My mother received PT/OT after an hospital stay. No one really wants to recuperate at a facility but rather be home. I must say that Orchard Post Acute with a loving, compassionate staff made her stay easier to tolerate. The CNA'S were more than happy to help with dressing and personal needs. They and the whole staff truly went above and beyond to make her stay pleasant. The PT department was so awesome! They truly had a motivating attitude towards all of the patients. You can see and feel that they really care about people. What was special and touched me deeply was her red carpet send off. The staff gathered around and played her favorite music as she left the facility. It's these things that make a facility special. I was tearful as I witnessed the care my mom received. I highly recommend Orchard Post Acute for your loved one. read more

This facility does not even deserve one star. Do not put your loved one in their care. The staff has horrible attitudes, and they don't even try to hide it. They won't return phone calls regarding updates on your loved one or have care to call you if having issues with your loved one. I stated many times to call if there was an issue with our loved one and no one ever called the family to let us know. The facility is not well kept and is dirty. They allow the residents to yell, cuss, & blast rap music as loud and as long as they want. They stole multiple things from my loved one. This place needs to be SHUT DOWN. It's really sad to see the elderly not being taken care of properly and rotting away in their beds. This was overall an awful experience , I don't think it gets any worse than this. read more

My Dad was admitted here during the COVID pandemic. We were not allowed to visit him and had no idea the living conditions were so inadequate. I received a call one day informing me that they had dropped him and he hit his arm and head. We finally got him released and I was absolutely horrified. He had lost so much weight and was skin and bones. I cried when I saw him. He passed away shortly after. I would never recommend this place to anyone. read more

If there was a rating of zero stars that would be my choice. I was admitted there on February 6, 2026 after having lumbar spine fusion l1 to l5. Care quality was zero. I had to wait 20 minutes for someone to ask what i need. Tell them my pain medication. I will get your nurse. Nurse never came and I had to press button again and wait still no Nurse. It has now been 40 minutes and still no Nurse. I didn't get my pain medication until 50 minutes. I didn't have a walker and I needed to go to the bathroom. Person came in and tried to lift me without my back brace. I screamed in pain. Next morning at 3am I had to go to bathroom. Pressed call button. I was going to have accident. I had to get my back brace on and go to bathroom by myself. I barely made it. They tried to make me put on a brief. I am 63 and I am not going to put a brief ob because you all are lazy. I pressed the call button from the bathroom and no one came. Made it back to bed by myself. I had to throw my soda in the hallway to get someone's attention. Staff is lazy and unsanitary. Couldn't flush toilet after pooping because it was going to overflow. Toilet leaking water with puddle of water beside toilet. Got my hospital gown wet on 2 different occasions. Got the wheels on my walker wet Did not have me labeled as fall risk. DO NOT SEND YOUR LOVED ONES OR EVEN YOUR WORST ENEMY TO THIS PLACE. NEEDS TO BE SHUT DOWN.​ read more

In the time that I've been with Orchard Post Acute I have seen nothing but caring and friendly employees. You can tell that the people that work there have a passion for what they do. Not only are the staff amazing, but the facility is tidy and well kept. The open hallways are always welcoming and free from clutter. Resident's seem happy and well adjusted. read more

I've worked for Orchard Post Acute for nearly two years and like most jobs, there are up and there are down. However, the ups outweigh the downs and I've had a great experience overall. What I appreciate the about this building is the workplace culture and the ability to know all the residents. I am so unbelievably disgusted with this facility, not only as a family member but as a past CNA/PCT. Take my word, do NOT trust this establishment with your family member. My grandfather, a veteran, was admitted here following recent health complications at the VA, he was at this facility all of 2 days before returning the hospital. In that short time, BOTH his wallet and keys have "magically" disappeared. While I understand that mistakes happen, and things get misplaced, this is so far from the correct way to handle things. Instead of attempting to help, this facility and its employees are dodging phone calls, hanging up, pushing blame around, and pretending as if nothing happened. Unfortunately, because of this, we have resorted to involving both the sheriff and OMBUDSMAN. After 36 TOTAL deficiencies found in state surveys over the past 6 years, one would think Orchard Post Acute would have the sense to make some changes, but this establishment CLEARLY has no care for its residents, their family members, or the veterans of our country. read more

