I have to fly out and try and help my mom get out of rehab but also get care for my dad. Enter Tyler from Transitional Care Solutions. Free service helping people find the right place for dad or mom. He was delicate with his guidance and understood our financial situation. After just two days we we had four interviews set up for us to find a board and care for dad. He needs 100% assistance today. I can now breathe, I can now sleep at night, knowing my 96 year old mother is safe and happy for the rest of her life, THANKS to this incredible team. My husband and I moved to Laguna Woods from Los Angeles 11 years ago to buy a condo next door to my mother (85 then) to help her. After several years it became clear that she needed even more than we could supply to keep her safe and healthy; her advancing age was seriously compromising her mentally and physically each year into her 90's, including blindness last year. AL and Memorycare were wonderful, but eventually not affordable, and also not able to handle her serious declining healthcare needs in-house. As I sought solutions for my mom, it was my GREAT fortune to be recommended to John Menzies Clark (Pathfinder4Care) by very credible sources. He came to our house (free consultation to evaluate your individual situation and needs) and I remember that when he said he could take our case, I felt this huge weight being lifted from me. This excellent Team provided all the help and resources with the very detailed paperwork forms as well as their knowledge for the many types of senior residences as it related to my Mother's needs in every category. (I managed Internal Medicine and Nephrology offices in LA for years, yet I need their help, expertise and personal knowledge of the local Asst Living, MemoryCare, Nursing Homes, Board and Care residences which we toured with him.) At age 96 she is now settled into a wonderful skilled nursing facility a mile from me, she is happy, they are taking very good care of her, and none of this would have been possible without John, Shawn, Christy and their many years of experience and relationships. I eagerly recommend them to anybody I meet who is facing this difficult situation of "what is going to happen when ....", or other precarious situations as well. They have many fields of expertise to cover all elderly challenges for families. I share my good fortune all the time by recommending Pathfinder so that I can spare others the heavy, worrisome weight that they bear ... like I did (but not anymore - whew). My husband was released from ER and sent to a Skilled Nursing Facility for further care and…read morePhysical Therapy. He was placed at Trabuco Hills in Lake Forest, CA. In Orange County, CA. We were very pleased with his care. The Staff were very helpful and caring. The food was very good and they could prepare it for those needed different levels of assistance. The facility is very clean. They manage the medicines carefully for each patient. And the Physical Therapy was daily. Our Case Manager was Gil Gapay and he was excellent. He oversaw Peter's care and when getting prepared for discharge to a Board and Care Home, he arranged: special Transport using a wheelchair, arranging Home Nurse Care, Physical and ordering a new Wheelchair (coordinating with our insurance). I would recommend them. It is not an easy situation for any family to go through. Navigating all the details can be quite overwhelming. Having great help makes all the difference.

My father was admitted to Trabuco Hills Post-Acute after being discharged from Hoag Irvine. Against our better judgment, and under pressure from insurance, we agreed to the transfer. It is the biggest regret of my life, worst two and a half days of my dad's life. On Friday, January 30, 2026, during a routine visit, I found my father visibly distressed. He was agitated, complaining of pain, had a severely red and swollen left eye, and his blood pressure and blood sugar were both dangerously high. Given his recent stroke history, this was extremely alarming. What was even more alarming was the staff's response -- or lack of one. I asked if a doctor was on call. There wasn't. I asked if he had been seen by a doctor. The head nurse did not know. She then called the physician because there were no notes or treatment plan documented. We were told the doctor "thought it was just an infection." They downplayed a potentially serious medical situation while my father was clearly suffering. Because I did not feel he was safe in their care, I made the decision to have him evaluated in the ER to rule out something more serious. Instead of support, I was met with pushback and attitude from the nursing staff. During this urgent situation, a member of administration, Mike Perez, came into my father's room -- not to assist with medical care -- but to pressure me into signing admission paperwork. Meanwhile, I could not get clear medical answers or a doctor to properly assess my father. When paramedics arrived, I requested he be transferred to his usual hospital in Orange. The head nurse declined and attempted to go around me during the transfer process. After the hospital evaluated and treated my father, Trabuco Hills refused to accept him back. Even ER staff at UCLA Medical Center were extremely apologetic about the situation and expressed disbelief that the facility would decline to readmit him after only four hours in the ER. They had difficulty reaching the facility, and when I finally got through, Mila, a night nurse informed me that because I advocated for my father and he had been in the ER briefly, they had already given his bed away. Given. His. Bed. Away. As a result, my father spent days in the ER in limbo waiting for placement at another facility -- all because I insisted he receive proper medical evaluation. I will never regret advocating for my father. He is now in a better facility where he is treated with dignity and appropriate care. But no family should have to go through what we experienced. If you love your family member, think very carefully before choosing Trabuco Hills Post-Acute. And if you have no choice, know your rights and contact the Ombudsman's office. No patient should be treated as disposable.

Words fail to relate my appreciation and gratitude towards them. it is amazing that each and everyone of the staff, that I had the joy to interact with, were very caring and very professional and knew their job My very dear friend and his wife stayed there and they would comment on how appreciative they were of Anna and her staff. Anytime I went there to visit or called they always seemed to have time to answer my questions and let me know how my friend and his wife were doing. I would absolutely recommend them to anyone who has a loved one that they would like to have the best personal care and the most compassionate care. We found Sacred Heart after reviewing several facilities and getting good recommendations directly…read morefrom other facility managers as well. As their primary caretakers, we decided to place them in Board and Care when they could no longer stay by themselves and needed 7/24 monitoring. We enrolled them (my best friend and his wife) at Sacred Heart and monitored them closely for the first few months and have never regretted the decision or disappointed in the facility or the staff. The facility itself is located in a pleasant residential neighborhood and has been totally remodeled - very bright znd cheerful atmosphere. The staff really cares. My friend and his wife often expressed their thanks and appreciation of the food and care they received there. We were kept up-to-date on their condition . The facility took responsibility for obtaining and adminstering meds and cooperated with their primary doctors, nurses and hospice workers who were not affiliated with the facility and who visited regularly. I cannot say enough about the caring and professionalism of the staff. The adminstrator, who also provides patient care herself, is wonderful to work with. If you are looking for Board and Care, you should put this one on your list and check it out
