    I spent 3 weeks at Avamere and was shocked at the overall condition of the building. The building…read moresmells moldy & musty, and sure could use a lot of remodeling. There are some very nice caregivers that work there, but having to deal with personnel shortages, and all the other issues, it often wears them down by the end of their shifts. The physical therapy staff is great, and the event planners do a decent job, but the atmosphere has just enough tension to keep the patients restless. The food was the worst thing about being there - truly awful and not that plentiful on some days - heck, I lost weight due to constraint (the one good thing). Even though my room had two patients, the room next door also shared the bathroom. Four older ladies with wheelchairs/walkers to navigate an old narrow bathroom door was miserable. I was given a list of rehab care facility ratings (while in Hospital) and Avamere Sunnyside was at the bottom - yet other Avamere care facilities were quite highly rated...go figure!

The staff there was hoard it took them 25 to 30 minutes to come help my dad to the bathroom, well as you know when you get older you can not hod your movements as long as you use to, making a person of that age wait 25- 30 min. for some one to come help you go to the bathroom is just too damn long. I would not send my dog there. let alone another human being

    Recently brought a gentleman to this facility And was very surprised by the lack of professionalism…read moredemonstrated by one of their employees. The lady forced herself between me and the patient and proceeded to abscond with him. I informed her that it was my wheelchair that the patient was in and I would like to operate my own wheelchair to which she abruptly said "no" and proceeded to tell me that she needed to do some of her own duties before I could take my chair with me. I informed her that I was merely transport and I was short on time and did not have time to run around the facility with her to, she attempted to proceed anyway. There are further considerations to mention regarding liability in the event of incident. The longer the patient is in my equipment likelihood I could be held liable for any incident. As a courtesy, I try and accommodate them as much as I can, but if I am short on time, they need to stop trying to intimidate me into accommodating their schedule. It would be different if they had requested my services and were paying for them, but they are not my customer.

I got excellent care and occupational therapy (OT) and physical therapy (PT) 5 days a week. The staff treated me very well and I learned a tremendous amount in PT & OT. There will always be times where a wait feels a bit long no matter where you are, but you must realize the nurses know how to prioritize the severity of the requests. I feel that if my other knee ever needs replacing, I will certainly plan to stay at Evergreen Windsor the next time to recuperate as well. Thank you Windsor Staff, for the great care! (Colorful Resident from room 28) 9/23/16 Insurance only covered 20 days of inpatient rehab, so I have 6 more PTs scheduled at OHSU-as outpatient. My first OHSU evaluation therapy was yesterday and the therapist said the progress I made from the therapy at Windsor was excellent, and this is after having no formal therapy for 9 days. I have been exercising at home the way Windsor PT and OT taught me, but wouldn't have scored so well at OHSU had it not been for my Windsor stay. Thank you Windsor staff. My new knee and I will always be grateful!

Chris would get six stars if I could give them. That said, the new "intake team" is grotesquely incompetent, loses paperwork, can't identify ICD-10 codes, blames coworkers when simple tasks aren't done, and lies for days about when work that used to take NWRA desk staff five minutes is going to be completed. After giving them my business for years I gave up and am looking to go somewhere else for PT because the clerical staff's incompetence is standing in the way of me getting the therapy my surgeon prescribed. It's a shame the low-level workers are so awful because they have some of the best physical therapists in Salem. Too bad their data entry flunkies do everything in their power to prevent you from getting to work with them.

While working on my shoulder in 2019 (I was laying on my back on the table and he was sitting on a stool on my right)while working on it my hand was in the air between us and my eyes were closed. I felt fabric under my hand suddenly. He had layed the back of my hand on his crotch. I was in shock and I froze. I wish I had the nerve to say something right then but I have extreme anxiety. I tried to yank my hand back but he kept holding on to it like nothing happened. He would also bring up random questions like "What would your stripper name be?" Or say other inappropriate things. I went to the police and filed a report but the police said "we believe he is socially awkward so we won't press charges." You can not be socially awkward like that with clients. It is not okay. I have been too afraid to post my review but this is still tormenting me 2 years later and I don't want it to happen to anyone else.
