Despite concerns about being released to a nursing facility, rather than their own home, the family was told that "death was not imminent" and that our family member could benefit from a brief stay (no longer than 20 days) in a skilled nursing facility/rehabilitation center to build strength and undergo both speech and physical therapies. At this time, our family member was non-verbal, unable to ask for assistance or press a nurse button, feed themself, or move freely to and from the hospital bed. Our family member required a high-level of care that Corvallis Manor did not provide. The family was told that there would be no patient onboarding until the following Monday. This meant our family member would not be meeting with any doctors, physical therapists, speech therapists, and that there would be no patient plan for at least three days. The family did not receive any sort of introduction to the facility, onboarding, or even a brochure with contacts/additional information. A family member was able to get in touch with the doctor assigned to our family member's case via his on-call number (Dr. Ruden), and was told he no longer worked for Corvallis Manor. Our family member was assigned a doctor who did not work at the facility. Following a surgery at Portland Adventist, our family member had a drainage tube at the surgery incision site. This tube needed to be flushed once a day and the bag needed to be maintained and emptied twice per day. Staff were unable to perform the flushing of the drain and had practically walked away from the situation when they could not figure out the medical device. Family, although learned on how to drain and remove the bile/fluid, had to request this to be done by staff, as nursing staff was not present unless called upon. Additionally, a member of the nursing staff (Shannon) said she did not have any orders to flush the drain. Our family member was diabetic, and the family was routinely testing his blood sugar at Corvallis Manor. Nursing staff did not test blood sugar until requested. When our family member was testing high (341), the family called nursing staff and it was 30-minutes until the nursing staff was able to take a reading. The blood sugar level reached above 400 when tested by nursing staff, a dangerously high reading, and the nursing staff was unable to administer insulin until 1.5-hours later, after the family pressed the staff to make a decision on the amount of insulin to administer. At Corvallis Manor, our family member tested positive for Covid-19. Despite Covid-19 protocols being expressed, such as receiving meals in styrofoam and being bagged separately when discarded, for example, these protocols were not followed, leaving others at risk. On the morning of Sunday, September 10, the family arrived at Corvallis Manor at 8 a.m. to find our family member nearly naked in the bed with the blankets on the ground. When asked, staff could not explain how long he had been laying like this. His wife decided to stay overnight on Sunday after this. On the morning on Monday, September 11, the family was told by staff that our family member would be released, after the family had pressed for a home release into hospice since admittance on Friday, September 8. A meeting with a hospice nurse took place and our family member was set to be released at 9:00 p.m. via gurney transport. 9 p.m. came and went and the family had to press staff to look into the transport and release orders. Shortly after, transport arrived. Our family member was released into home/hospice care and was transported home late on the evening of September 11. He passed away on Tuesday, September 12 at 3:30 a.m. Please let this be a warning to other families, do not allow your family members to be admitted to Corvallis Manor.     As a CNA for several facilities, this is one of, if not, the worst care centers I have every seen…read more My aunt was there for over a year and this is what I saw/heard. There was little to no communication with family. My aunt could not eat a lot of the meals and the staff would not get her food that she could eat. The nurses would stop or change medication WITHOUT DR'S KNOWLEGE OR PERMISSION (this happened at least twice). It would take staff on average (and yes we timed this over the course of several months) 45 minutes to 1.5 hours to answer her light. Which means she would be left with a wet or soiled brief or in EXTREME pain for that long. There are several other things I could mention such as part of her charts going missing; my aunt getting sores because the aides refused to listen to her or clean her properly, her not getting showers for WEEKS at a time; etc. Overall I would NEVER recommend this facility to ANYONE. If I could give negative stars I would.

