5 stars for my doctor, the staff and receptionists, and the facility/logistics itself. It is truly rare that a doctor's office (including staff) are all sublime and provide such a great experience! Let me go through each of the following: +Dr. Shah: Dr. Shah is really cool; he introduced himself to me as Dr. Jay, and we are from the same home town, age, and have a similar life background in terms of moving around from the Bay to LA! It was great having that rapport since it was my first time meeting him. But most of all, the best thing was that he ordered multiple tests to help me get to the bottom of my abdominal issues that I have been struggling with for more than half my life! After going to the ER a few times and finding nothing major in my CT scan, I was worried he would dismiss my stomach issues, BUT he did the opposite thing and helped me with diagnosing multiple potential problems. I REALLY APPRECIATE IT!! I feel like I can breathe a sigh of relief and finally get to the root cause of all my abdominal issues! This has not been the case with previous providers...(hence why I switched). THANKS, DR. SHAH! Oh also, Dr. Shah is funny; he made a funny joke on his business card lol. He also was able to take a look at my ankle as I know his expertise is also in sports medicine! He gave me detailed information on stretches to do for my ankle and also some resistance bands. WOW! THANK YOU! Going above and beyond - totally didn't expect that! He also did not rush me and answered all my questions. +Staff/Receptionists: I truly believe it is rare that if you leave a voicemail/message for the receptionists at a doctor's office, that you would receive a call back. Not only do I receive a call back each time, IT IS WITHIN THE SAME DAY! That is WONDERFUL! Keep up the amazing job!! It means so much to be called back; I almost couldn't believe it when I picked up the phone. The young lady who met with me before Dr. Shah was also very friendly and made small talk with me. +Facility & Appointment Logistics: The facility is nice; I think it's probably just a few years old. I experienced no wait (I was even seen EARLY). WOW. Everything was prompt, efficient, and thoroughly explained! I also appreciated how I received discharge papers with such details documented and on my labwork, there were even specific instructions on what I should avoid taking before the bloodwork. I was also told in detail about fasting. My previous provider did not do that! Overall, it has been a wonderful experience so far with this provider and clinic! I am so happy because it is very close to where I live too. Feels like a dream come true to finally have a great doctor and experience with a physician's office! I'm only giving the 3 stars due to the front staff personnel. I've been in healthcare for years and when the attitude is given by the front desk, it just ruins the vibe for the whole visit. It's my first time here, I try to get to the location and due to some insurance mishaps my appt location and time had been changed a few times. I arrived to the Crossroads location and quickly found out I arrived to the wrong one. Rushed over and even CALLED the front desk to let them know I was arriving late as a courtesy. As I walk in the front girl screening the appt just started repeating over and over how I was late and the tone was just annoyed right off the bat. Even another lady trying to come in was given attitude so quick too, but watched her as she picked and chose who to be nice to when walking in and out. ON TOP OF HAVING A WHOLE CONVERSATION ON HER CELLPHONE IN THE LOBBY out loud in front of us. Luckily they took me. Accepted my insurance and payment, the girls behind the window were a little nicer at least. The back office staff , especially the girl that took me back was much more pleasant. The conversation was there and the overall vibe was better from her. Finally seen Dr Baez. Super pleasant. Still took me in for my rather short 5 min visit and I was very satisfied. My husband and I have been going to Dr. Olson's office for a few years now, they have been wonderful!!! On April 3 I broke the fifth bone on my right foot urgent care was wonderful. I ended up going to Dr Shah, he was very helpful, I had to get a dye CAT scan of my foot he called down to the hospital ahead of time, made process much faster !! My follow up appointments were great !!! he would tell me things I would remember during the healing process that made it so much smoother. Also put me onto Physical Therapy after and I just can't say enough good things about this place.!!! I would have done this review much sooner, but we were in the process of selling our home, and it went really fast !! We will miss this medical group as they have been so good to us !!! No one ever really enjoys a trip to the doctors I'm very happy I chose PVHC. Since switching to PVHC network I have never left dissatisfied. What I really like the most is the fact that I leave with a summary report every time I visit. I also like that if you use PVHC network your labs/reports/visit summaries can be accessed by other doctors that you visit so that they may be aware of your health history. I'm very happy with the service I receive here. Dr Shaw does everything in his power to help me feel comfortable while also ensuring that I understand what information he's giving me. One thing that bothers me is that female doctors here tend to be booked way out in advance for almost 2 months sometimes. This is still a decent place to go to and way better than my old clinic. The male doctors here have a much more open schedule and I'm usually able to book an appointment with them pretty fast. read more

