Subsequently, we found our she had fairly advanced terminal cancer. The level of care and kindness shown by the entire staff (thanks, Kelly, Nedra, Michael) really touched our hearts. It's not the fanciest place I've seen, but the kindness, love and level of care mean that your loved one will be well taken care of. Our mom ended up in hospice, and eventually passed away in January, 2020. Throughout her suffering and our grief, the staff was kind, supportive and always helpful. They took most excellent care of her, both physically and emotionally. Thanks to all of you.

Why I even bothered after this point is beyond me! 1st appointment for an evaluation was fine. I thought maybe I had misjudged the place. 2nd appointment was unsatisfactory. The PT told me that they were terribly understaffed and she looked like she was ready to cry. After that visit, I decided to go to the trouble (and the new wait time) of getting a new referral for a different rehab place. So now, a month later, I get a bill for "services" totaling over $400. Services? For what? I would suggest you find a different PT office, if you are looking for help. Summit on 19th Ave, Silver Lake area may have been a great office 6 months ago but in my experience, it was a huge waste of time. I WILL be disputing this bill!!

The new team finally got records noted and called us about: "Resistance to care". This notation was never updated from his stroke on 3/30 - he was admitted at Mtn. View in mid April - no updates in 45 days on this chart mention. Yes, he had resistance to care - that was the case for about a week to 10 days after his stroke on 3/30. Mtn View kept this on his chart after 45 days of admittance with Zero updates, thus we had to explain to the new provider. The new place thinks he may be resisting care! A Disjointed facility with some exception such as the Speech Therapist, excellent. At Mtn View we get two opinions from two staff members and another opinion from another 3rd staff member. No cohesive uniformed rolled up decisions - poor weekly communication if any. Mtn View should look at Fields or other upgraded local facilities vs. it's unappealing worn out shoe with no laces appearance. They told us they have a broken down medical bus and it might not make it to the physician's appointment but they would be sure he would get there the next day as Mtn View had scheduled the appt weeks prior. Then they canceled his appointment a few hours later without notifying his family and rescheduled for another week out. His physician was displeased with their empowered decisions and texted us: "I'm not aware of a facility bing able to cancel medical appointments for patients at their facilities. That seems like a huge liability on their end" - quoted verbatim directly from his physician. The in house personnel who made this decision and I have a recorded conversation starts with the letter "M" I will leave her name out at this at this point. Now, one other upside (the reason for two stars) is one of his medical conditions did improve which means proper in house medical care. However, on the whole, we would advise RUN, don't Walk.

They left patients sitting in their own pee and poop for almost an hour while they stood around the nurse station just talking. People's clothes go missing and when it's brought up nothing is done to help find and return the missing clothes when if they have the name and room number on the clothes!! My father visited the Providence Everett ER on 2/19/26 and 2/20/26. Both visits left A LOT to be…read moredesired. During the 2/19/26 visit, no one even bothered to enter the visit into MyChart. I had no idea what was going on. Your Emergency Room goes out of its way NOT to talk to patient families. I am my father's Power of Attorney because of his severe dementia. On 2/19/26, I called three times to speak to a member of my dad's care team. I FINALLY got to speak to someone and found out Dad had already been discharged back to memory care. Why isn't your Emergency Department involving the POA in the patient's care? On 2/20/26 my father returned to the ER because his wound was bleeding way too much, he was not himself, strong personality changes, pain in ribs and profound facial swelling. I called the Emergency Department and on the third try finally spoke to someone who said they would add a note to Dad's chart to call me. No one ever called and I did see the note to call me in MyChart. As I mentioned, I am Dad's Power of Attorney and his dementia prevents him from being able to articulate what is wrong. I live in North Carolina. If I could be at his bedside, I would be. Another two hours passed and nothing. After my 7th call, I finally speak to a guy at the nursing desk who tells me my Dad's nurse "has her hands really full right now" and could he take my number. I gave him my name and number and told him that as Dad's Power of Attorney I needed to speak to his care team to explain why he was brought back and what we hoped would come of the visit. I told the guy helping the nurse that Dad had such a personality change in 12 hours and an oozing head wound that it was worth further investigation. The nurse at my Dad's memory care facility believed he had a concussion. Dad's aide believed he had a concussion. After I spoke with them about changes in Dad's behavior, as I too was thinking he could have a concussion. Not one nurse or doctor ever took the time to call me to discuss my father or why we wanted him looked at again. I checked MyChart and not a single test was run on my father on 2/20/26, the reason staff wrote that Dad was brought to the ER was incorrect. Basically, my father sat there, no testing was done, and he was returned to memory care without a single person at Providence contacting me about my father TWO DAYS IN A ROW!!! It seems illegal that the Power of Attorney is not contacted when care is administered to a person that has no idea why he is there. On a personal level, I am also my father's daughter. Is it the policy of Providence to keep family out of the loop when caring for patients? My father's memory care facility called me to tell me my dad was released and was being transported back to his room at memory care. SO, obviously the phones at Providence DO work if they can call the memory care facility but not the daughter/POA. I can't articulate how angry I am that I was excluded from my father's care. Memory care informed me that the ER removed the staples in Dad's head (he fell on 2/13/26) that the ER told him on 2/19/26 weren't ready to be removed yet. Information about the readiness for Dad's staples to be removed was in the After Visit Summary from 2/19/26. This tells me your nurses and physicians do not read chart notes. To conclude, my father was brought back on 2/20/26 for concerns he might have a concussion, but staff wrote down that the visit was about a bandage and removed staples that were not ready to come out and never spoke to me. Your facility told more to Dad's memory care facility than I ever heard. Staff never contacted me during either visit. I intend to escalate this complaint.
