There are a lot of nice ppl here but what really gets me mad is that staff gets wrongfully terminated for things that never happened when liars want to spread rumors .....Henry is cool, I love that guy, but please be careful who you get close to or even talk to cuz they may get fired for doing their job and that is really really horrible, but their are some nice people like I said Henry,(the good looking one)...LOL Stacey, Kim, Janae and the PT/OT'S they definitely work you hard to get you ready to be able to go home, but for the liars & sneaks & the back stabbers all I'm saying is be careful.... for everyone I mentioned INCLUDING the person who got fired I love you all & I appreciate everyone who took care of me!!!! read more

BCPA is a great place to come to when you need rehabilitation the staff are very friendly the CNAs Brenda Edith Janae Javan and Winnie are great with the patients the Social Worker Henry is funny he always has a song in his heart and would put a smile on ur face the wound care team Charles and Femme are very gentle and caring when it comes to changing ur bandages pt And ot are super awesome I love my PT Monet and other Pt shipra and my OT Chet Chet and Roblyn and other OT Nicole and Mewah are super nice they all care about the patients they work with. The staff is very professional and friendly. I'd like to specifically mention the hard- working CNAs, who take care of my mother and are always very gentle and nice with her (Blessy in particular). My mother is wheel-chair bound and has an after- stroke cognitive impairment. My sister and I visit her every day and always see her looking well groomed and cared for. I do see that staff is often overwhelmed and people are forced to multitask. It is not uncommon to see the same staff members serving food in the dining room and then providing personal care. I feel like the facility needs more staff! We did experience the loss of clothing a few times and that is another issue in need of more scrutiny. We take our mother outside to catch some fresh air every day, but unfortunately, it is not a common practice for the rest of the patients. I doubt if patients, that are not as often visited by family, ever catch any fresh air at all. Definitely something that needs to be considered, especially during the cold season. And the last but not least, the food variety. My mother can only take blended meals and I do not see much difference between lunch and dinner every day. A yogurt could be an option for a patient like her, but it is always mashed potatoes, blended protein and blended vegetable. Every single day! We are thankful overall. Many friends have mentioned that they also had had to make this difficult decision and transfer their parents to the post-acute care, but from what they've seen out there, Boulder Creek is still the best. read more

I have elected to amend my previous review *6-27-24 in that I wanted to give credit to specific medical/administrative efforts in securing a long-sought Neurological assessment appointment relating to previously (and still) chronic Vertigo/cerebellum injuries. Sometimes wrangling through the attending physicians' timing and massive amount of insurance protocols, identifying the proper classifications, and making the already slow process move more efficiently ...that can be daunting. So it's a huge Thank you to Stacey O, for exhibiting your prowess in such matters, and for my appreciative benefit. If your insurance forces you to go to this facility and you have no choice that is one thing, but if you have a choice and /or paying out of pocket, please don't do it. My mom checked into Boulder Creek on Wednesday March 17 at about 3pm for 2 weeks minimum. A nurse Erica called to get verbal consent for treatment. It was 1 nurse and BUT the standard 2 nurses. She also told me to bring my mom's advance directive and clothes on Thursday before 4pm and I would also be able to see my mom through the window. I get there right before 4pm (which coincidentally is shift changing time). Lita, the activities coordinator told be i shouldn't be there, the nurse was wrong, i needed an appointment, but they would allow me to see my mom this one time. I hear nothing else from anyone in the boulder creek facility. My mom told me on that her check-in day a male staff asked her how she was able to get in the facility. He also was rough with her, but we let it pass. Sunday March 21 the same male staff was rough and rude with my mother. He told her she was not the only patient in the facility and left her in a soiled diaper for over 45 min. Monday March 22, my mother told her dialysis social worker what was going on and they reported this as elder abuse. It was reported to the facility, the state and the sheriff. Monday afternoon about 4pm the director Dean and a nurse called to advise me that they got a claim and was investigating the claim to fullest and anything i needed they were there to support us. Tuesday March 23rd i got a call from a case manager that they wanted to have a team meeting to welcome my mom to the facility, introduce me to everyone providing care for her and to discuss my mom's care and make sure we all had a plan of care for her success. We had the phone meeting the following day Marc 24 and everyone was just so nice and wanting the best for my mom. They had tentative scheduled for different activities planned. Wed night, after the team meeting, my mom calls me and says she was told she was being discharging Saturday. Thursday March 25, Veronica the case manager calls me and says, my mom's insurance was no longer covering her stay after Friday March 26, and we needed to pick my mom up on Saturday before 4pm. I told Veronica we were already informed of this info on Wednesday after the Team call. Veronica was adamant that she was the point of contact and she JUST got that info right before she called me on Thursday. I contacted my mom's insurance on Friday but didn't get a call back until Monday March 29 but they said they did NOT know my mom was being released and they did NOT stop paying for her stay. The Insurance said that 9 days was not a time frame they would even stop paying. The insurance contacted the Boulder Creek facility, and no one had a valid reason as to why my mother was released. My mom had to go back to the hospital (checked in) on Mondays 4/1 because she was still having complications. Also to add they released my mom without her oxygen tank that she was admitted with and it took hours before someone was able respond to us. I called the facility and was told they were "too busy" to talk to me about the tank. I texted the director Dean, who 1st said my mom wasn't on oxygen, 2. Asked if it was really our tank. And 3. Finally said he would look into it. Then I get a call from Jane who said they found it... But when we went to pick it up, they misplaced it, again. read more

