We were so happy when my mother was transferred to Archstone after a broken hip. Everybody there was so kind, especially Vicki and Raymond were exceptional. It is a very clean facility and overall, it was just a pleasant place. We looked at other facilities, but no one matched Archstone. I never met a CNA as perfect as Vicki, she always went way above and way beyond, and if we could've, we would've taken her home with us to continue caring for our beautiful mother. If you or a loved one is non-verbal or can't communicate and are receiving care here, please find another facility. I witnessed multiple instances of verbal abuse and after reporting to Karen of Admissions, I was told "we already know about ___'s behavior but his sense of humor is different" and "he's been employed there for a long time." Interesting. I was told residents have rights? Some of those residents may spend their last days at Archstone and imagine if their last memories were of some sh*t head talking down to them and mocking their behavior. Unbelievable. read more

My dad had surgery and was originally sent to chandler post acute rehab which should be shut down it was so awful. We called archstone to have him transferred and Karen was amazing. She went above and beyond to get him out of chandler acute ASAP. Our experience with Archstone has been exceptional!! The place is clean, the staff is attentive. Nursing and therapists have been top notch. I'm so happy with the care my dad is receiving, I felt it necessary to leave a positive review. Only one nurse and one assistant and they couldn't come and see what was wrong in an hour and fifteen minutes? I am definitely thinking I chose the wrong facility for rehab. read more

It took me 4 months to find a suitable care facility for my Mother after moving to AZ in 2008. She has been well cared for and is thriving in this facility. Many of the staff have worked here for years . The turnover is minimal. Rooms and bathrooms are dated, however the common areas have been beautifully updated. I highly recommend! . read more

My Father-In-law stayed at Archstone for acute care for 2-3 weeks. His nurse Ray was great--caring and did his best to offer excellent care. Yolanda? at discharge is superb. My Father in law liked the food and people he encountered. He unfortunately had a recurring infection while there and Archstone was right on top of it and got him to a hospital post haste.Lindsey, the activities director is so sweet. She was excited to bring her Dad's Wood Working magazines from home when she realized he is a woodworker. read more

I would not recommend sending your loved ones here, unless you want them to die a lonely slow death, bleeding on the floor after they fall from the bed, the call light having been active for over 45 minutes. This is a sad facility. It's beautiful and nicely decorated with a beautiful courtyard with seating and a waterfall, but don't be fooled... the care is pretty much hands off and neglect is rampant. My mother was admitted here after suffering a debilitating stroke. She was able to sit, but not walk. They put a catheter in and then never cleaned it or monitored it, and she ended up being sent to the hospital with a severe kidney infection. The lack of care worsened her condition. After finally removing the foley, she requested to get up and use the commode, the answer was "you have a diaper on. Just pee in your pants." And then they'll change your diaper once a day. So our geriatric patients are sitting in their own waste, and contracting infections and sores!! Where is the dignity?! There were days she didn't eat because she couldn't feed herself. The food quality was awful anyway, so on the days I went to help her eat her lunch or dinner, I don't blame the patients for not eating. Bland and no protein. Usually a cold cooked vegetable and rice or pasta. Definitely not the meals these folks deserve. I also observed one patient, who had a jaw infection and couldn't eat, so she shoved her plate away, and then the LPN came over and took her away without bothering to check to see if she had eaten. No labs were ever checked. No blood thinners were monitored, no medication administered. Most of the residents here are non verbal, which makes neglect easy to get away with. My mother could say a few things and what she told us was horrible. "I hate this place" "I have a concussion because I have fallen several times" "I want to go home". It's heart breaking. I don't know if they are simply understaffed, or just don't care. Many times, they line the patients up in wheelchairs in the hallway. Some of them unable to even sit up straight are slumped over, moaning and drooling. Sadly staring off into space, waiting for life to end. On one visit, as I made my way past the wheelchairs to visit my mother, a lady in one of the rooms starting screaming "HELP ME!!! Please somebody help me!!!!" Her light was flashing in the hall and nobody responded!!! People in scrubs walked past and went into other rooms, and nobody paid any attention to the pleas for help! My children were with me that day. All of them were affected by what they saw. Sadly, she spent 10 days in the acute care unit at the hospital and then was transferred to Phoenix where she is in the neurological institute because the UTI she contracted at Archstone (because of unsanitary conditions and NO care) got into her blood stream and crossed her brain!! Her blood thinners were not monitored, so the infected, thick blood deprived the brain of the nutrients she needed! She was getting better when she got to Archstone and now she's dying!! The doctor says she isn't well enough to go to a skilled nursing facility and recommends hospice as an option!!! I am SO sad and ANGRY!!!!! I checked on the national nursing home registry and Archstone has several serious deficiencies from their most recent inspection, and at least one from each inspection before that. My mom's crappy care cost us $200 a night!!! (Better places are half as much!!) There has GOT to be better skilled nursing in Chandler! Archstone is the short road to the cemetery!!! read more

