On the surface, the facility presented well with a clean appearance, beautiful rehab gym, pool, carpeted hallways, chandeliers, and a private room option. There was, however, a slight odor in the air during the tour that we tried not to focus on. We ultimately chose the facility because we were told my mother would receive three hours of therapy a day. Unfortunately, after getting her settled in, things quickly became concerning. To be fair, some members of the daytime staff were wonderful. FIVE STARS for nurse Shannon, occupational therapist Mary, physical therapist johnathan, the social worker, and several daytime aides (Jeanette D, AnnJohn, and Maxine) were kind, attentive, and professional. I truly appreciate them. The nighttime environment, however, was a completely different experience. Patients could frequently be heard screaming for help. Outside of the staff members I mentioned, many employees did not appear engaged or fully competent. Leadership was also not transparent, and when concerns were raised directly, communication became evasive instead of solution oriented. Our family was present from approximately 8:00 a.m. to 8:00 p.m. daily to make sure my mother's needs were being met, which turned out to be necessary. At one point, my mother had an outside medical appointment and required paperwork from the facility. We were given documents belonging to an entirely different patient, including that patient's photograph. The explanation felt careless and deeply concerning, especially given the seriousness of patient privacy and identification. There were also several instances where my mother was nearly given the wrong medication, and long periods where she waited extensively for assistance. The odor in the facility became significantly worse at times, particularly later in the day. Additionally, the therapy schedule did not match what was promised. Instead of receiving three hours of therapy daily, my mother was receiving roughly 40 minutes combined between services. This experience taught our family that appearances can be misleading. While some staff members genuinely cared and worked hard, there were too many operational, communication, and patient care concerns for us to feel comfortable recommending this facility without caution. I'm grateful we made it through the experience and thankful for the few staff members who truly supported my mother during her stay.

Our family experienced catastrophic neglect that resulted in an emergency hospitalization and life-threatening conditions for my aunt. Here are the specific, documented issues encountered during her stay: Severe Medical Neglect & Infection: My aunt developed an unaddressed Stage 3 bed sore that became severely infected. Despite showing clear signs of distress and blood in her stool, facility staff failed to respond or intervene. Emergency Response Failure: During a family visit, my aunt was crying out in agony with zero assistance from staff. The facility failed to call emergency services. Family had to call 911 directly. Upon admission to the ER, she had a 100.5°F fever, a heart rate of 180, abnormal lab work, and was evaluated for sepsis directly linked to her infected pressure wound, along with heart failure complications. Lack of Basic Hygiene & Care: Staff forced my aunt to wear diapers rather than assisting her to the restroom because it was "easier." She was routinely left sitting in soiled diapers for hours until family members had to step in and change her themselves. Ignored Call Lights: Staff members explicitly admitted to us that they have too many patients to manage, resulting in call buttons being completely ignored when pressed. Safety & Falls: She fell out of bed while under their care, and staff brushed it off as non-consequential. Unsanitary Environment: Food service is deeply concerning--we have photographic evidence of insects on her lunch tray. This facility is actively failing its duty of care. Her hospital transfer was the direct result of systemic negligence. Please look elsewhere for long-term or rehabilitative care.