I went in today for a ingrown toenail removal for both my toes and my experience was great! Dr. Shah did an amazing job, it did not hurt one bit other than just feel pressure and clean cuts!!. He walked through each step and checked up on me often to see how I was doing. He has a great sense of humor which helped me keep calm. Dr. Shah specializes in sports Medicine and I HIGHLY recommend him. He was able to see me right away alongside Dr. Baez. My primary doctor is Dr. Olsen but he was already very busy and I needed this procedure done right away and Dr. Shah was available and I don't regret it one bit. Also the staff is awesome! Every time I schedule an appointment I arrive 15 minutes early to give sufficient time to be seen. Every time I arrive I wait 1 to 1 1/2 hours additional time to be seen this is ridiculous I can never see a physician here it doesn't matter the time or the day I come. 4 people arrived after me and were all seen before me. Went in to see Dr. Sha Thursday, thank God he had an opening, I explained to the nurses how much pain I was in and what had happened, and with no delay she let me know they could take me in right away. When I went in, Dr. Sha, started trying to examine my shoulder (has no mobility what's so ever) and he was so cautious while doing it, making sure he wasn't causing me anymore pain than I already had. Explained my X-rays very thoroughly, taking his time, explaining exactly what was going on with my shoulder. Immediately took me out of work, understanding they were giving me a hard time, not caring about what I was going through, saying "that is absolutely unacceptable, being in as much pain as you are and not being able to move your arm" (beyond grateful for that, kinda hard to put a bra on with just one arm, Praises to those that can!) gave me a steroid shot to stabilize the pain (pain was BAD) and gave me a referral for PT right away (got that scheduled too) it's Saturday morning now, and I've been actually able to sleep a little more every night since seeing Dr.Sha (was literally only sleeping two hours a night off and on since the Injury to my shoulder, about 4 weeks ago) beyond grateful to Dr. Sha and his team (everyone there is always so nice and helpful!) especially his nurse! Super cool, very nice and extra helpful! Glad I was able to be seen, and now glad he's my Specialist!! I am giving two stars due to the admins and front desk personnel, particularly regarding their poor tone and attitude when speaking with me on the phone. My primary physician, Doctor Erik Olsen, is fantastic. I am starting graduate school at a University, and the school is asking for my medical history and immunization record. Being that I am unfamiliar with how to obtain these things, I decided to call the office of my primary care physician (PVCH Chino Hills), hoping that they can either provide me with some information or guide me to the proper people. In speaking with them, they sounded like they could not wait to get me off of the phone. They made me feel as though I was burdening them with my questions. For example, as I was asking for clarification regarding whether I needed to call past physicians to grab my record, the personnel at PVCH interrupted me, stated that they need to transfer me to the person in charge of medical record, and then proceeded to transfer me before I could even finish asking the question. They completely cut me off and abruptly transferred me in the middle of me speaking to them. I understand that they may not be the proper person to provide information regarding medical record, but they could have AT LEAST allowed me to finish my question, THEN politely tell me that they would need to transfer me to medical record, and allow me to acknowledge the transfer before actually transferring me. I later called back to the office to confirm a follow-up appointment. They had made me an appointment during my previous call but never confirmed the time. After confirming my appointment time, I also asked them about mumps, measles, and another vaccination that I may need. Once again, before allowing me to finish my full sentence, they interrupted me and stated that I could inquire all of that at my appointment. Although that is true, at least allow me to finish my damn sentence instead of making me feel like a f**king burden. Finally, after I thanked them for their time, they simply hung up the phone without any acknowledgment back. Have some f**king patience, empathy, and phone etiquette when speaking with patients. People like myself are unfamiliar with these processes and rely on you to guide us. Interrupting us, abruptly transferring us, and then hanging up without saying a word is just sh*tty health service. read more

