First visit. Pain was overwhelming - but my tech kept pushing me! It will all be greater later!

**January 2026 Update** I had high hopes when the Ensign…read moreGroup purchased the facility in August 2025. Those hopes and the hopes of others have diminished over the last 6 months. The Administrator told several of us that he wouldn't be reading our emails and we were to address issues to the managers. The managers would be running their departments like their own businesses. I contacted the company Compliance line (9/25 & 1/26) as well as the California Department of Public Health over dirty silverware after repeatedly contacting both the Administrator and the Dietary Manager. In December I sent a separate email to the current Dietary Manager that the proper procedures (as explained in September by the Infection Control Nurse) were not being followed by some employees due to the condition of the what was received. Her response, "I will review with my staff. Also, we are going to rotate in new silverware" (12/26/25). Yet, I continue to receive dirty utensils as recent as 1/15/26. The residents routinely are missing things from the meal trays (butter, milk, condiments, utensils...) causing the CNAs to leave the floor and deal with hostile kitchen staff. Food preferences are often ignored (request to have small portions and food choices). I have repeatedly emailed about this as well as meals being early/late as well as menu changes with residents and staff not being notified. This is a big deal because the CNAs rely on scheduled meals to manage their time. I've come back from a shower and the food carts were in the hall 20 minutes early. Cold food on hot plates, food over cooked, mushy rice, flavorless meals.... I realize that this is institutional food, but there is a lack of attention to what goes on the plates. Milk is regularly out of inventory and residents are given cereal without milk on more than one occasion (a grocery store is next door from the facility) or chocolate milk to put on their cereal. The Administrator was copied on each email I sent to the Dietary Manager. I have been nice and I've been aggressive in my language regarding these and many others and neither approach has worked. I also have transcripts of weekly meetings with the Administrator and kitchen management where these issues and others were discussed. The Social Services Department has not processing medical referrals submitted by the facility Nurse Practitioner. One that was submitted in April and never processed. It was resubmitted in October and again January. Another was submitted in October and has not been addressed either. I manage my own appointments, but the facility must submit them to a medical facility. This is but one of many I have with the department. I decided this time I wasn't going to follow up to see how long it would take. During a November meeting with the Director of Nursing I had in November he stated that referrals were discussed daily. He directed Medical Records to see me and I requested the information showing that the October referrals has been requested and in the system. As of today the referrals haven't been processed. Housekeeping is also deficient. Most of the housekeepers do not thoroughly clean my room (others have the same issue). The showers have mold and there are often strange drippings on the walls. The lifts (standing & Hoyer) that are used for those of us who can't stand or need assistance standing were purchased that aren't adequate. These Hoyers don't get high enough to get people in their chairs or out of bed get stuck under beds. This issue was relayed to management several times yet somehow they were purchased anyway (initially they were rentals). The oldest lift is the one that most CNAs use but it has issues as well. The three Hoyer lifts I used in the last week all of them were dirty and looked like they hadn't been cleaned in some time. One had crusty finger prints and another had a layer of grime where the bar was welded. It's not unusual for the batteries not to be charged (I've had 3 batteries not be charged in a single day). Noise levels in the facility by staff at shift changes and other times are excessive. It often sounds like they are having a party while residents are trying to sleep (6:45am and 11:00pm). Management has been notified several times but that only helps for a short time. A refrigerator that was purchased to be used by residents (mandated by the State of California) sat unused for months while we waited for the rules of use to be written. Some of us have still not received the rules despite mentioning this to several managers. The screens on the windows and sliding glass doors have been broken since I came here 2 1/2 years ago. For these and many other reasons I would not feel comfortable having a family member receive care at Courtyard.

Fabulous place to bring your child for up to date therapy through play!! The therapist are loving,…read moregentle and at all times professional. Appointments are quick to meet the needs of the clients. The office staff work with the parents to meet financial needs and to ease the stress of the parents. Not many places you would see the Executive Director sitting on the floor playing with the children, but you will see that at CTC. I have witnessed children carried through the doors and after getting Therapy at CTC, I have seen those same children WALK back out those doors! I have seen mothers cry because after sessions of Speech Therapy they heard their child the first time say "I love you mama". That's priceless and that's what CTC helps happen all the time!!

