specifically during this outbreak of corona virus where elderly people are more vulnerable. read more

I agree with past reviews that this place seems very quick to want to kick patients out. I did feel the PT was good and mom made progress here (after a short stay in the hospital for broken hand). However, I did not feel she was ready to be discharged and the PT person also seemed concerned about where mom was going. I emailed the social worker and the Dir. of Admissions, but was told "medicare has strict guidelines.". I also spoke to the social worker in person who eventually told me I could appeal the case, but by that time, the arrangements had been made to move her back to her assisted living. Also, the AL had agreed after evaluation that they could take her back, which I was surprised, but they told me there were things they could do to "ensure her safety." Long story short, within 48 hours, mom fell and broker her right clavicle which really has set her back to this day. I even wonder if the shoulder break happened before she left CareOne because mom was complaining about pain before she left. Other comments will add was I did think mom's nurse was very nice, but did feel they were short staffed. Mom did enjoy her roommate which was good. This place has that smell and I noticed a latex glove under the heater. When I mentioned it to the staff that it had been there for a week, the next time I went back, it looked like it was just shoved further under the vent. So, not the best in terms of cleanliness. read more

The main goal for my dad being here was for the Physical Therapy post- surgery. It was excellent. The therapists he had were top notch and much progress was made. Same can be said for his occupational therapist. I agree with other reviewers that they seemed short-staffed, especially with aides. Could be partly due to Covid. I agree that the response to call buttons is a problem. I tried to overlook that since the most important thing for our family was a successful PT program. For this, they excel. After speaking to someone who visits these types of facilities in the area, this is one of the better ones. read more

My friend referred this place after raving about the facility, therefore I had my mom sent there after her hospital stay. I was blown away by the staff from the admissions, social workers, nurses, housekeeping, doctors, therapists. Ive had nightmare experiences time and time again in other facilities and cant say enough about how professional and helpful the entire staff is here in Wayne. The place is also very clean and happy environment. Im so grateful and can sleep at night, knowing shes in great hands read more

Though Care One at Wayne was an adequate facility for my grandfather's recovery, their handling of the Covid-19 crisis was absolutely unacceptable. Two weeks into my grandfather's stay, they began barring visitors from entering the facility, which they did act early on, but on FaceTime calls with my grandfather it was obvious that social distancing practices were NOT taking place within the facility. The physical therapists who assisted in the FaceTime calls were within inches of his face, wearing NO masks. Who knows where employees go after work, what family members they come in contact with, and what they may be carrying. Temperature checks may be effective for symptomatic carriers, but with a long incubation period for presenting symptoms and asymptomatic transmission, it is useless. Not until the last week of March did employees become mandated to wear face masks and PPE. (Side Note: During a FaceTime call, the PT who was managing the call for my grandfather was wearing a mask but sneezed centimeters away from his face and did not bother to move away nor cover her mouth additionally.) We relentlessly brought up our concerns to the social worker, Jennifer, and other staff members as cases in nursing homes and long term care facilities rose in the state. We were constantly reassured that the facility was 'air-tight' and taking all necessary precautions. Speaking with another employee, we were also told that there would be no new admissions of patients to further prevent bringing the virus in. When cases grew dramatically, we moved to get him discharged, but were met with resistance from the staff even though he was cleared to leave within a few days. He was in perfect health and had no reason to be there, besides his own willingness, and potentially be exposed to the Coronavirus. The additional time that he was to spend in the facility was by his own choice of extension and not necessarily recommended by the health professionals or mandatory. He simply enjoyed the friendly staff and had good insurance that would cover it. In my opinion, patients were overly comforted about the risks of contracting Covid-19 in their "safe little bubble". As Gov. Andrew Cuomo said, 'it only takes one ember to start a wildfire', and one ember it did. On April 2nd, our worst fears were confirmed when we received a call from Care One alerting us that there was a positive case of Covid-19. We were told that the positive patient had been brought in from the hospital days earlier with an already suspected case of the virus. Why would a long term care facility, already vulnerable, accept a symptomatic and presumed case of Coronavirus? This was entirely shocking as we were assured no new patients were being admitted from the hospital. Staff informed us that this patient was being isolated and on the opposite side of the facility that my grandpa was on and that any potential outbreak was under control. At this point, it was absolutely necessary for my grandfather to return home, even though employees had told us that they felt 'at ease' with the situation. During the discharge process, we asked the social worker to be informed of any outbreak arising in the 14 days following his departure to help us better monitor him and assess the level of isolation he'd need when returning home. She said she would and made note of it, but never did. We thought maybe it was a good sign until the New Jersey Department of Health released the numbers for infections and deaths at every long term care facility in the state. We were appalled to learn that Care One at Wayne had 12 cases, and as of writing this their total is 30 cases and 5 deaths, likely not including employees or those patients who were discharged amid the outbreak. It is absolutely disgusting that within the 14 days of my grandfather's discharge he was not notified of any outbreaks to take further precautions with his health. He is 91 and lives with his elderly wife, who could have also been put at risk. For a healthcare facility to sweep cases under the rug and hide it from patients and family members is irresponsible and could prove fatal. This is exactly the type of behavior from these types of facilities that led the Department of Health to publish every case and death in long term care. Trying to hide and diminish infectious outbreaks to avoid negative publicity does exactly the opposite. Grocery stores, including local Trader Joe's and ShopRite's, have done more to notify customers of possible contact with positive employees and shoppers than Care One at Wayne, a healthcare facility did. Just to make this clear, the issue is not that Care One at Wayne contracted and had an outbreak of the virus, as no facility is immune. It was inevitable. Instead, the issue lies with informing vulnerable patients about possibly fatal health risks within the 14 day exposure/incubation time frame. read more