We recently moved my 83 year old mother in law to Archstone skilled nursing for long term care. She is an alert, completely intact with her cognition and friendly. This is a tricky placement as she needs to have a feeling of normalcy and not an institutional environment. We found Karen in admissions to be very caring, supportive and really has gone above and beyond to make her feel happy and valued. The nursing staff and CNA's are excellent. Her call button is consistently answered within one minute! The food is average to good for long term care. The facility is homey and the patios are beyond beautiful. We work in the medical profession, and I personally have worked in dozens of nursing centers, so we are not naive or inexperienced at evaluating skilled nursing. In addition, they do NOT have a problem with bugs. If we had seen this or our mother, we would not stay. read more

My daughter is in Archstone care faciity in Chandler Arizona. I have noticed that the meals have changed in there. Apparently they have changed the staff in the kitchen and the meal portions are very small. Most of the time you get the wrong meal or such small portions which is not right or fare to the residents at all. Then evening meal is served around 5:00 p.m. and then you do not get breakfast until 8:oo a.m. the next morning. I believe that one's health and well being are not being met and would appreciate you looking into this. read more

My father was admitted to Archstone after having open heart surgery. He had a rough go of the surgery and was suffering from hospital delirium from being in the ICU for quite awhile. I expressed this to staff that my dad was newly mildly confused as well as hard of hearing. They ignored him. He fell twice. The food was mediocre at best (however the dining staff were great). They didn't recognize my father was having a stroke, they passed it off as dementia. My father was a fully independent and capable man prior to admission. Something was clearly wrong and I expressed it multiple times while asking for a physician for two days. They sent me two psychiatric PAs who were very pleasant but apparently ill advised to place a geriatric and confused man on antidepressants. Jesus. I made the staff call 911 finally after I had had enough of them ignoring his symptoms. My father was clearly digressing, and I was right. He was severely anemic and bleeding into his brain. The nurse that I spoke to the most, Ray, was very nice; the nursing assistants were quite nice when they weren't rolling their eyes, but for God sakes....I called for help for 10 minutes the night I got my dad the hell out of there and the hallways were dark with no one to be seen anywhere. NO ONE. When someone finally did come, she said "just a second" and walked away. I yelled NO and demanded 911. Start talking about calling 911 and every one and their mother comes through the door fumbling with their own equipment. One nurse had the nerve to tell me, his daughter, his care taker, and MPOA who also happens to be a cardiac nurse that "this is him, he's confused, that's his normal" I was livid. If I had not acted that night, my father could have been dead by morning. When we got to the hospital, his hemoglobin was 5! Five!!! Do you hear me, Archstone???? He fell twice, you think maybe that helped that situation?? Only after fall number 2 did they finally put fall mats in his room. When I asked for foot pedals for the wheelchair, the CNA (who was great I might add) had to go dig through a storage closet somewhere and brought us two different length pedals because that's all there was. WTH? His breathing treatment mask was frequently on the ground, his bathroom wreaked of urine on admission, and you had residents lined up in the hallways because it's easier to watch them. This is a common practice in nursing homes where it is notoriously overworked and understaffed. Administration---hire more people. The people you have now are not bad. Train them. Give them better patient ratios!! You will continue to have horrible patient outcomes with the way you manage. I've even spoken to people who previously worked here and they guessed what the problem was before I could even tell them. You have a great COTA and PTA. Don't lose them You'll screw yourselves even more and for God sakes...get them a real therapy gym so they can actually do what they so clearly are very capable of doing. There was one woman, my brother spoke with her more than I did. I can't remember her name but she dealt with resident activities and coordination. She was great. She truly enjoyed her job and she put alot of effort in to try to incorporate our father into daily activities and finding him a dining hall buddy to try to acclamate him to being in rehab better. Put handicap buttons on your doors. You guys have a beautiful courtyard!! You can't get to it if you're in a wheelchair unless someone opens the door. Really poorly designed facility. Also, the whole policy behind eating with family members must be done in a private dining hall is BS. You have one dining room...there are multiple families that eat with residents. Come on. Dumb idea. Oh and please go and buy a real ice machine. You have a cooler full of ice in the hallway....God only knows whose poop or flu covered hand has dipped into that thing. Zero stars. Horrible care. Horrible facility. Staff should find a different profession since they know nothing of true hospice care and the director of the facility is incompetent and dishonest. They moved a relative bc they stated they were unaware of an infection he had yet they made us wear gowns upon our first visit, they were fully aware. With all we are going through I would expect a baseline level of competence and empathy. None was noticed during his stay. It starts at the top and filters down. All they care about is money not your loved ones. read more

