SLAUGHTER HOUSE! If you want your family member to die, send them here! They are the worst facility I've ever seen in my life! My Sister was not on hospice and she died and they did not even know it. My daughter found her dead. And then they asked her if we wanted to Resuscitated her she was already gone for a while. It took an hour for them to come when she would push the emergency button. She needed her pain medication and they said that she would have to wait because they were doing shift change. Then 45 minutes later they asked again and they said we have three other people to administer medication too, so she'll have to wait. Called the facility five times to talk to the administrator three times before my sister passed away and two times after and they still have not returned my call. Call the police to get an investigation so we can get an autopsy to find out the reason for her death. She was only in there six days. read more

Remember palliative care means terminal sedation. They will slowly kill your parent (without consent ) really uninformed consent)(for easy medicaid approval) keeping them from food and fluids .They will die from no food , fluids, morphine, and Lorazapam every hour. They will die alone and in a frantic state knowing they are being murdered. Technically they are. You will regret trusting this home. If you love your parent do not bring them here. I tried trusting them and i will regret this for eternity.They should be shut down. Unethical and sick ,and greedy corporation. I hope my review keeps others loved ones safe and alive. You only have one precious life and now my beautiful loving mom went through murder and fought to survive in the last moments where they forcefully held her down and injected morphine even though she refused and fought for her life. she died fighting for her life being held down. she was an amazing mom and they took advantage of her brain bleed/ neurological dementia. god knows and i know what they did. the police and detective laughed about it like i was crazy. work in hospital i know what they did! i have medical records to prove it. please run if you love your parents. i wish i did not put my trust in them. my mom was kind to everyone. gave the shirt off her back. they need to do the right thing and confess like rn rhondi did. at least she was honest. i hope more family members notice what they are doing or did to their loved ones. they need to stop killing!!! they need to not treat them like an animal to put down. i feel sorry for those who did this because you can not take murder back, its done. family will live with this forever and it cannot be taken back. think of your loved ones and how you would feel. i give my patients love and respect even though they are confused and combative. Its not them they are sick. You are sick to be apart of this murder of the weak. ps.. they say family does not want to realize that they are dying its a lie. my mom had perfect vitals and was put on hospice with self care uti. no sepsis lungs clear everything great but uti and immediately on hospice with target date may 2. she died may 1 we had been told she was ok, just bad mood. too bad that utah authorities are in on it. so sad what this world jas come to. i miss her and pray everyday for justice. read more

Do not go to Orchard Park! It is filthy and the food is horrible. My sister received a horrible infection from their carelessness. The Drs. also are very unattentive. They are understaffed and it is dirty! My sister also felt very unsafe in this facility! read more

I give it the magic 5 star rathing, within the context they operate. Anyplace that appears to have to accept Medicaid as their primary insurance for most for their patients isn't going to have 5 star luxury facilities. I've seen nothing but positive things My father was here a couple years ago after an illness, and had to go through withdrawal from opoid overuse. He was also infectious for the first 10 days and had to be in quarantine. My mother is currently here for rehab after a stroke. Both "visits" were in the 2-3 week range. In my interactions, the staff have been nice, accommodating, and friendly. They have attended promptly when the big red button is pushed. They have administered medication properly. They have done the thankless job of helping when there are problems with incontinence. I'm not seeing problems. I see, likely, underpaid people doing a tough job well. read more

I would NEVER recommend this facility. My daughter was there after a hospital stay, They didn't even know she was there for nearly 18 hours, she missed 3 meals, had no medical care and that was just the beginning. She was ignored a lot of the time - she was mistreated by one of the "nurses" - She was supposed to get follow-up medical care (after the hospital stay) and she saw a doctor ONCE a week! She had to wait 1 1/2 hours one time to get help from the bathroom. Terrible - they put out that it's a rehab center - I think it's really a "nursing home" - The first meeting with any "head nurse" - director? - etc. was well into the 2nd week - and the nurse thought she was there for hospice! And when she finally checked herself out - they sent her home with someone else's medications! read more

If you don't want your family member with dementia drugged and tortured do not send them here. When a patient acts up they will fool you into palliative care so they can better control their behavior then take advantage and force a cocktail of drugs to off them. They hide from the hospice/palliative care that the patient has a POLST and wants all life sustaining treatment then hospice will not be able to let next of kin that DO NOT have power of attorney know their wishes and trick them into checking the DNR box on the admissions form. They were forcing morphine on my mom when she was refusing. I was told it would be on file in case she wanted it for pain but nurses started pushing it the very day I signed the papers. There's another review recently where they were pushing a patient with meds after being asked not too and they even pulled the family member aside and asked if they wanted to drug them till they passed. Luckily that person got there loved one into a different place. They never let us know my mom was transitioning and also the day of her death they never let us know it was time. We didn't get a call until the one that said she died. They said they held her down and medicated her and she died within minutes. They said she was having pain but didn't know what kind. My sister asked Rhondi and she told my sister that my mom wasn't having pain and we wouldn't have wanted to be there because she was like the exorcist! Who even says something like that? She also giggled after my sister asked if she was sure my mom was asleep and not dead when they left the room and said hehe yeah she was definitely asleep. This place needs to be investigated. We still don't know what type of pain my mom was having that led to her death since we were told different stories read more

I don't usually leave reviews but I just thought I had to for this awful place. If I could give less than a star I would. Don't buy into anything the sell you. They'll promise you prompt care, a clean environment, and to take great care of your loved one. What you get instead is a place that is constantly short staffed, rooms that aren't cleaned well, mostly rude staff, changing policies with no regard to how it affects your loved one. They even tried to change our loved ones room to a little crappy shared room because they said they felt they could make more money with our room then they were collecting from Medicaid. We get constant calls from our loved one that they aren't being helped. Sometimes even going past an hour waiting to be helped to go to the bathroom. If something breakdowns in the room, including your nursing call light, you'll be waiting weeks if not months before it's resolved. If my loved one hadn't been there for over a year and on her last breath she would've been moved somewhere with people that actually care. Don't make the same mistake I made and be fooled by their nice building. This place is a joke and I'm reporting them to the Better Business Bureau. read more

Please do not send your family members here, not even their basic needs will be met. They are understaffed and some of the staff are very rude, or brush you off when you ask something. My uncle would go weeks with out being showered or his oral hygiene done. When he would push the button for the CNA to come they would take 45 plus minutes to get there, and seemed really put out to help him. It made me sad watching the way they treated him and I just don't want anyone else's loved ones to go through the hassle our family has gone through! (Ps it was so bad my family had to hire a home health nurse from another company to come in and take care of my uncles hygiene needs, needless to say there was major neglect) read more

I wish I could have given them less than 1 star. My Brother has been at Orchard Park Rehab for several months. They rarely bathed him or helped him brush his teeth. His call button was out quite often and his room was not clean. There were a couple of CNA's that were good but they were overworked and underpaid. The rest of the staff was rude when we would ask for help and it would take up to 45 minutes for someone to come when he pushed his call button. And for heavens sake hope that you have private healthcare because if you are on government assistance they will shove you in a small space with no privacy. read more
