    My husband received a permanent crown placement on February 20, 2024. In April 2025, the crown came…read moreout. He contacted the office to schedule an appointment. At that time, he was informed that he could not be seen due to an outstanding balance for three missed appointments--something he had not previously been made aware. He was instructed to email the office manager, Alfred, at drsharmillaanand@gmail.com to discuss possible arrangements so he could continue receiving care. He sent an email on April 16, 2025, but never received a response, and the issue remained unresolved. On August 22, 2025, Alfred replied. He offered my husband an appointment to come in to have the crown addressed. On August 25 he went in for services and paid $41.00 for the re-cementing of the crown. This November, the crown came off again. When he called to make an appointment for another re-cementing, he was told he would be required to pay $169 upfront before being seen. When he questioned the out of pocket expense he was instructed to communicate only through the office manager via email. He emailed Alfred expressing concern about being charged again for the same issue less than three months later. He and Alfred conversed back and forward via email. Alfred responses were selfless responding firmly, stating that the fee could not be waived and that upfront payment was required due to prior cancellations, again incorrect information. No appointments had been missed. After my husband expressed he felt he was treated unfairly and not pleased that he was being charged again. Alfred stated that if he wanted to move his services somewhere else that upon receipt of a signed records-release authorization, they will be happy to provide copies of records to him or to another provider of choice. When I learned from my insurance company that the cost of re-cementing in August was only $41 out of pocket. I wanted clarification on why the new quote was $169. My husband and I went to the office to speak with someone in person in hopes someone can help and handle the issue in a case by case bases. We met with Jordan, who informed us that everyone had already left for the day (at 2:00 pm) and that no one was available to speak with us. She stated that ALL concerns had to be communicated via email. Jordan expressed no willingness to assist. While speaking to Jordan, I mentioned a billing error the insurance company notified me of that day. I was informed that the office billed a root canal for tooth 18, while all the work had been performed on tooth 19. The insurance representative explained that this was considered a significant error and that they would take the appropriate steps to address it. Out of frustration with the ongoing issues--immediate financial demand just for him to be seen, poor customer service, selfless responses from Alfred via email, unwillingness to assist and questionable billing practices--while in the office I told my husband that the situation and the interaction with Jordan was "pissing me off." At that point, Jordan asked me to leave the premises. There was no harmful intent behind my tone to my spouse. At no time was I rude, loud, disrespectful, or disruptive while in the building nor towards her. We were the only customers in the lobby. I explained to her that it is extremely difficult to address any concerns when the office insists that all communication must be conducted only through email. We then left the office. Immediately after leaving, I sent an email requesting that Shamilla call me. A few minutes later, I received a reply from Alfred titled "LEGAL NOTICE - OFFICE CONDUCT & COMMUNICATION POLICY." In this email, he claimed that my husband and I had arrived without an appointment and had been loud, confrontational, and disruptive toward staff. He also stated that Jordan reported I was planning to pursue legal action against the practice and because of that indication an intention to pursue legal action against the practice, they will not be scheduling any further appointments for him. When asked can the matter be invested due to that was false. He proceeds to send me a list of "To be clear". Also he mist pay $96 for his records. Needless to say, we were banned from the office based on false allegations. My husband's long-standing relationship with his dental provider was abruptly and unnecessarily destroyed. This type of treatment is unbelievable and completely unacceptable. We have never been treated in this manner before, and we strongly believe that San Marco Smiles should be held accountable for this mistreatment.

I regret ever stepping foot in this dental practice…read more The office was outdated, not clean, and there were human trafficking signs in the bathroom, which should've been a huge red flag to run out of that place before they touched my teeth! We only went here since they are an in-network provider with our insurance company. Little did we know, they have a history of unethical practices of submitting fraudulent claims to the insurance company to get paid for procedures that were never done. Once we discovered what they were doing we asked why they billed our insurance and were paid for procedures that were not performed? Immediately ALFRED, the office manager in charge of billing responded in an email discharging us from their dental practice and suggested we get an attorney. Then ALFRED continued to argue over email in the most disrespectful and horrendous way insisting I get an attorney. the response was bizarre and out of touch... for a moment I thought maybe this is some sort of AI computer program generating responses. Then I called the office and they just referred me to the ALFRED email. We have a zero balance owed, which is clear on our statement. Lucky for us, we only went to the dentist office once for a cleaning, and did not have any work done. Now I'm asking for them to release my dental records and ALFRED hasn't done it. Stay far away from this place!!! there are plenty of great dentists in Jacksonville that have beautiful offices, top notch dentists with ethical standard.

I was very…read moreclear from the start that the crown's fit, color, size, and position were critical, as I'd been struggling with chewing, talking, persistent facial pain, headaches, and even feeling the old crown pressing against my lip. I trusted Dr. Cohlmia to address these issues, but unfortunately, the experience was both disappointing and deeply upsetting. Dr. Cohlmia removed the old crown, rebuilt my tooth, and placed a permanent crown. While the temporary crown offered some relief, the permanent crown caused immediate problems. My headaches and facial pain returned, the crown was visibly whiter than my natural teeth, and it protruded against my lip. On top of that, it "clanged" against my bottom teeth while chewing and caused me to bite my tongue, leading to significant discomfort. I scheduled a follow-up appointment for an adjustment, and while it helped briefly, the same issues returned within days. When I called for a second adjustment, I had to follow up multiple times before I was seen. During this appointment, Dr. Cohlmia's demeanor had changed drastically. He appeared visibly annoyed, and his tone was dismissive and gruff. He argued with me, repeatedly saying, "I don't know what you're complaining about; the crown looks great." I tried to explain that the issue wasn't about how it looked but how it felt, as I was still in pain, unable to eat normally, and relying on Tylenol to get through the day. Rather than addressing my concerns, Dr. Cohlmia became increasingly hostile. He raised his voice, called my concerns "ridiculous," and sarcastically suggested refunding me, saying, "How about I refund the crown because nothing is wrong with it?" At this point, I felt trapped, embarrassed, and overwhelmed. Dr. Cohlmia was standing over me, visibly angry, and his dismissive attitude left me in tears. The patient rooms lack privacy, so this entire exchange could be overheard by others. When I agreed to the refund just to end the confrontation, he stormed out of the room yelling, "So ridiculous!" My embarrassment only grew when the front desk staff asked me to open my mouth and show my crown to them in front of other patients. To make matters worse, the office couldn't issue the refund that day and refused to provide written confirmation, simply stating I'd receive a check by mail within 30 days. The entire ordeal was humiliating and unprofessional. Dr. Cohlmia's care fell far below any reasonable standard. From dismissing my pain to his unprofessional and aggressive behavior, he showed a lack of compassion, understanding, and basic respect. I trusted him to help me with a difficult situation, but I left feeling worse physically and emotionally. If he wasn't capable of handling this case, he should have said so upfront instead of responding with hostility when the crown required multiple adjustments. I wouldn't recommend him to anyone seeking competent or compassionate dental care.
