    Major continuity of care problem, please call your insurance if you are having problems with Active…read moreMarin PT switching out insurance rapidly at the end of 2025 or billing you a ton for appointments you booked before 12/28/25. I learned this behavior is not allowed and violates both patient notice requirements as well as Continuity of Care laws and No Surprise laws in the state of California. Forcing a post-surgical patient in severe pain to make a rushed financial decision at the front desk is considered administrative coercion and poor medical ethics. I had foot surgery in mid November and was referred for post-surgical PT for hip and low back at my 12/22/25 follow up with my surgeon, due to issues that developed from an uneven gait in the walking boot. I contacted my insurance for referrals and was referred to Active Marin PT as well as a few others that were IN NETWORK. I was in pain and not sleeping well, so when I called Active Marin PT on 12/23/25, and was booked in for eight appointments beginning 12/29/25, I was very relieved. In my booking call, I provided my insurance information and Active Marin PT confirmed they were IN NETWORK. I showed up, in pain, on 12/29/25 and was told verbally that Active Marin PT had ended their contract with my insurance the day before, 12/28/25, but Active Marin PT would file something called a "gap form" on my behalf to bring their services back IN NETWORK. I was very confused as I had been booked for eight (8) appointments before they ended their contract on 12/28/25, and because PT was part of my post-surgical care. And I had made sure with Active Marin PT that I was IN NETWORK. I was induced to continue with care because Active Marin PT stated a plan (the "gap form") to deal with their contractual changes so my services would be covered In Network. Importantly, no one from Active Marin PT contacted me BEFORE of my appointment to explain they had cancelled their office's contract with my insurance (Blue Shield of CA, not a small provider) and obtain my consent to different charges or give me alternative options. I was instructed to keep my three appointments that week and wait out the gap form, and frankly I was in terrible pain that was making it hard to walk, and needed help at that point. I have since learned that the medical provider must give written notice to all patients when a master insurance plan is changed. I also learned that the Continuity of Care doctrine means my care continues for a period of time at In Network rates due to the booking date of 12/23/25, and I learned about the Post Surgical Care laws. Both laws disallow Balance Billing, or upcharging the patient to enrich the medical practice above what insurance allows. I followed up daily on the "gap form" both with my insurance and with Active Marin PT; after the first week (3 appointments) I became concerned about the lack of progress and I cancelled the rest of my appointments. The reception did not seem to have a handle on when or how the gap form might be resolved. I also asked how billing was going to be handled and no one seemed to know. I moved on to an In Network PT who is great, but I had to wait a while to get in for care. Active Marin's behavior cost me additional time in pain. Flash forward - not only did Active Marin PT NOT bill for the three visits at In Network rates ($15 per visit for me, total $45), they Balance Billed a total of $590 for three appointments. Before I knew the laws, I offered to negotiate the bill. Per my insurance, these appointments would have cost me $15 per, or $45 total. I offered $250 (5X what my bill would have been to try and be fair) in resolution. While I understand business contracts can change, Active Marin PT owed me full and complete disclosure of this change BEFORE booking me in for 8 appointments, taking me on an emotional roller coaster while in pain, confusing me with insurance jargon, and forcing me to start my care over, all while Balance Billing and violating California Continuity of Care Laws and California Post-Surgical Care Laws. Active Marin PT refused to even discuss the problem or work out a solution with me, instead inserting their billing coordinator into a quasi-lawyer position to go back and forth with me. It is important for the community to take extra steps with this business and triple check insurance contracts have not changed before each appointment or risk expensive and fake Balance Bills and a dismissive and rude attitude. If this happened to you, please file a complaint with your insurance company so you do not pay false and expensive Balance Bills like I did. I am out $590 and a lot of headache due to this bad behavior.