They were able to take care of my urgent need and provide me an appointment right away. As I arrived it was an easy process. The facility itself was clean. I would give it a zero, if possible! I was a new patient and they said I was 10 minutes late, eventhough they were screening people before entering and made me "late". Didn't know I had to take account other people's screening time. They claimed they have a "10 minute late policy and deny people if 10 minutes late" which is BS. I checked with my insurance company and there's no such thing. The office personnel and doctors make their own policies & apparently they toy with schedules, too. Dr. Olsen, who wasn't even my primary, decided I should reschedule and settle for an appointment the following month. No thanks! I will never return to this ridiculous place! My Doctor Mr. Jay Atul Shah, D.O. is one of the best Doctors if not the best Doctor that I've ever had. I've been a patient of Dr. Olsen for many years now. I've moved away from chino hills not too long ago but I decided to continue seeing Dr. Olsen as my primary care doctor, primarily because Dr. Olsen is friendly, accommodating, and compassionate with his patient. I was scheduled a 330PM appointment, which if you lived in Southern California you know how bad traffic is and on top of that I had to drive over 30 miles to see my doctor. The drive took 1hr 10mins even though on google it told me 40mins when I left the door. When I arrived I had to do Covid screening in which there was a person ahead of me. By the time it was my turn, the guy tells me I'm late by more than 10 mins and I won't be seen. I think it's ridiculous as a medical professional that you would deny service for being 10 mins late. Where is the compassion that you keep advertising? It's like there are so many variables in peoples lives these days including gas price that prevents people from getting to their appointments exactly on time. And yet you can't flex for more than 10 mins. And it wasn't even like the office was packed with people waiting. What if I came in with symptoms of illness that needed immediate attention. You would still deny care because I'm late? I'm switching primary care. I have a ppo, you gave me a reason to never come back. read more

Had an appt with Dr. Erik Olsen today. My appt was at 4:10, but of course coming from OC to make it on time on a Friday proved quite challenging. I called ahead of time to let them know I would be late. The young lady was nice on the phone, but did say it would be up to the doctor if he's able to take me late. I showed up at 4:30 and Erik was kind enough to see me anyway. He's a great doctor. Very kind and professional, but relatable at the same time. He wasn't judgemental when I told him how very much I loathe exercising and eating healthy. He gave great suggestions and because of him, I'm on an official exercise challenge thingy (I found it on Pinterest). It's just walking, but baby steps.... I have been trying for a month to schedule an appointment for a Rheumatologist, as I was referred by Dr. Agena of PVHC-Chino Hills Crossroads office, due to test results showing connective disorder disease. I have been put on hold, hung up on, left messages, without a call back, told that the referral was faxed to the Rheumatologist, who says they have not received it. I don't know what to do next. I have another appointment with Dr. Agena on 9/20 to read my bone scan, however, it took me so long to get that referral appointment scheduled, that I won't get it done until 11/4/19. I plan to keep my appointment to talk to Dr. Agena again to try and get my test results faxed to the Rhematologist. They tell me they have a new computer system, as of May this year, but this does not explain their lack of training, consideration for patient care, or even common courtesy. read more

