Nurses routinely dismissed this, saying she was simply "tired." In reality, she was being administered PRN medications without our consent, leaving her heavily sedated. They have refused to release her medical records with his a violation of the state of Illinois law. During a surprise visit at 10 p.m., we found her in such poor condition that she had to be rushed to the hospital, where she remained for seven consecutive days. Not once did we receive a call from Peace Igbinovia ,, the Executive Director/Director of Nursing, during this crisis. Family concerns were consistently ignored, and promised care services were never delivered. The facility is grossly understaffed, and the competence level of the nursing staff is dangerously insufficient. Integrity -- or rather, the lack of it -- is evident and was even openly acknowledged by the Executive Director aas she noted that several of her nurses were dishonest and unethical, yet she refused to hold them accountable. . Medical professionals urged us to remove her immediately and seek alternative care. We did, and she is now thriving and receiving excellent, compassionate care elsewhere. Families deserve honesty and safety -- Autumn Leaves, under its current leadership, provided neither. If you care...you will look elsewhere. Your loved one deserves better.

But I'm writing this review to express that my family will forever be grateful for the exceptional care that our father received at Autumn Leaves of Orland Park. They made my father feel like this community was his home and we were treated like family. The wonderful caregivers, the nurses, management, cooks and even the custodial members of this community were so kind, caring and loving to my father and our family. I interviewed so many places before I found Autumn Leaves, and at first, it seemed too good to be true, that we had found a place where the staff genuinely cares about its residences and treats them with kindness, patience and compassion. But we certainly had found that place! Autumn Leaves has such a dedicated team; all hands on deck from every single worker! I never saw a staff member on their cell phone while working; they all make sure that the residences eat their meals, get plenty to drink and give them the help that is needed throughout the day. My father's laundry was always done in a timely manner, his room was always kept clean, and his bed made every day. I had a stereo in my father's room, and the caregivers made sure his Frank Sinatra music was turned on every single morning for him without fail while they would get him ready for his day. His medications were always given on time as well. And during the night, the staff would check on him multiple times. If you're looking for a wonderful place for your loved one, look no further than Autumn Leaves. Our family was blessed to have our father here!

    NIGHTMARE AT PORTER PLACE As a nurse for 20 years, I must…read moreadmit that Porter Place is the most poorly ran facility I have ever encountered. I refuse to allow another family to go through what my mother and I experienced at Porter Place without A WARNING. My mother WENT THROUGH HELL and dealing with the staff took up so much of my time and effort as a nurse and her Advocate that I felt as if I worked at the facility. ADMINISTRATORS AND LACK OF OVERSIGHT: The top administrators, the executive director, Yosanne W.; the director of nursing, Kathyrn P.; and the ADON, Charli M. are not committed to providing quality care, nor do they have the experience and knowledge of what that actually looks like. I know this because after numerous complaints on the lack of quality care my mother was receiving, the subpar care continued over a period of months. After I placed a camera into my mother's room, I found out that the lack of care was worse than I thought. The caregivers were not even providing basic care to my mom. They were not cueing or assisting her with her morning ADLs - washing her face or brushing her teeth. Oftentimes she was not receiving her showers or even asked if she wanted one. I was nice enough to label my mother's dresser drawers to make it easy for the staff to help Mom get dressed in the morning. Yet, some caregivers omitted her bra, wig and some days her socks. When I repeatedly mentioned this to the DON, her response to me was, "Well, I have relayed this information to the staff, it is on them to follow through!" Say what?!? You mean to tell me that a person can come here, and get paid for not doing their job? Of course, I received no response. I have also witnessed the caregiver staff walk around with ear pods in their ear, listening to music, talking on the phone, arguing and cursing to whomever is on the phone while working. I have had to tell one young lady to turn her music off as she had it blaring on her phone while trying to help get my mother dressed. HIGH TURNOVER ALERT: The staff turnover rate is extremely high. As a nurse who has managed a facility myself, huge staff turnover is a telltale sign that something is wrong with the administration. My mother lived at Porter Place for 14 months and 90% of the staff has changed multiple times, including the top administrators mentioned above. Yosanne and Kathyrn are new and Charli recently left the facility. This very expensive facility will hire anyone off the street to work as a caregiver, mostly "unqualified and with no experience." In fact, Porter Place does not require their caregivers to be Certified Nursing Assistants (CNAs) which is ridiculous. Families pay a premium for their loved one's care and they hire high school grads and other young people with no experience and provide them with 3 days of "on computer" training to take care of our loved ones. Dementia/Alzheimer's residents require specialized care from employees who are specifically trained to handle different types of behaviors; and, who are capable of patience and professionalism. Most of the nurses are new LPN grads and PRN or as needed nurses. The few good nurses they had when we first moved in, left. When asked why did they leave, their response was they could not take it anymore. The top administrators' haphazard way of running a facility is bound to run the best employees away. PLEASE BE ADVISED: Porter is very expensive as they have multiple levels of care. If families are paying $8,000 - $10,000 a month, we deserve to have qualified and vetted personnel working with our loved ones. I found out through another nurse that Porter Place doesn't even bother to check references of their newly hired staff. She was hired on the same day as her interview. This is one of the reasons for the huge staff turnover. They will hire anyone who needs a job and when that person finds something better, they quit. LACK OF CONTINUITY: Every morning, my mother had a different caregiver in her room. There is no continuity of care and to make it worse, the regional manager, Nick T. told me it doesn't matter because they have dementia. LEVELS OF CARE OR A HOLE IN YOUR POCKET? Porter will accept your loved one at a lower level of care to get you moved in, and in a few months, they will go up on the level of care claiming that your loved one is requiring more of their staffs' time. They use a point system. The more points the more money and it is all subjective as the ADON or DON decides how many points to give your loved one. It does not mean more staff is on the schedule, only that your loved one requires more attention or redirection. For example, your loved one may have to be told to come and eat dinner 3-4 times as opposed to one time. They will use any excuse to charge more money. The point system is designed to nickel and dime you into the poor house. I will try to continue my review.

