My wife was there recently for 7 days. When she would ring the help button it would take up to 45 minutes for someone to come in and check on her. Nurses, CNAs, PT & OT staff would all just pass the room and not see whats the problem. What if it was a medical emergency? The physical therapy staff are fantastic. My father was in Care One in the late spring of 2025 after having a bypass surgery in his abdomen and long hospital stay. We had hoped that the stay there would help him to become more ambulatory with PT. From the moment we arrived, we realized it was a mistake to have chosen the facility. The night staff left him sitting on his bed immediately upon checking in while we waited for someone to come with medication. It was like he was forgotten about as soon as he arrived. The room was dirty, the following day I had to report the bugs seen in the room. His roommate clogged the toilet so it was unusable. And staff told my father they could not clean it so he was left without a bathroom until the following day. My dad was a fall risk and no one came to help him go to the bathroom even when he rang for assistance. He fell one day while I was at work and he had to scream for help and laid there while no one came. One of the biggest issues was with medication---his blood thinners were not being administered consistently. We were told the pharmacy was out of them. He ended up with clots in both legs that put him back in the hospital (eventually had strokes as well). They also did not carefully monitor his blood sugar and I found him to have extremely high blood sugar more than once. The nurse assigned to him acted like I was an inconvenience for asking for help and I had to call his doctor repeatedly to get anything done within the facility with his medication. I could also hear staff yelling at some of the patients with obvious memory issues and it appeared that many of the patients are ignored. My dad ended up in the hospital and we never returned to Care One. We chose home hospice at the end of his life which was a much better care setting for him and our family. One of my biggest regrets was ever allowing him to be admitted to this facility while he was ill. It was a place of nightmares and if you have a family member there I recommend staying with them as much as possible so you can advocate on their behalf. Even better, don't take your loved ones here. The people who own this facility should be ashamed of themselves. read more

This place was absolutely awful. They over medicated my father and were never around to help him use the bathroom. He fell numerous times because there was no supervision. We couldn't have gotten him out of there quick enough. They also lost several of his belongings including his hearing aids. read more

I'm a medical transport driver and I was curious how the food was here so one day I asked if I could try some and they charged me $5 to have lunch here. The first course was a lovely vegetable soup with big hearty chunks of veggies. Needed a little salt but was quite good after that. Then it came with a cheese burger and fries. The fries could've been crispier (they are baked, not fried) but the burger was good. You could taste the meat and the cheese, the pickles were thick and crunchy, and the bun was fresh. Only complaint is that it would've been better with some thick cut bacon in it. Also came with unlimited drinks, I chose apple juice. read more

We just had to go rescue our grandmother from SCARE one. She looked half dead and no one noticed. They fought us every step of the way to get her to a hospital, insisting they could provide equal care (why weren't they). Hospital was annoyed of the lack of care given and the fact that she had an infection and stress on her heart . Not to mention they argued me with what my grandmother looks like and acts like. If you want your loved ones neglected, send them here. read more

Please do not send your love ones there for therapy, it's dirty they have uncaring nurses. Some nurses don't sterilize their hands before and after entering. CareOne is well named, 1/10 stars. If you are looking at some of the lower ratings from when this place was called The Willows, you should be very afraid, because the quality in every area seems to have gone downhill since they went from non-profit to for-profit. In particular the quality of the food is worse than a public school lunch for elders who need more nutrients and less junk in their diets. Which would be understandable if the room and board was not so pricey. Medical care is dubious and I have heard tales of probable malpractice from friends with loved ones here. It is very creepy to walk down the halls since the facility became CareOne as so many residents look overmedicated and in wheelchairs compared to just a year earlier. read more

