Because I no longer have accchs. They are able to bill accchs $677 for every visit, but now that I pay cash they only make $179 per visit. They forgot to send in my drug tests multiple times! I knew I'd report them to the state board and accchs months ago. I've been talking about all the sketchy shit to my mom and disabled friend for months. So I knew I was going to report them from the start. I am apart of the reason the fbi shut down a ton of sober living homes for human trafficking and double billing. I hate when people break the law and don't follow rules! I'm autistic it's hard to ignore. It's very easy to see when an accchs scam is happening and I felt it happening the second he gave me 10mg oxycodone without starting me on 5mg first. And then he kept letting my pee tests expire and not send to lab. Today the doctor says he is cutting me off pain meds because they don't feel comfortable prescribing them long term. I asked them if I broke the pain contract, they said no. I later get an email saying I'm permanently discharged because I broke The pain contract. They kicked me out of their practice because I am self pay. Discrimination. They're also not billing using correct address' and he's practicing in multiple states with multiple address. I talked to my pharmacist right away and told them everything I experienced and they told me YES this doctor is doing illegal shit. 1/26/26 they said I no longer need to do drug tests to receive my meds, that's illegal. My pharmacist is helping me with filing the report to accchs and the state board. Oh yeah he also gave me a 14 day supply of meds but wrote on the bottle it was a 7 day supply of meds. So I felt like he was trying to trick me or something. I'm not a drug addict so I use my meds properly and this little weird shit he did, did not affect me, I never took more than I was supposed to. But prescribing meds wrong on purpose is illegal as well, malpractice. I've been on prescribed controlled substances since I was 8 years old, I am well aware of how it all works. I've been permanently disabled for 12 years also. And then he tried to get me to do as many x rays as possible before my insurance cut off. He found scoliosis and then cancelled me as a patient immediately. Why would someone do that? He wouldn't even discuss my imagining he ordered. He made me pay $179 to get discharged today. So yeah I knew immediately the second he prescribed meds wrong and gave me 10mg oxycodone right away that he was crooked and I WAS FUCKING RIGHT!!!! If you ever feel weird at pain mgmt ask the pharmacist about it. They can definitely reassure you if you experienced something illegal. Other than that, while I was being prescribed oxycodone for a few months, the appointments were fast and scheduled normally. Just nothing the doctor ever did was normal. Letting my pee tests expire multiple times, writing wrong medication information. Accchs would only allow a 7 day RX and he gave me 14 days and put 7 days on the bottle, so that's called LYING. Lying about how much you're prescribing. All I see is red flags at this place.

My God. This place is horrible. Sandy Gallo NP, was my provider at this facility. By far she is the…read morebest Practitioner for pain that I have ever seen. Compassionate, empathetic and understanding. So here goes my reasoning for the 1 star review as I am unable to give negative stars: 1. You have a better chance of being struck by lightning than getting someone on the phone. 2. Your medical records will never be received when sent here. 3. I have PPO medical insurance and don't need a referral to go anywhere. However, I was rudely told several times i needed one. I did as I was asked while my insurance and primary care provider did not understand why and thought it to be foolish. 4. When Sandy Gallo left this practice and I asked where she went so I could follow her, I was told they had no idea and that she just randomly left. THIS WAS NOT THE CASE. I FOUND HER. I have never come across a place that openly and directly attempts to discredit another medical Practioner. 5. Martha at the front desk was amazing at helping me schedule appointments. This is a plus. THE ONLY ONE. 6. The staff is full of the most disgruntled employees ever. They are slow. Very slow. 3 out of 4 appointments I was forced to wait with no answer as to how much longer I would have to wait. When I say wait, it isn't like 30 mins extra. When I say wait, I'm speaking of several HOURS more. I had my 2 year old with me and that is probably my fault, but she was losing her patients, haha. My first appointment was a 3 1/2 hour wait. UNACCEPTABLE. 