Staffs including nurses not knowing what to do. You call for service, ask for x-ray results or report, or medication, they always say okay I will get back to you with a big smile but it will never come. You have to really insist with many requests and keep on asking, maybe you can have an answer. The NP Tim is very nice though, and the female Indian doctor is also super nice, but she comes 3 times a week. She was very responsive with our family medical questions though via phone if not in person. read more

THIS PLACE IS THE WORSE. My 93 year old mother was basically ignored because her personal caregivers that the family hired had to do all the work. The employees didn't think they should have to do their job. The nurse assigned to my mom yelled and demeaned my mother's caregiver treating her like she was stupid. this nurse was so bad the caregiver was told by other staff members that she had been reprimanded over 8 times for horrible work. She even tried to cram mom's medicine down her throat while she was resting. During mom's 4 days there she was rushed back to the hospital twice. after the 2nd trip we had to put my mother in hospice and she passed away two weeks ago. IF YOU HAVE ANOTHER CHOICE DO NOT PUT YOUR LOVED ONE IN THIS PLACE. read more

Do not send friends or family here! Terrible place, hard to say a nice thing. Sent my mother here and came across a lot of issues with the staff and just overall horrific treatment. First night there, she was left in soiled linens for hours. This was a recurring theme. Staff disappeared at lunch and during overnights for hours at a time. Memory patients were left in the TV room to yell at passersby all night. Call times were often multiple hours, and the halls were filled with yelling patients begging for help. Baths/showers were roughly 1-2 times per week. She got physical therapy, perhaps 2-3 days per week, which should have been 5+. The idiot doctor there took her off meds without informing the family, and she predictably had complications. Ended up back at the hospital, which, all things considered, was a relief. The social worker here is terrible and capable of no more than handing you paperwork. Discharge was a mess. Multiple times, they tried to get a signature to "change" her insurance - it seemed very shady trying to get to personal billing and off insurance. The unqualified nurses did not know how to work basic equipment, a situation that required a trip to the ER to re-install a valve. In one instance, a mistake ended up with a urine-covered floor. The food is terrible to the point of being inedible; the food was always delivered cold. You fill out a menu, but they just give you whatever. Tried to use Uber Eats, but deliveries got "lost" at reception. Called the cursing director, never got called back. Don't read the positive reviews, must be friends/staff or very old from a better time. RUN FAR AWAY. read more

The photos are lovely but it is a nursing home. They are only as good as the staff and the management. I heard that the staff was over worked but it didn't seem that way to me. My sister was there on hospice for her last 10 days. We had a complete lapse of care for two of those days with her catheter. Also we struggled to get staff to administer morphine when we observed agitation that looked like pain to us. The staff had not observed it and questioned our observations. At the end the staff could not have been more caring. We were fortunate that we only had her there for 10 days. The family needs to provide private duty nursing to make sure that care is provided. read more

If you have an elderly parent in need of rehabilitation due to surgery or a fall, run, don't walk away from this place! They are simply collecting Medicare money while their patients lie in bed to rot. My father was recovering from a fall and a hospital stay. Instead of improving, he got worse because nobody got him out of bed and had him walk. He lost so much muscle strength that he is currently in a wheelchair, worse than he was when he went in to the Concierge. There was never a plan of care shared with him, no goals for recovery, extremely poor communication. He had several nurses who were ok, and a couple who were really good, but that experience was ruined by the administration here. They messed up records and couldn't get things right with Medicare. When he was released, there were no discharge instructions. While he was there, he rarely got a hot meal. Nobody ever took him out of his room except for OT and PT. It was so depressing. He is struggling to recover from a fall that had no permanent damage because of the lack of care at this place. As if that wasn't enough, his wife took a fall too. She suffered from dementia and he was her caregiver. When she fell, she was sent to rehab to recover and they were able to share a room. Her care was worse than his and she declined so much while she was there that 2 weeks after her discharge, she has passed away. So as I said before, if you're considering a facility for rehabilitation, RUN away from this place. Your loved one won't be the same if they stay here. I'm only rating one star because of the couple of nurses who did provide good care. I don't understand why they stay here. read more