I have not gotten physical therapy until now due to piss-poor PT practitioners I have encountered in the past. One ortho surgeon insisted surgery was my only option and a far more credible ortho surgeon kept steering me to physical therapy since recovery from shoulder surgery is nearly a year long and very painful. I brought my daughter to Mill Valley Physical Therapy for knee rehab after her meniscus surgery. The PT pro was doing excellent work for my daughter so I thought, 'I'm already here; maybe I can try PT again while my daughter is working on her recovery.' I made an appointment to coincide with my daughter's next appointment. Fingers crossed! At my first appointment, my assigned PT Pro began by going over the MRI report, which she'd already read, and explained it to me so this non-medical professional (me) could understand which muscles are torn, which are in distress, where bone spurs are rubbing and degrading ligaments in my shoulder and, then!, explained all the secondary effects my body was likely feeling as a consequence of the main problems evident on the MRI. In short, she put in the effort before my appointment to be thoroughly prepared to begin fully informed treatment of my shoulders as soon as we met. My PT pro proceeded to work on my shoulders to ease the years-long pain I've endured. By my second appointment, I was already feeling relief in my shoulders and she began treatment to the secondary pain spots. Suddenly, I was feeling better, I was taking fewer muscle relaxers to sleep, and I was sleeping through the night without pain. I had not slept through the night without pain in years! I still have work to do to achieve my goals for my shoulders but I am highly confident, nay, extremely confident, my PT Pro will steadily guide me to heal and strengthen my body to the best of my capabilities. Also, the front office staff are super friendly and accommodating to get my appointments and my daughter's appointments to happen concurrently. They also regularly update my orthopedist on my progress. The other PT Pros not treating me or my daughter all know both my daughter and me by name and greet us enthusiastically and actively check in to see if we feel we are improving and asking if we need anything else for our comfort and rehabilitation. Sorry, this is a super long review but there are so many inadequate PT practitioners out there, I felt duty-bound to share with my Yelp Comminity the rare, professional, and highly competent group of professionals at Mill Valley Physical Therapy who are worthy of your trust.

I was a little skeptical at first because the name sounded a bit funky, but it's actually Ewa's initials (Ewa Zaborosky) - and turns out to be a great predictor, in a way, as well as a great marketing device. This was indeed "easy" rehab - as these things go. Ewa has a wealth of knowledge of different sources of physical (and balance) problems and solutions, and a kind and gentle way about her that has been hugely helpful, unlike some of the "drill sargeant" types I've seen before. She has a very lighthearted and playful sense of humor that always brightened the day. Most importantly, she actually got me restrengthened without aggravating other problems, which is really saying something - and helped me tremendously with the balance issues. The exercise portion of her work is primarily Pilates-focused, which is a marvelous system for both athletes and regular people of all ages, but she also brings in many other techniques. This is not "sports medicine" PT that looks like typical commercial gyms, and is thus much more helpful for those with issues like arthritis, along with strokes and various other nonorthopedic problems, but even injured athletes would benefit tremendously, since that system was initially developed for elite dancers. For those times when the kinks just won't work themselves out, and early in the course of therapy before physical exercise is appropriate, Ewa is exceptionally skilled in hands-on techniques, and has a marvelous articulating table that really helps keep the pressure off the bad parts and keep everything at angles that work best. She has truly got magic hands, and a range of other soft tissue modalities that together seem to just smooth away pain and stiffness. She holds a PhD in physical therapy (I hadn't even known such a thing existed!), and is always going to different continuing education classes, so you are always assured that she is up to date on the latest. Sue is also a very skilled therapist who was equally helpful in other ways, and Iona, the receptionist, and Jeff, the fitness trainer, round out the regular crew, all of whom are always just terrific and as nice as can be. They also offer private Pilates lessons and a monthly membership that allows ongoing use of the facilities once discharged from formal PT, which is quite a bargain considering how few local gyms even allow access to the Pilates equipment at all without signing up for expensive classes, which, once you know what you're doing, are just not necessary all the time.

There are flyers with prices as soon as you walk in so there are no surprises. By the way, the prices can't be beat! I was welcomed at the front door and was asked to call a number to secure an appointment. I suggest calling the number if you have questions or need to communicate accommodations since the person on-site was not fluent in English. You can book massages, Accupuncture, and cupping. I ended up booking a therapeutic massage for 90 minutes at $160. This is a great price considering local places can charge upwards of $300. I knew this was going to be a simple and no frills experience. There is no sauna, relaxation room, or fancy spa additions. The credit card machine was broken so cash was requested. I booked by appointment using the phone number, hit the atm, and then made it back for my appointment. There are three tables sectioned off by walls and a sliding door. There was no one in the space besides the two of us. If there were others, you would probably hear them. The towel laid out on the table was very old. That is my only complaint. The massage was great and I had every knot worked out during this session. This masseuse worked diligently to get into my muscles. If you need a deep tissue massage, this is the spot. The time flew by quickly. Again, this is a simple place to get a massage and be on your way. I'll be back again! Pro tip: I suggest bringing headphones if you want to listen to music. Otherwise, you'll hear the restaurant noise next door.
