Our family member, age 92, arrived from Portland Adventist to Corvallis Manor on the afternoon of Friday, September 8 via gurney transport. Despite concerns about being released to a nursing facility, rather than their own home, the family was told that "death was not imminent" and that our family member could benefit from a brief stay (no longer than 20 days) in a skilled nursing facility/rehabilitation center to build strength and undergo both speech and physical therapies. At this time, our family member was non-verbal, unable to ask for assistance or press a nurse button, feed themself, or move freely to and from the hospital bed. Our family member required a high-level of care that Corvallis Manor did not provide. The family was told that there would be no patient onboarding until the following Monday. This meant our family member would not be meeting with any doctors, physical therapists, speech therapists, and that there would be no patient plan for at least three days. The family did not receive any sort of introduction to the facility, onboarding, or even a brochure with contacts/additional information. A family member was able to get in touch with the doctor assigned to our family member's case via his on-call number (Dr. Ruden), and was told he no longer worked for Corvallis Manor. Our family member was assigned a doctor who did not work at the facility. Following a surgery at Portland Adventist, our family member had a drainage tube at the surgery incision site. This tube needed to be flushed once a day and the bag needed to be maintained and emptied twice per day. Staff were unable to perform the flushing of the drain and had practically walked away from the situation when they could not figure out the medical device. Family, although learned on how to drain and remove the bile/fluid, had to request this to be done by staff, as nursing staff was not present unless called upon. Additionally, a member of the nursing staff (Shannon) said she did not have any orders to flush the drain. Our family member was diabetic, and the family was routinely testing his blood sugar at Corvallis Manor. Nursing staff did not test blood sugar until requested. When our family member was testing high (341), the family called nursing staff and it was 30-minutes until the nursing staff was able to take a reading. The blood sugar level reached above 400 when tested by nursing staff, a dangerously high reading, and the nursing staff was unable to administer insulin until 1.5-hours later, after the family pressed the staff to make a decision on the amount of insulin to administer. At Corvallis Manor, our family member tested positive for Covid-19. Despite Covid-19 protocols being expressed, such as receiving meals in styrofoam and being bagged separately when discarded, for example, these protocols were not followed, leaving others at risk. On the morning of Sunday, September 10, the family arrived at Corvallis Manor at 8 a.m. to find our family member nearly naked in the bed with the blankets on the ground. When asked, staff could not explain how long he had been laying like this. His wife decided to stay overnight on Sunday after this. On the morning on Monday, September 11, the family was told by staff that our family member would be released, after the family had pressed for a home release into hospice since admittance on Friday, September 8. A meeting with a hospice nurse took place and our family member was set to be released at 9:00 p.m. via gurney transport. 9 p.m. came and went and the family had to press staff to look into the transport and release orders. Shortly after, transport arrived. Our family member was released into home/hospice care and was transported home late on the evening of September 11. He passed away on Tuesday, September 12 at 3:30 a.m. Please let this be a warning to other families, do not allow your family members to be admitted to Corvallis Manor. As a CNA for several facilities, this is one of, if not, the worst care centers I have every seen. My aunt was there for over a year and this is what I saw/heard. There was little to no communication with family. My aunt could not eat a lot of the meals and the staff would not get her food that she could eat. The nurses would stop or change medication WITHOUT DR'S KNOWLEGE OR PERMISSION (this happened at least twice). It would take staff on average (and yes we timed this over the course of several months) 45 minutes to 1.5 hours to answer her light. Which means she would be left with a wet or soiled brief or in EXTREME pain for that long. There are several other things I could mention such as part of her charts going missing; my aunt getting sores because the aides refused to listen to her or clean her properly, her not getting showers for WEEKS at a time; etc. Overall I would NEVER recommend this facility to ANYONE. This place is a "SHIT HOLE" and if you have family that you love there, check on your family... I worked agency at the facility on Sat 12/7 and when I first walked in I knew the kind of day I was going to have. I received a piece of report of a cna.. and he gave me the wrong pt information. LIKE a hospice patient that wasnt an actual hospice pt, said he had just did rounds, a pt he told me was contient was infact INCONTINENT... He was saturated til the piss was dripping from mattress. The brief all the cotton and jelly along with his wound dressing fell out the moment I went to change him.. I had a Med Aide ask me if a pt had a dressing i laughed and looked... she got cocky at the mouth "If youre changing thing you would know that".. I said, "No nurse if youre in his Mars&Tars you should know which pt had wounds... but just a FYI he has a colostomy and thats in his belly not coccyx... It was a very unorganized day and I can truly see why they facility is full of agency workers. Then what made it official.. The med tech was butt hurt so i guess she sent the nursing manager a message stating 2 pts complained about me. And she need me to leave the facility..(which was already my decision after the way the work in the facility).. So the moment I left the building went into my car to write a report about the facility they blocked it from happening.. So i called my agency and notified them of the choas in the facility.. i truly dont see how the state is allowing the facility to stay open... read more

