If you have scrolled down and seen several one-star ratings for this place and now see 5 stars, don't be surprised. There is a reason: This nursing home used to belong to a different person/company who was heavily fined and is now banned by the state of California from operating such establishment. That person clearly did not care about the facility or the well-being of the residents and patients. Hence, you see a lot of one-star ratings for that timeframe. The previous owner is now gone, along with many of the staff. She sold the business to the current owners. The new owners are highly regarded in the Vacaville and northern California area. Even though it has only been a few months since they took over, if you drove by, or stopped by, you would be able to see, feel and experience the significant changes they have already made to Orchard Post Acute. Orchard is a wonderful place for mom or dad. the new owner has done an amazing job with redoing the inside and outside the facility. My 92 year old mother had a bladder infection that sent her there the first time, she had the best of care. The physical therapy staff was just wonderful, they got her up and walking again. She was there for month came home for a week, but found that we could not take care of her, she requested to go back to Orchard they welcome her back with love and compassion. The whole staff was just wonderful, from the kitchen, activity and nursing staff + hospice. She did pass from natural causes. I would highly recommend Orchard Post Acute Care. read more

Orchard has a New Director with compassionate staff, dedicated to providing a comfortable stay for the residents. The activities hall is bursting with energetic staff and residents from the time it opens until it closes at night. Bridging back purpose for thoes who otherwise would stay inside their rooms. The OLD stigma of the facility will take a while to go away, however the renovations are apparent that this facilities Management cares about the environment and all of the residents. My suggestion is stop in and see for yourselves that ORCHARD is completely transformed and it's atmosphere shows that this leadership goes the extra mile and leads the way by example. If you have a loved one or if you're in need of a Rehabilitation center rest assure this Management is the same Management as VCRC. Your family and yourself will be in excellent care with Physical Therapy daily and an open door policy any time. A place where your voice and opinions are valued. My husband was recently admitted to the hospital (March 2024) with a diabetic foot ulcer and was inpatient for 10days. Upon discharge from the hospital it was decided that he would need further rehab including PT/OT and wound care. He was admitted to Orchard Post Acute for this care and stayed for 3 weeks. During his stay he was cared for by very caring and attentive staff. The entire staff including nursing, techs, custodial, dietary, etc were outstanding in their care and treatment of my husband. We were very pleased and impressed with the pleasant attitude, attention to detail and the professionalism of everyone who came in contact with him. Upon learning that we would need hospice away from home and having previous experience with rehab and long-term care facilities, including hospice, I had our case manager submit a request for a bed/room at several places that I was interested in. First stop, Orchards Post Acute Care in Vacaville! I met with Aaron, the director and asked for a tour of the facility. I was impressed with the welcoming environment and the professionalism of this place throughout, and the staff onsite, the cleanliness and the responsive attentiveness to loved ones that reside here and call this place home! There are plenty of activities to participate in for residents and if your loved one is unable to participate in a group atmosphere, they will visit bedside and spend some quality time with them one on one! The food is good, prepared fresh in the kitchen, snacks and drinks are provided. You can bring a favorite dish if you like and it is encouraged here! Be involved while your loved one is under care here! You know them best and can provide valuable information to the staff to help them help your loved one even better, especially the first couple of days! Highly recommended if you want your experience to be a positive one! Be involved if you are able! Any concerns you may have, they encourage you to talk to your nurse or staff, they will listen and work to make adjustments where needed and as they are able to, for the best of care provided! I recently was hospitalized with several significant issues. I have had some exposure to nursing home in the past and did not relish the idea at spending time at one. First I tried a nursing home that was close to my residence and quickly found out that a few extra miles was not too big of a price to pay to be in a quality facility. I next went to the facility listed above due to its proximity to my daughter. This time it paid off. Not only did I get to see my daughter and her family but also had an outstanding experience at this facility. I did not meet one employee at this facility that was not not friendly and caring. I performed my own test to see the comparisons because of the rumors I have heard. At the first facility I pushed the nurses button and then waited. 5 min. 10 min. 15 min. Finally between the 15 and 20 min mark someone showed up. At the above facility I did the same thing expecting a similar response time. To my surprise someone was in my room within 60 seconds of pushing the button. I consider the quality of care from all of the employees to be top notch and very caring. I would highly recommend this facility to anyone requiring this type of care. read more

