I went to Newbury Smiles about three months ago for a routine cleaning. At that visit, I was told I had "deep pockets" in my gums, but no one mentioned cavities, an urgent treatment plan, or any significant issues. I was also never shown any X-rays, and I did not meet with the dentist -- only the hygienist. I returned in November for another cleaning, and immediately after the appointment I was suddenly told that I had three cavities between my teeth on both sides of my mouth and that treatment was "imperative." I was completely blindsided. When I said this was the first time anyone had mentioned cavities, the dentist insisted that I had been told months earlier -- which is simply not true. I clearly remember that conversation, and none of this was discussed. I wasn't shown X-rays at that time because they hadn't even been reviewed yet. The dentist only looked at my August X-rays in November and then accused me of "neglect." His exact words were: "Neglect adds up, and it comes in the form of tooth decay. You've done a good job, but now you have a lot of damage to repair." That comment alone was unprofessional, dismissive, and deeply unsettling. Then they slid a $2,500 treatment plan across the table as if this had all been previously agreed upon. I felt pressured, blindsided, and very uncomfortable with how the entire situation was handled. Something didn't sit right with me, so I went to a highly rated dentist at Westlake Dental Care for a second opinion -- a practice with consistent five-star reviews. They were incredibly welcoming and thorough. They were shocked by how few X-rays Newbury Smiles had taken and reviewed. After examining me and reviewing new films, they told me: * My gums are healthy * My teeth are in great shape * I do not have cavities * I do not need fillings * I do not need any of the treatment Newbury Smiles recommended The contrast was night and day. I'm grateful I trusted my instincts and sought a second opinion at Westlake Dental Care!!

They both are excellent oral surgeons; I was treated by Dr. Nicholas Nikchevich. I recently had a very difficult extraction of a cracked old root-canal molar performed by Dr. Nicholas Nikchevich, and I cannot say enough wonderful things about him. I have been afraid of dentists since childhood, so I was understandably nervous about having this tooth removed. Dr. Nikchevich made the entire experience remarkably comfortable. I had nitrous oxide (laughing gas) along with local anesthetic, and I was completely comfortable throughout the procedure. The extraction itself was much more complicated than expected because I apparently have what my mother used to call "roots that go all the way to China!" My roots were extremely deep and broke into small pieces, so Dr. Nikchevich had to work carefully to remove everything. Throughout the entire procedure he repeatedly asked me if I was OK or wanted to take a break. I kept telling him, "No break -- I'm fine!" Dr. Nikchevich warned me that my face might swell, that I might have bruising, and that my jaw might ache afterward. None of that happened! He prescribed a pain reliever, but I only needed three pills during the first two days and didn't need it after that. I will be getting an implant from him in three months. What impressed me just as much as his surgical skill was his thoughtfulness. He called me several times at home afterward just to make sure I was doing OK. That kind of personal follow-up is extraordinary. I have been seeing dentists for 78 years, and I can honestly say that Dr. Nikchevich is one of the very best I have ever encountered -- both professionally and personally. A+ and a gold star! I would give him ten stars if I could. If you are nervous about oral surgery, I especially recommend him. He is extraordinarily skilled, compassionate, reassuring, kind, and genuinely cares about his patients.

Had he done what the ER doctor initially recommended when assessing my fractured mandible, I wouldn't be dealing with the complications I now face. On December 16, 2024, I suffered a fractured mandible and was initially treated at Saint John's Pleasant Valley Hospital. The ER physician diagnosed it as a clean break and informed me that it should have been easily corrected with an open reduction procedure. When I consulted Dr. Siavash on December 26, 2024, he ignored the recommended approach, instead describing my injury as "shattered"--a completely different diagnosis than what the ER physician had told me. For reasons I cannot comprehend, Dr. Siavash was completely against performing the open reduction procedure, even though it was the clear, logical, and medically sound approach to correcting my injury. Instead, he **wired my jaws together for four weeks**, ignoring the need to properly set the mandible at the fracture site. This decision led to predictable complications, including malunion and malocclusion--complications that should never have happened if he had followed a standard surgical approach. His consultation lasted only 8-12 minutes after I waited over two hours in the examination room to see him. Given the seriousness of my injury, I expected thorough attention and care, yet I received rushed, dismissive treatment. Even more shockingly, when I asked if he wanted my dental records before the operation, he dismissed the idea entirely and instead asked me to "just send a picture of me smiling." How does a trained maxillofacial surgeon ignore critical records and opt for a casual photo instead? This is jaw surgery, not a cosmetic consultation. Adding to my frustration, Dr. Siavash was the only option available to me through Ventura County Health Care Plan (VCHCP), meaning I had no opportunity to seek a second opinion within the network. Later, another surgeon's office informed me that Dr. Siavash doesn't even treat trauma-related injuries, raising serious concerns about why he was entrusted with my case in the first place. Because of his poorly executed treatment plan, I am now accruing additional billing expenses from VCHCP as I seek proper treatment to correct the avoidable damage his surgical approach caused. His refusal to perform the correct procedure resulted in lasting complications that never should have occurred. His negligence and unwillingness to follow the standard, medically recommended approach call into question his competency as a maxillofacial surgeon. The errors he made were not minor--they were completely avoidable, predictable, and unacceptable for a specialist in his field. No patient should ever have to endure such carelessness and disregard for their well-being. Dr. Siavash had every opportunity to fix my jaw correctly, and he failed. Instead of following the standard procedure, he ignored the best approach and left me with permanent complications. To put it simply, in golfer's terms, he had an easy putt to make, and he tried to sink it with a nine iron. At this point, I want my treatment to be managed by a different physician--one who understands the importance of proper jaw function and precision in surgical care. Given my experience, I firmly believe that Dr. Siavash should no longer be considered a trusted maxillofacial specialist within the Ventura County Health Care Plan (VCHCP). If you need a surgeon who prioritizes precision, thoroughness, and patient outcomes, I strongly urge you to seek an alternative specialist. Do not trust him with your care. He blew it and I am paying the price for his negligence, or worse.
