I am an Advocate for a patient there. Any concerns I've had have always been addressed, & when I learn about them from the patient. A bit slow with follow up and correcting/updating correct health information. A family member of the patient and I took a tour of the facility while we viewing patient's progress! The majority of time visiting and when TCB most staff were pleasant without any major issues! Between the run down facility and the rudeness of some of the staff, I'm very glad my sister is out of that place. I personally witnessed a staff member call a patient an "MF" as they exited their room, which isn't all that surprising given the quality of some of the people they hire at that place. There were some people who were absolutely lovely, though. I must say that she was very happy with them, especially the PT staff. They were in the minority, unfortunately. This place needs to be bought by Cedars Sinai or some other entity with enough money to make it nice. My sister was very unhappy with the accommodations. First read all the other negative reviews of this roach infested "rehab." Tania Pineda is the most incompetent, uncouth person in the "skilled nursing facility" business. if you care about yourself or any person you may be facilitating a stay here, DO NOT DO IT. The staff, nurse Tina, the only RN pressured me to choose a mortuary for my loved one. The director, whom is as low class as they come, is a danger to patients. They don't give patients enough space to heal. the rooms are packed like sardine cans. Patients are heard screaming and crying in pain. Complete neglect! The paint is peeling from the walls. THERE IS MOLD AND MRSA. The food is served cold and smells like wet cat food left in the sun for days. The "Interns" are rough and will injure patients if not from handing them, from neglect. No physical therapy or occupational therapy is given until you complain to report them. the bed rails are covered in feces and every inch of this place reeks of feces. life sustaining meds are not given as prescribed. I truly wonder if this death mill is paid a commission by the local mortuary? the health department needs to shut it down and this scary place has been reported by doctors ! grade : F- read more

OK, my experience here at Prime Rose has been over the top 100% I was here also last year when I had open heart surgery and could not walk and the therapy here was awesome. I mean they got me back walking again was able to go home this place also is good for anyone with medical problems That maybe their doctor don't understand or I don't think you should really I'm not a doctor but I don't think that you should have your legs cut off until you come to Prime Rose . They are excellent Therapy. I want the food you know most hospitals and places don't have real good food, just care when you want to go and pay your rent you're able to do that. I'll go home and check check your apartment check your home. On Tues. DEC. 11th I was admitted by ambulance to this care facility via Little Company Of Mary after a week of intensive care after being hit as a pedestrian. The hospital trusted their staff with my care and sent me their via ambulance. I was supposed to be admitted by 6pm, but it did not happen until close to 10. At 10 pm, my AMBULANCE arrived, they had trouble parking and because of medical waste in the street out front, they did not want to even roll their equipment through the mess on the street in front of the facility, to take me inside. (NEXT DOOR was the "Inglewood Trailer Park") ( a person came out and yelled at the AMBULANCE crew for "Parking near their entrance"..I was taken by gurney inside and deposited on a bed that HAD NOT BEEN CLEANED, just a pad thrown on the bed and a HUGE stain on a pillow. As I was wheeled in, there was medical waste, bloody gauze, an open, urinated Adult Diaper on the floor, and trash. As I was wheeled into my room, the smell was OUTSTANDINGLY BAD, urine, feces, and the bed they put me in was stained with urine, the "Pad" a washable one was put down, and I was told, "That's OK", and they offered the pillow (Photo attached, stained, with the last person in the bed). Saying "It will be ok"..i can't MOVE, I WAS STUCK. Paint coming off the walls, open holes in doors and walls, ceiling tiles and insulation falling through the ceiling, exposed wires and filthy. They have a 2 star GOVERNMENT RATING. I called "LITTLE Company Of Mary. "IMMEDIATELY, THEY Removed me, took me onto HOME CARE, and I'll have visiting nurses and PT at home. They found it APPALLING! MY medication was requested, to no avail. Their answer was "It will be 24 to 48 hours until it gets here". I was offered water, out of a pitcher that HAD NOT BEEN CLEANED, and had visible "Dirt and Museic " debri on the cup, attached to the jug. A dirty wet towel, with debris, was brought to me when I asked for a cold towel for myself, stained....i WANT MY MEDICATION IN FULL. I want this fixed. My next calls, and my attorney's, will be to "Kumquat LLC" AND YOUR PARENT COMPANY. My Mom (Lynelle Robinson) has been a patient/resident of Primrose Pos-Acute for two plus years. Her experience was one that made me feel comfortable in leaving her there for exceptional quality of care. The staff (who are too many to mention) treated my Mom with the up most care and diligently worked with rehabilitating her in hopes she would be able to walk along, feed herself and discharge in my care. The nurses for like family yet stayed professional. The custodial staff never fell to check on my Mom and always made sure her maintenance issues were resolved asap. She was compassionately know as Madam President. My Mom even though she had serious conditions never let it effect her input and activities in the meetings. She always looked forward in