My family member went there to recover from surgery. He has special dietary needs due to being on dialysis and having diabetes. His dietary requirements were not met. The food they serve is poor quality and tastes awful. He had one nurse he really liked and she did a great job. The rest could not be bothered to provide good care. The activities are almost non existent. We spoke with several full time residents who told us there is not much to do and they spend most of the day in their bedroom watching TV. To top it off there were roaches in the bedrooms. I would not recommend sending your loved ones here. read more

Understaffed RN's and CNA's! Not fair to patients or staff!! I would look elsewhere!

We moved my father here after he had been hospitalized and following two different short term rehab facilities. He was ALTCS pending and Karen was very helpful in making room for him while we awaited his approval. I personally think it is a very nice facility and has a very homey feel to it in some of the common areas and courtyard in the center. My mother visited it with me and she told me if she ever had to be in a place like this, this is where she would want to be. We got my Dad moved in and it was great that they would do his laundry (although he did lose a lot of clothes). They have good therapists and I thought the staff was all very attentive, patient and competent. My dad complained about the food but every time I was there, I thought the food looked pretty good and I commented several times that whatever he was eating was better than what I had for dinner. A few times the food came and he said it was terrible, so I started tasting it and I thought it was good. I personally think sometimes he was just looking to complain about anything because he was not happy that he could no longer live at home and be independent. They really tried to engage him with group activities and events and he refused to participate, so unfortunately he missed the chance to be a part of a community and find others with common interests. The biggest challenge for him was the fact that he had to have a roommate and his room was quite small. ALTCS does not pay for private rooms, and he was very unhappy about that. After about 2 months, we ended up moving him to a group home in a private residence and are now paying extra every month for him to have his own bedroom, but guess what - he still complains about the food and now he is getting home cooked meals 3x a day, and he does not have 24x7 medical staff caring for him. I would recommend Archstone to others, my advice is to be realistic about what it is and what to expect. Their primary focus is to keep their residents healthy, stable and safe, and they go above and beyond to try to make it more than that with their common space and social activities. If you are placing a parent or loved one here, you will have a different perspective than they do about what is really important and trying to set realistic expectations with them up would be a good idea. read more