    This is a very personal and tough review to write because I'm in the medical field (have been for…read moreover 30 years), so I know how tough it can be to care for patients, especially during the time of Covid-19. But even so, there needs to be some "basic" expectations met when being cared for at a healthcare facility such as The Regency. My mother had a horrible fall at home and broke bones in both her legs. Add to this that she has a degenerative muscle wasting disease that limits her mobility, and you've got a recipe for problems (which is why we chose a care facility - The Regency - close to her home for rehabbing before returning her home). Our family's expectations were that they would do physical therapy, prevent further problems from developing, treat her with respect and compassion, and be honest about their care and her progress. None of these things occurred at all... Let's start with these items and where The Regency failed with all of them: 1. Physical Therapy: initially this started out okay, but with the limited mobility that is my mother's baseline, they initially tried to over work her and actually caused more pain. Then, later, they claimed that she and/or we were refusing therapy (not true). Our expectations were that they'd do PT appropriate for each patient. Even later, they had a shortage of staff and PT was rarely done, meaning her rehab had basically stopped. 2. Preventing further problems: Because they began to rarely release her from bed, she developed bed sores (2 of them) near her coccyx. These were finally reported to us and we asked to know what was being done. Barrier cream and frequent turning, we were told. But when I spoke to my mother (we did get her a cell phone so she could call us), she said that she often had to call and asked to be turned because they would either forget or claimed they were too busy. Turning with two broken legs also caused some pain so she would ask for some pain medicine. Again, frequently this would take hours to arrive or not at all, meaning that I or my father would have to call the charge nurse and ask what was going on. Which led to our third problem... 3. Treat with respect and compassion: Some of the staff were moderately ok with care. But many of the nurses were either dismissive or downright mean (Two of them actually yelled at my mother because of her calling and reporting problems to me and my father). Really? Yelling at an old lady with two broken legs? 4. Honesty about care and progress: We had an initial call with the staff which sounded promising and upbeat about the third day my mother arrived. But this was a smokescreen for what truly goes on behind these doors. Call bells going unanswered, pain being left untreated, physical therapy being delayed or not done at all, bed sores developing due to inept care, and a lack of communication about what was really going on all led us to remove my mother from this Hell and back home with in-home care. It is truly sad how horrific the care was here. They sound all sugar and spice and everything nice on the phone. But do not be fooled. This place may be a death sentence for your loved ones if left in here and they are immobile or nearly immobile. Update: My mother is home with round the clock in-home care and she's doing much better. Bed sores have healed, pain is under control, and she gets out of bed twice daily. She is also having PT two to three times a week.

My dad died from a bed sore that was left untreated for months. Corey the head nurse/accountant stated after a month the bed sore looked like it needed medical attention. Prior to this they tried to put my father on hospice because they felt like they couldn't help him. Rather they didn't want to. The nurses are never around the staff is lazy and half the time you have to assist the residents yourself. They give the patients food away. Once in awhile we would get my dad a healthy meal for letter and it would be gone! They never ever informed us of my father's falls or injuries. The old accounting manager (no longer there shocker) also admitted to double billing. He admitted to billing people and any private insurance as well as state medical. Then the very elderly he said he would tell them just don't pay it if you have no assists. Thank goodness this is all recorded. I could go on and on but all you need to do is look on Yelp and Facebook Reviews. Not to mention giving peoples rooms away and moving their things in hopes they don't ever come back. Please don't send anyone here! As far as the rooms go, they added a dying man into my father's room hoping my dad wouldn't make it out of the hospital the first time. The man ending up dying that made my father's PTSD so much worse. The guy in his room had 5 days to live he was in a diaper and my husband had to carry him to the bathroom because no staff was available. He saw a elderly lady in a wheelchair and she asked if he needed help? My husband said he was looking for a nurse, "her response, good luck!" Not to mention the food they serve cold food and their is a live living pig in the dining area. I asked the cook why is this poor pig inside and not in a barn where he belongs? His response, don't ask me this whole place is messed up. I pray for any person left their to live. In closing they met my father's oxygen machine run out of distilled water five times in the days I went to visit. I had to literally show them how to do their job! These are the last people I would ever send anyone I cared about to. Negative 100 stars!

I've seen residents wait for medication for hours, I've seen them be passed over on their shower days or forced into the shower. I've seen them cry in pain when they're meds have been looked over and I've seen residents wait for their call light to be answered for up to an hour. As for employees, I was made to feel like human garbage. I was overloaded with work, having to pick up slack from the shift before me, (which is not irregular in this line of work) but when I couldn't take care of what was left for me from the previous shift, I was threatened with write ups for wanting to get my shift's assignments done. The charge nurses' attitudes were always sour and they thought little of the CNAs that were running their backsides off, constantly behind and overworked. Even certain residents were favored and taken care of more promptly simply because of their threat to the company. I'm sad to say that I left after a month of employment. I truly wanted to prove myself as a newly certified nursing assistant, but my dignity and my sanity were worth more than that. Be warned....reconsider your options if Avamere is ever recommended to you as a place of rehabilitation, or employment. I have worked in multiple facilities as a caregiver for the past 6 years, and I have never worked in such a disorganized building, or under such rude nurses. I have NEVER been so disrespected as a human being.