They had some negative reviews and this worried us when my wife had to go there for rehabilitation. They must have new management or ownership. My wife received outstanding care. The physical therapist was better than my wife had in the hospital. The food was very good and the staff were responsive and very supportive. It is a shame the negative reviews aren't deleted. This is a good place to have the loved ones cared for, however there is a boatload of confusion, it may seem as to how to allow people to come, and go to conduct visits, and or business as I have seen many times people standing for as long as ten minutes, outside, trying to gain entrance. This would seem to be quite an easy fix, yet there they stand,..still. these people are important, just as you or I. There are people inside that need their visits, there are jobs that need to be done in caring for the individuals that reside within, and they all wait expectedly, yearning to be in out of the weather to be there for the reason that they have made the trip. read more

If you are looking for a place to go rehabilitate yourself or a loved one. PLEASE DO NOT TAKE THEM TO CORVALLIS MANOR. I had a double hip replacement and was taken there to get well. What an awful place to go. It smelled like death when I was brought in from PEACE HEALTH. When I was in my room, there were 2 people looking at my wounds. (keep in mind I had to go thru test to make sure I had no mersa bacteria on me.) As they are looking at my wound some young kid comes in and starts sweeping about 10feet away. I had to stop him and then ask why they didn't. They just looked at me like I was dumb. Then the night nurse comes down to my room and tells me that if I use the nurse call button it could be 5 to 90 minutes before I got any help. Then the same nurse almost doubled my pain meds. I had to ask for ice packs for my hips. I had to ask for my meds. Needless to say i got my primary dr to get me out of there. I normally do not write ratings, but my experience with Corvallis manor has not been positive in any way. I have been trying for over 3 weeks now, to contact a patient there whom is in hospice and is failing rapidly. I first tried to visit over a month ago, and because of corvid, She could only be brought outside, which is understandable, which she was not strong enough to do. i called the administrator to ask for a compassionate visit to her room, which I am entitled to. I was told that she would get back to me. She never has. It has been over 2 weeks. I have called her twice more, leaving messages and call back numbers, to no avail. My friend is dying. I called again today, for the 4th time and was kindly told by the director of nursing services that i can visit my friend anytime i want. The administrator, Mary has never returned my calls. My husband just spent 9 days there and was treated with respect and dignity. His call light was answered in a timely manner. The food wasn't too bad, just small portions ( which wouldn't hurt most of us ) I even had one lunch and one dinner with him at no charge. They even paid for the medical transport from McKenzie Willamette hospital to the Corvallis Manor for rehabilitation from a big back surgery to remove a tumor from his spine. We found all the workers to be cheery and supportive. I'm not sure what death smells like but we didn't smell anything bad. The only negative was that they don't have air conditioning and it was very warm. read more

If I could give NEGATIVE 100 stars I would. Took my mom here and had the WORST experience in my entire life. I would go daily to check up on her and she was always uncomfortable and not taken care of. The food was from what looked like dollar tree and was not what she should have been eating to regain her strength. The workers there were all rude and gruff and were very annoyed if I ever asked them to get my mom something. She had to share a bathroom with 4 other sick elders. The whole place is OLD and very run down. I had a traumatizing experience here and so did my family. She did not get better being here she got worse. Which was no doubt the reason for her death soon after we got her out of this place. read more

I went to Corvallis Manor from Good Samaritan Hospital on February 14, 2020 after having double knee replacement surgery. For 6 days I had to endure hostility, incompetence, inedible food, long wait times for pain meds or assistance, as well as having to show some nurses how to put the ice packs on my knees or assist me to get off the bed to use the walker. One time I even called 9-1-1 to get them to call the facility because I couldn't get anyone to answer the call button for an hour and a half in the early morning hours, and I didn't know the phone number. Some LPNs who were new to their job really cared and were sweet and compassionate, but the others were cold and insensitive. Overall, it was a horrible experience, and as soon as I could get myself in or out of bed, I went home. Fortunately, I had frozen several soups and filled my fridge with nuts, and fresh fruits and vegetables which my friend brought me on a daily basis. If you have a choice, don't go to Corvallis Manor to heal from surgery. It is a depressing environment. read more
