What I've come to understand about hospice orgs is that they seem to attract nurses who may be…read moreburned out from other jobs. This results in transactional care, checking off boxes to comply with Medicare but a lack of genuine compassion for the patient. (hospice enrollment recommendations below) Hospice affords more normal hours than working at a hospital. However, the trade off is probably that the nurses may have to drive 100 miles a day all over the county to serve patients. It's a crap shoot when it comes to the nurse case manager (weekly nurse) you get based on when you enroll in hospice care. And it's a crap shoot for the nurses too, who gets assigned to them. (see #4 below) Transitions was recommended to me by someone I trusted. I didn't know what I didn't know but my initial instinct during the first transaction with the intake nurse seemed off to me. She didn't listen, was not paying attention to what I said (about my Mom's care needs) and not educating us on how we should be looking at the whole hospice experience, especially medication management. We lasted a month and a half before I went to another company. The whole point of hospice is having continuity of care. The nurse case manager kept scheduling other nurses to come in her place which consequently caused us to have to orient a new nurse almost every week. Recommendations for Hospice Enrollment and Ongoing Management 1) Trust your instincts if your first interactions with the hospice org aren't stellar. Unless your family member is very close to death you probably have time to interview another company. 2) How medication is administration is explained and distributed (by mail or pharmacy pick up) is critical. 3) Understand that the first several intake people who call or visit may not be the regular providers you will be seeing. They have to check boxes for Medicare so they may only be marginally helpful. 4) The Clinical Manager who manages schedules and nurse performance is THE most critical role in the process. This person MUST be a great problem-solver, compassionate, caring, understanding and a good listener. Spend time up front describing your loved one's personality, likes and dislikes to hopefully get the best match possible nurse case manager (weekly nurse). 5) Once you get through the initial process, if you don't like the hospice care you're getting it's fairly easy to transition to another company (assuming you have POA for your loved one). 6) Make sure your POA and Healthcare POA are updated. If you had documents drafted years ago, they may not be specific enough for today's requirements. 7) If you have a life alert system you can give the hospice company number to the life alert system company so 9-1-1 won't get called in an emergency. 8) LPN's have more authority with medications than RN's but Medicare requires RN's to visit at least once every other week. 9) Hospice nurses understand that the closer to the end of life someone is, the less interested in eating they will be. Also, people at the end of life will sleep more. So keep track of eating and sleeping because the nurses will be most interested in that in addition to pain management. 10) Document changes in health in advance of nurse visits even if it's jotting small notes on a calendar because over time all the days blend together.

Shared room with shared bathroom, little privacy, often difficult to sleep and awakened at 5:30 am to make sure the night shift can get patients ready for therapy before the day shift arrives. Food generally not good; some residents order food delivered. Administrator was rude at check in after not explaining all of the facility policies to me. Discharge planner referred me to a home health agency, Amedysis, that is not in the UNC system, which is odd for a UNC hospital. This agency apparently cannot access my doctors' records re: my rehabilitation goals through UNC MyChart. If you are a UNC patient, ask lots of questions before agreeing to referral to Amedysis.

The facility office manager but mainly the newer medical assistants however were unprofessional and could not believe my experience today; quite rude, gossip behind your back, and in this visit made me feel uncomfortable. Everyone else has always been great. Update: I received a call and an apology and ensured no more gossip would be tolerated. This is truly appreciated.

However I work in the healthcare field and I have seen a lot of things that could have been avoided if people just would have gone to the doctor, so as a result I am a firm believer in yearly check ups. I am also a huge advocate for finding a doctor that you like and trust. Unfortunately due to changes in the healthcare system, doctors rarely have time to spend with their patients. There's always a demand for them to move quickly in between their patients to get to the next one. My husband started going to Dr Grace Tang several years ago and he is her number one fan.....if doctors can have fans..... . She recently moved to this practice and he spent months researching trying to figure out where she had gone. I was also in the search for a good primary care doctor, and when I saw how adamant my husband was about finding dr. Tang so he can continue going to her, I knew she had to be a fantastic dr. (My husband is super picky.) So I made an appointment for my yearly physical. Let's start with the front office staff, on the phone when setting up my appointment they were very professional, and answered all of my questions regarding insurance, billing, and my goals. Upon arrival they were just as professional, kind, and greeted me with a smile as I came in. Unlike most practices, I only had to fill out a couple pieces of paper. Dr. Tang's nurse was also very nice, and professional. She took my height, weight and blood pressure, reviewed a few questions with me, but left the majority of my history for the doctor to do, which is very unusual. Usually the nurse obtains all that information and if the doctor wants to know they have to read it. But that is one of the things that I really appreciated about this experience, the doctor sat & listened to my health history, and wrote her own notes. And then there was the infamous Dr Tang! She was gracious, classy, thoughtful, and actually cared about what I was saying. She sat down and took the time to listen and ask questions. So very unusual from the typical revolving door experience i'm use to. And last but not least came the most dreaded experience for me, lab work! Before we go any further you should be aware I'm a huge needle phobe. I hate needles with a passion.... needles bring about massive amounts of anxiety for me....truly it makes no sense, but that's how it is. Dr. Tang suggested that I stay in the examining room and lay down for the blood draw instead of going to the lab draw room. For me this was an excellent idea! And then in came Brittany. I'm not sure if Brittany was a phlebotomist or nurse who does lab work ( as a result, henceforth she shall be referred to as the blood sucker!) but either way she was amazing! She kept me calm and relaxed with her down home compassionate and sincere demeanor, and she's really good at what she does. I barely felt the needle go in and she worked quickly despite all of the blood that I needed drawn! When it was all said and done I told her she could draw my blood any day. BEST BLOOD SUCKER EVER! !!!!!!!! And believe you me people that's not something I ever say! From start to finish I had an excellent experience at this practice. I highly recommend it to anybody who's looking for a doctor (and staff) who want to take time with their patients and get to know them, and treats them like human beings, not like a herd of cattle they have to get from one location to the next as quickly as possible. Oh! I forgot to mention their patient web portal. I realize more places are turning to this, but I still have not seen it as often as I thought I would. However! They have a simple to navigate web portal where you can email the doctor with questions, see your lab results, and see the suggestions the doctor gives you regarding your lab results. And they do lab results fast! My husband had his results by the next day, with recommendations from the doctor attached.... I totally find this impressive

I had an appointment the next day and she tweaked a med and now I am fine. This wasn't the first time she called me over something. Once she called me on her way home from her car and she sat in her garage for 5 minutes until we finished!. Okay, people, when is the last time you had a doctor call you at home?????? WHEN??? I have no idea if she is accepting new patients but if you are looking for a practitioner, ask if there is a waiting list and GET ON IT!!
