There is open line of communication; types of care covered are too numerous and personal to list here, but can run the gamut from: personal care, light housekeeping, meal prep assistance, mobility challenges, making sure that the floor has no obstructions when client is walking, general CNA (Certified Nursing Aide, like I used to do many moons ago), and HHA (Home Health Aide) designations, as well as contact with nursing and other staff. The facility is beautiful, welcoming, and the staff I met seemed caring, professional, and genuinely invested in residents. I left feeling relieved and hopeful during an incredibly stressful time. Unfortunately, everything fell apart after I paid the deposit. Communication became nearly nonexistent. Days would go by without updates, and the assessment I was told would take place never happened. Instead, the assessor, Bernice, apparently contacted my mother's rehabilitation facility and asked the social worker for her cognitive score. According to the social worker, when she provided the information, Bernice's response was simply that my mother "seemed like a better fit for Memory Care." What makes this especially frustrating is that the rehab social worker specifically stated that my mother was relatively independent, was not a flight risk, did not require personal care assistance, and primarily needed medication management. No one from Vitality ever called me to discuss concerns, ask questions, or explain, or even tell me their decision! In fact, I learned that my mother had been declined only when the rehab social worker called to provide my mother's discharge date and asked where she should be discharged. When I explained that I was waiting for Vitality Hudson to complete its assessment, she informed me that Vitality had already declined my mother. Imagine the panic and anxiety of learning, from a third party, that I needed to start the entire assisted living search over again--from 1,500 miles away--with only one week's notice before my mother's discharge. I immediately left a voicemail for Kyle, the sales representative I had been working with. Throughout the process, he seemed kind, responsive, and competent. However, four days have passed since that message, and I still have not heard from anyone at Vitality. What saddens me most is not that Vitality determined they were not the right fit for my mother. That is their decision to make. What is disappointing is the complete lack of communication, follow-through, and basic courtesy during an already difficult family situation. If someone as persistent and organized as I am can fall through the cracks, I worry about what might happen to a vulnerable loved one in their care. Sometimes blessings come in unexpected ways.

We moved our mother to the facility (then, Windsor Woods) in July of 2020 and she remained for 4.5 years. I visited frequently, usually for several hours each visit. I highly valued the facility, itself, and actually witnessed a compete building remodel of the common areas in the summer of 2023. The daily activities, food, and services offered were beyond expectation. The staff, of course, is the key to any successful business, particularly elderly care facilities. Over the past 4.5 years, I met and truly cherished a plethora of amazing people, on staff. Med Tech Annie Lambert was beloved by Mom and truly went above-and-beyond her job description and duties in caring for her. She is priceless and will be sorely missed. Director of Wellness Bernice S. was the consummate professional that was always thorough, thoughtful, responsive, understanding and reliable. Bernice is the cornerstone of the community. Wellness Supervisor Lisa L. was so appreciated for her thoughtfulness, attention-to-detail, responsiveness, and could be relied upon to answer any and all questions. Both Bernice and Lisa maintained continual communication with me regarding Mom's health, medication, general well-being, and more. We are both satisfied with her and her staff, as they are caring. They also have a website where you can review your charts, stats, make an appointment, etc. The only downside of the practice is she stays overbooked and your have to wait up to an hour past your appointment time. This is fine with us, but for some who have to get back to work, they may have a problem. I have seen her come out and take someone who is limited on time back to the exam rooms. All in all, I like her and her staff, and the care we are given exceeds ones we have had in the past.

She may occasionally be running behind but I know it is because she is listening and taking time not to overlook anything with her other patients. I don't feel like a dollar sign to her, she makes me feel welcomed and I believe she truly cares. I trust her so much I recommended her to my oldest son to her who is also in health care and he shares my sediment that she is a very good doctor. She is very thorough and I have complete trust in her. I do have to reschedule my appointments more than I like due to work conflicts and her staff is always able to help with my scheduling challenges.

Hospice Diamond team wants to put my mom in danger she is a high risk of falling they want to…read moreremoved the 4 rails off of my mom hospital bed and I was told that you can not removed the 4 rails while the patient is in the bed. Hospice Diamond in Brooksville is telling me that these 4 rails on her bed is illegal. I want to keep my mom safe on her first night home from the hospital hospice diamond order two matress and 4 rails the delievery came over and put it together my mom came home from the hospital and put her on the bed the next morning she was on the floor. She had emergency surgery she fell and broke her tail bone the fire rescue came over and removed the matress and kept one in the bed they put my mom back on the bed and told me to keep the 4 rails up for her saftey I told hospice diamond about that they did not care they want to removed the 4 rails and put my mom in danger This is abuse of a patient. DO NOT USE THEM THEY ARE HORRIBLE.

I absolutely would recommend Healthsouth Rehabilitation Hospital of Spring Hill ("Healthsouth")…read morewithout reservation. My 87 year old mother was at Oak Hill Hospital and needed inpatient rehabilitation services (physical and occupational) before returning home. I researched not only Healthsouth but the other rehabilitation facilities in the Spring Hill area. In making the decision I would give this advice to anyone else needing to make a similar decision: * Know the difference between a Rehabilitation Hospital (e.g. Healthsouth) and a Skilled Nursing Facility (Spring Hill Health and Rehabilitation). The care provided at these two different kinds of facilities is VERY different. Healthsouth's sole focus is on rehabilitation, generally 3 hours per day for 5 days each week. No one stays and lives at their facility long term. This is not the case of a Skilled Nursing Facility (basically a nursing home that also provides rehabilitation services at a much lower level). * Know that if the patient is covered under Medicare they have the right to choose the rehabilitation facility they want to go to--assuming, in the case of Medicare Advantage plans, that the facility is covered by the patient's insurance. * Go visit the facilities you are considering. One visit and I was convinced Healthsouth was the place for my Mom to receive the best care. A bright, upbeat, cheery environment with patients and staff engaged and enjoying their therapy sessions. The size of the gym speaks for itself...so many different exercise machines, equipment, tools, games, aids, etc. to stimulate and keep the patients' engaged and interested in their own therapy. * Know that Oak Hill Hospital's physicians, employed by Access Health, will try every way possible to direct you to a facility other than Healthsouth, specifically to a facility serviced also by Access Health physicians. Don't fall for the rationale and excuses they will give you as to why your family member should not be going to Healthsouth. Be strong. Be your loved one's advocate to ensure they go to the proper facility that will give them the best care. After two weeks my Mom is going home tomorrow. She is so pleased and happy with the care, professionalism, respect, empathy, and focused attention she has received while a patient at Healthsouth.
