    Great patient care. Staff is friendly and helpful. All departments are very involved and great with rehab and long term care.

    Good Care, the team is responsive and compassionate and Knowledgeable. Would recommend SNF for rehab to everyone!

    My husband received great care at Fresno Community and transferred to a rehab center closest to…read morehome on a Friday. I met him there after laundering his clothes, same day. As I surveyed the facility I had a bad feeling. Unsanitary conditions and lack of care. I literally cried the whole way home feeling I abandoned him in a place that my intuition was telling me to get him out of there. I pulled over half way home and called him. I told him I would come back and bring him home. He stated he would stay through the weekend and see how it's goes. My intuition was dead on because the next morning he had a friend extract him from the facility. Here are some reasons. Treated as if he was there to get oxycodone, could not be further than the truth, nurses weren't licensed as portrayed, told he would have to schedule which day he'd like to shower. The safety bar in the bathroom was the exact opposite, it was an unsafety bar as it appeared attached, when utilized it came right out of the wall and he almost fell over. When he reported it to an attendent, she said "oh well". As I spoke with him on the phone, it was so loud in the background, only to discover it was the employees chatting loudly in the hallway. Having been in a car accident, 10 broken ribs, collar bone and head trauma the unlicensed woman said she had to check his genitals! Fresno Community nor the Fresno VA Hospital that he stayed at for a few days after Fresno Community didn't find a reason for that. The woman that bullied him has a name close to Mydole, not sure of spelling. Any Veteran in need of care, please do not go to this facility that has a pretty driveway and great security gates to give false impression of what is inside. Please don't let your family member stay there!

The staff made such a difference in a very caring warm environment . The dining experience was wonderful also , I loved the extra effort coordinated either activity director and the chef to add some fun and joy to my mom and others recovering . I will be forever grateful for Morning Star and helping my mother at such a difficult time regain her strength and keeping her spirit upbeat .

There's the Director who doesn't care about the patients or the families that visit the patients. These are family and friends visiting and they deserve to be comfortable while visiting. They have those metal chairs that you sit on at bbqs or play cards. The Director of Nursing doesn't like to be asked questions. She may have a heart of Gold. Good luck finding it. The person helping Social Services is indescribable. She struts around the facility is screaming tight tight jeans. During our status meeting we were told upon discharge they would provide us with a hospital bed, shower chair, walker and a home health nurse. Well that didn't happen. This is what happened. We were told on April 29, 2026 that our insurance was terminating our stay by May 1, 2026, but I could appeal. I appeal that afternoon and was told it would take 1-3 days. I was naive to think the appeal would be approved because my husband was not ready. On Friday, May 1, 2026 at 3:30 I was sent a text that it was denied. She said the discharge would read against medical advice. How is that when we have no choice. Sge ended the text by saying thank you for understanding. Understanding what. There was no choice. Duh We had to leave on Saturday, May 2nd which was Saturday. Where was the hospital bed, the home health nurse, shower chair and walker. I brought it to her attention and it was crickets. I get to the facility on Saturday to pick up my husband and I contact the woman and she says are you there because it was before 8am. Why would I watch full day to pick him up. We were told we had to l ave so we had to leave. If Theodore tie was worth his salt he'd make sone changes. 1. More comfy chairs. He could apply for a grant to get better chairs. 2. The Director should have to introduce herself when new people arrive and nit keep her nose into a computer. 3. The woman helping social services should have to dress more professionally abd not argue with the patients about a lawsuit the patient guest know what she's talking about and gets into an argument with the patient. She needs to learn how to talk to people or better yet be transferred somewhere or lose her job period. Although you don't expect the four seasons the food could be better not tough , burnt or mushy vegetables. All the people that have to be there they should be given better food. I have never been treated like we were being used as a coverup and we couldn't get what was promised to us. Where's the empathy, compassion and caring. Oh I know it's Oneida's when you get a scoop of ice cream from the Director.

They are the only facility in the Fresno area that would accept someone with his diagnosis. When I removed him from his previous facility that was rated 4 stars by Clovis Community, he was so weak that he could not sit up or stand because he had lost nearly 30 pounds because of their poor care. The receptionist at Sierra Vista was very welcoming. The staff was friendly and I could ask any staff member about his condition. At his previous facility I was only allowed to speak to one person and she never gave me answers to my questions regarding his weight, BP, etc. Sierra Vista's dedicated Physical Therapy staff was able to get him up on his feet and walking again. I am extremely thankful for them doing that! When he was discharged to go home many of the staff hugged him goodbye or gave him a kiss on the cheek. They will be happy to know that he is still up and walking. Thank you Sierra Vista!

I was there three months b/c of a car accident. If I had to put some one in this home I believe I would feel good about the general care. I of course would have to bring meals for my loved ones because honestly their food was not edible. HOWEVER **** if you are there with a wound this is not the place to be. My wound nurse decided to oil up my wound then proceeded to tightly bandage my wounded leg. This was not the surgeons orders. Wound nurses are suppose to be a specialized skill. Well this one wasn't. I went home thinking I was finally on my way to recovery. Three days later my wound had opened and I could see the hardware in my leg. Hugh hole. Returned to E.R. Turns out the infection had turned to MRSA and I had to be readmitted to the hospital to open the wound, clean it out, remove the rod in my leg. So now I'm back to square one. After the hospital I went to Willow Creek. Not my choice but because I now had MRSA I needed to be in isolation. DON'T GO HERE. Everyone is lazy. You rarely see your CNA, your nurse is NEVER on time with meds, and sometimes my call button would be on for 1 1/2 hours. Not the kind of place I'd send my elderly. This place is ABSOLUTELY THE WORST! This is a long account and I'm skipping the next 4 weeks. The long and the short of it is: I loved the people at Sierra Vista. Watch out for the wound nurse, make sure you can deliver meals to your loved ones. And remember to stay away from Willow Creek. I would have given this a 5 because I loved the people. I subtracted for the terrible food and the unskilled wound nurse.