Do yourself a favor and do an internet search, they have bad reviews from past employees, residents and even the health department. They treat the elderly very badly! Back in February I wrote a review for Boulder Creek. I wanted to post an update. My father passed away in May and passed away at Boulder Creek. He was on Comfort Care (a hospice alternative) and I want to thank everyone at Boulder Creek for providing the BEST POSSIBLE CARE that he could have received. Everyone did an outstanding job and I can not be happier with the care that he received. I recommend Boulder Creek without any reservations. This place is incredibly hard to get answers from. I've left multiple messages for the director, a doctor, and anyone with answers. My daughter is in Boulder Creek acute care. She just accidentally found out she has Hep C. Her social worker was printing out somethings to help her file for disability. She's been in there since 2 October, at least 6 weeks. No one ever told her and she can't even find out if she's on meds that can cure it, even though she and I have been asking questions for a week. Hep C is curable only with medication. This place must just want her to die. Avoid them like the plague. They're incredibly irresponsible! read more

My mother, who is severely demented, has lived at Boulder Creek Post Acute for the last 7 years and she has received wonderful care from the staff. Everyone that I have dealt with there has been efficient and caring. They have done an amazing job during the pandemic with very few patient Covid cases. Nursing homes, in general, are not fun places to visit. For the visitors it's hard to imagine living there ourselves. What I looked for was a place that was clean, had activities for the residents, and had caring employees. Boulder Creek has a beautiful outdoor space where we can visit with my mother almost year round. My mother's room is always spotless and we are encouraged to decorate her room to make it look more like home (and believe me, me have). I can stop a staff member and ask where my mom is located and they will inevitably say, oh Irene, she's over here, meaning the random staff members know who my mother is in this large facility. It was a hard decision to place my loved one into a nursing facility but I have the peace of mind that my mother is receiving excellent care at Boulder Creek. read more