I would like to give a 3.5 Star rating, but it won't allow me. I think it's unrealistic for a care facility to have 5-stars. My idea of 5 star is unrealistic and very costly, so please take that into consideration when you see my rating of Archstone Care Center. The ones who caring for our loved ones 8 hours,12, even 16 hours is praise worthy. Archstone Care Center has a large number of 5-star staff. The Archstone family/team truly work together to make sure my/our loved one(s) is in a positive, safe, and comforting environment. I am thankful for the care she receives from the nurses, CNAs, laundry staff, cleaning crew, maintenance team, activities department and the business administration team. read more

I had my mother here over a year ago. I had addressed my concerns with the Administrator. He did email me back quickly. After a few emails back and forth I was disappointed in his replies. I reread the emails and I had expressed concerns of his employees and how his staff speaks of him. I also mentioned my mother had passed away and that is why I didn't email him right away. Not once in any of the emails did he offer his condolences or even ask how our family was. All he was consumed with was who spoke ill of him. It really showed me that what is said of him might actually be true. Knowing this person is responsible for the way Archstone is ran and he didn't show any care towards my mother or our family saddens me. I would not recommend it here. The nurses were quite rude and loved to be on their phones. The aids don't seem properly trained. The only time I saw the nurses and aids run around quickly was when the survey team was there. An employee rushed into my mother's room to make sure her call light was in reach and working. Shouldn't have that been checked before a survey team is there? I don't know what a survey team is but that's what I heard the nurses talking about. The director of nursing does not like her job. Any time I had to speak to her she was rude and very short with us. I don't know if it because I am an RN myself and know the ropes. I had to speak with Jaime the speech pathologist once and she reeked of alcohol. It smells everywhere in the building. This place is the most unsanitary, understaffed, patients are neglected, care is abusive, infested with insects (roaches and scorpions). Nurses and aides are lazy. Call lights are not answered in reasonable time. Please do not put your loved one here. The care is SO poor. This is not a biased one time experience this review is based on multiple visits there day and night for extended periods. The state should shut this center down. My mom has been in Archstone for the past 8 years, and we have been very pleased with her care. I like that the employees are stable and are very caring and loving towards my mom. The building is old but you would never know by looking at it as they have done alot of updates over the years my mom has been there. Never judge a book by it's cover! read more

Surprised at facilities condition and lack of weekend resources. Dad was admitted on a Saturday night. Once admitted he was told he would not be cleared to walk (per his surgeons instructions) and would remain on bedrest until assessed by Dr. That is fair enough but Dr. wouldn't come in until Monday. Other facilities have the capability to assess and provide services on the weekend so if you need these services I would keep looking. Nurses were kind but they were limited until he was assessed. Director of Nursing was extremely rude to me when I requested he be transferred and refused to address it prior to Monday. They didn't meet his healthcare needs nor was he discharged until Monday. Wrote a complaint letter to the facility they never responded. Pictures are worth 1,000 words. I was only in one room during this two day stay but it is in need of a much needed update for safety and compliance reasons. I had my husband bring bleach from home to clean the room. I didn't want to leave my Dad there alone until discharged. read more

Zero should be an option. Facility was very helpful and accomodating while transfering my loved one there from the hospital. But as soon as she was admitted, the care depleted. Here are things we observed within three random visits at different times of the day: urine soaked socks on her foot that was already infected with a bed sore, immobile patient being left alone and with door to room closed while out of bed on a precarious commode without reach of a nurses button, oxygen compressor without intake filter (in fact, the filter itself was on her bathroom floor...in urine), urine dripped all over floor near commode, hand wound uncovered when orders were for it to always be bandaged, and my favorite...on the last visit the RN started a confrontational argument with us in our loved ones room when we arrived to give our loved one the oxygen mask she had requested. There was not a single supervisor on duty that night, nor a doctor on call. Even though we are listed as POA, and our loved one is fully coherent and lucid, the RN argued with us for 20 minutes refusing to allow the oxygen mask. Two nights previous our loved ones oxygen levels dropped to 84 without the mask, multiple staff told us they did not provide them, but we could bring one...and when we were finally able to bring it this was what we faced. Our loved ones oxygen level was on 3 when we arrived and she said she wasn't getting enough...the RN turned it down to 2 and then refused to tell us what the orders said in the computer. This place is a nightmare and first thing in the morning we will be on the phones looking for a transfer. Our loved one is a tiny, late 90's female with absolutely no other health problems other than nuralgia and having just been through a partial hip surgery. She is not even a difficult patient. Prior to this she still lived by herself at home. But after a week in this place she has pneumonia and signs of congestive heart failure. Is there a rating LESS THAN ZERO?? read more

