It took me 4 months to get a new patient appointment. When I saw Dr. Allmon she assured me that if…read moreI am sick they will get me in same day or next day. I called and left a message a month or so ago when I was sick and no call back. I called last week and left a message to get a rx refill and no call back and no refill given. I called 2 days ago leaving a message that I am sick and my name, date of birth phone number, they never returned my call. Today I made a mission to get a hold of them. Called left a message. Called again got a person, was placed on a hold and then when they answered my call was told they do not have any appointments. I absolutely would not recommend this office.

Per Baptist Memorial Hospital-Collierville, my intake/symptom forms from 2014 to 2017 were "shredded" by you or your office within Baptist Memorial Hospital-Collierville's 3 year policy and conveniently when I was most ill. I requested my records when you changed practices in 2017. Former, current and future patients, I implore you to: 1. TAKE NOTES OF YOUR VISITS. 2. TAKE PICTURES of EVERY FORM YOU SIGN OR COMPLETE, INCLUDING YOUR PATIENT INTAKE/SYMPTOM FORM. ***This intake/symptom form, one form, is your only voice - you the patient. My voice was erased.*** 3. TAKE PICTURES OF ANY HANDWRITTEN INFORMATION YOU PROVIDE TO YOUR DOCTOR. I did provide handwritten information on a single notecard for each visit. These notecards were also not included in my medical record in any form from 2014 to 2017. Why not give these notecards back to me? 4. If you are unable to write or have memory issues, RECORD YOUR VISITS for your own recollection of what was discussed and what you need to do. 5. REQUEST a copy of your MEDICAL RECORDS. 6. Request a Complete Thyroid Panel. Request your lab results. Do your own research. Thyroid disease affects 1 in 8 women. I still cannot get all of my records while under your care requested August 2017. Where are MY records? YOU MUST BE VIGILANT ABOUT YOUR OWN HEALTH CARE. YOU ARE YOUR BEST ADVOCATE.