I have to say I wish there was a higher rating I could select. The new ownership at Post Acute has brought a top notch team and experience for local Acute care. Recently we had to find a spot for my Grandma. We were so worried about brining her somewhere we didn't know. But from the first call over there they made us feel like part of their family. I have never experience a post acute care facility like this and I have over 20 years of EMS experience which they did not know going in. I have a high level of customer service expectations, and they excised all of them. They are genuine, and truly passionate for patient care. My Aunt recently stayed at Orchard Post Acute following a hospitalization. We couldn't be happier with her stay and care! We were initially concerned for her to stay there base off past reviews, but it was apparent with the new ownership and leadership that they had made so many physical improvements to the facility as well as overall care. I seriously don't understand how people are giving this place five stars. The staff is great, but they're extremely understaffed. Good luck in getting someone to check on your loved one in a timely manner when they push the call button. The room has a jack and jill bathroom which is shared between 4 patients. In the five days my mom was there, not once was it cleaned. There was blood splatters in the sink, the toilet was disgusting. The floors are black from grime and dirt. The beds are out dated, and uncomfortable. If you have a question, nobody has an answer. It's always the same thing, they're off today, but tomorrow they should be back. However, nobody ever gets back to you. My mom was there for five days and not once received the PT she was supposed to receive. The day I checked her out (AMA), we found out she was never admitted. I understand it's under new management, and their employees are doing the best they can being short staffed and all, but not providing a clean environment is totally unacceptable. My dad has been there for ten days. No one returns calls. When you go down there everybody tells you a different story. Once we got a hold of his nurse she knew nothing about his condition, or his treatment. She kept saying we had to call someone but would not give us a name. She was rude and giving tons of attitude Unfortunately my dad can't speak well so we have not idea how he is being treated. But if they are treating him like they are treating us it's horrible. Please spare your family member. Don't let them stay here. read more

My brother and I just had our mom transferred from Vaca Valley Hospital to Orchard Post Acute Care. She was there for two months. Nothing I can write can express what a great experience it was. I heard bad things about the past management but leaned the owners of VCRC bought OPAC the beginning of 2023. We had a great experience with our dad at VCRC so we gave OPAC a shot. They were all FANTASTIC!! Mom was well taken care of by any nurse that attended to her. The folks from PT were very patient and good. Betty, the activities director had the place hopping, did her best to get mom involved. Bryan, the head nurse was very involved and over saw everything so well. She even got several visits by Aaron, one of the owner/directors. Mom is pretty particular about her food and she said the meals there were really good. They are doing a great job renovating the facility, it's looking awesome. If you have a loved one in need of care, send them to OPAC, you will not be disappointed. I just want to give a shout-out to OPAC for the transformation under the the new ownership in a short amount of time. It's amazing! If you take a drive by ( and you should ) just from the outside appearance you can see the changes! All the hard work, time, money they have put into it, definitely shows! It's much more attractive and inviting to the community. ( where did that mystery van go anyways?). They have changed things on the inside as well! We just had to call 911, to return my cousin back to the hospital because she was so badly neglected it borders on criminal. For the longest time all she could hear was loud music playing from the staff while she was left with a call light that did not work. They only heard her screams in-between songs. Please do not allow your loved ones to be transferred to this shameful place. Their just collecting a check and will not care for your loved ones. We have notified the police. read more

My mother-in-law was admitted in March 2024 after a brief stay in the hospital for sepsis and cellulitis. When the doctor told me that's where she would be going, we were very upset as our aunt had been there many, many years ago and it was a horrible experience. The doctor assured me that the new owner is the same owner as Vacaville Convalescence and Rehabilitation, which is where I wanted her to go. He told me that Orchard is much better now. Well, Orchard Post Acute Care has been ABSOLUTELY AMAZING! The STAFF are INCREDIBLE at what they do. They do their job with the UTMOST CARE and PROFESSIONALISM. I have seen them almost every day for the last 7 weeks while visiting my mil so I had plenty of opportunities to really scrutinize them. I could not be more IMPRESSED with the level of care they provided. So many staff members stood out that I would be remiss if I didn't share their names: MJ, Fa, Melissa, Ashley, RoseMarie, Mae, Jenny, Crystal, Kate, Samantha, Eon, Z, Kaylyne, Shiena, Taylor, Erica, T, Brenda, Courtney, Eric, Tiffany, Betsy and also Dr. Richardson. I know I may have missed some of the WONDERFUL STAFF and I apologize for that. Not only did I see them provide medical needs but on more than one occasion, I saw EMPATHETIC staff members share a sweet hug with a resident as they were leaving and saying goodbye. It is in an older building but the FACILITY itself was CLEAN and had NO SMELL. I was impressed because I expected it to have the urine smell that many of these types of facilities tend to have. I would definitely recommend Orchard Post Acute Care. It would be my first choice if I ever need to use a facility such as this. I do have to give extra kudos to Melissa the Physical Therapist. She was so knowledgeable and encouraging but also didn't let my mil make excuses and because of that, my mil was able to accomplish so much. read more

