I was just released from Pacific Acute. I have been in one other facility and am familiar with two others. This was the best. The food was the only thing that didn't measure up to the rest of the facility. The nights were quiet which was really important. Staff was adequate for the number of patients. I spent 2 weeks at Pacific to address my disability. What was an important factor to me was the size. 47 patients and only 2 to a room. The crew were wonderful and I received top notch PT and OT. I interacted with a lot of patients during Bingo. It was very refreshing. I even got to watch Monday Night football in the common area. They had many great activities. I even got a great manicure. The most pleasant surprise was the Chef. Retired military and one hell of a creative cook, especially the deserts (even though he is a 49ers fan) vs my Rams. When it was time to leave they helped me pack as I had brought way too much stuff (clothes). A special shout out to Michelle and Mark for taking such good care of me. My husband has been recuperating from two major life-threatening surgeries at Pacific Post Acute for many weeks and he has gotten amazing care! The owner, administrators, nurses, aides, P.T. and O.T. staff, cleaners, food prepares, just everyone has been incredible attentive, compassionate and kind. I see that the staff get along with each other and have made friend with each other. They are happy working there and even travel long distances to work there which tells me they are treated well too. That means a lot to me! I have tears of joy thinking of how well taken care of my husband has been! They even recognized during his stay that something was wrong and perhaps he needed urgent hospital care. They insisted he be taken to the hospital...I believe they saved his life by doing that! Once in the hospital he was rushed to a further emergency surgery. When he was discharged from the hospital Pacific Post had saved his room and bed for him and all his things for him. They welcomed him back so lovingly. The food has also been good and the P.T. wonderful! They give him his meds everyday as directed, on time and correctly. Telling him exactly what it is he is taking. I would highly recommend this facility, though physically not the newest and fanciest, to anyone in need of skilled nursing care. They didn't give me my meds when I left and they kept skipping giving me my meds when I was there. They may have worsened my condition. I don't have any knowledge of that for sure but I know that I was supposed to take a seizure medicines and they kept skipping them. When they signed me out they didn't give me any of my medicines. So I didn't take any that night or the next morning because I didn't have them. Then when I called them the next day they insisted that I had to go get them and they couldn't deliver them to me. Sending a loved one to a care facility is one of the most emotionally challenging decisions a family can face. For me, placing my father in a facility was the absolute last thing I wanted to do. Too often, care homes are viewed as places where people go to decline--not to heal--and the thought of that weighed heavily on me. When my father was discharged from UCLA Medical Center, he was in rough shape, 94 years old, not able to walk due to a bad urinary infection that he was hospitalized for. ,I was given a list of recommended facilities. I began researching each one thoroughly, reading Medicare reviews and personally visiting them. The process was emotionally exhausting and, frankly, disheartening. Many of the facilities I visited felt cold and institutional. Staff members seemed unhappy, and the environment felt more like a place to endure than to recover. Then I toured Pacific Post-Acute in Santa Monica, and everything changed. From the moment I walked in, I sensed a different energy. The staff at Pacific Post-Acute didn't just work there--they clearly cared. People were smiling. The team was engaged. I saw patients up and about, socializing and moving--unlike the scenes at other facilities where most patients remained bedridden and withdrawn. There was a palpable sense of dignity and purpose here. I had the pleasure of meeting Mark Tubalinal, who gave me the initial tour. He was not only warm and informative, but incredibly responsive to every follow-up question and request I had--of which there were many. Mark made sure I felt supported every step of the way. His empathy and professionalism made a huge difference in helping me feel confident in the decision. Now, three weeks into my father's stay, I'm beyond relieved and grateful to share that he is thriving. He's walking again, mentally sharp, and gaining strength daily. The transformation is nothing short of remarkable. I'm currently a residentst at Pacific post acute. I have fibromyalgia and woke up unable to walk, even one strep. one of the. administrators, MARK, took on all of my problems and solved them with grace and empathy. My father was in this facility for less than a week. They stole his phone and money when I called to speak to someone regarding the matter the charge nurse Alba Martinez has poor communication skills was rude and told me she couldn't help me at all. Don't trust these people with your family I reached out to the facility to try and speak to someone regarding the matter and I was dismissed. My 100 yr old mother spent 3 months in this rehab. I had selected this facility, due to its close proximity to my home and having the only private room available among several other rehab facilities. My 3 month stay was a mixed set of experiences, mostly unpleasant thanks to a rude 64 yr old supervisor, I will simply identify as Ms. S. Ms S supervises the CNA's (cerfified nursing assistants) & therapists, etc. She spends the day constantly berating all the help, patients, and visitors, every day. She needs to be reminded that she is not respected at all by verbally abusing everyone, & misusing her power. Example: As a daughter of a patient, 1) I was constantly told off to remove all stored adult diapers & linens from the closet & counters. Well, someone tell Ms S that I don't work there?!! 2) I went to the head nurse to approve my personally feeding my mother, which set Ms S off. Ms S reaction: literally forcefully dumping my mother's food tray on the table without a word. (Thanks for making us feel welcome) 3) Standing with hands on hips while changing my mother's soiled diaper & verbally expressing disgust that my mother couldn't respond quickly to her verbal requests - - until I told her off that my mom has Alzheimer's. Then Ms S does a quick psycho 180 & tries to back peddle with faked kindness. Ms S needs to be reminded that these rehab patients need help & patience, or please do everyone a favor & retire. You obviously do not like your job. Please visit as much as possible to advocate for your loved one. I had to speak up for other reasons: 1) inept pill dispensing person: She forgot my mother's Rx eye drops 3X & would show up at 2:30 pm for my mother's "morning"(?) Rx & eyedrops 2) I found my mother's call button cord wrapped around her neck when I arrived & told the nurses that my mother has Alzheimer's; keep the call button away from her! 3) I found ants in the room & 2 on my mother's pillow. Problem was solved with handyman's ant traps - -but what would have happened if I wasn't there? 4) Administrators tried to boot my mother out of this room 2 different months into a shared room, due to some bogus reason. Had to stand ground & refuse. 5) I inquired if Mom's night eyedrops were given the day before. Well, the pill dispensing person involved proceeded to tell me off next day. 6) I found my mom freezing. Room door was closed(!). Blankets fell partially off, but my mother, having Alzheimer's, couldn't help herself. I had to block door open with tray table following nights. Taped instructions on table to check on my mother every night. Problem solved. On the plus side: 1) Patient food is ample; chef makes great pastries(!) 2) Therapists were great & are kind. Would I recommend this place? No. . Unless you are retired & can physically check conditions daily & advocate. read more

