    When entering the facility the first day I was there - at about 9:00 am, there were no staff in the…read morelobby. I wandered about the facility looking for Dad's room, noticing many room doors open until I finally found a staff person who directed me to Dad's room. It was at least 30 minutes later when someone came to Dad's room asking for my ID. The second day, I walked into the facility at 8:45 am and there were no staff to stop people from entering. I was able to walk through the facility unchecked - with many room doors wide open. I walked to the lobby several times during the day and 90% of the time, there were no staff to observe people entering or exiting the building. My concern here is a safety and security issue. it appears that anyone off the street can enter the building unchecked where vulnerable adults are housed and would have direct access to the interior of resident rooms for at least 30 minutes before staff checks in. It also appears that it is possible that any of the residents can leave the facility unobserved. I observed and took pictures of a fall hazard at Dad's garden door. There is a 1-2 inch step down to brick pavers when exiting Dad's room to the enclosed garden area. This is an exact replica of the fall hazard at Dad's apartment mailbox area where Dad had fallen twice onto a hard surface while getting his daily mail. While sitting with Dad at dinner, I observed Dad opening his packet of plastic utensils and setting them beside his dinner of a corn dog and fries (healthy meal?). The woman sitting next to him, took Dad's utensils and placed them with her own utensils. Dad reached to get them back and she hit his hand and covered the utensils with her hand. Dad got angry and told her "those are mine." Dad was very agitated. I told Dad that it was OK and I would get him another packet. There were no staff present to observe this exchange so I found a staff member to get another packet for Dad and told her what had happened. She brought another packet for Dad. I observed no action taken by the staff member to prevent this from happening going forward. I had Dad place the utensils on the other side of his dinner this time so Wilma would not take them. My expectation was that staff would be present to witness these altercations but there was no one. At dinner, I observed that no residents talked to each other. They ate and went back to their rooms. I asked Dad if he ever talks to anyone. He said no other than to say hi if someone says hi to him. No one at the facility was making sure Dad's hearing aids were used and charged even though I mentioned it several times. Smoke alarm was chirping all night Thursday into Friday until about 10am. Dad said it kept waking him up all night and he didn't know where the noise was coming from. When I put his hearing aids in Friday morning, he could then tell it was coming from smoke alarm (yet another reason to focus on Dad's hearing improvement). Staff was not aware of the chirping during the night or the next morning until I told them around 9:15 and a nice man changed the battery around 10:00 am. There appears to be only 1 staff member at the facility at night for over 25 residents. I was not allowed to observe what happens from 6:30 pm to 9:00 am even though I was told that I could enter by using the ring doorbell after 7:00 pm. I asked about that and they said the ring doorbell was broken. There was no water to my Dad's room Friday morning. Couldn't flush the toilet or wash hands.. I found later that the dining room had flooded a few days earlier and they had to shut off the water to repair it. It is impossible to reach anyone on the weekends. The manager set up a timeframe for me to call my Dad on Tuesday and Friday between 2-4 pm because they said they were being inundated with calls. 60% of the time, no one answered the phone during these times and I was unable to speak to my Dad. They seem very short staffed and make it very difficult to communicate with residents. This is a big red flag to me and a terrible solution for my Dad as he is used to my calling him everyday since he was diagnosed with dementia. In the 12 hours I was there, there was no socialization among the residents and Dad said over an over again that there was nothing to do and nothing on TV. The few activities I observed were heavily religious based although couched as Christmas Carols or Bible Study. I asked the staff member who gives medication if she takes Dad's blood pressure before giving him blood pressure medicine. If his blood pressure is below certain levels, Dad is not supposed to get blood pressure medicine because it could get too low. Staff member said they don't take blood pressure.