If I could give 1 star I would. I don't normally write reviews and have never written a yelp review before but I feel that people need to know about this dreadful facility and the appalling level of care being provided. My mom is there for rehab and ended up contracting covid and is getting abysmal care. As of 1/5/2021 there are 28 residents and 9 staff infected. Staff are going back and forth between covid and non-covid residents which they are not supposed to do and is probably why it is spreading. They are understaffed. It is nearly impossible to get through to the facility and no one calls back. My mother was left overnight for over 12 hours in a soiled depends! She is bedridden. Her food is late and cold. She is supposed to be having an abdominal wound cleaned and bandages changed and they are not doing this. She tells me that caregivers yell at residents who are in distress, telling them to shut up. I've contacted the county Ombudsman and they are investigating the neglect that is occurring at Orchard, not just my mom but I'm sure all residents. I've also contacted Solano County's Congressman Mike Thompson to alert him to the situation at Orchard. The owners of this facility also own Hayward Gateway Rehabilitation that was being called a "death camp" back in April from all the covid deaths. If I can give zero stars that would be my rating. This facility has provided a low level of care, medication not administered correctly, non operable HVAC, and the patient not receiving meals or snacks. This facility should be shut down. My brother was a patient there for a week & half and the place was horrible. He was rushed to the hospital under their care after two days of complaining of chest pains, vomiting, urine brown and was given OxyCodine , Acetaminophen on a empty stomach all in a three hour period. He later had to have his gallbladder removed. The heater went out in the building for the entire weekend of 11/20-11/22, it was freezing my brother room was next to the back door that the staff continuously kept open. I was given permission from the nurse to bring in a heating blanket, since the engineer didn't return to work until Monday. That evening when I brought in the blanket, I was told he couldn't have it from the nurse on the evening shift. There was no protocol in place to have someone fix the heater on the weekend. The residents had to suffer the entire weekend in 40 degrees temps at night. He was forced to sleep in his clothes due to the coldness. The staff could never get the transportation schedule right, On two occasions he went to dialysis, and had to wait 2 and half hours for pick up. This resulted in him not receiving lunch, and not to mention not offered a bag lunch, nor snacks to take along to dialysis. The Nurse Frankie was horrible, when I called to ask why my brother transportation was late, he replied we were wondering where he was. The Nurse Renee, gave wrong information regarding my brother scheduled Clonidine weekly patch, it was determined the information was incorrectly charted, and she was unable to follow up with the nurse who charted that his medication was administered. She could of been proactive and checked the date on the patch to ease our family concerns, and the patient concerns regarding the patch on the same day. Two days later, I received a response that it was not administered due to his blood pressure. That was clearly a lie, it should have been noted in the chart. The nurses are clearly unconcerned about the patients well being. The door bell didn't work, I informed the staff and when I returned the next day it was removed from the building structure. The food is horrible, and no snacks are given in the pm. Thank God for DoorDash because I had to order soup for him to eat. When I went to retrieve my brother belongings, they had thrown in other patients items. He did not share a room, nor requested his items to be washed by the facility. I did his weekly washing. I did point out that these were not his items, or his size of clothing. Looks as if they just threw the items in bags and boxes without referring to the property sheet. I declined the plastic hangers, barbells, and plastic tub and hygiene supplies. If you are picking up your love ones belongings bring gloves as I did and go through each of the items. I also did not appreciate the soil underwear that was placed in a tied bag that wasn't even my brothers. I reported Orchard Post Acute Care to the Ombudsman Program and CCL and completed the RCFE complaint form. There is no rating low enough. My friend was transferred here by Kaiser. She was put in a room with three beds, and still could hear nothing but loud voices coming down the hall on the first night. Swearing, screaming, uncontrolled people all around her. She got extremely fearful of spending any more time than she had to, and ended up having her son come get her on that first night. This place needs alot more help than the prior reviews intimate they already received in 2016. NO ONE SHOULD HAVE TO STAY IN THAT ENVIRONMENT , PERIOD. read more

