I requested my records when you changed practices in 2017. Former, current and future patients, I implore you to: 1. TAKE NOTES OF YOUR VISITS. 2. TAKE PICTURES of EVERY FORM YOU SIGN OR COMPLETE, INCLUDING YOUR PATIENT INTAKE/SYMPTOM FORM. ***This intake/symptom form, one form, is your only voice - you the patient. My voice was erased.*** 3. TAKE PICTURES OF ANY HANDWRITTEN INFORMATION YOU PROVIDE TO YOUR DOCTOR. I did provide handwritten information on a single notecard for each visit. These notecards were also not included in my medical record in any form from 2014 to 2017. Why not give these notecards back to me? 4. If you are unable to write or have memory issues, RECORD YOUR VISITS for your own recollection of what was discussed and what you need to do. 5. REQUEST a copy of your MEDICAL RECORDS. 6. He was attentive, truly listened, and consistently responded to messages within 24 hours. When I tried other clinics, I realized how exceptional his care was. Recently, I returned to BMG but had to see Dr. Ongsingco as Dr. Wallace was not accepting new patients. Unfortunately, my experience has been the opposite. Dr. Ongsingco seems disinterested, rushes through appointments, and takes nearly a week to respond to messages, if he responds at all. I highly recommend this clinic--but only if you can see Dr. Wallace.