participating in everything that pertained to Primrose. We always talked about how satisfied she was with finally finding a safe facility that were able to address her critical needs which the pass 6 facilities were unable to handle.....due to renal kidney failure and other critical underlying diseases my Mom unfortunately passed away October 20, 2020...with all her many different diagnoses...NEVER once did she test positive for COVID-19 which was my biggest fear. Sam the administrator who always kept me abreast of the guidelines and regulations due to COVID assured me it wasn't my Mom that he was concerned about, it was the outsiders coming interacting with the patients which assured me she was in good hands. In closing I just want to say my Mom's condition was rare yet this facility out of 7 were able to meet all her needs......the pandemic however was beyond everyone's scope of tolerance but they made it work until her final hour. I respect everyone who came in contact with my mother's care and my God continue to Bless this facility Primrose Post-Acute and the staff that were like a bonus family to me. My husband was sent to this awful place a week before Christmas from Kaiser hospital at 9pm at night because he needed physical therapy after being admitted into Kaiser with an infection. He is undergoing chemo currently at Kaiser. The staff at Primrose are lazy and unconcerned about the patients. The rooms are not cleaned properly so I brought my own cleaning supplies and cleaned his area. The first night My husband asked his nurse for pain medication after waiting over an hour just for someone to respond to the call button. He was not given any pain medicine that evening because the nurse said she would be back and never came and helped him. The next day when I went to visit they told me I could only stay for 30 minutes, After going in his room I understand why the 30 minute visit rule was put in place,his portable toilet had not been dumped, the room spelled awful and his lunch was cold because it was sitting on the table and no one came to feed him even though the instructions were clearly noted to feed him. I immediately complained to the male administrator and told him I have no intentions of following the 30 minute visit rule because I would be showing up every day for lunch and dinner to feed my husband. I saw the awful reviews about this place when I was at Kaiser with my husband but the facility I wanted him transferred to was full. I only agreed because I knew it would be for seven days and I would be going daily to sit with him. It pains me to think about the patients at this AWFUL place that have no family to check on them. read more

I had a difficult time trying to find a skilled nursing facility to care for my mom so I could return to work. IHSS didn't give me the caregiver hours needed so she could continue to stay with us, Primrose was the only agency that fought for me and Mom. I explained to Brenda-Admissions, what my mothers health challenges were and our challenges as a family to care for Mom properly and although she initially was denied entry, Brenda made it happen. The staff went above and beyond to make Mom feel welcomed and comfortable. My mother isn't the easiest person to take care of but she never once complained about being at Primrose. We finally found a Wonderful Assisted Living boarding care for Mom and she was excited to move into the beautiful house which was to be her next home. As excited as she was, she told me she missed her care givers at Primrose I will forever be grateful for everything they did to assist me in getting my mom cared for. do not send your loved one here. the cna's are lazy and liars. i removed my grandmother from this horrible place. she passed 8 months later but i truly believe she would have died sooner had she stayed. she was here for 30 days. she had dementia, and they performed duties with her as though they had no clue about the illness. she could not feed herself, but they would leave the tray in from of her and come back after 15 minutes to pick it..i came after work and asked the cna how did she eat, she lied and said very good, i asked did she feed herself she lied and said yes. she can't feed herself, i made it a point to get to her at feeding time so that i would be sure she ate. it would take her 30 minutes to eat because of her chewing. the staff was loud, during late hours dropping equipment, totally disturbing the patients, and theuy kept it cold. i popped up many times at 9pm, 10pm 1130pm one time i came at midnight you should have seen the look on their faces. one of the nurses told me visiting hours are over i told her to check with the heasd nurse and policy visiting hours are 24 hours. they are rush, rush with the patients, no compassion or skill. their was one nurse and a physical therapist same ferinandad-they were very nice and caring to my grandmother. as for the others, i hope they don't receive the kind of care they provided to mygrandmother...if you do decide to go with them make sure you visit daily....they take forever changing them and they did my watch my grandmother, she almot fell out of her bed numerous times. there was an incident where i came in and my grandmother had marks on her arm like someone dug their fingernails in her. i have a pretty good idea who, one of the nurses said the day before my grandmother dug her fingernails in one of the cnas arm, not on purpose..when you try to get my grandmother out ogf bed she grabs on you because she believes she is going to fall...i believe that nurse was angry because the cna said the cnas screamed when it happened..i believe she came ibn my grandmothers room and purposely dug her nails in her arm..i will take this strong feeling to my grave. beware of primose!!!! read more