My sister has been at Boulder Creek for about four years now. I am constantly so impressed with the staff there and feel so secure knowing she is receiving such great care. If I have a question, an issue, a request, it's always handled quickly. The CNA's talk to her, hug her, squeeze her shoulder, listen to her, and smile, smile, smile. It's not easy to work at a nursing home, but they are there, day in and day out, taking care of so many people with special needs. My appreciation for them is great. Lita is an Angel and the nurses and CNA's deserve a daily hug. My sister is happy there. What more can I say when I ask her if she likes living there and she lights up and says, "Oh, yeah!" Depressing. Bad Smell. Unskilled staff who do not care about the patients. It's a mad house. It's a scary place to be honest. Having said that, there were some amazing people there who were trying their best and I appreciate them. The case manager:Theresa seemed to have psychopathic behavior. She had no bed side manners and had no idea how to handle complex cases for folks who needed a little extra support. My brother was in Boulder Creek about two years ago. I know, this review was long in coming. Anyway, he went in there not talking, not walking or dressing himself or feeding himself, catheterized and having to be on a liquid diet because he couldn't swallow properly. We believe he got this way as a result of what we have come to call a triple-whammy: 1. He had stopped taking his psych meds, 2. He was experiencing mild dementia (per MRI cell damage in brain was minor), and 3. He had a raging UTI and was suffering from delirium. We didn't know what was going to happen or if he was going to stay the way he was. But we kept visiting and the staff kept working with him, the physical rehab people, a person who worked with him re his swallowing etc. I don't think they knew if he was going to get better, he was so unresponsive with everyone. But they just kept working with him, putting him on the exercise bike though he'd pedal for two seconds and then stop, over and over, and he always acted as if they were torturing him each time they worked with him. But they were professionals and never took it personally. And after a few weeks I visited him one day and decided I'd help feed him and I was never so surprised than I was when he suddenly took the spoon away from me and started feeding himself! From there, he just got better and better, though it was a slow process. I can't say enough about how wonderful the staff at Boulder Creek was, from the aides, to the nurses, to all the various rehab people. My sister and I are VERY grateful to them all. All those things my brother couldn't do when he went in, walking, feeding and dressing himself, he could do when he left and we are so grateful to them. read more

If you have a loved one please do not choose Boulder Creek post acute care. The nurses, social worker and director of nursing do not care for the patients. I can't believe the negligence I witness, our family member was admitted on July 11th and was placed on hospice. He was admitted around 5pm When we came back the next day to visit around 11 am, he was unresponsive. Hours before his admission to Boulder Creek he was at the hospital talking and even had his lunch and we were shocked that in less than 24 hours he was no longer communicating. We asked the nurse if he had received his medication, since we had never seen him like that. She stated he was just admitted and hadn't received his medication from pharmacy. I was shocked! That was over 16 hours of no medications! After I brought it up she went to the back and found his medication. Our family member was there from July 11 and on September 9, 2017 he passed away. During his stay, he had 5 falls. His iPhone and money stolen. The nurses towards the end were not managing his pain. He needed assistance feeding, we couldn't go for every meal. I requested 3 separate times to the charge RN for them to take him to the dining room since he needed assistance with feeds. I even wrote a note In his room asking for the CNA's to take him to the dinning room for each meal. Every time I would come in for lunch he was in his room. At that point I asked to talk to the director of nursing, she apologize but also brought up that the facility was not receiving the payment since he was also under Elizabeth Hospice. I couldn't believe what I was hearing, all were simply requesting was for him to he taken to the dinning room. He wasn't strong enough to cut up his meals. Our family member suffered with liver cirrhosis, and his abdomen would retain liters of fluids. This made him terribly uncomfortable, the nurses would not call Elizabeth hospice. My mother who would go to visit him every single day, asked 3 different occasions for them call hospice to send a nurse out. The nurses from the facility finally called the third day. My mother also spoke to the social worker, and asked if she could bring up her concerns to hospice and change his care plan. The social worker assured my mother she would have a meeting and bring up everything they had spoked about. She wrote everything in a yellow sheet of paper. As soon as my mom stepped out, my mother returned back to tell her one more thing. The social work office whose blinds are broken did not know my mom was outside the door once again. My mother witness when the yellow piece of paper she had written all her concerns was ripped and thrown to the trash. I couldn't believe what my mother had told me, I gave it a few days and contacted Elizabeth hospice to see if the facility social worker had followed up with them or better yet set up a care conference. Sadly I was told the social worker had not contacted them since the date of admission. Working in healthcare field myself I couldn't not believe everything that was happening. Thankfully we had Elizabeth hospice so help us set up a care conference where I addressed everything and confronted the social worker. She denied her ripping up the paper and said she was the one who set up the care conference. I told her that was a lie because the hospice was the one who had set everything up and there was no documentation of her contacting them since the day of admission. My 98-year old father who is blind and has a lot of needs is admitted to Boulder Creek. I like the facility because it is a one level facility with an enclosed, big, and clean yard. The staff is very nice. They always return my call when I want to speak to my dad. My father always sounds great over the phone. David, the Administrator, is super nice. He explains everything straight to the point about what's going to happen during my father's stay in the facility. He really cares for his patients. Because of corona virus, I was not able to see my dad for 2 months. I asked if I can see him just through a glass wall.  I saw my dad well groomed and very clean. I'm so thankful I found this facility. I have peace of mind every day knowing my dad will be taken good care of. I will highly recommend this place for the elderly. I wish I could give more stars but the maximum is 5. read more