I'd rather have sent my mother to jail than to Care One Holmdel. She would have received better care!!! My mother arrived with a broken hip and just needed OT & PT. She received good therapy and attention from Nikki and Ellie. Wonderful at what they do. That's where my good review ends. The night staff is horrendous and left my mother without pain meds (Tylenol) for hours despite asking over and over. One morning she had to hold her bladder so long that she called me (I live 1.5 hours away) to tell me "I'm about to burst". Terrible. I called Care One to tell them to GO CHECK ON MY MOTHER. Once I arrived, I spoke with Crystal who is in charge of the nursing staff. Her line to me was "call me any time....ANY TIME!" So guess what?? I needed an important question answered before the weekend. My first call was at 2:30pm and left message after message. Come Monday morning when I went back down guess what her reason for leaving on a Friday and leaving me to hang all weekend was....."I had to go to my cousins' son's graduation". Hmmmm, ok, great. NOT! We're all entitled to time off but when you tell me to "call any time"....I believe you. If this is how you represent yourself as a manager/leader, what standards does your staff have as you mentor them? The night staff is awful. No one wants to be there and they shouldn't. My mother suffered a medical emergency and pulled the alarm for help....no one came. Mom's 93 year old roommate SCREAMED for help....no one came. FINALLY MY MOTHER CRAWLED from her bed in to the hall way.....the uninterested staff stood over her for several minutes...no one talked to her, no one wanted to help her. HORRIBLE CARE....CARE NONE should be their name. Best move ever was to have her taken to Bayshore Hospital where she had a restful sleep and a decent meal. Don't get me started on the food. Where does this stuff come from? Is there a kitchen on site because if there is, it needs to be shut done. My mom lost weight in the 2 weeks she was there. Did anyone pay attention to the fact she "couldn't eat the food?" Slimy chicken, mystery meatloaf, grilled cheese that she couldn't even open the bread up to put mustard on because it was like glue. OUTRAGEOUS. DISGUSTING place and I feel horrified for anyone who goes there. YOUR STAFF needs to be replaced. THE NURSES AIDS DO NOT CARE for the patients. Do a clean sweep. One star given ONLY because of the OT/PT staff! read more

The quality of care my mother received was awful. She needed assistance going to the restroom since she broke her hip but they took so long to respond. Their solution? A diaper!!! Instead of helping when she needed the help, they put a diaper on her so they could come at their own convenience instead of actually doing their jobs. The food was terrible. The "care plan" wasn't created until we complained days after she arrived. The manager was rude, disrespectful, and not the least helpful. He said if we had a problem with the nurses it was our responsibility to follow the "chain of command" and speak to them (which we did to no avail). We are not employees and do not need to follow the chain of command! We are paying clients and should be able to go to management any time we wish. Don't put your loved ones here. Terrible. I wouldn't give this place a star at all!! Very dirty,only a handful of staff i would recommend. They are very unprofessional and lazy. Please if you have a loved one don't ever put them here. Liz a head nurse in charge always took excellent care of my mother in law. Destiny a aid took better care of my mother in law then most nurses. The only good thing that came from this shit hole is all the physical therapists. I had to give a star or it wont post... Don't Go to Care One in Homdel. Also it was very sad for otherPatients crying for help. Sitting in there soiled diapers waiting to be changed john a aid was new and he treated my mother in law horrible. I complained and told the administrator what he did. I requested from him to never step foot in her room.... I have a family member who has been here for a few months....the staff has been kind, patient, friendly and all around great! This is not a slick looking, modern facility, it's more homey and old fashioned. We have been very happy with the care that's been received here, and would highly recommend this rehab. read more

The CNAs and aids had an awful attitude. They often had to be reminded to give me my prescribed meds and when I did remind them, I often had to wait hours to receive my meds. I would give this place zero stars if I could. Sending any of your loved ones here is a death sentence. There are rude staff members, my mothers gold chain was stolen. When my mother passed away they left her roommate in the room the entire time paramedics worked on my mother, they did not even take the roommate out first. If my mother was brought to a hospital and had better care she would still be alive today. They did not provide her with the care she needed. Truly an awful place and should be shut down. read more

Although I liked Care One when we were looking for a place for my sister to Rehab. I found out a lot is to be desire of any care facility. First of all they were very accommodating to get your love one there. The place was ok. The staff were very caring and did what ever they needed to make my sister stay as comfortable as possible. The only issues we had was that she constantly complained about not being fed another and her discharge was a disgrace.! We were constantly having to bring her food cause she was always hungry. The portions for an elder person I can get, but when you're 63 yrs old the portion was starvation size. We constantly had to bring her food! The rehab was excellent and although she was on weight restriction they did get her up and walking before she left after being bed ridden for 18days. The discharge was a mess. No meds were available for her discharge and what they promised us wasn't given. (30 day supply) wasn't given. Needlless to say she was sent home with pain meds to get her through the weekend, Which was a disgrace! Please if you send your love one's her you have to sty on top of them to do there job! read more

