The different times i have come in to check on mother in law in Rehab.. she has been in bed not looking comfortable, wet soiled diaper always in a gown never in her clothes and i have had to ask staff to bring her a water cup. She seems to be medicated to where she cant think. I know they are short staffed and all. read more

My mother requested assistance getting out of bed and assistance going to the restroom because she is non-weightbearing and could not do it herself. She was told that the facility did not have anyone to help her get out of bed. Did not get someone to help until her physical therapist showed up for her therapy two and a half hours later. Several times, she requested assistance getting out of bed to use the restroom. She personally called for help, and I or another family member also walked into the hallway and up to the front desk multiple times to attempt to track someone down to advise that she needed assistance getting up and going to the restroom. From my mom's initial request until someone finally came in the room to help was over 3 hours. A different time, she requested assistance getting from her wheelchair to the restroom. She was advised that there was only one person available on staff to help, and she would need to wait until someone else came in. She had to wait for more than two hours to get help getting to the bathroom. Because she had to wait so long, she did not make it to the bathroom in enough time. The soiled clothes that she had on were taken off her and thrown in the closet in her room on top of clean clothing that I had already taken home and washed. We had been told multiple times (when we asked, of course) that there was a laundry service that would come around and get her clothing, but in the 20+ days she has been there- her laundry has never been done or even picked up. When she first arrived from the hospital for Rehab due to surgery on her foot- they did not rewrap her OPEN wound for 5+ days. The only reason it was rewrapped then was because my dad insisted that she had to have it done. They argued with him saying that no one told them she had to have it done. However, while in the hospital- she had gone into sepsis because her wound infection had gotten into her blood stream. So, I'm not sure how someone sent to Aspen for Rehab after a foot surgery that caused other issues would be allowed to sit wrapped in their own infection for more than 24 hours. They make appointments or have us make appointments for her for her doctor visits and then don't communicate her need for rides to/from the appointments. This has caused her to miss TWO of her appointments with her foot doc which is the primary reason she is in the rehab facility and can't ever be released to walk or do more progressive physical therapy until she is seen by her doctor. On the second missed appointment, we were told that they just forgot to put the information in the system for a driver to be able to take her. So, this has created and additional two weeks of her sitting- for no reason. She has not had one shower the entire time she has been there because they don't want to assist her to do anything. The lady in the room next to her fell in the shower (also in SKILLED NURSING) because no one was assisting her to bathe or even in the room. It was more than 15 minutes of her screaming and crying in the shower before someone came to help her because they didn't know she fell. The females that work there will tell you EVERYTHING you want to hear when you are there or call on the phone because they just want you to stop talking, but when you leave they are going to continue the same level of care and go back to doing what they want. It is extremely saddening how anyone thinks it is okay to treat another human being in this manner. read more

My Mother in Law was in skilled nursing for roughly 8 months. She recieved excellent care. So when it was time for my father to go into assisted living this was our choice. We compared other facilitys but had a very good comfort level with Aspen Trace. MacKenzie and Becky have been very helpful. I know my father is getting good care and he is satisfied where he is at. read more

My 74 y/o husband was admitted to Aspen Trace from the hospital for PT and catheter care. He had been suffering with and treated for recurrent UTI's which resulted in extreme weakness. The PT was adequate, however the nursing care was almost non-existent. We never had a care meeting to discuss a care plan even though I requested one several times. I asked the unit nurse manager to at least come in to answer questions, but she never did throughout his stay. His Foley catheter required daily flushing to help prevent another UTI. His nurse assured me there was an order in his chart to flush his catheter every evening between 6:00 and 10pm. I was told that was being done, which was a lie. It was not being flushed which can help prevent UTI's. He also had an acute problem when his catheter bag was not checked from dinner to early morning when my husband awoke in terrible abdominal pain. His catheter bag was completely empty, meaning something was blocking his urine. Obviously his catheter bag had not been regularly monitored and emptied per catheter care protocol. He phoned me in severe pain. We requested that the nurse call for an ambulance to get him to the hospital ER, but the nurse refused to do that, saying the physician on call (but not in the facility) had to authorize that. She searched for the bladder Ultra Sound equipment and when she finally located it, the US showed a completely full bladder. During all this, I was on the phone with my husband who was in acute pain. The nurse eventually removed the catheter and inserted another one so that his bladder could empty. This lack of care exposed him to more opportunities for bacteria to enter his urinary track. He was discharged from Aspen Trace 2 days later, but was only home 2 hours before he had to return to the hospital ER. He was diagnosed with another severe UTI, likely caused by the total lack of catheter care during his stay at Aspen Trace. The facility is newer than most and looks inviting, however neither my husband nor I would ever recommend Aspen Trace for any type of nursing care. The staff in memory care have provided wonderful care to my mom. They always have smiles and I am certain that take great care of my mom.