7. My wife called, WHO IS DEFINITELY ON MY HIPPA FORM AS AN AUTHORIZED PERSON TO ASK ABOUT MY STATUS AND FURTHER HAS full access to my record. My wife asked for my medical records and she was told in a highly rude and unprofessional manner by Maria (front desk), that she was not on my release of information list. I live 26 miles away. That's how much I love seeing NP SANDY GALLO. Additionally I was told that I needed to come in person, fill out a new form, and that it would take 3 weeks or longer to receive my records. The following is the actual rules about medical records: CFR, Which is the Administrative Committee of the Federal Register, rule of law regarding what I previously mentioned is as follows: (note: I am not an attorney, but I am excellent in research and finding rules and laws) ----------------------------------------- Title 45: Public Welfare  PART 164--SECURITY AND PRIVACY  Subpart E--Privacy of Individually Identifiable Health Information §164.506   Uses and disclosures to carry out treatment, payment, or health care operations. (c) Implementation specifications: Treatment, payment, or health care operations. (1) A covered entity may use or disclose protected health information for its own treatment, payment, or health care operations. (2) A covered entity may disclose protected health information for treatment activities of a health care provider. They could have disclosed the information to the new office and not violated any laws. When they told my wife that they could not do anything unless I came in, she asked the records to be faxed to the new office i was planning to attend. IMPORTANT!!!! SANDY GALLO IS NOW AT THE FOLLOWING LOCATION!!! One Health Medical System Urgent Care 901 E Jefferson St, Phoenix, AZ 85034 (602) 833-3199 https://g.co/kgs/iN9SiW I encourage you all who were a patient of Sandy Gallo to call your insurance company to let them know where Sandy is now and should you feel the way I do, please let them know. It is real easy and is mostly easy to in very few steps. I was previously and work comp insurance adjuster and flagged plenty of shady or fly by night types of medical practices and very often those providers and practices were dealt with accordingly. It is very important to speak your mind. There are many more of you out there. Make your voices heard so that something may be done. I will end this review here. If i were to actually write done everything I experienced for nearly half a year, I would be writing this review for weeks. Feel free to reach out to me should you have any questions regarding my experience with this horrible facility. I implore you all, DO NOT GO HERE. Thank you for taking the time to read my experience.

I was a patient at both the pain clinic and the primary practice. I had providers who were…read moregenuinely caring and good at the practice of medicine. The other staff however, were consistently horrible. The support staff was always rude and committed several HIPAA violations every single time I was there. The person at the front desk loudly called insurance carriers, announcing the date of birth, full names and addresses of the patients in the waiting room. We all would sit there and listen to her. On several occasions I brought it to the office managers' attention, however, nothing was ever done. Their very low standards are represented in their Yelp profile. The website is a bad link and the CEO's bio was clearly not proofed for errors. Don't trust your health to someone who can't even copy and paste their own website's link adequately. I no longer receive treatment from these facilities, as the providers I grew to love have now discontinued their employment there. Without those providers, it is not worth it to frequent those facilities. There were some pretty funny moments of possible malpractice; once a nurse practitioner told me that she was quite perplexed as to my diagnosis of a yeast infection, because only promiscuous women with multiple partners got yeast infections and since I was not sexually active, I should not have a yeast infection. Not only is she immensely mistaken, but she also failed to realize that a side effect of the antibiotics she prescribed is yeast infection. Don't go to a health care provider in which their combined knowledge of medicine is less than that of which you learned in high school health class.

Very easy fix with the concept of basic counting and addition. That would require common sense. Much easier to just make your appointment the following month on the same day. Doesn't matter if a month has 31 days or if the date landed on a Saturday or Sunday then just do it the following Monday. So you are a couple days past the 30 day supply given. Fabulous way to manage chronic pain for your patients. It is like a Olympic event to see how many patients we can see in one day. Quantity over quality by far. Also should give a warm and fuzzy feeling to anyone when reading through these reviews not one response or comment from the office. Just the sound of crickets, regret and embarrassment. My complete history was documented and the physical was thorough. My experiences elsewhere were full of expectations that devices would be implanted as I was handed brochures to read up about them so that when the sales pitch came, I was somewhat knowledgeable about what they had planned for my treatment. Although I had been diagnosed with Neuropathy, I wasn't diabetic. They only assumed how they would treat my pain without an exam and just using the BUZZWORDS, my cure was in the brochure (which included surgery, physical therapy and THOUSANDS of dollars). The entire office staff were kind, courteous and pleasant to work with.

