We chose to use in hospital hospice with this organization as my husband had been in the hospital for weeks and we thought it would be easier on him than moving him. It turned out to be a nightmare. After 3 days, we were told they needed to discharge him because he did not meet "Medicare criteria" for in patient hospice. Within six days, we had plans for the move to a freestanding facility. My husband passed away the night before the move was to take place. The day before the move was to take place, Hospice in Branford called to make arrangement to move him to their facility and that he "met the criteria". This experience was traumatizing to me and my family. I would discourage anyone from using this organization for their loved one. read more

Rude, unreliable an very unproductive. The Home Health Aide sat down the entire time; the nurses were rude, argumentative and not considerate of family members. OT, PT and ST were ok, but far from stellar. They were anxious to go and often complained of my location. All services never gave me a time frame that was consistent and appropriate for me and my needs. Big disappointment. read more

We used Constellation for my father 18 months ago. He came home from the hospital on Palliative care. The nurses, social workers, home health aids, case managers were wonderful. The equipment arrived on time. The representative was pleasant and professional. I am a nurse myself and was pleased on how smoothly the process went. My father transitioned to Hospice and the nurses were there to support us every step of the way. I slept on the couch to be by my father, one of the nurses saw that I hadn't really slept in a week. She brought in a night nurse so that could go home and try to get some rest. The night nurse was amazing. She encouraged my mother and I to get some rest and she would wake my mom with any changes. And as life would have it, my father passed away peacefully early that morning the same time I woke up to go back. The night nurse was so good to my father in those last hours. Her compassion for my family will always remain in my heart. Constellation is really a great agency. I was sad to read some of the negative reviews on here. Expectations and emotions run high in times like this. We always think that no one can take better care of our family member better than we can but the reality is, we cannot do it alone. Constellation helped us keep my father in the comfort of his own home and he passed the manner that one can only hope for; comfortable and peaceful. He did not suffer. I will always be grateful to Constellation for the great care my father and family received during this time. They are truly Earth Angels read more

Who would think to ever do a hospice review prior to using a company for hospice care. This company was just horrible to say the least. They did not aid in what they were hired to do. They did not show when scheduled and most of all when my mother's health was failing we could see it was, but there was no communication from them to the family in preparation of what was documented that the end was near. Last hours when we called to ask someone to come my mother was in terminal restlessness and they stated to call 911. She later that evening passed away and they never showed even after. They are a horrible company I found many reviews on here showing the same type of experience that is upsetting that they are still allowed to treat people this way. If you are doing research for hospice care like i wish i did, please do not use this company. read more

Do not use them for hospice services. Just when you need healthcare professionals who are professional, they are anything but. Hospice is supposed to make the patient and family's end of life care and planning easier. They created more stress by ignoring follow through. My LO waited several days for prescriptions because they dropped the ball. The nurses come uninformed and unprepared. The office staff makes promises that fall by the wayside. I do home health myself, but physical therapy. I usually bow out before Hospice comes in. I can't say enough good things about the nurse, Donna, who came out and did the initial assessment, made sure we had any medical equipment we might need. Got us additional medications for comfort, and supported us through the whole journey. The aide that Constellation was able to find for us was kind, compassionate, and competent, and made all the difference. My father's last weeks were spent in comfort, with his family around him, not needing to worry about symptoms or calling doctors, trying to get to doctors, to change medications or add them as his physical condition progressed. I think he actually gained more time with us, as he was not fighting for breath, or in discomfort from positioning or GI issues as Donna managed everything. Thank you, Constellation! read more

Terrible experience. They were suppose to provide services to a family member and update me weekly. I never received any updates. I called to remind them of the weekly calls and still received no calls. Do not use this service!!! read more

My hospice worker suggested a ridiculous medical procedure for my husband I said no I was the health advocate then she discussed his mideical info with my daughter and got a yes I said no he has suffered enough.She had no concept of HIPA laws .When I confronted her and told her my daughter was down to b notified about my husbands condition not to make medical decisions she informed the nursing home that I did not want my daughter to b called when he was dying.I had fractured my pelvis could not b there I needed my daughter and family to b there since I could not.Luckily my daughter decided to visit and could c he was dying but at least he was not alone.If it was up to my hospice workers lies and working against me he would have been alone with no family with him.She should not b in this business. read more