This is a Skilled Nursing Facility (or SNF as they're called) and from what I know, they focus more on rehab following a hospital stay now and not long-term care like what one might think of when they hear about a Nursing Home. I mention this because if a loved one was in the hospital for something other than a routine/elective procedure and after a lengthy hospital stay and you are told that the doctor is recommending transfer to a Nursing Facility instead of discharging them home, it creates a great bit of anxiety for both you and your loved one because "Nursing Home" is a scary, perhaps the scariest word for an older loved one but we were assured that this was indeed temporary and in the best interest of our loved one and in retrospect, it was the right decision. Their ultimate goal is to rehab your loved one and get them ready and strong enough to be discharged at home and have things ready for them at home (Oxygen, equipment, etc.) when they are. To be very honest, it won't be easy for you and your loved one especially with all the COVID-19 protocols that limit visitation - by limit I mean there are no visitations unless you go outside on the window or use Facetime but you can register to be an "Essential Caregiver" by taking a COVID-19 test, making an appointment to get trained for it, and then making appointments to see your loved one. You are however able to drop off things for your loved one. There will also be times that you will be very frustrated because communication could use some improvement. The problem is that the nursing staff take instructions from the hospital doctor (at least in our case) but don't really know what the doctor's longterm plans are. Your point of contact is the social worker who gets her information from the Director of Nursing who again, may or may not know what the doctor's goal and timeline is. If you call the doctor's office, they might refer you back to the Social Worker and so as time goes by, your level of anxiety keeps building up. At the end of the day though, I can say without hesitation that every single person that myself and my loved one came in contact with there was a dedicated and caring professional who had my loved one's best interest in mind. The rooms were nice, bright, and clean and resembled a nice hotel room. The food was pretty good from what I was told as well. When the time came to be discharged, they helped us set up the medical equipment and other things we needed. read more

The Nursing/Care staff amazing but the resources they are provided and staffing levels they have are below expectations for a caring / rehab facility. The meals provided doesnt match up to what was on the menu. My mom while she was there has 2 cell phone "disappear" Please make sure you check the bill weekly......my mother was going to rehab and on the day of her birthday, I was there all day, the said she went to rehab 2x! Inside, the people running this establishment should all be fired! We placed my mother there, for therapy, after she broke her ankle. The physical therapists there are wonderful, however, the nursing staff are rude, disrespectful, horrible caretakers who have no business being employed anywhere! Freddy, the man in charge is of no help and is incapable of running any business. We told him of how my mother was treated, not properly cared for. He was and is completely incompetent. This place is the pit of hell. I wouldn't recommend sending a cat there and I hate cats. First, they do not know how to read charts. They decided to change his medications without consultation or explanation for no reason at all. Every hour, "change your diaper"? He doesn't wear a diaper, look at the chart. I had to tell the nurse to empty the urinal after she walked back and forth past it several times. Maybe if she was not so concerned about the diaper she might have noticed. He's a diabetic who had a toe amputated. What do they feed end stage renal failure patient, carbs, and sugar. They took him to dialysis and when they brought him back, they put him in room that was under construction with no air conditioning. He was in there for hours. When they finally realized their error, they manhandled him into a wheelchair and then dumped him back in his original room. Finally, they wanted us to pay $114/day in addition to the insurance for the pleasure of treating my husband like a sack of potatoes. He was there for a day, we had him moved to Encompass where he got the help he needed. The need to shut this place down because their only concern is money and how much they can get out of you. If you have family members in nursing homes, snf's, or rehab please check on them on a regular. You have no idea what they are going through if you don't. Trust me, when I'm done the world will know about this. read more

The Nursing Staff in the Facility is TOP NOTCH as they go out of their way to make sure our Dad was cared for and THEY contacted us if something was up or there was a need to go to Hospital. We do believe they are Under Staffed as is so many places during this time. They person who greets us every time we enter the building is always sweet and has a name tag at the ready for us. The Folks in Billing are Top Notch and do what they can to help. However, I can not say The Administrative staff (particularly the Caseworker) are very helpful. WE had to continually Visit with the Caseworker and repeatedly ask for UPDATES (Which we never received). The Communication FAILED on an ongoing basis. WE were ALWAYS the ones initiating contact. We are VERY disappointed with the Administration side of things. Would I recommend you put your loved one in this facility - YES - only because of the GREAT NURSING and PT/OT/ST people are AMAZING! They lie to you and when one of them will actually talk to you they are so disrespectful. Management makes a big deal of meeting with the family and patient at the beginning of their stay but then you will never see them unless you are able to hunt them down. This is a beautiful building you had a horrible experience. 3 of the 6 wings are closed.They have an ice cream shop, a tea room, a coffee shop, and a movie theater. All of them are closed. The place is under staff with very few people that speak English. My wife was left in dirty diapers for 6 hours because the staff was too busy talking and playing on their phone to answer. read more

When my mother's nursing home closed in late January 2020, i visited several places. The Concierge was so pristine, caring, and beautiful that I fell in love right away. The nurses, especially Toyin, have taken great care of my mom and her needs. They call me whenever there is a question or issue, and keep me in the loop with her care. During COVID-19, we receive weekly calls to notify us on what is happening within the facility. They make every effort to make us feel connected including using FaceTime so we can see them even when we can't be there. I am so grateful for the Concierge. I hope everyone can receive such special care as they age. read more

The Concierge is wonderful. It's nice that the building is beautiful and clean, but it would mean nothing without the great staff. Jeffrey, Aundre and Laurel are wonderful. One of them would tell my family, look what miracles happened today! Nurses at the concierge are so lazy and neglectful. They don't care about the patient. They will say that they give the medication to the patient even if they did not give it. You press the call button and nobody will come. You ask for pain medication and it will take hours before they will come. When you go ask for help, they will roll their eyes and take time . Please do not take your love ones on that facility unless you do not care about them. The facility is nice but the the service is really bad. read more