My…read moredaughter received early intervention services & they decided to abruptly terminate them because of some misunderstanding w/ regional center, which wasn't communicated to us. My daughter regressed during treatment & the team actually recommended cutting her treatment hours! The speech therapist decided she didn't want to work with my 2 year old anymore after we nicely requested she use a more play based approach rather than forcing our toddler to operate toys while crying, (how is that speech therapy?) These people are beyond unprofessional. When our older child received therapy here, there was some miscommunication w/ our insurance concerning coverage. They didn't realize this for months and we ended up accruing a sizable debt- which was no fault of ours. They hounded and harassed us to no end - although we were making regular payments. I would steer far away from this place. Their vision is great, but the way they are treating clients is horrible. In my experience with quite a few therapy centers this is the ONLY place we have ever been treated so badly. I makes me so sad that we wasted almost a year and half of my daughter's crucial early intervention period with this place. Never again

Milestones Movement Therapy - Post-op Day 30: Heel slide knee flexion exercise

I signed up for a sports massage with Niels Pedersen, and after one lat-torturing (in the best way) session, I noticed that I had gained several inches on my strict toes-to-bar, another lat-heavy movement that I had been trying and failing to unlock for the last two years (but was not on my radar in relation to the massage therapy). I signed up for a second session with Niels, and the week after, to my astonishment (and to that of my cheering gym peers who witnessed my prior struggles), I suddenly can do strict toes-to-bar! Huge thank you to Niels and the magic/slight torture he bestowed.

Briefly, Ryan helped me regain near-normal knee flexion/extension by week 3 after a total knee…read morereplacement. He is amazing!!! Over the last few years, the limitations of osteoarthritis and other chronic degenerative conditions forced me to be more sedentary. I had multiple surgeries, including two bunionectomies, a spinal fusion and laminectomy, and, most recently, a total knee replacement. I started going to Milestones Movement Therapy because my daughter wanted to take a more holistic approach to my health, with Class IV laser treatments to promote healing and decrease pain as I recovered from my knee surgery and functional movement therapy to help me avoid future injuries. Working with Ryan gave me the opportunity to thrive; continue enjoying activities like hiking, paddle boarding, white water rafting, kayaking, and hula dancing; and maintain an active lifestyle as I approach my 80s. I am so thankful that he has been part of my recovery team! I think if I started seeing Ryan much sooner, I could have avoided the damage that caused me to need my previous surgeries. My first appointment was for laser treatment with Miles on post-op day 2. Before the session, my knee and lower leg were swollen like one giant, overfilled balloon, and performing my physical therapy exercises was so uncomfortable that I could barely get my knee to flex at 90°, even with strong pain medication. I was amazed by how much my pain decreased and my range of motion increased after that first visit! I started movement therapy with Ryan on my second visit (post-op day 4). He performed cupping, laser, and massage treatments to improve the circulation in my leg and around my knee, and I walked out of the office again with markedly decreased pain and stiffness. Over the next 16 days in which I had bi-weekly visits with Ryan, I felt stronger (less pained and more stable), stopped needing pain medication altogether, and was ultimately released early from in-home physical therapy when my flexion and extension measured at 130° and -4°, respectively. My outpatient physical therapist said "normal" flexion is 130° and extension is 0°, and he and my surgeon mentioned how surprised they were by the rate and extent of my progress. It is post-op day 30 now, and Ryan and I have started addressing some of my chronic postural/pedal concerns. I can walk without a cane, ride a recumbent bike, and navigate stairs. I am working on strengthening my muscles, improving the mobility of different joints, and trusting my new knee to support different loads. Ryan is a true professional. Every time I see him, he asks whether I have questions/concerns, actively and openly listens, and addresses things in a way that makes me feel very comfortable under his care, even while he pushes against the boundaries of what I think is possible. Ryan is extremely thorough when explaining different modalities and their benefits and knowledgeable about rehabbing the human body, so my family and I trust his judgment. When my extension plateaued at -10°, for example, and other health professionals were telling me to just focus on contracting certain muscles to get my leg to straighten, Ryan's insight into the rotational aspect of knee flexion/extension and knee mobilization treatments got me to -4°. Ryan is also patient, constantly showing my daughter different treatments and exercises to try with me at home; and funny, engaging, and motivating, always keeping my spirits high while we work through painful or physically/neurologically taxing issues. My daughter and I are very pleased with Ryan's quality of care and feel so fortunate to benefit from his expertise that we've told all of our friends and relatives about my recovery journey with him. Several of our friends have started or will soon start working with him. He and the rest of the team at Milestones Movement Therapy are amazing at collaborating to ensure you have the best opportunities for improvement. Working with them is definitely worth the investment!