Despite the fact that this facility has association with UCLA, I was not impressed with the level of care provided to my loved one. When my loved one was admitted, a staff member had the gall to say to her: "next time you leave, it will be in a hearse." Well, that staff member was right. My loved one passed away after a week in the facility. She was unable to feed herself, and I don't think the staff took the time to feed her. Also, the meals provided were inappropriate for her needs. Staff did not follow-up with us despite repeated inquiries and phone calls about the status of my loved one. read more

Our experience was very positive and my mother didn't want to leave when it was time to be discharged. No one likes to be in the difficult situation of having to go to a post acute facility, but Pacific Post Acute was there for us in a big way. read more

I made one visit and one phone call The phone call receptionist was very helpful and spoke with compassion, sounded like she knew what she was doing. My visit was unpleasant. It had a bad stench, seemed like some of the staff lacked a sense of urgency. I don't have trust here nor would I want to keep family in this facility. read more

The Facility and especially the Staff are excellent. This is the first time any close family member of mine has needed a Skilled Nursing so I was uncomfortable and unsure of what the place was going to be like for my Father, 81 yrs old. From the Nurses, Assistants, PT, OT, Speech Therapists, Social Services and everyone I forget to name, they were all so caring and truly worked hard to encourage him to take his meds regularly, go to therapy and eat good hearty healthy food. In 2 weeks he gained a needed 10 lbs and learned how to use a cane. Many thanks to everyone. This was the best choice for my family and I hope it will be for yours too. read more