This is the worse place you can possibly put your loved one. Had my grampa in just for respite care ( hospice) , they ignored everything we told them on how to manage my grandfather. We told him he needed to be fed, they didn't or at least did a piss poor version of trying to feed him. He always stunk like pee, his breath stunk ( no brushing of teeth). Two nurses were good, the others had attitudes and we're unwelcoming. They kept telling us he was difficult and didn't want to eat. This being said to an old certified nursing assistant who took care of the elderly for 13 years, ( i would have never told my families that) , you try and you learn how to approach each patient. The administration were all understanding about our concerns in our face then turned around and tells Hospice they couldn't handle his behavior ( what they really meant was they couldn't handle the families expectations). Seriously? 140 lb , 84 year old fragile non old man that is blind and is loving and kind but gets frustrated because he can't see and we changed his environment because my mother had gone 8 days with no sleep. Oh and we lost our grandmother his wife at the same time. All I can say is your loved ones whom worked and supported families all their lives should get better care. They love the patients that have no family so they won't get in trouble for ignoring them. Sad day in this world because this facility has won best facility of year 4 years straight. I really do not know who judges these places. In my eyes this place is a glorified, upper class dump. read more

My mom only stayed there for a week and I live 1,000 miles away and was flying down on weekends. She went as a rehab patient. During her time there, I would get three or four calls a day from different staff. I had to send her Healthcare Directive and POA to each different area. On day 3, their social worker called to identify mom's assets and start paperwork for moving her to the assisted care side of the facility. What! She came in 3 days ago for rehab and you're telling me you've done a complete medical assessment and came up with this? That was the last time I heard from the social worker. Then a nurse called concerned that mom had quit eating and drinking. What?! Then their hospice nurse called wanting to do an evaluation...o.k. Then the nurse asked if she was still at the hospital emergency room or cardiologist...WHAT???? Last time I heard, she was at Archstone. I called the emergency room and found out she was there and had be there for some time. I called Archstone and the poor nurse who answered the phone told me he was going to call me. That's good because she was taken out of your facility 4-5 hours ago for a doctor's appointment without notifying me. Modirectivzthcare directive stated she did not want to be revived. It was written by an attorney, co-signed by witnesses and notarized. The facility administrator came back and said she couldn't accept the directive unless it was an Arizona based directive. Hmmm, no other facility had issues with her directives. AZ's directive required the signor to be coherent, which my mom was no longer. If I hadn't had to watch a friend of mine to through a similar process where the facility tried to take control of her mom and keep her alive because it was financially lucrative, I probably would have just dealt with this...I was 1,000 miles away. Instead, I had sympathetic and caring help at the hospital and moved her to Hospice of the Valley. We were lucky to get in and they were able to honor my mother and treat her with compassion and dignity. read more

My Dad was admitted approx three weeks ago to Archstone. My father has been in some horrendous facilities. I have learned to ask questions, have open communication with staff, and help them whenever I can. First off Adam in admissions is very accommodating and he really seems to be improving the facility. I have loved the therapists and everyone I have reacted with. I had one issue with a particular nurse and it was addressed. The facility is clean and they are remodeling, the dining area and gardens are beautiful. The activity director Lindsey is phenomenal! I am there the majority of the day and see everyone working hard. The CNA's really try to accommodate and the nurses seem very competent. I think it does not deserve such a low rating. I would recommend one thing to administration, hire more CNA's to avoid burnout. They have the toughest job of all! A big thank you to Rachael the NP who personally called me to answer my questions at night after returning from a trip. Also Dr Ayela is great and personable!
