My mother was in for rehab in January of 2024. While the appearance of the facility is outdated, the service was good. The nurses were great. There was one nurse who was the bad apple out of the bunch. And I believe she was "let go" due to multiple complaints. According to my mother, the food was good as well. You didn't get a choice of what to eat, but everything was good. Especially, the ice cream (lol). Other than the appearance of the facility, everything we've experienced was above par. People also need to realize that, with all rehab facilities, the staff is cut down over the weekends. It's not the service. Things take longer because of the lack of help. However, that doesn't mean that you don't get the help that you need. My mother still received her PT, meds, food, etc., on time. I recommend Avalon for rehabilitation. read more

I had hoped that we had finally found the right rehab facility for my husband who is recovering from an amputation due to diabetes. A previous experience with this facility was very positive and I had so much hope in this facility. Generally the staff is kind and attentive. Unfortunately, after three days....they were unable to manage his diabetes and rather than listen to me as a person who has been successfully managing his diabetes for the past 10 years....they are by policy and protocol required to refer to a house physician who is not an endocrinologist. Nursing staff must wait for the doctor to find the time to call them back while my husband's blood glucose just kept getting higher. It's sad that the nursing staff isn't allowed to use common sense and deliver a shot of insulin that is more than the physician has ordered. Our health care system is so very broken. I guess my advice would be....if your loved one is a diabetic....you might want to choose another facility. read more

HONESTLY THERE SO MUCH BAD ABOUT THIS NURSING HOME. Pretty much if you don't care for your love one then leave them there but if you are like me and my family REMOVE ur love one ASAP. This place is so horrible my dad was there for 3 months and let me tell you the hardest 3 months of his life. WAITED 14hrs for a diaper change & when finally the nurse comes he tell hers and her response you have to wait I'm BUSY. The workers are heard in the hallway by patients how the staff will go back and forth with each other on who is going in next to help a patient in need. FOOD: There's no set schedule they come when ever to feed you breakfast can be at 8am 10am lunch can be at 12pm 1pm or even 2pm and dinner comes at 830pm 9pm my dad is diabetic his sugar went down drastically. This is to all staff members from this facility. I'll take out maybe two workers that we had great experience with, but for the rest if you took on this job, treat these humans with respect just like if you would treat one of your family members they're already going through a hard time humiliating them it's not OK belittling them it's not OK making them feel like if they're a bother it's not OK! because it's your job you're getting paid to do this job , they are ready in a tough spot some that can't even move from bed like my dad was GOOD LUCK AND I WILL PRAY FOR ALL Patients in this facility. Great job in facility overall for my father who had a stroke/chf. Kudos to the nursing staff for my fathers ongoing recovery.

Understaffed. Food is terrible. Clothes lost. Mom had light on for an hour for someone to take her to the bathroom. No one ever came until I went to the desk to complain. read more

Hamilton Continuing Care Center or HCCC is truly an unfortunate destination for loved ones to have to live out the rest of their lives. As you walk in you are greeted with a strong almost eye watering urine smell. The staff is a mix of, some friendly people, while the majority are either rude or arrogant. The residents shared rooms are tiny, I'd say the bare minimum under the ADAAG and make wheel chair accessibility difficult. The provided in room TV's are about the size of a large tablet making it a struggle for elderly people to see. But of course they will allow you to donate a better TV. The nurse call buttons also don't work leaving residents sitting in their own feces for hours before a staff member takes them to be cleaned up. The laundry service is a disaster, clothes are constantly lost or mixed up with other residents. The meals/food are un-appetizing, often cold and sometimes have un-identifiable objects in it. And are not cooked to any dietary guideline or restriction.Oh and like previously mentioned they meals are billed out at ridiculous costs. The staff doctors have more patients then they can manage and end up not visiting with the patients regularly or talking with the patients, nor talking to the specialized private doctors or even keeping up to date with the patient meds. The facility and staff do not notify the family members about anything other then getting their money. Never get any notifications about changing meds or changing transportation arrangements which in turn make the patient miss appointments that you then have to pay for. Truly disgusting. read more

Pleased overall with the care they are providing to my father. The nursing staff is companionate and helpful and informative with what we need answered. The admissions director Sue has been nothing short of amazing with all of our questions and needs. I would say that some of the aids need to be addressed and questioned in regards to there job duties. As a daughter with a father whom is ill, I do have to say to ANYONE, that you must also be present and there to make sure your loved one is cared for. This is not a dumping ground and to be present is to make sure your loved one is cared for properly. I would absolutely recommend this facility. read more

This is advertised as combo nursing/rehab center, however, it really is a nursing home with a rehab room with the avg. age of patient/resident around 80. Facilities are subpar with very sad looking common rooms and strong smells. If you are looking for rehab and/or have a medical condition, I wouldn't recommend this place. Staff is friendly for the most part but facility is run down. I would recommend having an advocate there if the patient is not fully with it because we had incident where dr changed meds without notifying family or patient and only found out when saw pills being given to patient. Absolutely awful since the meds were not suppose to be given per the patient's specialist. The food is HORRIBLE and more importantly is not cooked to dietary restrictions. They served a sandwich for dinner while charging insurance $300/ night. It comes ice cold too...which makes gross food taste even worse. Physical therapy is once/day and about 45minutes and not always one-on-one session. It would be better to do an outpatient program elsewhere. Going to Care One if need rehab in future...not here. read more

Clean with very caring staff. Shared rooms are entirely too small. Patients are allowed to store too many personal items in containers making It difficult for wheelchair mobility. read more

Update on my previous review....I did find two rehab rooms which had appropriate though limited pieces of equipment .

After taking care of my mom for 6 years, she is paralyzed, bedridden, very little speech, but is alert and understands you, I put her into sub-acute Hamilton Grove. DON'T EVER GO TO HAMILTON GROVE. It looks like a funeral home when you walk in, dark and dingy. Up in Rehab where she stayed for respite care, the aides were phenomenal. I wrote up a whole bunch of instructions and they followed them to a tee. Mom is on pureed food but it wasn't that great at all. She pushed a lot of food away. She had to go back another time, and I thought this would be for permanency. Was I ever wrong. They stuck her in a corner facing a wall. She looked so pitiful. I immediately called LIFE @ St.Francis Medical (what a great group of people to help the participant and the caregiver if living at home). LIFE removed her and I requested Hamilton Continuing Care. She is still is sub-acute waiting for a permanent bed. I am so very pleased how all my instructions are followed, how well the aides take care of my mom, the front desk receptionist (2) always friendly. The food is out of this world. My mom can't get enough. Even though pureed, I taste it first and you can actually taste what the food is that they say it is. Their Chef (s) is fantastic. Only thing on weekends you sometimes get leftover food but its still tasty. I am on medical leave for 2 weeks and can't drive to see her, but called people in the area to stop and visit her. Family and a few friends. She was also put onto an air pressure mattress and knock on wood is not on any pain meds like she was at home. She is quite happy. I think they are doing a great job, in fact, some people there known my mom from childhood. I expect my mom to live a pretty long life in HCC, can't wait to decorate her own room. Nurses, aides, chefs, everyone is very very nice. I would like to say that Dolores is an excellent aide for my mom in sub-acute. Warm and loving.