I understand that they have questions they need to ask as per their job and state/federal regulations on pain managment, however, Dr. Glenn sounded VERY accusatory from the git go. And i get it to some degree, I CHOSE to be on a medication that's typically given to people struggling with dependency/addiction, but id hope a doctor would be able to set their preconceived notions aside to effectively and correctly do their job and thats not why im on this particular med, im on it cuz It works better than traditional opiods and my prior pain managment doctor boasted about how its safer and id agree. But from the moment he saw my charts, things were off. I didn't realize right away that he didn't have ALL of my records, so when I was begrudgingly asked if I used recreational drugs, I said no, thinking that my open Marijuana usage was listed and he could see it, Dr. Glenn clapped back with "you've never done recreational drugs" (as if I was lying) I responded with "I mean as an adult, no. As a teen yeah, what teen doesn't" and Dr. Glenn asks what i tried as a teen (my last teen year was 13 years ago mind you) I gave him the information he was asking for. We move on to him asking if I'd ever received surgery for the things im complaining of, answer was no. Then I get told "well if I had been told I have significant narrowing of the hip space joints, I'd have gone and got surgery" i interrupted at that point and said "today's march 11th, I wasn't told about the hip space issues until february 25th... there hasn't been enough time to figure things out". Which i would assume, if he had access to that specific record, he likely had access to see when imagining was actually taken, but maybe im wrong. He asks me to take UA as normal for pain management and told me he wasn't gonna have me sign a pain managment contract "in case things didn't work out"... fast forward to march 25th, my next appointment with him (he wanted to see me when I was supposed to have a refill of meds) I drive the hour from my house to the facility (which Dr. Glenn was made aware in march 11th appointment that I lived an hour away) once i make it inside im told that they cant complete the appointment because they dont have my records. To which they send another request and ask me to reach out to my pcp, which I do before leaving the parking lot on the 25th. 26th I hear from my PCP that records were already sent, 27th I call az pain treatment to set up a new appointment, while on the phone with the receptionist, i explain that the doctor canceled my 3/25 appointment because he "didnt have all my records", to which the receptionist informs me "i must have misunderstood because they have all my records from what she can tell", i explained thats verbatim what the lady at the front desk said, my husband was standing right there when it was said, definitely not a misunderstanding" and she said "hmm thats weird, you chdck all our boxes for setting up an appointment so we will go ahead and set one up for you". The appointment was set for March 31st. The 31st rolls around and I at least got a phone call this time, telling me not to drive the hour away, because "Dr. Glenn thinks you're not fit for our practice, and that you need to go to orthopedic surgery" Being that my pcm used to be a pain managment doctor, they found that odd as they were trained to avoid surgery unless absolutely necessary, especially for someone in my age group. That being said, I feel more so that this doctor thought I had other issues, and wrote me off from the git go and if that's not the case, then his bedside manner is seriously lacking. Even if I had a drug dependency/addiction issue, being accusatory isn't actually helpful. But aside from that, ive been in therapy over 10 years and continue to go because of the addiction that runs heavily in my family, I wanted to make sure to never be like my parents and ive done a really good job at that, and I'll be damned if I get accused of anything im not doing let alone something ive actively worked towards avoiding my entire existence. Needless to say, im thankful it didn't work out and that I didn't sign a pain managment contract with Dr. Glenn. I do wish I had been given the opportunity to see Dr. Nasser, I hear he's nice and who I thought I was seeing until I spoke with my PCP again today. I've had about 4 procedures done with her and I have to say she and this office are the most efficient doctor's office I have ever been to. I show up ahead of time for my scheduled appointment and the office staff is already calling me back at the time of my scheduled appointment. That is unheard of. You are usually waiting for about 30 minutes to an hour before you even see the doctor, after your scheduled appointment time. WOW! What a difference. Each appointment and procedure went off without a hitch. She communicates very well and was very knowledgeable about my pain, where it was radiating, and what procedures would be best to help me find the maximum comfort for all my diagnoses. The facility is clean, all the nurses are kinda, the only thing we had an issue with every time was finding a deep and big enough vein for my IV. Each time they had to call the big dog RN nurse, to place my IV. Sorry everyone, my veins stink lol! Other than that, things really did go well. Each procedure was a bit painful because I was awake but it was bearable. I am about 2 months post-op from my last procedure and things are still healing. The pain has definitely gotten better, it takes a while to heal, but I am optimistic that things will continue on the up and up. Thank you for being so attentive and treating me like family. Not once did I ever feel like just another patient, just another number through the door. Excited for my follow-up after the beginning of the year to see how things are healing.