Zero stars!!!! The admissions person, Jeanette is unprofessional, inconsiderate and should not be in that position. My 90 year-old Father with dementia was being displaced in his current nursing home so it was an urgent timeline to find a new home for him and I applied to the Concierge. I was told by Jeanette there were seven beds available and send over his records asap. I did so first thing in the morning the next day and never heard back. I phoned three times in one week and left messages along with his current nursing home personnel trying to assist us. Finally I called the Social Worker of the facility the following week since the deadline for him to move had passed and she went to check the status for me. Jeanette called me and said my Father would not be admitted. I asked why since there were seven beds available when I applied and was told they were only admitting patients with Medicaid if they went through rehab admissions. That was not true because I knew a patient from my Father's nursing home that was admitted and did not do so through the rehab route. I asked why we were not told or phone calls returned and she said she told someone at the current nursing home. I spoke to that person and everyone else in the facility and no one spoke to her or received any messages from her. She never answered why I was not notified or my calls returned. She wasted a whole week of time leaving us hanging when I could have been looking for another facility for him. She didn't apologize or say one consoling thing. Totally unprofessional and not trustworthy at all. You don't treat this segment of society like it's just boarding a pet. In fact, the people that keep my pet are 100 times more compassionate and caring than this girl. Pathetic! read more

This place is a disaster! My mom stayed here while she had a pic line for antibiotics. At the time, they were having a huge change over and hired a bunch of people from a poorly rated nursing home in the area. They were even hiring right off the street. Mom developed aspration pneumonia there too. The nurse practitioner told me that it really wasn't a painful death. "It is how people died back in the day." I quickly snapped on her and told her my mom is not a damn dog! Treat her!! I had medical power of attorney. This is so sick and disgusting how low they have become. Trying to work with their administrative staff (from long distance via phone) was a nightmare. They were of very little help. Most importantly, the level of care was extremely below par. DO NOT SEND A LOVED ONE to this facility. If there was a lower rating that 1 I would have chosen that. Please don't bring family here ! We had a dear family member here almost three weeks because they were SUPPOSED to get him strong enough to use his legs . He never once was brought to the rehab center. They did some PT in his room but not enough to make a difference. The doctor was good , saw him once a week I think. He was told that his recovery speed was up to him , but when he asked for better PT, he did not get it. I think they try to stall so you can stay longer and pay to just lay there in bed. They never let him use the restroom, had to sit and wait for hours for them to come clean him up. He was able to get up with assistance, but they chose not to get him up. So , he got bed scores and wound care had to come . The nurses and aids ignored him when he talked , just walked out of the room. They argued in the hallways . We were not kept informed of how he was doing , could not get hold of people there. Fortunately, he had his phone and finally said get me out of here. The food was tolerable, sometimes it was leftovers. All in all, not the kind of care and REHAB you would expect from such a self touted place. Pretty enough but that is where it ends. It's really awefull to waste someone money and life stuck in a bed when they came for help. Extremely disappointed and will never use them again. They should be investigated and fined in my opinion. read more

Adequate. Just Adequate. Understaffed - they want to do a good job but there are not enough people - we waited more than 30 minutes for them (more than once) to answer the red button for assistance. We were told that if they were giving a bath somewhere else, it may take awhile and they will be there when they could. AND despite the fact that my mother is diabetic, they push the heck out of starches in the meals and in the afternoon brought her full sugar ice cream (2 scoops) without her asking for it. Their rehab staff is good and my mother is improving physically in that respect. Their afternoon program, however, is to stick the people in beds to take naps or in wheelchairs and left alone. Their advertising material in the room at check in states rehab seven days a week but that is not correct - no rehab on Saturdays or Sundays. Sure I've been told that all Skilled Nursing Facilities and rehab places are this way and it's a good thing I can be there often, but honestly, adequate is the best I can say about them. read more

My dad went here for two and a half weeks until we pulled him out.He was placed there to get physical therapy. Because of COVID, we couldn't visit, so we had to rely on what we could find online.The place is beautiful, but that hides the reality of the extremely poor care he got there.When we pulled him out, he was much weaker then when he went in.He had 3 PRESSURE SORES.They never gave him a shower (no hot water!!!!!). They just wiped him off every few days with wipes and called it a sponge bath. He never went to the physical therapy room. He was supposed to be getting stronger at walking, but they just told him to roll around in his wheelchair!!! They were very rough when they handled him, so much that they would peel his skin off when they would be grabbing him.He came home filthy, with bandaids all over. When he would ask for something, they wouldn't say a word to even acknowledge him, they just turned and walked out and never helped him with what he needed. Please don't place your loved one in this horrible place!!! read more