Needs a total renovation to start. Dirty, I was on a sheet with a hole behind my back and the top sheet was like tissue paper with a holey area (I have photos) for 8 days before they were changed. The nurses and physical therapist are great, but a lot of the CNA's could care less. You better have an hour after you press the call button if you need to use the bathroom.I also have evidence of bugs. After 2 days I was in tears. I couldn't get home fast enough. If you love your family never have them sent here. The menu is another thing. Tell the dietitian your likes and dislikes and it doesn't matter. read more

My mother was a patient there for 18 months on hospice. She passed away on 12/23/20. The took good care of her. I work in a AL/IL community and was always checking on here. I never found any neglect. The care staff showed compassion for my mom. The only problem I had was that they kept loosing her cloths, blankets and her teddy bear that we never found. They were not suppose to be doing her laundry, but they continued to do it anyway. I didn't hesitate to go looking for her stuff. That is the only complaint that I have. They kept me updated about her condition and answered any questions that I had. read more

My husband has been at Orchard Care Home for the past couple weeks. The staff has been great to him and for his care. Since day 1 they have been responsive to his needs, helping him learn to walk, talk, dress himself, etc. They provided a special bed mattress that helps with bed sores that he received at a different facility. They listen to him and he is part of discussing his rehab care. My husband was in the ICU for 14 days and then in a cardiac recovery room for an additional 7 days before coming here. He has to learn everything all over again. It is like having a 54 yr old toddler. Everyone at the facility checks on him, makes sure that we as his family are taken care of and keep us informed with his progress. When you walk in the facility the patients there seem happy and have life to them. They are not all depressed and just sitting there. They participate in activities and they love strolling the halls in their wheelchairs. I would recommend this place if you have a family member that needs that assistance care. read more

My mother came here yesterday - transferred from the hospital. Facility is a little bit beat up. But she is in a fairly spacious double room. The staff here are all exceptional! Incredibly helpful and responsive. The RNs and CNA's really seem to care about the patients and are happy and cheerful. The support staff is very responsive. The PT'sOT's and speech therapist all really seem to know what they're doing. They have a wonderful woman named Brenda who leads a program in the dining room. Everybody does simple exercises with her and then she leads a fascinating discussion about current and past history. I have a post grad degree and learned a lot. She has such a friendly, enthusiastic and cheerful attitude! Are nurse Kitty is awesome! The level of care she provides is unbelievable. read more

If I could give this place zero stars I would. It is the middle of summer and 82 degrees in my grandfathers room. The toilet plumbing is overflowing in his room. When we asked for them to get his medication, the "RN" stated she was too busy. My grandfather had a hip replaced, is a fall risk, and we were told that no one would watch him for his 24 hour watch, ordered by his MD. We have had to check him out because of the back up of the plumbing system, and are headed back to ER for re-assessment because of his fall. Orchard Post Acute care made a lot of changes this year. They got a new administrator in January 2016. This place now clean all the time, the nurse are top notch and the CNAs great they actually care for every patient needs. Overall most of the services here have been outstanding. They lost stars with me based on the medication piece. I came here for rehab after bilateral knee replacement . I arrived on Saturday afternoon and was placed in a room with two other noisy residents. As well my bed did not work, it wouldn't go up or down and my call button didn't work. In addition, I did not receive my first dosage of pain meds and other parts of my prescriptions til well after 10pm.... I was told they were waiting on the in-house pharmacy. That Monday I was able to speak with administration and was able to be moved to a much quieter room. They fell drastically off again when they let my pain meds completely run out BEFORE ordering. I had to send my husband to my regular HMO pharmacy the following morning to get my refill. Again I missed 3 doses of pain meds that I am prescribed every 4 hrs. On the flip side the CNA staff have been excellent. Physical therapy is a beast, but they too have been professional and polite. That's my rant, I pray your family member has a good stay. read more