I'll start by saying that I usually don't leave reviews for places, but after talking to people about Pacific, they kept saying I should do something for them so here we are! I had the unfortunate experience of having a family member hospitalized after a nearly fatal accident. This is the first time witnessing the recovery of someone so close to me and I didn't know if she was going to be able to make it back into living a normal life. After being taken care of at the hospital, she needed more therapy to get her strength back before going home. We didn't really know what to do but were recommended to look at a few nursing homes since she would have to stay there until she got her strength back. I hated touring nursing homes during this process because it always had a certain feeling and smell to each of the places we checked. I was nervous because it really felt like I was going to be leaving her somewhere that she wouldn't be happy. Then I toured Pacific Post-Acute. The staff were friendly and the building looked clean. I was greeted immediately when I walked in, and the person who gave me a tour did a good job at explaining how the building worked. We chose Pacific and I'm glad we did! She was able to get stronger and I was contacted regularly with the progress she had. She is now home and back to her normal self and I can't imagine that happening without the help of the great workers at Pacific! I do have a couple concerns that didn't really bother us but might if your thinking of bringing your family here. 1. The building is well maintained but it is pretty small. For us, this was a good thing since we felt like there was a more personal connection between us and the staff, but if you want somewhere with more space, this might not be a great option. 2. Parking is tough (Like everywhere else in Santa Monica). I can't really blame them for that since I don't really know how they can fix it, but it doesn't change the fact that you'll have to do some walking if you want to find a parking spot. Finally, I know there is a stigma with places like this, but if all nursing homes were like Pacific is, I don't think that stigma would exist. Thank you Pacific! read more

My girlfriend of 18 years was sent here from.UCLA Not a very good choice no one ever answers the phone you have to make appointments which would take two or three weeks staff was always in the patio having parties which I couldn't understand why there was no visiting when I guess everyone was having a party in the patio so they even have time for patience Rosie had a feeding tube in her stomach it was supposed to be clean twice a day it wasn't clean for over two weeks till I call the doctor from UCLA Then there was being cleaned and the dressing it was so bad it was Green brown and black, Rosie could they have gotten an infection. You went to take a shower with their brand new pajamas to put them on after she was done when did the staff came and told her they belong to someone else and took them when they were hers had her name on it because I bought them you went over to Santa Monica hospital for a week to two weeks all of her personal belongings Mississippi care or put it in the box with a lot of things missing they said well maybe they got thrown away why would you throw away a personal property of a patient no no no that's not what happened one of your staff took it which was a joke and then you try to say everything was in the box I would recommend this place to anyone and I have I have pictures to post of the filthy feeding tube read more

