This is a very problematic business. I had right hip replacement surgery in November 2025. Just out of the hospital, I was assigned PT/OT from Pathways Health. The therapists were scheduled for 2-3 visits a week for a month - which would have been 12 PT visits. They came once a week (4 times). None of the therapists would give a time/date for the next visit, he'd just call the night before he was going to come or sometimes the next morning of!. The OT therapists were very good, but after 1 visits each, they never came again nor did they or the office return our multiple calls. I've called the office, left messages. Do not count on these people. they are completely unreliable. In fact they compromised my health and recovery by not following through with good service. BEWARE. read more

Does not have proper staffing. Management never responds to calls at a timely fashion. Can not get a nurse practitioners to see you once. Short staff for physical therapy. Nurse checks vitals that's it. If asked a question never tries to get an answer read more

My sisters existing nursing agency of 6 years informed us they were unable to care for my sisters new needs. We hired Pathways as they promised they could help us. All was going good as we were getting used to a new home caring schedule with a PT, OT, Nurse and Home Health Aid. About 3 weeks into it and some man from Pathways called to ask if I drive my sister to the stores. I said, yes. Every so often I drive her to shop with an electric cart. He said this considered her to be "non-home bound". They are strictly a "home bound" company, and proceeded to say they were discontinuing services right away. They said it was a miscommunication somewhere and they are strictly a "home bound" care company. So, to sum it all up, we had a nurse in place for 6 years. We were promised by Pathways they could do the job. Due to a miscommunication on their end, they left us high & dry. No notice and no help to replace them due to their mistake. I highly recommend any other company except Pathways. Their PT, OT and Nurse were great. read more

My family and I decided to try Pathways Home Healthcare to care for my father after he underwent major surgery in Boston and was essentially homebound. We were told that a nurse would come by to assess him and discuss what other services might be available, such as physical therapy. After waiting three full days with no update, we finally received a call from a manager. During the call--which sounded like it was coming from a daycare center due to the background noise--we were told that the assigned nurse's car had broken down and that they were trying to find a replacement nurse. Two hours later, the same manager called again to say that the original nurse was now en route using "new transportation." When the nurse finally arrived, I asked about the car trouble, and he looked completely confused. He explained that he had no car issues and had actually been assigned other visits earlier in the day, which delayed him--even though he had known about our appointment since 7 a.m. That's when it became clear to us that we weren't getting the truth. We tried to follow up with the manager, but unsurprisingly, we never received a call back. At that point, we decided to remove my father from their care. This experience was incredibly frustrating and disappointing. The lack of communication, the misinformation, and the overall poor management were unacceptable--especially when dealing with someone's post-surgery recovery. I would not recommend this company, and I regret not doing more thorough research beforehand. Management needs serious improvement, even if operations are being run remotely. read more

My father is currently a patient trying to receive home health care after 3 weeks in acute rehab. I whole heartedly DO NOT recommend this organization. (I give it one star because the Physical therapist is amazing,and I can't leave zero stars)) There is a complete breakdown in communication, management never calls back or gives accurate information regarding appointment days or times. There are frequent no shows by RNs without a phone call. Blood work is not taken in a timely manner and there is never a date given to when it may be completed. There is zero accountability and no answer as to why services are not being completed. They state they are available 24/7 but when leaving messages on a weekend you don't get a call back, ever, not even when the work week begins. No one at the organization accepts responsibility for this and no one has any answers as to why a patient doesn't hear back. We are unable to get a schedule of when RNs will be visiting and even when they commit to a day the RNs don't show up or call to say they won't be there. My father should be seeing an RN twice a week and he has only seen one once a week for his duration with the business. Additionally most don't have his medical history, know what medications he is taking or know anything about him. Clinical mgr Andrew in particular will not return calls or answer any questions regarding when care can be expected. Blood work orders from the PCP do not make it to the RNs so over a week goes by without the blood being drawn. When trying to call and speak to a rep you will be put on hold and frequently hung up upon.. frequently. Sometimes without anyone answering, sometimes in the middle of a conversation. If this is the state of our home health care I fear for those who are alone and have no one to advocate for them. Those poor people are simply prey for organizations such as this. I work in health care and I could not imagine caring so little for my patients. This is the definition of sub-standard care. read more

While the therapists and nurse were exceptional, the nurse practitioner was incompetent and deceptive. She convinced my 24/7 aide to ask for time off, said she cover for him, then never showed up. Without authorization, she contacted my physicians, told my PCP his services were not needed, and continued to act even after I changed from Pathways to Partners. Add in losing other patients records and leaving then unattended..If you're looking for home care, I strongly recommend you look elsewhere. Charles C. . read more

Pathways is an amazing company that cares so much about their patients. Nurses are available 24/7 for emergencies and will do anything they can to keep the patients safe and in their own homes, where recovery is best achieved. read more

If I could give this home health agency less than one star I would. We started with them a month ago and the clinicians never show up. They were supposed to see us 2-3 times a week. My husband never got a through exam and the nurse left within 10 minutes. I tried calling for management and they never return my call. The woman who answers phone sounds like she just woke up and couldn't care less. The manager Tiffany said she would call several times and never did. Awful management and an awful company. read more

my home health nurse was useless. the physical therapist set me back in my progress which had been constant until i started seeing her. she preferred to gossip and give "information" about health - i found her to be a very UNintelligent person. i was not interested in listening to her. i wanted therapy to get better. I THINK THESE PERSONS WASTED MY TIME AND MY INSURANCE COMPANY'S MONEY. read more