This place makes me ill when I hear the name. Why the state has not shut them down... I have no idea. A family member was transferred to this skilled nursing from the hospital for rehab. The family member was left in their feces and urine and neglected for over 8 hours without any nursing assistance or medication. The bed had no sheets or pillows and the family member was laying on a bare bed. The family member said, "It feels like I just have entered hell." read more

My father was sent to this facility by Kaiser. The nurse assigned to my father said that she had 20 patients to care for a shift. My father hit his call button to get help to use the restroom and no one came. Once he soiled himself it took about an hour to get him changed. He tried to get out of his bed on Thanksgiving to use the restroom and fell on his knees and the facility told us that he fell on his butt, but the roommate told us it was his knees. We were told to bring him clothes and to put his name on everything we brought so that laundry could be brought back to his room and 90% of his clothes are missing! We still have not received them. The facility sent my father back to kaiser twice one for a fever which they gave him Tylenol before sending him so by the time he got to Kaiser he had no fever and Kaiser sent him back and I don't remember what the other time was for. I tried to get my dad transferred to another facility and Graylyn (kaisers liaison) told us that he was already doing his physical therapy and was not sure they could get him into another facility. My dad went back into kaiser for an infection in his leg and when they were getting ready to discharge him I told them he was not going back to that shit hole and they found a much nicer place in Napa. I wouldn't send my dog to Orchard! Every time I asked to have someone call me regarding different things no one returned calls read more

My husband went there for rehab to get back on his feet and walking after 8 days in the hospital with a blood clot in his right lung. 14 days later he was sent to the ER by ambulance with pneumonia. That should never have happened. read more

My name is V. Allen and my cousin was a patient in Orchard Post Acute Care several times. I was very satisfied with their care, professionalism and how clean the facility was kept. No Pee smells as you entered the door. After my third visit, most of the employees knew my name. I am also my cousin's care giver. I was very impressed with the nursing staff as they cared for each patient. Rob got bathe everyday, he ate in the lunch room, which was a plus because he was a messy eater. The P. T. Department was on point. He came home walking better than before. Each of Rob's visit there, gave me a feeling of relief. I knew he was being taken care of the same way I took care of him. I would recommend this facility to anyone who want quality care with quality people. Another plus for the staff, they always asked if I wanted something to drink. They cared about my comfort as well. My only disappointment was Rob's last Skilled Center was not Orchard, they were full, no beds. He is not presently a patient there and someone calls at least once a month to check on him. The Adminstration of this facility is chaotic and inattentive to the concerns of the residents, family members and staff. Not efficiently staff and workers are overworked therefore cannot properly attend to all the residents needs. Not thoroughly stocked when pertaining to supplies. Worn and overused items not replaced by facility until unable to use and even then the waiting period for a replacement takes months or is only replaced during time of the evaluation of the facility. Facility heavily reliant on donations from family members, residents and staff rather than taking care of needed items themselves. Too many relations and friends of the administrator in position of authority at said establishment who have no experience or knowledge of the positions handed to them. Much needed outside observation on times when administrator and heads of individual departments are not aware of in order for certain issues to be properly addressed. It must be noted that there are many employees that try their best to administer proper care to residents but are usually taken advantage of by the administrator and certain department heads and eventually end up quitting; leaving residents with an even less efficient and sufficient body of employees who truly care for the welfare of the residents. read more

we had no choice but to put my father at this Orchards Post acute care facility, because kaiser recommended this place. when we first got there , we didnt like the place. we asked the nurse to room my dad near the nurse's station they refuse to do so. my dad wa in the acute care from decmber 18 to febuary 2nd . He was a high risk fall patient . They didnt care . My father fell and fractured his hip 3 places . They call 911 on the 3rd 5am to take hime to kaiser. were he had surgery after 2 days and passed away on feb 11th. This place is careless, dirty, lack of staff. People are not trained. I recommed everyone not to put your family members here . They will not take care of your family. They used to pull my dad by his hands of the wheelchair to transfer to his bed.I witnessed it many times. I even ask if i could help. They really keep their patients dirty. I wish health department closes this place, if not i think health department is all about the money and not pt care.Its really sad that i lost my father if he wasnt going to fall he could have still been with us today.I will give below standard.CNA's at this facility feed they patients by shaving food in their patients mouth.Ridiculous place.