When it became apparent that I would need left shoulder replacement surgery, I called my P. T. at ABC Post Surgical Care. He told me he no longer worked at ABC but had switched to Pacific Acute Care. I had.successfully been through four surgeries with Tom di Palma their P.T. and refused to go anywhere where Tom was not. I cannot tell you how pleased I was with the facility, administrators, Drs, P.T. Dept. Staff, Adjunct Staff, Nurses, CNAS housekeeping and custodial staff. Pacific is a small facility. It is homey and many languages are spoken. The atmosphere is warm and the staff inviting and always helpful. Pacific is by far the best Care Facility in which I have chosen through word of mouth to recover. From Stephanie in Admissions to Rachne who will fulfill any special need you may have. On July 2nd of this year, my father, who had been diagnosed as having  Alzheimer's disease (AD), was transferred from UCLA hospital in Santa Monica to the Pacific Post-Acute Nursing Home on 17th Street in Santa Monica, a facility that advertises itself as a "five-star, skilled nursing facility that provides personalized rehabilitation and nursing services in a comfortable environment."  Naturally wishing to visit him in his new facility, I was informed that I would need to make an appointment to see my father; the first available to me was the 4th of July.  When I sought out his room on the appointed day, I discovered that he was simply not there.  Asking the staff about this anomaly, I was blithely told that he was certainly in his room.  "No, he isn't," I insisted.  Subsequently he was found to be visiting in another patient's room.  The excuse of the staff to their not knowing his location was a simple one:  "Sorry, we didn't know he could walk."   I thought, "Five-star, personalized services?" and asked if they had bothered to read his medical file before taking him in as a patient, only to be met with silence.  When I took him outside into the daylight I observed blistering all around his lips as well as under his chin.  I asked the CNA (Certified Nursing Assistant) how this had ever come to pass, and was told:  "I don't know.  I think maybe he was shaved with a dirty razor."  Of course this prompted my asking her if she didn't know who had shaved him, would she please find out for me, a query which was cut off by the response that in fact she had asked about this, but "Nobody knows who did it and you ask too many questions."  When I challenged her by asking whether in fact she was telling me that somebody could have come in from the outside and shaved my father's face without anyone knowing his identity, she once again answered with a simple "I do not know." Later, sitting in the back yard with my father, it was very disturbing to hear patients crying out for help with no one bothering to attend to their needs. The next day, the 5th of July, I went back to the Pacific Post-Acute Nursing Home to check on my father and discovered him to be sleepy and unresponsive.  I asked his CNA to provide me with a list of the medications that my father had been given since I saw him last.  Her answer was to tell me that she simply didn't know and that I'd have to ask the doctor on duty.  The supervising doctor did call me later and informed me that an error had been made, and that instead of having been given a 25mg dose of the prescribed medication, my father had been administered a 75mg dose instead.  He apologized for the mix-up and told me he'd had it corrected ... On the 6th I returned to visit my father, but when I entered his room I was shocked to find him lying half on the bed, half off, with his legs dangling over the side and entirely naked except for a wet diaper.  He was very cold and was asking for help but garnering no response from the staff.  I called out desperately for assistance and by the time the social worker, the RN, and the CNA all traipsed into the room they found me crying and trying to cover my father as best I could.  Somewhat beside myself, I asked them if this was how they were accustomed to treating their patients, and was given the now familiar, "We don't know what happened." From my point of view, as a clinical PhD psychologist, I couldn't help but find their indifference problematic, and as my rage swelled, I told them that my father was a human being, and yet he was being treated like an animal.  In fact, I conjectured, I supposed they treated their pets better than they did their patients; that said, I addressed myself to the social worker.  I noted that it was her duty to report this institutional negligence to a higher authority, while admonishing her not to be adversely influenced by the nursing home administration.  I reminded her that she had taken an oath of service, and that accordingly she was duty-bound to protect my father against the careless negligence he was obviously enduring.  She advised me that she had taken note of everything, and that she would faithfully report the facts of her findings.  However, my surmise is that she did not do so or perhaps the administration fired her before she could act--but in any case I believe a wrong has been done. I decided, given all that had happened, that my father's health and welfare were not being served at the Pacific Post-Acute Nursing Home and so, despite all the difficulties this would entail for me, the only responsible course was to take him home with me at once. read more

