The facility is very nice, fresh paint and good lighting. The new director is very nice and he does seem to care. But unfortunately it takes a while to clean house. 95% of the employees there are rude and only speak Filipino. I will stand at the nurses station waiting to ask a question and they will keep yakking and yakking and yakking amongst each other and completely ignore me. I'm not a nag. I don't constantly have questions or need assistance but when I do it would be nice if someone at least acknowledge my presence maybe say hello how can I help you something! anything!! Choosing the right skilled nursing facility was pretty difficult for me since I was placed in charge of it for my grandpa since my parents don't speak English that well and I had to make sure I did my research correctly and that my grandpa would be taken care of and I didn't need to worry. I met with the staff here first and they helped answer any questions I had. This was the first time doing this so I had tons of questions and the staff were so patient and kind answering my concerns. This is an amazing skilled nursing facility. My grandpa was placed here for rehab after his hip surgery a month ago. The therapy team here is great with their recovery program. He was there for three weeks. After my grandpa was discharged home, they even called to check up on him. Follow up is so important in any business and they made sure to do that which is a big plus for me. read more

9/23/17 Our family still has a great appreciation for Boulder Creek. Today we had lunch with my mom and the residents here outside in the courtyard. Lita and her husband provided lunch and took the time to make sure we all enjoyed ourselves. This place still amazes our family how the staff have so much patience and are so kind to all the residents. We can't Thank Boulder Creek enough how much we appreciate how great you have been caring for my mom. I always feel at ease when I have to leave because I know she's being well cared for. Don't. Choose another facility. I was there for two months following an amputation. The communication was AWFUL, and they fell short on too many essential services. I got my necessary rehab, but was treated like a child. No respect for privacy. The worst was the assigned social worker/case manager, Theresa. Messages were NEVER returned. Instead of addressing me with any information, she insisted on calling my wife. I was right down the hall. I would only catch her if I went to her office. Knowing that I was there temporarily, she knew that I would need a wheelchair upon returning home. She never ordered one, and I returned home without one. No communication. I even called the facility's director and left a message for him. No reply from him, either. Avoid this facility. Shameful. read more

This place needs to be shut down!!! Place leaks when it rains, smells like mold, food is horrible and some of the staff are just plain rude. No communication between departments, had to wait days for a TV and when the girl finally came in to tell me there was an charge and if I didn't pay she would have the TV removed. It's sad when family members need 24 hour care and end up in nursing homes, especially when the homes are not so great. This place reeks of urine and my mom's belongings go missing often. The front desk person, Diane, is always unpleasant when I call to talk to mom and acts like she's doing me a favor by connecting me to my mom. Find another place for your relatives. read more

Boulder Creek did an amazing job onboarding my elderly father who is almost completely blind and nearly 100 years old.David, the primary administrator who assisted my family in admitting my dad to Boulder Creek, was amazing! He explained the entire process to us, answered all of our questions, and it is clear that he cares so much about his work and those who live in Boulder Creek.The facility is very accommodating-- one level, and an enclosed peaceful yard for its residents. The staff is very welcoming and helpful. Not only did they assist us throughout the onboarding process for my dad, but they allow us to maintain communication with him by helping us talk on the phone. My father always sounds so healthy and happy over the phone.We are so thankful that we found this facility. Even with COVID-19 quarantines, the facility still allowed my family and I to visit my father through a glass wall, where we can see that he is well taken care of (groomed, fed, nice clothes on, hair combed, and happy). I am a volunteer chaplain at Boulder Creek and come once a week with 5 others to lead a worship service. We have been warmly welcomed with the greatest support by the activities staff and nursing care staff as well. The facility is always clean and the front desk always accommodating. We spend an hour sharing God's love through song, the Bible and praying with the residents. We have been received well and enjoy our time with the residents. read more

My dad spent 2 months at Boulder Creek to recover from a stroke. In the early weeks he required care to sit up, eat, dress and use the restroom. Two months later he is walking safely with a walker and is nearly independent. The Director, David G. and his team are keenly interested in quality care for each patient. The CNAs are kind and attentive, treating each patient with respect. The nurses who round daily, morning and night, dispensing medicines were genuinely connected and showed great concern for my dad and his condition. The occupational, physical and speech therapy programs were outstanding. The gym is large and well-equipped. The therapists are professional and extremely dedicated to the progress of their patients. Their reporting on my dads progress was extensive and very helpful in collaborating on the appropriate discharge date. The courtyard is well-maintained and a nice retreat for patients. There are areas for improvement (i.e. case manager communication/training, call light attendance/timeliness), however, the positives greatly outweigh any negatives. We plan to go back in a few weeks and personally thank the staff that provided us such good care. My dad forged friendships with the staff who cared for him which kept his spirits up and kept him motivated to keep healing. read more

HORRIBLE. If you want your loved one or yourself cared for and paid proper attention, here is not the place. I stood in the hallway listening as a man prayed out loud and yelled for help for more than 10 minutes as nurses and CNAs walked by his room without batting an eye, even after I told them more than once that they should probably go help him. I nearly went in myself to see what was going on/ needed with the patient. Horrible patient care/ customer service, they should be ashamed of themselves. For the staff at Boulder Creek: EMTs are always watching when we visit Skilled Nursing Facilities dropping off or picking up patients, and some of us don't look the other way and won't hesitate to report the neglect or abuse we witness. I've known about this facility since it was Monte Vista. Now, Boulder Creek, with a new, young director, it is raising standards daily to be a top notch facility. Lita, the Activities Director, has been there for many years. She is wonderful and compassionate and cares dearly for the residents. I have visited patients here for many years. Now, a Hospice volunteer, I see my patient twice a week and always am impressed with the care I see given to the residents. read more