DO NOT PLACE YOUR LOVED ONE HERE! Due to health insurance options we had very few to pick from and chose this place because we figured it was under new management and the one positive review it received. Well the positive review obviously was written by an employee because it nothing has changed. With the exception of an LVN Darren and the front desk receptionist, all employees are rude and lazy! The care here is substandard at the very best! They have lost my loved one's paperwork, are not managing his pain as they were unable to give him because they ran out and never ordered from the pharmacy, their oxygen equipment does not work...just to name a few things. This does not include the fact that yesterday a resident (it was apparent she had dementia/Alzheimers) was walking out of the front doors. (I talked her back in). The employees are lazy, rude and have no compassion. I have no idea why the state of California has not shut this place down or investigated employees, but you bet I will find out why! Update after initial review: I had to have 911 called from the facility after I found my brother barely breathing due to their lack of care and non working oxygen machine. He died a few days later. Although my brother was put there while in hospice I can say without a doubt in my heart, mind, body and soul he would have lasted a little longer had it not been for this facilities' lack of care, judgements and non working equipment. If I were to do it all over again I would have placed him somewhere else to have a few extra days, weeks or months with him. read more

I had a family member here and my family and I had a great experience with Boulder Creek Post Acute. Jan, my family member's nurse, was extremely attentive to my family's needs. He had excellent bedside manner and was always friendly and professional. He never hesitated to make the stay for my great aunt as comfortable and safe as can be. The CNAs that helped Jan attend to my great aunt were also super helpful and friendly, making this rough time for my family not so tough. As for the facility itself- it was very clean and huge! There were 3 nursing stations available (or at least from what I saw during the time here), had 2 large dining rooms, and had a room for rehabilitation therapy. This place seems pleasant enough when you first tour it but when you actually get there it's a whole different stor. They are extremely disorganized and most of the employees are inept and unprofessional. When my husband got there his nighttime meds were due at 7 pm but apparently nothing was there. I had known about the transfer since the early afternoon. When I went to the nurses station to ask for his meds she looked surprised and then frightened and then quickly started typing an admitted that he had never been ordered!!! But They said it "WOULD" be coming from the pharmacy". It never came!! A guy from the pharmacy showed up in the parking lot at midnight; by what time my husband was beside himself, but apparently my husband's medicine was not prepared yet! So he didn't get his medicine until the following morning. He went into withdrawal As he is on regular pain medication. So didn't get a wink of sleep. turns out the pharmacy that delivers all the way out to POWAY is located in El Cajon! And funny name for a pharmacy: SUPERIOR. Not! My husband had a stroke on his left side but wouldn't you know there's no grab bar to the right of the toilet--not even up to code! And There was a filthy moldy ( actual mold) used urinal right on top of the toilet, yuck. At 48 hours of this nonsense He was fed up to the max When he got transported back to the hospital because they just couldn't seem to get it together and my husband demanded to leave, they arbitrarily decided to take him to a local hospital rather than the one he had originally been discharge from. Then enroute apparently they changed their minds again and decided to take him back to the original hospital without even telling me! so I was halfway to Poway to get his stuff when I happened to call and find out where he was going! (no one communicates at that place not even WHERE your loved one is on the planet. When I asked why I hadn't been called they lied and said they tried to call me! (absolutely no iPhone record of any missed call; no attempt to call) So then she tried to say that "maybe your number had been written down wrong" or that they didn't have the number (what? ) I personally gave it to the head nurse and not only that but it was on the paperwork for the previous hospital. Total ineptitude and deception. I drove back up to retrieve his belongings. They had bagged it up. An electric razor was missing and so was the bedspread I had brought him. I didn't even bother to ask about it I just got the rest of the belongings for my husband and left! My advice is to never ever dare put anyone you care about in that "boulder creek"institution read more

