    Our relative used to be in independent living. When she went into memory care two years ago, things…read morehave gone downhill. Mind you, independent care wasn't the greatest, but it is very, very difficult to transfer a loved one with dementia to another facility. Therefore, we are still at Heritage Crossing. We have addressed issues with them several times. I had addressed an issue a week and a half ago in regards to my loved one that her room was filthy. Her bedsheets were filthy and they needed washed. The person I addressed this to was River, which is a new memory care director from what I'm told. My husband also was in contact with Rebecca, which is the director. I myself have tried to call corporate several times. You will never be able to get a hold of them. Ever. They make it so that all you can do is leave a message. UNLESS you are looking to sign a family member up at one of their communities then they will talk to you. Fortunately, I was able to get through with Amy who handles the billing and she was getting in contact with Caitlyn who is a vice president up at Corporate. I had to call Amy back today to let her know that I still hadn't heard from Caitlyn. Amy assured me that Caitlyn would call me back that she is very busy. Mind you, Caitlyn was made aware through Amy what my issues were. But Caitlyn is too busy. I gave Caitlyn plenty of time to call me back. Once you are a resident there and you are paying them $7000 a month for memory care, this is what you are subject to. Please see the pictures below. The first picture is blood and urine on my loved ones bedsheets and linens that have been there for at least two weeks. They have not been washed even after I asked. The napkin that you see on the bed is "stuck" to her bed. The picture of the floor and the toilet... that is feces on the floor and on the toilet seat, there is also dried urine. The clothes... They fold them and then they throw them wherever they can in the room, not organized at all. The room is absolutely disgusting. I have several more pictures that I will be sharing online. This is just one instance. I have several instances. I am absolutely at a loss for words and thoroughly disappointed. Not one person... not one has reached out. Instead, it is going to be my responsibility to clean up after my loved one that I paid them for. Beware before you commit to this facility. Talk with a residents family, especially back in memory care that has been there for a while. You should be able to get all aspects of the community. Not just what they tell you to get you in there.

    We had a very bad experience in our short time at StoryPoint of Medina…read more The predetermined 'enhanced' support that was sold to us, and was so desperately needed did not happen with any consistency and when concern was expressed by us, rather than improving services we were met with a lack of empathy and no consistent improvement--in my opinion a managerial issue We had been talked out of the assisted living we originally sought and into 'enhanced' independent care. After a couple weeks we expressed concern that assisted living was needed. There was no follow up from StoryPoint staff about his assessment for assisted living or to make arrangements to have him moved there despite our concerns for his worsening weakness. He was hospitalized shortly thereafter. We decided that we were not comfortable sending him back to StoryPoint upon discharge from the hospital and were fortunate enough to work with another assisted living facility to get him admitted there upon his discharge. Unfortunately he passed away in the hospital. No one called to check on my dad while he was in the hospital for well over a week. StoryPoint does not have corporate information listed on their website or anywhere I can find so it is concerning that there appears to be no real oversight about decisions being made at individual locations.

They have been unfairly trying to throw me out for so long, finally this weekend they resorted to sending me to the hospital for a blood clot that was getting bigger and then call the social worker at the hospital and tell her not to bother sending me back. All because they refused to send the correct paperwork to my insurance. And this comes after one of the social workers threatens me with physical removal and gets about 2-inches from my face... And also they try to Stage a illegal home inspection at my Mom's house when it's clearly the duty of the therapists to do that and they have nothing to do with it..... I did nothing at all to deserve this treatment i was always nothing but nice and respectful to all the staff and the aides and whatnot and everybody got along with me.... So yeah if you wind up going here don't bother defending yourself because this is how you'll be treated if you do. Also beware that some of the aides here are abusive ( one of them left me nude in the bathroom and covered in excrement) and some of them will lie on your chart and say that you did more than you can actually do....     I am a court appointed guardian of a gentleman in Baths memory care unit. I received a call from…read morehis nurse three days ago. My ward was unresponsive and paramedics were called to administer Narcan. I requested details of the incident but received no return calls. I went into Bath yesterday to check on my wards condition and to ask questions. Again, no one knew anything about the incident. I've called and asked for the administrators name and I went in person to the front desk requesting the name but no one knows the administrator. Strange! I had the finance woman's email and I did email her to pass my email along to the proper department. Why doesn't Bath have Narcan on hand? I can buy it at the drug store. I've requested drug interactions in the past. Most residents are on numerous meds. I need to know what happened. Today still no response therefore I had to file a nursing home complaint with the State and I had to call the Ombudsman at Area on Aging. If someone had simply returned my calls I would not have had to report to these agencies to get answer's. Also I checked Medicare.gov. This home has a ONE star rating on Medicare. Gov. The 3.5 rating on their home site is made up of several employees giving them 5 stars. Not residents or their families. Please do your research before placing your loved one at any nursing home. My ward was already placed here when the court appointed me as his Guardian.