During my pregnancy, I used to visit the women center for gestational diabetic to adjust my high bold sugar. This Center is amazing me with their organizations. I like all staff as they cooperate together and they have nice deals with the patients. For example, my nutritionist always explain to me about my blood sugar readings and she gives me a lot of solution to solve the elevated blood sugar. After several trips to the ER, my hospital made an appointment for me at this facility (suite 115) for a follow-up ultra sound. The woman who assisted me (older hispanic woman, curly hair) was incredibly rude and completely ignorant to the reason for my visit. She kept saying "you can't just walk in here and schedule yourself." Those were her actual words. I was honestly baffled. The appointment was made by my hospital a week prior, and I was only instructed to go there. I never just "walked in" demanding an appointment. I informed that I myself have never called this facility, that my hospital scheduled an immediate ultra sound due to probable ectopic pregnancy (which is extremely painful). But because I didn't have a physical order from a primary physician, she pretty much just said too bad, and AGAIN stated that I can't "just walk in" there. If she took the time look at the appointment, she would have seen that this was set up through the hospital, not me. Pomona Valley never once informed that I needed an order from a dr, they said they sent everything over for my appointment. I was very confused- I just recently moved from Dallas so I do not yet have a primary physician in CA. I've only been to the ER because of this recent experience, and every dr I've called doesn't have anything available for at least 2 weeks out. Yet I'm supposed to have an ultrasound within 5 days. So the hospital scheduled for me to come to this, and I'm turned away. But it was the way I was turned away that really disgusted me. You should really have more thought into the way you speak to people visiting your office, especially when they are in life-threatening positions. Additionally, I asked her for my paperwork back and she continually stated that I had everything, without looking. When the other receptionist was back, she easily located my paperwork on the desk and handed it back to me. Very awful and traumatizing experience. You should be ashamed. read more

I was referred by another clinic for their physical therapy and for some reason they have very poor communication skills and did not notify my plan that a specific signature needs to be on the order. They will not even make the appt and look out for the order my provider is facing to them. I am very happy NOT to get care at this facility. read more

Do not recommend this place at all. Made an appointment in the office on Chino and when I showed up I was told my appointment was in the office on Grand, even though I requested it at the Chino office. I wanted my results so I was willing to drive to the other location. One of the MA's called the office to let them know I was on my way and to make sure I was going to be seen since my appointment was made in the wrong location, only to show up minutes later and be told that I was not going to be seen at all. I can't say if the doctor is a good physician or not, but she definitely denied me the opportunity to be seen and received my results. They need better communication and inform their patients where their appointments are. Will not be coming back! read more

I was DENIED medical treatment or the ability to see a doctor although I had a scheduled appointment and valid PPO insurance. The office staff lacks any kind of compassion and the insurance billing clerk is incompetent. I have SERIOUS concerns about my physical health, which I expressed, yet no action was taken to call my BCBS PPO insurance to clarify (something I did in the parking lot after and they confirmed I was covered to see the doctor my appointment was with). This office is terrible and I will never trust my well being, or that of anyone I care about, to anyone there. read more

If I could give this place zero stars I would!For this being a chino hills office you would think it would be up to par. The waiting room feels dirty and stale. It's extremely unappealing but it gets worse once you get to the counter and have to deal with the incapable, pathetic, idiotic, rude, inappropriate, worth---LESS than the gum I stepped on in an alley reception staff. Too much? Go in and see for yourself. Better yet don't and take my word for it. Completely unprofessional and uneducated. Not only do they not know how to communicate but they can't properly send a fax either.. I rarely go to the doctor so I've put up and been at this office for about 3 years now. I've finally had it at my last visit. Simple enough. All I can say is the female behind the counter was lucky that day there was a glass window separating her and I. Inadequate staff ruins great doctors.. not that's there was any there.. but Dr. Olsen and Dr. Oakley were perfectly capable and great. Sadly, in the last year, I've not had a good experience with the office. All the doctors, including those whom come and gone over the years have been nothing but caring and wonderful on follow ups. Even the office has always been so cooperative with patients (me for one) to handle paperwork, setting/rescheduling apt. or urgent appointments (scheduling within 24 hours, if not same day). I would have given them 5 stars. Because they really did have a wonderful customer service in my opinion and I was proud to have a good care center in our community. However, I wonder if the office management has changed or what in the last year? The administration is not as caring and cooperative as they used to be. They have attitude towards reschedules or urgent appointments (like when you are in pain and you know it is getting worst by day, better see a doctor for it). If they are always going to send me to urgent care with attitude, then I wonder why do I bother having a doctor (and that is in a care center where if not one dr. the other should be able to fill in).