I have been living at Boulder Creek Post Acute for 5 years this July 4th. I wanted to speak on the integrity of this facility. I really wanted to give a review of my time here. When I came in I was told I was put on hospice and couldn't walk and I was ill. I was an ambitious patient and between my doctor and physical therapy, I was walking on my own in a year or so. Without the encouragement of the staff including the doctors, I wouldn't have been able to get back up on my feet. Along my way, I was privileged to meet many smart and inspiring nurses and positive and dedicated CNAs. When the pandemic happened, without missing a beat, this place changed overnight including all the new protocols, masks, etc. Through it all the nurses, CNAs, physical therapists, social services, kitchen, housekeeping, and all the departments, all stayed focused and worked hard, and were positive. Recently as things have gotten back to normal and I noticed all the beautiful smiles on all the wonderful people who have been so gracious and have shown such compassion and kindness to me and all the residents. I feel so blessed to have known so many talented and special people at this time in my life. I walk every day only wishing I can be as kind to all of them as they are to me. Thank you for taking such great care of me and all the other residents. The staff at Boulder Creek is hardworking, down-to-earth, honest, and caring. I've learned many valuable life lessons while staying here that have altered my way of life to become better. I'm very grateful. read more

On saturday 2/23/19 my brother in law passed away at this location . I have never experienced such a below standard of personal treatment for a marine vet or just human life in general at his time of passing. We were told that we had 6hrs to remove his body and find a funeral home that would pick up his body and this is at 10pm on a saturday night. I was told numerous and reminded time and time that we had to follow procedures and protocol. I asked numerous times for this paper of procedures and protocols and I was denied. Also asked for a complaint form and it was declined they said to do it online. I ask for business cards and they only gave me 1st names. My brother in law has no family in California so all I wanted to do is honor his life as a Marine. Myself and my family was treated with so much disrespect makes me think this is how they treat everbody? Is this their policy to treat people like this? After a human expires this is how we treat them now since they no longer employ them? read more