We have had a good experience with Aspen Trace. My dad started in the rehabilitation wing for a fall and has transitioned through Aspen's respite care unit and then to the Assisted Living unit. Katie Hacker, Residential Marketing Director, has been very helpful with our transitions and coordination. She is very easy to work with and very compassionate!! The nursing staff on the Assisted Living side has also been great at caring for my dad. A special shout out to Roz, Cindy, Tammy, Debbie and Julie on the care unit team. They all do their jobs with great care and compassion and I am very grateful. read more

I would not recommend trusting your loved one to the care of Aspen Trace. My father was in the rehab part of the facility for one month. He was in worse shape when he was discharged than when he went in. In fact, the reason he was discharged was because the insurance company felt like he wasn't making enough progress for them to justify paying for further treatment. When he was released, they agreed to transport him home. We assumed because he couldn't walk or transfer from a wheelchair to the bed, someone would assist us. Instead, he was literally dropped off at the curb. I have video. Aspen Trace was supposed to arrange for in home care but calls to Aspen Trace to inquire when that help would be coming were never returned. My mother and I were left to figure out how to care for his catheter (which is another thing they left uncared for) and how to move him for toileting and grooming. This doesn't even scratch the surface and would be way too long to list the many ways they failed him. I will say that the person who went in there was able to walk before their accident that required rehab. He was in good spirits and hopeful for a good recovery. When he came home, he was weak, in very poor mental health and malnourished. He was sent to the ER 2 days after his discharge from Aspen Trace and died 15 days after that. If you care about your loved ones DO NOT put them in this poorly run rehab. They are incompetent and lazy. Half the time, no one can be found. You ring the nurse button and they may come 1/2 hour later if you are lucky. They cause more problems, than help patients. They don't communicate with each other, important medical for your loved ones is not relayed. No one knows what is going on in there. If you ask them to check something, you get an attitude. Very unprofessional arguing and complaining at the front desk about other co workers. They'd rather stand around talking than be checking on the patients there. They don't have simple supplies, they have to hunt for them. You might as well care for them yourself. The worse place read more

It's a newer rehab/recovery facility on the south side. We needed to admit my mom for a short 2 week stay. I read about the facility after researching it a little. I had to pick a facility quickly because the hospital was releasing my mom from the hospital in less than 2 days. The facility was rated a 5 out of 5 overall across the board in the info I was given from the case worker. Our initial tour of the facility was pleasant enough, so we decided to place her there. My mom's room was clean but very bare bones, as there was absolutely no decor. I don't know if that's standard,.....but OK. It was a bit dismal. The bed wasn't the best quality either. It was basically 5 inches of memory foam. It did move up and down so a person could sit in it. My mom seemed to like her therapists and the in-house doctor. They appeared to be attentive to her and that was real important to me. There were pleasant and interesting activities posted and available to the patients and residents. My mom participated in a few activities they offered. She seemed to enjoy that social aspect/portion of her stay. Most of everyone on staff that I encountered who worked there seemed cordial and professional enough. I am docking the 5 star rating to a 4 solely because the food is absolutely sub par quality. As a prime example, we had a terrible and quite shocking experience visiting once when we ate with my mom. They gave her two complimentary tickets for two free guest meals. She warned us about the food, but I thought that it surely cant be THAT bad. It's got to be better than a grade school lunch or hospital food right? WRONG. The service we received was really pathetic and lacking. We were treated very poorly. The main server basically ignored us even though it's supposed to be like 'a nice restaurant type of dining experience'. At the beginning, No one acknowledged us and I literally had to go over to the food counter area where I found three employees socializing. I was ignored for quite some time until I finally spoke up. They were very aware of me standing there. It was very rude, annoying and disturbing. We had chicken tenders with a salad and a potato tater tot cake wedge. The tenders were bland/average tasting and cold. The potato cakes were literally soaked and dripping with oil. It's like someone just poured grease over them before serving them. It was absolutely appalling and just so gross. They were completely inedible. The salad was not appealing either. It must have been sitting out, because it appeared to be mostly wilted. The dressings they offered us were sub standard. We were supposed to get another veggie side and a desert. We got neither, and had to ask for the dessert. We tried to get her attention because she didn't bother to check on us. When she finally came over, she told us the kitchen was closed, and although very hesitant, she would go check. She came back with a few plates with some tiny pieces of substandard dry cake with hardly any icing on it. I didn't eat mine, because at that point I was definitely disgusted and done. I ended up bringing my mom occasional food from home since she wasn't on a restricted diet. It was hard to think about her having to eat the swill food offered a majority of the time. If we get a request for a proper review of the place, I will be sure to mention everything I mentioned in this review. I would recommend them to anyone, but I truly hope they improve the quality of the food offered to the residents. It's such a critical part of the equation. read more