I have used Constellation several times to care for my mother after hospitalization. The first few times I was very pleased. The nurses and therapists are great to work with. However, management has become unbearable. Specifically, mom was released to be cared for at home rather than subacute rehab on suggestion by the hospital case manager. However, the manager said that all they were "required" to do was have the nurse and therapist evaluate mom and that she couldn't provide any additional care because mom's primary is a nurse practitioner who works under a number of physicians who could sign off on her treatment plan. When I asked "Debbie" if she'd called the number i gave her she said she hadn't because "they are difficult to get a hold of." I should add that my mom had the identical condition and treatment through Constellation 4 months ago. And that her primary care hasn't changed. This is an appalling situation to be placed in and I will NEVER recommend Constellation again. Stay away, I'm sure there are better options available. read more

I live in Manhattan and used the NY based office after total knee replacement surgery. I was very pleased with the care and services I received. The nurses who visited me were thorough and caring. I received occupational therapy to help me navigate my apt and daily life and physical therapy several times a week. The therapists were amazing - both effective and kind. Nurse Cora removed my surgical staples, and was efficient but compassionate during the somewhat painful process. All in all, I am very pleased with the assistance I received during a particularly difficult time for me. I would have no hesitation selecting Constellation again, if the need arose and recommend it without reservation. read more

We had an awful experience with Constellation Services while they provided hospice services for my dad. While there were a few really delightful people that we got to know, the overall services, customer service, and care was terrible. Living across the country during the six + months they worked with him, I found myself having to fly home several times a month to address problems they created. They didn't listen to my dad or our family, created a toxic environment and were rarely helpful. They created a lot of chaos. I often hear about the care and peace families find with hospice - this was the opposite. We stopped working with them in the final months of his life and never heard from them to inquire about why or his condition. By contrast, we worked with another Hospice provider for only a few weeks at the end of his life. These folks were amazing and reached out on numerous occasions months later to see how the family was doing. If possible, stay away from Constellation. read more

Was referred to them by Stamford Hospital after needing home care assistance while recovering from a total hip replacement. Overall very good experience, lots of care set up - visiting nurse, pt, home health aide. Everyone sent was very kind, compationate abnd professional. Everyone worked with me with scheduling that worked for both of us. Physical therepist (Michelle) was the best Ive had so far, and Ive had 5 surgeries in the past 6 months - 3 spinal fusions and now both hips replaced. She was patient, well educated and just an overall joy to work with. Really motivated me! The home health aide (Sherry) was also a pleasure to work with. She went above and beyond what she was scheduled to do (assist with showers and getting dressed). If there was time left over she went right to work cleaning for me without even being asked to. Overall, very positive experience, and I look forward to working with them again now that I've just had my other hip replaced. Would definately recommend to family and friends going forward. read more

The home health aides don't show up on time and sometimes they never show at all and this is after they confirm that they would arrive. It appears as though the only thing you can count on is that they will be dishonest in their communications. Absolutely disgraceful. read more

I had heard of this company since I live in Connecticut but personally did not have a need to utilize them. My parent live in NY and I saw on the website that they also were in NY. I called the number which did put me through to the CT office and they assisted me in getting in touch with NY. I initially called to ask some questions and was received by Gina who was a pleasure and she assisted me to the point she could and then connected me to a department that answered the rest of my questions and gave me the information I needed to help my father get the care he needed after coming out of a rehab facility in the next 4 to 6 weeks. The period between the first call and his coming home was filled with a lot of questions for myself and my family and I was able to communicate a few time with Constellation and they assisted as best they could with ensuring it was a smooth transition. My father ended up coming home and he received many services including nurse's, and aide and various therapies. The staff were all professional and caring. I asked a lot of question and always got a response whether it was specifically answering my question or directing me to an appropriate source. After nearly 2 months of care the services ended and my parents were prepared for a new stage in their life of dealing with medical problems that will be with them forever and will need to be managed. I would absolutely recommend them and will make sure to utilize their services if ever in need. read more
