BEWARE OF THIS PLACE!
On Day 1 that I went there, which was on Sept 8th, I met with Mia, the social worker. It was then that i in person, saw the Power of Attorney form in his chart and briefed her on my dads' condition, as well as our wishes, and the condition of his home. It was also then that i sent her a copy of the psych consult to her email address which she received as i sat there with her, and witnessed her print it out and place notes into the computer.
Days later, i get a call from home health that BartowCenter is discharging my dad because insurance will not pay any more days and he has completed his care. Come to find out, it was Mia that made the call to the insurance in the first place to tell them his care was complete and he could leave if he wanted to, which prompted them to stop services. I immediately called and talked with Mia's assistant, who told me that she doesn't have any POA and it was never received, nor was there any record of a Pysch consult. I argued with her and then she hung up on me, telling me she cannot talk to me and will not talk to me, that my father doesn't want me involved. I then made threats that by law she has to talk to me or i will get an attorney and sue them. I called back and spoke with the financial officer, who said that she would look for the forms and I could send them to her which she received. A couple days later, my wife and I receive a call from Mia that my dad is again placed on discharged and he would be discharged that day, and that i had to come pick him up immediately. If we did not, we would have to pay $10,000 up front immediately. We immediately contacted DCF who put a hold on his discharge. She told us she "didn't care if he got a taxi to a crackhouse or lived on the streets, it wasn't [her] problem."
My father had a severe stroke in Nov of 2016 in his right basal ganglia that compromised his reasoning and judgement which led to him becoming septic hours from death and above the knee amputation. His foot cut off back in Feb.
My father has had two psych evaluations that both deemed him unable to care for himself and was documented by several health professionals.
Mia, the social worker has put my dad at risk of his life so many times. She also has went of her way to manipulate my dad, the nurses, the staff, and the doctor, Dr. Dean, who she told him and the Administrator, not to talk to me and my wife until he met with her and the administrator.
Dr. Dean then 45 minutes later, went in and spoke to my dad for 5 minutes, deeming him capable of making his own decisions with the malicious intent to revoke my power of attorney.
We had an intervention which once again halted the discharge but no disciplinary action on Mia who lied saying she never talked to me and has no idea of a consult or documents.
We insisted on another consult in which only a nurse practitioner came in and said to us that she was not qualified to determine capacity, it had to be a psychiatrist or psychologist, while telling Mia, she didn't want to make waves.
Mia has now refused to send the transfer requests to Jacksonville as ordered and is refusing to adhere to knowledge that concerns my father's well being and safety as he has no place to go, and his home is unlivable and unsafe, as well as he has no leg and doesn't know how to get around yet. She also convinced the dcf investigator to let her come along, and manipulated her also that i get yet another call from my brother telling me they called him, he is being discharged, upon learning that Mia convinced Maia, the DCF investigator that he could leave. Maia, lied and said i was supposed to be there and i never showed, which days before gave her the numbers and info of family members to call to be present as well as the company in charge of cleaning his home and repairing it. No call was ever made or received. The supervisor was called and complaint has been filed. Mia is now giving my father legal advice to revoke my POA.

On another note, most of the nurses have really cared for my dad as have the rehabilitation people. The Dr and social worker however are corrupt. read more