My mother died here. The place is awful. She had Alzheimer's and everyday I came there it was if she was left for dead - They lost her teeth, had a stranger's shoes on her feet about 2 sizes too small, they left her staring at the walls and slumped in a wheelchair. I would give Care None zero stars if I could. My father wasn't here long but the staff was very uncooperative and even more so uncommunicative. But their biggest failure when my father transferred to a much better facility they forgot to send his clothes with him, and when they finally did 5 days later they sent the clothes dirty. You think that's bad in the process they lost his dentures. They said they were sending a dentist to fit him for a new set which they never did. If you are looking for kindness and compassion for your rehabilitation, this is the place to come. Without getting through a long story , I was transferred here from another facility. It was the best gift that could have been given to me. From the nurses to therapists , aids and entire staff , nothing less than 100 percent professional.Have I mentioned the food? Fantastic!! TERRIBLE. Do not bring your loved ones here, it is basically signing them over to a death sentence. Two of my family members died while in this care home facility. While some of the staff is nice, a lot just slack off and are very rude. My grandmother wouldn't be changed for hours sometimes and the one staff would yell at her for wetting the bed. My grandfather couldn't pick stuff up or move really and he was very dehydrated and i asked if they were giving him liquids and she pointed to a table a foot away from him with juice on it and said it's available for him to get if he wants it. HE COULD NOT MOVE. Awful place, don't ever bring them here. Sucks the money out of you as well for the most BARE MINIMUM of care. Awful read more

Care One employees did a wonderful job taking care of my father, Don Johnson. Robert, Director of Nursing, was kind and played an active part. Joann, his day aide, went above and beyond and gave him fantastic care. She will be missed. Mary from speech spent countless hours with him. The Director of Activities, Michele, was great in giving my dad attention and provided really good entertainment and activities. The head Chef made visits to his room to check for any special requests. It was appreciated. Sadly, my father passed at home but my family will always be grateful for the kindness shown to us at Care One Holmdel. read more

My mother was a resident and I saw Careone as a family member. Then I broke my hip and saw Careone as a patient. I went into the hospital again recently and was very nervous when they sent me home. Luckily for me, I was very educated from my last stay at care one and I knew I could call them to help. Everyone welcomed me with open arms when they saw I needed some extra TLC. My days are filled with great food, activities, and most of all the therapy I need. The nurses were able to regulate my medications and the cna's took great care of me! I cant not thank everyone enough. I cant single out one person as the greatest because everyone here is educated and respectful. read more

The therapy was very good, I was very pleased with that part. The nurses were good, One very good one. The aides were not good at all. The activity's were good. The room was clean, but the aides did not keep up on keeping my husband clean and dry. Bed was at times made but still wet from him having and accident during the night. 3.5 months all CareOne did was overdose my mother on medications that she did not need, leave her to sit in her bowel for hours which caused her to have several infections, didn't clean her properly, returned calls back when it was easy for them. My poor mother would call me crying saying she was sitting in her bowel movement for 3 hours . I saw my mom at one point at visiting , she couldn't even lift her head up or look at me straight because she was so doped up on different downers. I was never told of new medications given to her . Social workers never helped me with anything all they helped with was telling me I need to find a new place for my mom bc insurance was running out . Pressuring me everyday until I had enough and got her transferred into a better place . She was evaluated at new location and they couldn't believe all the medications they had her on. My mother is back to herself and can actually speak now. I am definitely going to take my complaints elsewhere besides Yelp. Do yourself a favor do not send your loved one here. read more

Care One should be called Care None as they are a disaster. My Mom was placed their for rehab after colon cancer surgery. She has an ostomy bag and the nurses at Care One have no training or idea as to how to handle this. As a result of being in residence there for one week she ended up with a bad fungal infection at her ostomy site and we had to transfer her to a different facility. The managers kept making promises that were left unfulfilled with her being in worse shape after a week of rehab there rather than better. They are mismanaged, understaffed, under qualified and generally uncaring. This place should be shut down pending investigation by the state. read more

I have to give them one star because you cannot submit a review with no stars. I wouldn't send my worst enemy to Care One in Holmdel. Every single staff member is incompetent. They ignore and neglect their patients. My father was there several years ago for over a month, until were finally able to find someone to care for him at home. I've seen animals treated better than they treated their patients. I reported them to the State of NJ and unfortunately the results of their investigation are not divulged. Stay away!! read more