If I could rate this facility a negative star I would!!! They have horrible communication with family, my grandmother was non verbal after a stroke and was injured by a CNA while being lifted improperly which caused her shoulder to fracture! They then waited 2 days before sending her out to get a xray/sling, 81 Years old sitting in pain for days before being treated! Then sadly when she recently passed, we were not contacted by the facility until after she had already passed and we had been notified by the hospital. If we would have known sooner maybe we would have had time to say goodbye before she passed. When we got to see her, she was in clothes that were not hers, a men's shirt that was on INSIDE OUT and BACKWARDS! All her clothes we provided had her name labeled on each piece so we know it was not something we purchased and took to into the facility. Absolutely disgusted by the fact that my grandmother left this earth being treated like that, I hope and pray the facility closes soon! read more

First, the staff was good, they did the best they could with what they had. Food was so so. The place was filthy. They never mopped the floor the 22 days there. They swifed it. The worst was my Dad went in because of infection on leg Dr wanted to monitor it.. He came out with five (5) new wounds one worse then the one he went in with. The wound Dr said it was not properly initially cared for so he was going to lose a flap of skin. And it would take a while to heal beacause it was to the bone. read more

I have worked Orchard for over 2 years. We have a great culture and work environment. We have wonderful caregivers that treat the residents with respect and compassion. I have personally seen the therapy team works miracles with the residents helping them improve their function and abilities. The building has also been recently remodeled so it looks and smells very nice and clean inside. I would definitely recommend Orchard Post Acute to my family and friends. read more

My grandma came out so happy. She went in with a broken leg and hand and came out able to walk. I grew up going to geriatric facilities to volunteer and they were nothing compared to this. The facility smells clean. Dolores the ADON answered all my questions re my grandmothers health. The nurses were always so cheery like they were happy to work there. If any patient ever asked for anything they would never neglect them. They would stop what they were doing to give care.It's about rehabilitation here. I truly appreciate this place. Orchard Post Acute is a God send. My dad was in a prior facility who on multiple occasions neglected him and it resulted in multiple bed sores, uncontrolled diabetes, and dehydration. After being discharged from the hospital in January 2024 for pneumonia, my dad was placed at OPA. My dad has been carefully monitored by the CNAs, nurses, the Assistant Director of Nursing (Dolores) and his attending physician. My dad is unable to use the call button so he 100% dependent on the staff to check in on him and take care of his basic needs. I am happy to say that my dad is changed regularly, liquids are encouraged, he is fed by staff, the CNAs and nurses are friendly and will ask if there is anything we need or if we have any questions. My dad is now thriving. Dolores closely monitors my dad's condition and symptoms to communicate them to his doctor. Since being admitted my dad is more lucid, his arms and hands have stopped shaking, significantly less hallucinations, increase in memory and is now able to relearn to feed himself. I cannot thank God enough for directing our family to reach out to OPA who at the time just had one bed open up. I truly believe this was God's plan for my dad. I recommend OPA to my friends and coworkers. OPA is a hidden gem!! read more

This place is extremely questionable. They did not return my fathers clothes (although he did come home with the shoes I brought) They gave him questionable meds that he was NOT taking when he went there. I don't even know if he ever wore the clothes I brought him and he came home in the same clothes he was in when he went. He was there for 2 months!!!!! Now I am wondering if he ever even bathed!? Be fore warned about this place. I am going to contact a lawyer and see about suing for elder abuse! read more

Just recently had my grandmother at this facility and I couldn't have chosen a better place for my loved one . She spent a good 7 months there and I was more than satisfied with the care she received . The cna's and nurses were on it every time we needed something.
