We retained Cindy Hasz and Grace Care Management after really going through the ringer with my dad who is recuperating in a skilled nursing facility. From the first phone call she provided knowledgeable and eye-opening insights. Her authoritative advice saved us immeasurable time and money. We were navigating a complex medical situation, compounded by an inappropriate termination of Medicare insurance coverage. All of a sudden we found ourselves navigating a complicated new Medi-cal application process, while the facility was trying to get us to transfer him out, pretending he wasn't eligible to stay on Medi-cal. .I'd give a 10 ! Cindy Hasz greatly helped us with our Mom 2.5 years ago. As our Mothers Dementia has progressed Cindy again directed us and made the right decisions. Her referral for treatment for our Mother changed everything. She understood our needs seemingly before we knew what we needed. She can come to the home to oversee Diabetic care and Blood Pressure issues. For those who cannot accompany their loved one to doctors appointments Cindy will take them and be their advocate. I thought this business was a different business when I walked in. Cindy came almost running up to me and assured me i was in the wrong place, while she pointed at the door with both hands and waved and said "bye bye" with both hands! I looked at her in disbelief at how rude she was being to me, but she continued shoving me out the door. I engaged Cindy Hasz, a certified geriatric care manager, to became my mother's care expert in what would be the last 18 months of mom's struggle with Alzheimers. She was referred to me by an elder law attorney and when I first talked to Cindy I could tell I'd found someone who not only understood mom's disease and home care needs but as a nurse could help our family navigate the medical care minefield for mom that dementia patients have to traverse. Her expertise guided our family in all those ways during those 18 months. Initially Cindy monitored mom's narrow ability to remain in her home safely. Cindy's Grace Care staff came to mom's home daily, prepared her meals, dispensed her meds and assisted mom with her grooming and incontinence issues. When mom could no longer remain at home, Cindy guided us through a circus of transitions from home to ER to hospital to skilled nursing to residential care followed by more hospital, more skilled nursing and more ER events. A number of times Cindy and her management staff were brought in to advocate for mom in order to resolve frustrating physician care issues that neither we her family nor her RCF could resolve on our own. And finally, in mom's last weeks, and after an increasing number of chaotic trips to the ER, Cindy intervened to obtain hospice nursing for mom: hospice care had been denied because mom had only "dementia" as a diagnosis. Cindy helped Kaiser ER physicians understand mom's need for hospice. As a result, mom had four weeks of peaceful and excellent hospice nursing care followed by a peaceful death in her own bed with her family and familiar caring residence staff by her side. Mom's Alzheimer's journey was more difficult than I ever could have imagined, and having the aid of a certified geriatric care manager like Cindy to assist our family was a godsend. Cindy was always a dedicated guide and mom's advocate. We were so very fortunate to have found her and I can whole heartedly recommend her and her care staff to anyone. read more

We started working with Cindy and Grace Care Management when my elderly mother was being discharged from a skilled nursing rehab unit back to their apartment. It was the first time that either of my parents needed any sort of assistance and caregiving. After interviewing several senior care managers, I felt so lucky to have found Cindy. She showed so much compassion towards my parents and towards me being a "newby" at being responsible for frail, elderly parents. I highly recommend Cindy as a care management consultant for anyone needing help managing the health care or concerns of an elderly loved one. My mother and I first heard about Cindy when we got news that my father's Medicare benefits were prematurely being cut off by the Skilled Nursing Facility (SNF) he was at after he had survived a massive stroke and the SNF didn't think he was making enough "progress" in therapy. He still had over a month left of his 100 days of coverage, which he badly needed to use - the injury to his brain being so severe - so it just didn't make sense. The cost to pay privately for him to stay at the SNF was exorbitant $355/day - and he was no where near being able to be cared for at home. We were stunned, angry, perplexed, thought the SNF would help us through the process of managing my father's recovery, had no idea what our legal rights were, nor what we should do. I called around for an attorney, and one of the paralegals I spoke with suggested I call Cindy. I had no idea there was such a thing as a care management consultant, nor why we would need one, but I figured I would give Cindy a call to see what she would say. That is when everything turned around for us. Within a few minutes of listening to her, I knew we needed to meet her in person. Talking to Cindy was like a breath of fresh air - friendly, knowledgeable, and informative - she gave us what the SNF told us was not possible: hope for the future. Having a loved one go through a massive stroke with extensive brain injury is a devastating experience. There was so much to learn and figure out, and the SNF had only given us the most dire prognosis - that sending my father to a nursing home was our only viable option, something which we would never let happen. He had worked far too hard in his life and deserved to live his days at home. Cindy helped us navigate the rough waters of our health care system, dealing with Medicare and Skilled Nursing Facilities so we could get my father home. She explained the for-profit business model of SNFs, why they cut you off from your Medicare benefits, and how to fight back through the appeal process. She told us about how medicare is really supposed to work, and the legal case that was supposed to eliminate the myth of the "improvement standard" for continuance of benefits. For the first time, what we heard made sense. We took everything she told us to heart and acted on it. For my father, she connected us with two excellent therapists to work with my father. Very little of what the SNF therapists did made sense, was never challenging, and some of it even seemed like it was meant to sabotage my father's progress - like adding a confusing, unrelated cognitive tasks at the end of a physical task. With our private therapists, my father began to make noticeable improvements. Cindy told us how to best leverage our funds for the long run to aid my father's recovery without going bankrupt. And when my father had a seizure and was admitted back into the hospital, Cindy told us what needed to happen for his Medicare benefits to kick back in again. My mother and I made it our mission to make it happen, and with God's help, we were successful. That small tip alone saved us from having to pay privately for another 1 1/2 months at the SNF - saving us a good $20k of money. Cindy was always responsive and positive - a friendly, helpful face in times of crisis. She came to our aid when a therapist decided to blow up one day and yell at my father. She attended all of our care meetings at the SNF (which we had previously dubbed scare meetings), giving us the comfort and support of knowing we had someone there who was on our side, and a witness to what was being said in the meetings (we had been told that we could not record our own care meetings, which was likely a lie), and countered their information from her own decades of experience in the health care industry. Having Cindy on our team made all the difference in the world. My father is now home and we have figured out a way to take care of him so that it doesn't break their bank. She recommended an excellent Home Health Services, who she personally interviewed, and we continue to consult with her as we move forward into the future. None of this would have happened without her. I recommend anyone struggling to take care of an ailing elderly loved one, no matter what the circumstance, to give her a call. read more

This company is amazing. Cindy and her team have extensive knowledge on the ongoing care for the infirm and elderly. Our brother had a massive stroke at 61 yrs old, with a Thai wife who spoke little English and a two month old baby. Cindy evaluated Gary and guided us through the whole process of facility placement, immediate healthcare concerns and finally ongoing in-home care upon discharge. She evaluates the needs of the patient and helps the family navigate through the complex and confusing healthcare system. More importantly, she can foresee problems before they arrive. Cindy had experience in stroke victims in the past so her help was invaluable. She made recommendations that other family members rejected but she was always correct in her evaluations. Other family members were in denial but Cindy helped navigate their reasoning back to sanity.