Our experience with my mother being admitted here was horrible. The physical plant is cold and in poor condition. Many on the staff are from an employment agency, and are not full time employees, and lack the compassion, care and training that I am sure the full timers do. My mother was left in her own feces for 3 hours, and after 3 requests for help, someone finally did. We had major service and care issues almost daily. The food is cold and poor in taste. We finally had to move her after 2 weeks as her condition was getting worse, and not better. Carmel Mountain rehab is what saved the day for us, and was the exact opposite experience. read more

The staff at villa monte vista were caring and friendly. They treated my mother like family. She went home earlier than we expected thanks to their wonderful rehab staff. The admission department made the transition from the hospital to their facility a smooth one. My mother-in-law (who is 87 yrs) was in this facility for two weeks. The staff was caring and professional and the facility was clean; however, the condition of the facility is in great need of a facelift. For example, my mother's room had several holes in the wall, furniture was old and warped, holes in sheets. Food, if you can call it that, was borderline unedible. When kitchen was temporary closed for repairs, we had to bring in food because the tuna surprise sandwich looked disgusting. I would not recommend Villa Monte Vista to anyone. The owner of this facility should be ashamed. read more

These are very friendly staff to talk to too, and my boys 'Oma' has always been in a joyful mood when ever I visited, unless she was asleep! I have had the lady there for over 2 years now, and she is even off of hospice. The office staff had been very helpful in the past, and I hope they continue to do a good job. read more

This was a dreadful place. My mother was there several years ago and I am just now able to share the experience. The staff was slovenly, sullen, and lazy. My poor mother was forced to lie in her own fecal matter for hours, and when I arrived, I had to literally beg for assistance, and no one offered any apology. They basically abandoned her to her room and even allowed her to fall out of bed twice, bruising her face and head. On another occasion, I arrived to see that no one was monitoring the patients. There were several lights on outside the rooms indicating a call for help. Another visitor and I went searching for staff, and found them celebrating an employee's birthday in a common room. Also, my mother was humiliated at having to be bathed by a 20 year old male employee, so I interrupted the process and finished bathing her myself. To be fair, I reiterate that this was several years ago, and I sincerely hope things have changed, if not, perhaps this post will help. read more

This is where my grandma got transferred a month ago after her surgery from Sharp hospital. The staff were friendly and kind enough to take care of my grandma and help her back on her feet. A couple of things that needed to be adjusted just so i would've given them a better review. First thing, the dietician and the nurse that both works in the kitchen needed to communicate properly about my grandma's food. Both my uncle's and i talked to the dietician about "No meat", "No tomatoes", "No greasy foods" its because of her age (she's 91). Alas they failed to meet the requirements what my grandma wanted so that she can get better for her health and well being. The nurse brought in chicken sandwich with a slice of tomato which we already told them about it. The following day i visted my grandma on her dinner time in between 5-6 pm. The tray that was brought in by the nurse has tomato soup and a greasy grilled cheese sandwich cut in half. I told the nurse who brought the tray that she can't eat after she looked at her food. And they wonder why she's been losing weight for the past week or so if they just know how to "communicate" with each other or "Eye to Eye" contact. Nonetheless, its a good place for seniors to enjoy their stay at the rehab center. read more

My husband was at this facility after hip surgery. I couldn't have been more pleased with the quality of care and the pro-active nature of the staff. My husband was a difficult patient at times and they took great care to make certain he stayed safe at all times. They were always available when I had questions or needed assistance. This nursing home is belong to 3rd world country. They don't have a enough supplies for patient care and they are not compliant in infection control protocol. I've seen an isolation resident is on the same room with a healthy resident. Beside, they don't have a hand disinfectant in every room. They are very poor in patient care and I will not recommend this place. read more

This place they call a good long term facility it isn't. When I worked there in Dec/2016 I witnessed multiple residents with pressure soars, one I recall had five sores. They were always out of wipes, towels, wash cloths, diapers/depends, soap, a lot of residents didn't even have every day street clothes to wear. They have a specific closet they go to in the hall to get lost and found clothes that hardly fit half of them. The staff are racists/ they are always speaking in their native tongue but hardly never speak English to anyone of other ethnicity. On many occasions due to not having towels or wash clothes, I and other cna's had to use gowns to give peri care. I had one cna that told me on my first day to laugh at the resident who were screaming and cursing while caring for them. I had another time were I let the supervised nurse know that one resident is asking for pain medication and she gave attitude and stated that I already gave him medicine. (With major attitude, unprofessional-ism) Many more issues, this place should not be in business they have been going through new owners, and administrators like crazy. And of course I save the best for last..... I gave them two weeks notice, I go up their a second time to pick up my last paycheck the lady stated she mailed it. A week later I call up there with my mom and asked where my paycheck is. The administrator called back and said he would cut a check. He didn't so I had to call wage and hour. Wage and hour called them to see whats going on and they told wage an hour that it was mailed three days ago. Now almost two weeks later they stated that the check that I never received had been cashed, so someone fraudulently cashed my check. So now wage and hour and another company is investigating this issue. I hope the person from there is penalized for what they did. I'm highly upset this is food out of not only my mouth but my special needs sister, and my mom. I have never been done like this and cannot understand why people do this...... read more
