Nurses, CNAs, PT & OT staff would all just pass the room and not see whats the problem. What if it was a medical emergency? The physical therapy staff are fantastic. They work well with the residents. It's hit or miss with the nurses.

We had hoped that the stay there would help him to become more ambulatory with PT. From the moment we arrived, we realized it was a mistake to have chosen the facility. The night staff left him sitting on his bed immediately upon checking in while we waited for someone to come with medication. It was like he was forgotten about as soon as he arrived. The room was dirty, the following day I had to report the bugs seen in the room. His roommate clogged the toilet so it was unusable. And staff told my father they could not clean it so he was left without a bathroom until the following day. My dad was a fall risk and no one came to help him go to the bathroom even when he rang for assistance. He fell one day while I was at work and he had to scream for help and laid there while no one came. One of the biggest issues was with medication---his blood thinners were not being administered consistently. We were told the pharmacy was out of them. He ended up with clots in both legs that put him back in the hospital (eventually had strokes as well). They also did not carefully monitor his blood sugar and I found him to have extremely high blood sugar more than once. The nurse assigned to him acted like I was an inconvenience for asking for help and I had to call his doctor repeatedly to get anything done within the facility with his medication. I could also hear staff yelling at some of the patients with obvious memory issues and it appeared that many of the patients are ignored. My dad ended up in the hospital and we never returned to Care One. We chose home hospice at the end of his life which was a much better care setting for him and our family. One of my biggest regrets was ever allowing him to be admitted to this facility while he was ill. It was a place of nightmares and if you have a family member there I recommend staying with them as much as possible so you can advocate on their behalf. Even better, don't take your loved ones here. The people who own this facility should be ashamed of themselves.

He requested kosher food, and was provided with a very limited diet that essentially consisted of stuffed shells and sandwiches - a very poor diet, especially considering he was supposed to be building back up his strength and endurance after pneumonia. Communication (the lack thereof) continued to be a major problem. After yelling and screaming several times, we finally got little bits and pieces of information from the facility. While at Cranbury Center, he contracted Covid (and was sent into "isolation" as a result), and then suffered another bout of pneumonia. The facility at least called us to tell us about these occurrences. Finally, he had shortness of breath and a low oxygen level, which resulted in the staff releasing him back to the hospital. Thank goodness he was finally able to "escape!" To add insult to injury, the facility then sent my parents a bill of close to $3000, since they'd kept him for more than 21 days and his Medicare only covered the first 21 days. The facility's lack of professionalism and transparency have been absolutely appalling, and my father on more than one occasion told me he felt like he was not going to survive his stay there. We have told the hospital that, if he needs to be released back into rehab, under absolutely no circumstances was he to be released to Cranbury Center.

I came to the Elms because I needed to get rehab for a total knee replacement. Bad choice from the…read morebeginning. I received rehab for 1/2 hour a day (Only if I asked} At no time did anyone come to my room to schedule a time to go to PT. If I strolled into the PT room I would quickly shown a bike to sit on. How is that possible, no appointment??? I always had to ask for my sheets to be changed, never did they make it part of their daily routine. Often, I just gave up asking. Lastly, the reason I felt compelled to write this review was for the following incident that must be exposed. I received breakfast as usual around 8:00 AM on Sunday April 11th. When I received my meal it was not eatable and was cold. Things that are cooked should be at least warm. When I as for it to be warmed up the attendant REFUSED TO GET IT WARMED UP BECAUSE SHE DID NOT WANT TO WALK BACK TO THE KITCHEN TO SOLVE THE PROBLEM. The administration ASKED the attendant to apologize but refused. Anywhere else on planet earth immediate expulsion would have been called for. Instead, the attendant thumbed her nose at the administration and merely a useless grievance was filled out. {We all know where that will end up) In conclusion, if you want evidence that the system is broken here it is. My mother was a long term care patient for almost two years. Care was barely adequate, we had to…read morealways advocate for her to get things done. Facility is old and it shows. Bed linens are threadbare and food is horrible with hot food/coffee served lukewarm at best. Some of the staff are outstanding but the good ones are overworked and understaffed. A lot of staff just go through the motions with an attitude. A lot of temp staff, especially on weekends and holidays. Got a call after thanksgiving that mom was discovered in bed with two black eyes. They tried to claim she fell out of bed. Mom was bedridden so even if she fell out of bed, how did she get back in on her own. Administrator was useless and continues to be so to this day. Avoiding our calls as we try to get a copy of the incident report. Police were called and they are just as useless, saying there's no proof that a criminal act was committed. We transferred mom to another facility. It's disgusting the way the elderly are treated.

When we started our search for a nursing home for Dad, we knew we wanted somewhere in close…read moreproximity to Oradell, because most of us live there. Dad did not need a home that looked 'pretty or fancy'; for us it was about the quality of care he was going to receive. The Armenian Nursing and Rehabilitation Center was a place most of us passed on a regular basis because it was on the way to Shop Rite. It was recommended to us by one of our in-law's families whose parent was a resident at the time and had so many nice things to say about it. When we visited, we were impressed by the staff and had a good feel for the facilities. It was important to us that Dad received one-on-one care and we were offered a private room on the sub-acute wing. We checked Dad into his room and within a week they had moved him closer to the nurse's station so they could keep a closer eye on him. Dad was 88 at the time and suffering from dementia and physical limitations. Fortunately, he still knew who we were but needed assistance walking and getting dressed. Before moving Dad into the Armenian Home, he had attempted a few escapes at home. The staff made sure he was always wearing a monitor so they would be alerted if he left his chair or bed at any time. Over the 4 1/2 years that Dad lived at the Armenian Home our family would visit regularly. There are six of his kids, plus spouses, as well as grandkids and friends that would stop in at any time of day to visit or help feed him. We were always greeted by the staff at the front desk with smiles and friendly banter and the aides were all very friendly and accommodating. If we wanted to feed Dad, they would make sure that we had everything we needed, especially apple sauce, his favorite. You see, later in Dad's stay, it became more difficult to feed him and maintain a healthy weight. Fortunately, the staff members were aware of all his favorite foods and knew just how to get Dad to eat when we couldn't.....they have their tricks!! We cannot say enough about the overall care that the nursing staff gave Dad throughout his stay. We were always notified promptly if there was ever a concern about his health. Everything from a head cold, to a fever, to a reddish color on his big toe. In the 4 1/2 years that Dad was a resident at the Armenian Home, he never had to be taken to the hospital. They were always able to care for him and keep his health in check. The nursing staff was always there in case we had any questions or concerns. When we decided to bring Hospice in to help manage Dad's pain from his lack of mobility, the nursing staff worked closely with them and was in constant contact with us if there were any changes in Dad's condition. Never once did we doubt the quality of service Dad was receiving. During the last week of Dad's life, many of us were coming and going, spending hours just congregating in his room. The staff allowed us to spend as much time with Dad as we wished; even permitting overnight stays when we felt the end was nearing. We recall a staff member from the kitchen stopping by and asking us if we needed anything to eat or drink. The entire staff went above and beyond to make us feel comfortable. We are so grateful to the Armenian Home and especially to the staff on Dad's floor for the care he received. From the aides to the nursing staff and all the doctors that monitored him, they all treated him with love and respect. We had total confidence and faith that Dad was in good hands, which gave us great comfort and peace of mind.