Zero Stars. I brought my dad here in May 2019 and it's the biggest regret of my life. He wasn't given water when he asked for it. The food was terrible and my father couldn't stomach it. Instead of them calling us to ask us to bring him food, he just didn't eat and they disregarded it. I wasn't aware until my dad told me that night. They never gave my dad his medication while he was at the facility. As it turns out without my dads medicine he fell into a diabetic coma type of sleep. The medical staff treated him like he was lazy and just sleeping. They took no measures to see if something was wrong even after my mom asked for help and explained this was a diabetic coma. The staff member said, he's just being lazy and sleeping. We're not babysitters to make sure he gets up and eat. If he doesn't eat that his problem. My mom was in complete shock. She had to call my brother for help because the staff weren't listening to my moms concerns. My father needed to be rushed back to the ER but they failed to call 911 for an ambulance. Instead it took hours for their contracted ambulance to take my dad to the ER. The lack of medicine, delay in access to ER medical treatment further worsened my father's chances of recovery. I didn't know they didn't give my dad any of his medicine until I went to pick up his things including his medicine and found his bottle of pills complete. Not a single dose of his medicine was given to him during his stay. Despite the medicine needing to be given every 4 hours. I tried filing a complaint with the facility and they never contacted me. I spoke with the Ombudsman today and they shared with me that they receive complaints about this facility on a regular basis. Too many complaints. They also explained that we have to contact DPH to file complaints with them in order to have sanctions placed on this facility but complaints that are recent (with 6 months). I wish I had pursued this but my father passed shortly after this and almost 3 years later it's still extremely hard for my family. I just don't want anyone else's loved one to have to go through what my loved one went through. Like I said this is my biggest regret that I can't take back. read more

My dad was transferred to Pacific Post Acute after being in the hospital for what we believed was a stroke. He has had excellent care with an amazingly dedicated team. His Physical Therapists Matthew and Eddie have been incredible, ensuring that he make a smooth, strong recovery and giving him personalized and dignified care. His nurses Kiya and Cindy are also incredibly accessible and super helpful. Chris who runs the business side is the kindest, most helpful human imaginable. Our situation has not been an easy or straightforward one and Chris has held our hands and walked us through a daunting and often overwhelming process every step of the way. The staff is cheerful, welcoming and truly just kind. I appreciate all of the care they have given to my father and our family. read more

I was released two days ago from Pacific Convalescent Hospital following surgery at UCLA Santa Monica. I will admit I was in a great deal of pain and all I wanted to do was go home but I was not ready to go home alone. But the care I received there prepared me to go home and I am doing very well!! Doctor Garcia and her team of nurses, healthcare providers and physical therapy team put me back on my feet. Once I was permitted real food following a clear liquid diet and then a pureed diet, the food was really very good! I am sure, if the necessity ever arises again, I would return to Pacific Convalescent Hospital without a moments hesitation. This place is shady. A loved one's hip was dislocated at the facility. I went with that patient at the hospital and noted that a hammock used to transfer patients was underneath my loved one. In ER I called the director of nursing and asked what happened. She put me on hold and then on speakerphone as she sat with another worker. She then said she did not know what had happened and that she would call me. She never has to this day. Their doctor, who was also called before the patient arrived at the hospital, was also a no show for days after the dislocation. Would not recommend. UPDATE I called Pacific several times after the call I made to ER. I was not able to even get the name of the DON. However I remember her face and our conversations. I know the name of the doctor. It is curious that you are so responsive now. You are incorrect Bryce. It happened at your facility. I still have the instrument that was used to transfer my loved one. How insulting you are to keep insisting, here and in your private messages, that I was mistaken--that it could not have happened at Pacific Post-Acute. It did. It's hard to forget a place that you researched and that you visited several times. You must not think much of caregivers or they care they put into find the best place for their loved ones. I would never agree to a random place. You CANNOT intimidate me.You will not bully me. You will not mansplain me. I am not a corporation. I am a family member who is even now dealing with health issues of a loved one. Your institution has NO compassion and by that I mean YOU. You had no interest in responding to me when I called. And by you, I mean the people I spoke with. I am not certain that you were there when I called. It is only because of the YELP review that you are responding to me at all. I will be sure to make an official complaint. I gave your institution many chances to respond to me before. My experience is that your institutions deny, deny, deny. You are even denying my father ever came there. Now why don't we get a third-party involved? Tell me the best governmental agency to make a complaint regarding a SNF and we can make it official. UPDATE: You have refused, via private messaging, to provide the best governmental agency for me to express my concerns. Your associates would not respond when I contacted them privately and now, after the Yelp review, you insist that I call or email you. Your tone suggests that going via a third-party would be best. Since you don't even think that it's your place, I am surprised that you have refused to provide that information. You all did not provide any help when it really mattered. read more

