    I've worked for Orchard Post Acute for nearly two years and like most jobs, there are up and there are down. However, the ups outweigh the downs and I've had a great experience overall. What I appreciate the about this building is the workplace culture and the ability to know all the residents. OPA is a 98-bed facility which makes it easier for staff to get to know all the residents by name and build relationships.

    Not organized. So many staff just walking around. Call light ringing for 10 mins, I timed it. Dirty.

Our family is so appreciative of the professional and loving care she received. My mother's doctor was accessible and kind and his recommendations for comfort care made her last days peaceful. I especially want to thank Dr. Sidhu, Kathy the facility director, the nurses (especially Tiffany) and all the wonderful aides who do the daily care. Even the gate guards Dave and Julian were wonderful. Even though my mother was bedridden she was always clean, well fed and given her medications as directed. Tabitha in the adminissions for the skilled nursing said herself out of her own mouth! " you dont fit the demographic due to your age" I just had a full hip replacement it's my second hip surgery plus I have dentures, hearing aids, walker/wheelchair combo. 6 titanium spine fusions severe osteoarthritis and degenerative disk decrease and i was completely approved in every way tell the Evil Tabitha who clearly should not be in adminissions in any facility did complete age discrimination! It really broke my heart and made me very sad and depressed to treat someone like this to discrimination of age seriously. I have been to a few skilled nursing places in NV and here in California and I have never experienced such hate! Save yourself the trouble if your under 70 your not getting in as long as the evil Tabitha is running adminissions here.

    I am heartbroken and outraged by the gross negligence of the skilled nursing facility where my…read more91-year-old grandfather was receiving care for his physical therapy. He was supposed to recover there and return home, but because they failed to properly monitor his catheter, he developed a severe infection. He was transferred back to the community hospital, and the infection entered his bloodstream. Ultimatley, he passed away. At one point, we spoke with the director in charge late in the day. While we voiced my concerns, she didn't even make eye contact--she packed her bag, shut down her computer, and got ready to leave. It was clear she had no real interest in addressing the issues, and that moment reflected exactly why the facility runs the way it does--with indifference and a lack of accountability. They never followed directions, and any time I asked questions, no one knew the answers. They blamed the previous shift instead of taking responsibility. No family should have to suffer a loss because of such neglect. I urge others to stay far away from this facility. Their inaction cost my grandfather his life.

They are the only facility in the Fresno area that would accept someone with his diagnosis. When I removed him from his previous facility that was rated 4 stars by Clovis Community, he was so weak that he could not sit up or stand because he had lost nearly 30 pounds because of their poor care. The receptionist at Sierra Vista was very welcoming. The staff was friendly and I could ask any staff member about his condition. At his previous facility I was only allowed to speak to one person and she never gave me answers to my questions regarding his weight, BP, etc. Sierra Vista's dedicated Physical Therapy staff was able to get him up on his feet and walking again. I am extremely thankful for them doing that! When he was discharged to go home many of the staff hugged him goodbye or gave him a kiss on the cheek. They will be happy to know that he is still up and walking. Thank you Sierra Vista!

    The CNA's are the best. The Physical Therapists were great. Nursing Staff were always ready to…read moreattend to your needs. THE FOOD IS TOTAL GARBAGE. I was there three months b/c of a car accident. If I had to put some one in this home I believe I would feel good about the general care. I of course would have to bring meals for my loved ones because honestly their food was not edible. HOWEVER **** if you are there with a wound this is not the place to be. My wound nurse decided to oil up my wound then proceeded to tightly bandage my wounded leg. This was not the surgeons orders. Wound nurses are suppose to be a specialized skill. Well this one wasn't. I went home thinking I was finally on my way to recovery. Three days later my wound had opened and I could see the hardware in my leg. Hugh hole. Returned to E.R. Turns out the infection had turned to MRSA and I had to be readmitted to the hospital to open the wound, clean it out, remove the rod in my leg. So now I'm back to square one. After the hospital I went to Willow Creek. Not my choice but because I now had MRSA I needed to be in isolation. DON'T GO HERE. Everyone is lazy. You rarely see your CNA, your nurse is NEVER on time with meds, and sometimes my call button would be on for 1 1/2 hours. Not the kind of place I'd send my elderly. This place is ABSOLUTELY THE WORST! This is a long account and I'm skipping the next 4 weeks. The long and the short of it is: I loved the people at Sierra Vista. Watch out for the wound nurse, make sure you can deliver meals to your loved ones. And remember to stay away from Willow Creek. I would have given this a 5 because I loved the people. I subtracted for the terrible food and the unskilled wound nurse.

She quickly got to know and identified his needs- I had never seen my son calm down and focus like he did on our second session. Equally as important, she gives excellent education for us as his family. She explains the process and helps develop tools so we can help him at home. Now we understand his sensory needs, and we have a sensory diet to follow. We have tools to actually help him when he's disregulated, where we used to feel powerless. We've been to other OT's, but worked to come back and work with Priscilla as soon as we could. She had truly helped us understand our child better, and forever changed our lives in the process.