I went there after a fall down the stairs. Then went for spine surgery. Came back for rehab. Got an infection and was treated with IV strong antibiotics. After a month became very nauseous and couldn't eat. Lost 20 pounds and they never checked why. Finally I said I wanted to go to the hospital so I was rushed there. Turns out I had kidney failure and needed dialysis. Care One never picked up on this. I could have died if I didn't insist on going to the hospital. Now I am finally home after one more month of rehab a reputable place. I have nothing good to say for Care One in Wayne. read more

I would give Care One no stars if that were possible. The care my mother received there was neglectful and often worded on elder abuse. Billing was so confusing that they ended up overcharging for non delivered services. My mother contracted infections there and the sanitation was very spotty. I pulled her out and put her in a much better facility where all her issues were quickly resolved. read more

My husband was admitted to Care One Wayne in late January 2019 for rehab after a fall and hospitalization for fractured ribs and vertebra. He arrived at Care One via ambulette from the hospital at approximately 7:30 p.m. We were taken to his room and he was transferred to bed by the transport staff. After waiting approximately an hour with no Care One staff even acknowledging his arrival, he put his call light on. An aide came and asked if he needed something. She apparently alerted the nursing staff and a nurse came and said she would do his intake even though she was not his nurse. The next day my husband had nausea and vomiting and when he became agitated that evening the APN wanted to send him to the hospital. She agreed to check him for UTI first. When they couldn't get a urine specimen, they tried to do a bladder scan but the ultrasound scanner was not working. They catheterized him, took a specimen, and started an antibiotic. I kept asking for the results of the urine culture and after 3 days the nurse finally said the specimen was contaminated so they did not have results. My husband's mental status was deteriorating and on the fourth day of his stay the physical therapist said he had barely been able to participate in therapy after having a couple of good days. The next morning, when I arrived at Care One at 9 AM my husband's room was empty and I was informed that he had been transported to HUMC. The nurse said she called me and left a message on my cell phone but my phone did not show any call or message. When I got to the hospital, the infectious disease doctor said that the antibiotic he was given could have caused the disorientation, and that that antibiotic is not allowed to be given at Hackensack University Medical Center. The doctor questioned if he had actually had an infection. He was admitted to the hospital, and I returned to Care One that evening to get my husband's clothes. The nursing staff said they had been waiting to hear from the hospital if he was going to return after being checked in the ER. I was appalled when I went to his room and it had not been cleaned. The bed was unmade and the floor was covered with debris. In the course of my husband's five night stay, there were some positives. For the most part, the nurse was attentive and both the occupational therapist and physical therapist were great. Unfortunately, his hearing aids and glucometer disappeared from his bedside dresser in the middle of his stay. He had not worn the hearing aids because of the nausea and vomiting. I checked and saw them in the drawer before I left for the evening. The next morning he called me and asked where they were. When I arrived, his nurse said they had checked everywhere, including the laundry and food service. Both she and another nurse said they, too, had seen them in the dresser. My husband was unable to get out of bed so he did not take them from the dresser. I have attempted to speak to Jim Gardner, the administrator of Care One in Wayne. After being told that he was in a meeting, just stepped out of his office, etc., I left my name and number and asked that he call me. I never heard from him. I wanted to know what their process is when hearing aids go missing. I also wanted to share with him the good and not so good experiences we had with his facility. To me, it speaks volumes that he never contacted me. I would never recommend Care One Wayne. read more

My mother stayed here after a surgery several years back. I would never in a million years recommend this place to anyone. My mother would ring the bell for the bathroom but no one would come. She soiled herself repeatedly. Not sure if it was due to the fact that there was inadequate staffing for the number of patients, or if the staff were uncaring/unhelpful. From my interactions with the staff, it seemed like a combination of both. Her experience there was so traumatic that it triggered an eating disorder that dropped her weight 30 pounds. She has never rebounded from the experience and she has not recovered from the eating disorder to date. Don't send your loved one here to recover! That would be my advice to you from my poor mom's experience staying there. read more