I loved this place for my mom before. She had exceptional care from so many and all management and staff knew her and me. Now you walk in and it's like night and day. There are still some phenomenal staff who should not go unappreciated (Bre, Porcha, Dell, Mercedes, Alexis, Dawn, Tamika, Mariah, Kiana, Keesha, Aneidra, Jomiece, Brittany, John - spelling could be wrong and I apologize). These people will go above and beyond. As far as the others - it's a paycheck and their behavior shows that. They don't care about the residents or the family members. My Mom was on hospice and her final days there were miserable for her and me. I spent day and night to be with her. The lack of compassion and humanity is overwhelming. The lack of professionalism is mind blowing. "Attitude reflects leadership" and let me tell you from personal experience, it shows! I would never recommend this place to anyone and it's heartbreaking because this place used to be a place of peace for me, knowing my Mom was so well taken care of. It changed. It showed. I hope they learn from this and make the appropriate changes for the resident's sake and their families. Original post...how things changed..... Where do I begin on how incredible this place is. My Mom started to decline and I had to make a tough decision on what to do next. I met with Kelly to go over options and tour the facility and attended a free support group for families at the facility. It was a perfect match from day one with Kelly, Brittany up front (now promoted), and meeting with Tamika during our in home assessment. I cannot say enough about Nurses Bri, Keesha and Aneidra. Porsha is a gold star who is always there; whether on a unit or up front. Dawn up front is my son's favorite person to see and she always has a special treat for him. Tori has gone out of her way to make my Mom comfortable on numerous occasions and Danielle (god love her) always ensures my Mom's room is clean and picked up. Those not named are such as special as the others but I'd be writing all day. If you're looking for a place where staff truly care - this is it. Also, Laporsha - thank you for stepping up into your head role. You're killin it!

    A relative was moved to another unit after a fire near theirs. Remediation took over 6 months…read more They have returned their original unit but now have an insect (roaches?) infestation. We recently visited and found roaches and their debris all over the kitchen. We cleaned as much as we could, and advised management of the situation. In 3 weeks, we've contacted them 2 - 3 times and nothing has been done. There is a person scheduled to clean weekly and they see and say nothing? We sent in our own exterminator, he was treated rudely by the staff. Really? Not acceptable.

    What was once a 5-star, beautiful facility with accommodations for independent living, assisted…read moreliving, and memory care has deteriorated into a 1-star, unsafe, unsanitary, and poorly managed, pest-infested community where seniors are treated with disregard and disrespect by management. * Management - The Executive Director is unfit and unqualified to work in a senior living environment. She consistently communicates with residents in a rude, dismissive, and condescending manner, seldom engages with residents, avoids addressing legitimate concerns, and demonstrates a clear lack of compassion or professionalism. Her leadership has contributed significantly to the overall decline in morale and quality of life for residents. * Theft from resident apartments - Multiple residents have reported missing items from their apartments. Concerns brought to management have been ignored or dismissed, leaving residents feeling unsafe and unprotected in their own homes. * Bed bugs, roaches, and ants - This became a recurring problem throughout the building after management allowed the garbage chute to remain backed up from the fourth floor to the ground floor for two weeks. Despite multiple complaints, management failed to act, claiming a part needed corporate approval. Corporate approval delays prevented timely repair. It was repaired only to break again a few weeks later. Infestations have continued despite repeated extermination treatments in my apartment and in apartments on every floor. This has created unsanitary and unsafe living conditions for residents. Many residents have had to discard mattresses, sofas, and other furniture due to this problem. Others have been forced to temporarily move out until it was safe to return. Several residents have suffered bed bug bites as a result of these ongoing infestations. This neglect from management has made it extremely difficult for residents to feel comfortable and secure in the building and in their apartments. * Outdoor walking path - The path used daily by residents for exercise remains obstructed and unsafe. Despite numerous requests, management has failed to remove debris or address low-hanging branches that make the walkway difficult to navigate. Pool - The pool heater has been broken since July, preventing residents from enjoying activities such as water aerobics. Despite repeated complaints, management refuses to spend the money to repair the heater, denying residents this important opportunity for physical exercise. * Activities - Activities that were part of the enticement of moving to this community have now been assigned to residents to handle themselves, rather than being organized and managed by staff as in the past. Residents are also responsible for setting up and taking down chairs, tables, and other equipment after each activity. * Washer and dryers - With two laundry rooms on each floor and several machines available, residents should not have concerns about doing laundry. However, very few machines work, and there is rarely a washer and dryer functioning at the same time. Seniors are forced to search other floors only to find those machines broken as well. Management has been informed repeatedly but has not responded or made any effort to replace the machines. * Communication from outside the facility - Family members of residents are restricted from communicating with the Executive Director or other staff to obtain answers regarding the care of their loved ones. The main phone line at the front desk often rings with no answer, or when answered, calls are purposely disconnected. * Lease - Residents were given a new lease to sign; however, when requesting a copy, they were told they would receive it soon. The leases sat on the Executive Director's desk awaiting her signature while residents were told she was "too busy" to handle it. It took four months before I finally received a copy of my lease, raising red flags about hidden violations or altered agreements. * Thermostat/Heat - The building cannot regulate heat properly, causing extreme temperature fluctuations throughout the day and night. When residents raise concerns, management's only guidance is to "open a window" if they get too hot, creating uncomfortable living conditions for seniors.