The 1 star is really a zero. But I have to give it at least one star in order to post a review. Three weeks ago my father was here for a total of 45 minutes. My father is on Medi-Cal and Medicare. Before I chose this place for him I read the reviews and looked at the pictures on Yelp. There were a few bad reviews but also a few good ones. For the most part it looked acceptable to me. We wanted a place where my father would feel safe and where he would get the treatment that he needed. My father was transferred from a local hospital to this facility. My husband and I arrived about a half hour later to meet him there, arriving to an unlocked deserted Lobby and unlocked doors into the room areas at 630 pm. Anyone could have walked in. As far as his room goes, I've seen better rooms in detention facilities, as I worked in them. My dad is a fall hazard and his bed had no rails and the furniture looked worse than thrift store furniture. Thrift stores wouldn't even sell this furniture . There wasn't even real sheets on the bed or a real blanket. It was more like a moving blanket. When we tried to address the issue of a new bed they told us maybe they could try to do it and get him a new bed. When we asked when, they said they didn't know. He would have to sleep in the bed they gave him originally with a head injury and a broken arm. Maybe that was an unacceptable answer? It was a true hell hole. Another patient in the room told us if we knew what was best for us we should not leave him here. As we were leaving the gentleman he said we've made a really good choice. My father was admitted to Boulder Creek in late January after about 20 days in the hospital after a seizure, fall and diagnosis of dementia. Our family was reeling from this diagnosis and was surprised to learn that if you have a HMO Medicare coverage you really don't get a choice of a facility--it is wherever they have a bed! We had begged for a local facility so family could visit easily. Boulder Creek staff assessed my dad in the hospital and determined they could handle him ( due to dementia and sun downing behavior this is not always possible at some rehab centers). When we arrived at first all I saw was the physical building and was disappointed. It is a old building which is slowly going through remodeling. The front lobby is beautiful but the rest of the building has sections which are old, sterile and in disrepair. The community/ dining room used by the residents should have been remodeled before the lobby! First some of our problems: The TV in my dads room changed channels whenever his roommate changed his TV. That took 2 days to get fixed. The wheelchair provided to my dad had broken foot rests and never rolled easily. My dad fell once and was put on 15 minute checks if family were not present. That helped and I do admit my dad was stubborn trying to get out of bed to use the toilet on his own -but then again, call lights are often not answered for at least 10 minutes. Of course there were positives too: The staff had some amazing bright spots. The new Director , David provided me with contact information, I used it about 5 times during our 13 day stay and he was always promptly responsive , professional and kind. The charge nurse (Aura? So sorry never got the spelling) was wonderful, as were most of the other nurses we encountered. Our favorite CNA was Jan who always took time to smile and talk to dad and never tired of answering our endless questions as we navigated this new reality of my dad's health. One thing I know--I wish I could remember all of the staff's names to recognize the positive ones here. A suggestion..all staff should be required to wear a standard large name tag with their name and title easy to be read by patients and family members. The staff individually were mostly positive experiences but they appear to be understaffed and some staff members seem to be indifferent and too busy to look up, acknowledge a family member trying to get help or even make eye contact and smile. Our personal experiences: My dad was there to gain his strength back. I imagined PT as the focus. Often if dad was asleep when PT came to get him, they let him sleep and then didn't always come back to get him later in the day. My dad was restricted to a wheelchair as there were not enough staff to let him walk with a walker and someone beside him. The food was fine, my dad had no complaints even when he was on ground up food due to swallowing issues. The SLP graduated him to a regular diet and he enjoyed it even more. Activities were hit and miss in quality and if a patient doesn't track activities on their own they are not encouraged to come participate. We were fortunate to have family members there staggered throughout the day to advocate for my dad's care. I think our level of involvement resulted in a better overall situation for my dad. We knew we would be taking my dad from Rehab to a memory care facility. When a bed became available at our memory care facility of choice, we started the discharge process from Boulder on a Friday. We knew that due to forms needing signatures from Drs etc it would not be until Monday and Theresa our case worker/manager promised us this would happen. On Monday a series of no communication and outright lies from Theresa delayed the discharge to the new facilities. After asking multiple times for the forms and discharge paperwork my dad finally was discharged in the afternoon. It was a shame to end our stay at Boulder on this unprofessional, rude note. Bottom line, Boulder Creek is a work in progress. David appears to have the place headed in the right direction. The staff has some amazing caring individuals. They need more of that type of employee. Look past the physical condition of parts of the center--after all it is the care that matters! (And why due to a few outstanding staff I gave 4 stars. ) Lastly if your loved one cannot advocate for themselves, be involved and ask for what they need. Most of the staff there is willing to help you get it but you cannot wait for the care to come to you--you have to ask for what you need. Attend the care conference, take notes on medication, ask questions and you can advocate with the best interest of the patient in mind. read more

My Father has had a severe fall in early July of this year. To say the least it was a devastating situation for my Father and myself as he had four fractured ribs and a fractured nose. He also had recently been diagnosed with dementia. I was so relieved and at ease by the caring and attentive staff at Boulder Creek. The nurses and staff were so friendly and attentive to my fathers every need. Theresa who was the case manager along with Dr Chang and were a Godsend to my family and I and gave us renowned hope.
