    Update: 1/22022:…read more Not how I wanted to start my new year with a reduced rating.. Why two stars? I don't have time to reminisce when I chose to go with Yolo. I am so upset, and livid. Not just for me, but because my son's count on me for guidance and I failed them. The last week, constant calls were made and we were relying on the right direction from the staff and the correct information.. We were told if we wanted to take my ex off of hospice, we would just have to call them. I just wanted to get him in the hospital, because of not drinking or eating (3 days), and now blood in urine (2 days increasing). I called yesterday morning, and they said they would meet us at the hospital to sign. No one ever showed, and finally got a call after calling them. Why am I upset? When the staff came out and attempted to give him them his needed medications, they failed. Yes he was agitated and refusing.. but yes this man has a brain tumor and his refusing or saying no, along with confusion and extreme aphasia.. just stop trying?. As each call got worse his care team never came up with an action plan.. Another gripe, every time I was told they would call me back, never happened.. Even the hospital shook their head, because of this situation. I would never want our situation to happen on anyone, so please keep calling when they don't return calls.. and push for them to be more active .. be the voice for your loved ones..

The staff understand the elderly and all of their needs. They have cared for them, while making them feel as though they were the part of a larger, extended family. The grounds are lovely, and Dad has the joy of watching nature and taking part in the gardening and watering of the tomato plants. Mom enjoyed the interaction with the other residents, and particularly liked taking part in the art activities~~~something that I had no idea would be such a favorite. Their room is spacious with large windows. They have all the issues that typically plague the elderly---they need help in the bathroom, bathing, transferring. They cannot medicate themselves. Meals must be provided. From a monetary standpoint, it has been a good exchange for the support that they have needed. We had some fluid moments, and I am very pleased that Spanish Bay accommodated their needs at a price point that was fair for all. After being their caregiver for several years, it wasn't easy letting them go. I have to say that Spanish Bay has been as supportive of me and my family as they have been of the folks. Growing old is difficult. You lose the ability to navigate life and the many different decisions that have to be made daily. Spanish Bay is exactly the environment that suited their health and social needs. If you looking for a true home for your elders, this is it!

    **January 2026 Update** I had high hopes when the Ensign…read moreGroup purchased the facility in August 2025. Those hopes and the hopes of others have diminished over the last 6 months. The Administrator told several of us that he wouldn't be reading our emails and we were to address issues to the managers. The managers would be running their departments like their own businesses. I contacted the company Compliance line (9/25 & 1/26) as well as the California Department of Public Health over dirty silverware after repeatedly contacting both the Administrator and the Dietary Manager. In December I sent a separate email to the current Dietary Manager that the proper procedures (as explained in September by the Infection Control Nurse) were not being followed by some employees due to the condition of the what was received. Her response, "I will review with my staff. Also, we are going to rotate in new silverware" (12/26/25). Yet, I continue to receive dirty utensils as recent as 1/15/26. The residents routinely are missing things from the meal trays (butter, milk, condiments, utensils...) causing the CNAs to leave the floor and deal with hostile kitchen staff. Food preferences are often ignored (request to have small portions and food choices). I have repeatedly emailed about this as well as meals being early/late as well as menu changes with residents and staff not being notified. This is a big deal because the CNAs rely on scheduled meals to manage their time. I've come back from a shower and the food carts were in the hall 20 minutes early. Cold food on hot plates, food over cooked, mushy rice, flavorless meals.... I realize that this is institutional food, but there is a lack of attention to what goes on the plates. Milk is regularly out of inventory and residents are given cereal without milk on more than one occasion (a grocery store is next door from the facility) or chocolate milk to put on their cereal. The Administrator was copied on each email I sent to the Dietary Manager. I have been nice and I've been aggressive in my language regarding these and many others and neither approach has worked. I also have transcripts of weekly meetings with the Administrator and kitchen management where these issues and others were discussed. The Social Services Department has not processing medical referrals submitted by the facility Nurse Practitioner. One that was submitted in April and never processed. It was resubmitted in October and again January. Another was submitted in October and has not been addressed either. I manage my own appointments, but the facility must submit them to a medical facility. This is but one of many I have with the department. I decided this time I wasn't going to follow up to see how long it would take. During a November meeting with the Director of Nursing I had in November he stated that referrals were discussed daily. He directed Medical Records to see me and I requested the information showing that the October referrals has been requested and in the system. As of today the referrals haven't been processed. Housekeeping is also deficient. Most of the housekeepers do not thoroughly clean my room (others have the same issue). The showers have mold and there are often strange drippings on the walls. The lifts (standing & Hoyer) that are used for those of us who can't stand or need assistance standing were purchased that aren't adequate. These Hoyers don't get high enough to get people in their chairs or out of bed get stuck under beds. This issue was relayed to management several times yet somehow they were purchased anyway (initially they were rentals). The oldest lift is the one that most CNAs use but it has issues as well. The three Hoyer lifts I used in the last week all of them were dirty and looked like they hadn't been cleaned in some time. One had crusty finger prints and another had a layer of grime where the bar was welded. It's not unusual for the batteries not to be charged (I've had 3 batteries not be charged in a single day). Noise levels in the facility by staff at shift changes and other times are excessive. It often sounds like they are having a party while residents are trying to sleep (6:45am and 11:00pm). Management has been notified several times but that only helps for a short time. A refrigerator that was purchased to be used by residents (mandated by the State of California) sat unused for months while we waited for the rules of use to be written. Some of us have still not received the rules despite mentioning this to several managers. The screens on the windows and sliding glass doors have been broken since I came here 2 1/2 years ago. For these and many other reasons I would not feel comfortable having a family member receive care at Courtyard.