My grandmother has been to multiple facilities which included both great and difficult experiences. We were referred to this location by a family friend, and have been pleased with the professionalism and care that has been provided to our family member. My grandmother has been happy with the staff which in return validates that we made the right decision choosing this location. What an awesome group of health care professionals! The CNA's,like Gloria and Abena, the physical therapists,occupational therapists, speech therapists, like Daniel, Tom, Elizabeth, the director James, the nurses like Kevin, Salima and Garett, the activities coordinator,Esperanza, the accountant,MaLou, the receptionist, Stephanie,the doctor assigned to the facility Dr. Garcia, were all caring, helpful individuals. Everyone there went out of their way to make my mother feel comfortable. They all watched out for her, treated her with kindness, love and respect. Would someone tell me why that god awful Berkely East where my mother was previously seems to be recommended???? This is my mother's 6 time in rehab over the last 10 years, so I know of what I speak. I have a relative at this facility. Having been here many times and at different times, I feel the place is marginal and the staff is questionable. The staff appears bothered to help patients. Today, a room bell (patient asking for assistance) rang for 10 minuets before anyone responded. My children were worried that no one was helping the person who is asking for assistance. I have witnessed a CNA yell at a patient, the staff being dismissive to patients and there is more socializing between staff than anything. Really do your research before placing anyone here. read more

My experience with my mother's care at this facility has been top notch. All the staff are attentive and caring and go above and beyond with the care they provide. Director James along with manager Tonya and nurses: Gloria, Martha, Kevin, and Salima, OT: Carmen and Tom have been amazing in helping support my mom through this journey. I called abs spoke to Brenda who helped my family with all the questions regarding this facility, she was so kind and informative. I felt comfortable speaking to her and having my grandma there. If I can keep another family from sending a loved one to this place I will. My sister was in need of hospice care and was transferred to Pacific on July 21, 2020. As I wasn't on the emergency contact list I asked my nephew to add me, which he did 3 times. It took over 10 days to get on the contact list. I never actually talked to anyone with any medical information. After numerous calls and promised returned calls, my sister contracted pneumonia and was transferred to the UCLA Santa Monica on August 2. I never found out any info regarding my sister and her condition. More calls, more crap "sorry in meeting" "just left for the day" "wasn't there when she was moved". This is a facility you entrust your loved ones. I don't believe they should be in business and please don't believe the website as a lovely and loving care facility... they care less than zero. As per the other yelp reviewers, my husband and I also found Pacific to be extremely top-notch! On first entering the facility one notices the pleasant fresh smell (unfortunately a rarity in some other nursing facilities...), the abundance of lovely light, and an overall upbeat and pleasing atmosphere. The outstanding amenities include very large rooms, a gym/physical activity program, physical therapy, and an enclosed garden where residents are even encouraged to participate in gardening. David provided us with a very comprehensive tour and Esperanza the activities director seems perfectly suited for her position. As mentioned in the other reviews, the staff (amongst whom we also met Brian an Tanya)is cheerful and welcoming. My aged and terminally ill family member was cared for at Pacific Post-Acute, and my conclusion is that they provide a level of care that's the next best thing to having medical equipment at home and one's own family trained as healthcare professionals looking after your loved one. Pacific's professional management and staff truly care that much. These are wonderful and dedicated people who deserve our trust and gratitude for what they've freely chosen to do. I have been very impressed with the good people at Pacific. Both the administration and care staff are genuinely nice people who care about the residents and the care they receive. The facility has a very home-like feel in a residential setting, including a garden area in the center. The facility is always clean and welcoming. I would recommend it to my friends and family if they are in need of a rehab facility for themselves or their family members. read more

Pacific Post-Acute is a well run skilled nursing facility. With around 50 beds, the staff know the patients well and anticipate their needs. The staff is very helpful and welcoming. The rehab team is fantastic including, PT and OT. We loved Dee, one of the CNAs. read more
