They would encourage more alcohol, extend her massage, add hot stones or other extras, and then increase her bill. From the conversations I personally overheard, it was clear they viewed the fact that she was a doctor...and therefore assumed she had money..as an opportunity to get more out of her. That completely changed the way I view this business. Offering an additional service with a clearly stated price is one thing. Privately discussing how much more you think you can get someone to spend because of their profession or perceived wealth, while assuming your customers cannot understand you, is something entirely different. After hearing these conversations myself, I no longer trust their billing or upselling practices. If you choose to go here, I would pay very close attention to exactly what you agree to and what you are ultimately charged.

When you're providing a service, especially one people are paying for, you should give your client your full attention and take pride in your work.
