I didn't mind filling out a health history, but when I questioned 10 pages of non-applicable forms she told me if I didn't fill them all out (including an unlimited info release to anyone the doctor chose - and agreeing to pay outrageous collection and legal fees) then I couldn't see the doctor (mind you this was just for a consultation that was going to cost me $125).
