They confirmed my cell # and said they'd text them right then for me to fill out but then they had me sign just an arbitration form then almost immediately put me in a room early and I never got that text to fill out the intake forms and the provider came in early. The provider I'm sure did the best they could with zero information and health history for me, but honestly it was a wholly unproductive appointment and I 100% regret coming. I cried in my car after my appointment because I was so frustrated. I waited weeks for this appointment and missed work for this appointment and spent time preparing by sending and ordering records and it was all for nothing. My suggestion is bipass the office staff completely and go old school and bring hard copies of your referrals and any health history/medical records and make your own list of things like medications and hand them directly to your provider so they actually know info about you and why you are there.