i gave this one star because there was no option for zero. Having to make a quick decision, we chose this place because it was close to home. My poor husband had five broken bones in his left leg and a broken right shoulder. He was bed-ridden and there for rehab. When they brought his tray, they would put it on a table right inside the door. He would call for help because he could not get out of bed to get to it. They usually came 20-30 minutes later. Once a nurse stuck her head in the doorway and said, 'we have twenty patients each, I'll get to you when I can'. Cleanliness? Because of the broken shoulder and not being able to use his arm, he had to scoop his food with the palm of his hand. He asked if he could at least have a wash cloth or napkin to clean up and after four requests he got one. After he was there one week they had never changed his gown or cleaned him up. Let's face it, if you have one nurse taking care of 20 patients you don't have time for stuff like that. I went in and raised 'heck' with them and they changed his gown and washed his face, Not his hair. After being there ONE MONTH he had never gotten his hair washed and when he got home he had the same hospital socks on that he had on one month earlier in the hospital. UGH! After going in and complaining about the placement of his tray the next day the gal brought his tray in and slammed it on the table next to his bed and said 'There, tell your wife you got your food!' Two days after I got him home he was admitted to the hospital with pneumonia. Of course they dropped him off at the end of our driveway in the pouring rain. They didn't even pull into the driveway up to the house. When he left the hospital and went to Aspen Trace he weighed 203. When he got home he weighed 171. He lost 32 pounds in a month. I have the hospital records and his medical records from Aspen trace. I have proof. They said that he was suffering from malnutrition. Of course if you don't get your food and you can't eat you're gonna lose 32 pounds. He was there one month and saw a doctor one time. This is a man who had had 10 heart attacks and strokes and severe diabetes and he saw a doctor once. Rehab? yes he got some rehab but usually they would put him in a chair and leave him for three hours. And the humiliation he went through. He told me once that he screamed and hollered for help he couldn't get to a call light he was stuck in the middle of the room in the chair and had to go to the bathroom no one ever came and they scolded and humiliated him because he had gone to the bathroom in the chair because he couldn't get help. There's no reason for that. One time when they came to give him rehab they had to 'clean him up' first because he had slept in his own feces because he couldn't get anyone to come help him or clean him up. My poor husband went from a big burly guy to a small weak man laying in his bed hollering for help. I heard him on the phone and it broke my heart. I asked him why he yelled and he said because they never answered the call light and he tried to get their attention. Let me tell you how they check on their patients when they are admitted. I would think they would look over their medical records and know everything about them. One day he said a nurse came in and was giving him some kind of electromagnetic procedure on his broken shoulder. She had shut the door and he asked her to please open it because he just sat in that room all day and at least if the door was open he could hear people outside. She replied to him 'oh no I can't do that! There may be someone walking down the hall that has a defibrillator and this could set it off.' He said he looked at her with total disgust and said I have a defibrillator in my other shoulder and she got very very upset and took the machine and ran out of the room. They later sent another nurse to check his shoulder to make sure he really did have a defibrillator. Like he would lie about something like that. Set it off? It could have thrown him into a heart attack! By the way I finally did hear from them. You know how usually hospitals or places like that call and check to see how the patient is doing? Well my husband came home the end of October and passed away November 9. Last week which was the first week of February Aspen Trace called to tell me that they found all his clothes and would I like to come pick them up. They had never called to check on him to see how he was doing or anything. What a joke they are. Please please do not put your loved ones in this place. They don't care, a patient is just a paycheck. Especially now with Covid they know that the family is not there to see how the patient is being treated and check on them. They just put in their eight hours and go home and nobody cares. I will never ever forgive myself for putting him in that place. It is on my mind constantly how he was mistreated his final month alive. read more

My 86yo mom was in Rehab at Aspen Trace last year. The staff does not know anything. They are NOT friendly. My moms walker was folded up next to the wall so she could not go to the bathroom. She walked out of Aspen Trace and was found by the landscaping company. I had her go to the hospital so she could be better taken care. When she was discharged from the hospital they would not take her back. Her items where packed up sitting at the front desk. I WOULD NOT put you family member in this rehab unless they can care for themselves. read more

This is a beautiful place. Had high hopes. My mother was there two weeks after breaking her back in a fall. They cleaned the room once. They never helped her bathe. They took two hours to answer the call light. She wet the bed several times because of slow to no response. The medication were never given on a schedule. Avoid this place. read more
