The facility did not give her a walker despite asking for one and was told if she needed to go to the bathroom to ring for help. She did as instructed, waited an hour and no one came. Rather than wet the bed she forced herself out of bed and into the bathroom - could have done extreme damage to herself. They also got a critical medication dosage wrong and when we tried to get it resolved the staff person and then the nurse - rather than offering to check with the dr - were obstinate in insisting that it was the correct dose. After some raised voices we finally got them to confer with the dr. The food was inedible - I brought lunch and dinner almost every night. Twice, in the week she was there, someone woke her in the middle of the night asking if they could change her diaper - she didn't wear a diaper. Rarely did any staff member introduce themselves and/or explain their role. Random people would walk in and start taking or asking questions and we'd have to ask them who they were and what their role was. It was very disconcerting to be woken up by a strange man in the middle of the night wanting to "change a diaper" that didn't exist. There were some patients/residents sitting in the lounge area in their wheelchairs every day with a plate of food in front of them but it looked like they could not feed themselves. I never saw anyone interact with them. It was heartbreaking. I like to believe that the staff care for the most part but they need a lot of training and/or additional staff/nurses. The room was clean The lady at the front desk in the downstairs lobby was always friendly.

After touring the facility, being told both my parents would be accepted and together , which was most important to my family & expressed deeply to the administration. I was encouraged to initiate the transfer process for my mother who is in another facility , after obtaining OCCO/PAS approval, and transferring one spouse into the facility all was approved , we were later informed that the second spouse was being denied due to "acuity level" and "payor "status concerns. What made this especially difficult was the lack of any clear clinical explanation despite repeated questions, while nearly every conversation focused heavily on finances, Medicaid pending status, payees, and financial responsibility paperwork & give us until Monday after a stressful week to discuss like it didn't matter . Families navigating long-term care are already overwhelmed emotionally, financially, and physically. Communication matters. Transparency matters. If there are concerns about Medicaid pending admissions, families deserve honest and direct communication before major decisions are made , especially when dealing with elderly married couples who may now face separation after relying on representations made during the admissions process. I strongly encourage families to: * read every admissions and financial form carefully, * understand the difference between POA and personal financial responsibility, * keep all communications documented * and ask detailed questions before making transfer decisions Be aware that federal law (42 CFR § 483.15) prohibits facilities from requiring a third-party guarantee of payment. As for the food , my father hates it and says it's disgusting . He said he has lost a lot of weight , he says his stomach is shrinking & he is not liking , or eating the food , he is hungry all the time however he said he can't stomach to eat the food . . I have ordered outside food to the facility upon my Dads request. My dad is Jewish , he's not at all kosher , however he liked the Kosher meals on wheels at home . Outside of PT and OT sessions, my father appears to spend nearly all day in his room/in bed with little to no meaningful activity or engagement , which to me is deeply concerning . Communication and responsiveness to the facility have also been extremely frustrating during our experience. This process has been heartbreaking and unnecessarily stressful for our family. Mother's Day weekend passed & my parents are now an hour from one another & separated due to what happened here . Disgraceful.

Communication is…read morepoor, my father, 99 with a broken back, was put in a room that was dark, no water, no pitcher, tv with one channel. Held captive by POA, I was powerless to move him. The aids came in one night to weigh him. Why? A man with a broken back to be hung in a net contraption? I watched him screem as they attempted to lift him and stepped in to stop it immediately. I unhooked the device and told them to get out. I had to find a nurse (no where to be found) to make sure they NEVER abused him like that again. Why in anyone's name would you put a man with a broken back inside a net? Zero compassion. No water ever in the room. I HAD TO FIND A CUP AND PITCHER for my dad. I was there often as well. He was ALWAYS thirsty when I came which tells me they were not attentive at all. He wasted away until he died with zero morphine on board because why? Oh they were "out". Grace hospice that partners with them, had to be called three times on 2 separate days to get care for my dad. Oxygen machine kept going off with a loud beep alarm. No one came and each time I had to reset it. It was extremely disruptive. It was horrible and I watched him die, his hand in mine, 8 hours without morphine and three calls out to Grace Hospice. The care is horrible, unattentive and abusive. DO NOT SEND your loved one here if they cannot advocate for themselves. This was the MOST HORRIBLE experience and I can never get it out of my head. My father is at peace but it was a horrible, suffering death. Type of Care: No compassion - all transactional Communication with family: Terrible Room: Dirty and dark Staff: Unengaged DO NOT SEND YOUR LOVED ONE HERE IF THEY CANNOT ADVOCATE for themselves. TERRIBLE PLACE.

    My father was placed here after a broken back confirmed by hosptial. He was 99 years old. He was…read moreplaced in a room, dark, TV with one channel, no water or pitcher in sight upon first visit. I KNEW this was not for him. (unfortunately I have no POA to move him). I went often and one night three aids came into his room, closed the door behind them and had a weighing contraption they proceeded to hook him up to. I said "He has a broken back", their reply it will only take a second. They put a NET on it and proceeded to lift him. He started screaming "Stop, please stop" at which time I stepped in and stopped what they were doing as clearly this was causing excrutiating pain. I unhooked the equipment and told them to get out after mandhandling my father. I had to find a nurse (none around) and instruct that is NEVER to happen again. He died in their care three days after that catastrophic event while I sat with him, no morphine in sight. 8 hours without any morphine, none on the premise. I called Hospice three times, it never arrived and he passed a terrible death 8 hours later while I sat helpless and held his hand. DO NOT BRING YOUR LOVED ONE HERE. The care is terrible, communication worse and its not a place for someone if they cannot advocate for themselves.

I was lost. I was a criminal. I was an unproductive member of society. I saw no hope in my life until I went there. Just to let everyone know my name is Damon. It's actually why I picked this place because it literally had my name written all over lol. I just have to say that this place, the idea, the caring staff who never gave up on me saved my life. It wasn't easy but that's what they taught me. Life is not easy. Although the more I learned from Damon House made my life a lot easier than what it was. The staff are so dedicated to saving lives it's hard to believe that you feel that a stranger you've never met cares for you so much. Cares for you to point that they give up their wants and needs to help you. I'd say give up their happiness to help you but I realized helping you made them happy. I love this place (although I didn't love it when I first got there lol). It took a while for me to realize the good in people and I have come to realize the GREATNESS in Damon House and their staff. The director truly cares about everyone and that sets the example for her staff. I thank God that I was dumb enough to pick that program just because I shared the same name! lol.
