Was discharged to this facility after a stent in the hospital from surgery. For the most part this place lacks the necessary skill/training needed to care for the patients under their care. From supervisors that lack the ability to communicate to their subordinates, RNs that don't know how to properly change a Wound Vac, to LPNs that don't know how to properly administer insulin. Then add to that by company policy there's certain prescriptions that only an RN can administer, but guess what...they don't always have an RN available to administer the prescription. I'm disgusted that the state continues to allow this place to operate. On the subject of rehab...they get by with just the bare minimum. With what rehab I got under their care, I could have ordered the needed supplies shipped to my home and used instructions found online. Then add to all of that that the inter office drama between staff members that residents have to endure and witness. And discharging patients with out the needed supplies to carry on the care they are needed. I could go on but...

Because that previous experience is so much fun and interesting working with great staff and wonderful residents, I thought volunteering there would be the perfect place. I called the office and spoke with a woman. I told her what I wanted to do but she was very rude. She transferred the call to another person. The next woman was just as rude as the previous one. I decided not to volunteer with Eastmont. I didn't want to surround myself with rude and grumpy staff. I did get a call from upper management. She was very kind. I appreciated her apologizing for her rude employees. Eastmont gives great care to its residents. I hope that never changes. Since renovating their facility/home, it is a beautiful and comfortable environment. I gave them a lower review so they will learn to hold people accountable. I believe everyone should be treated with kindness and being polite.

My husband has been battling Alzheimer's for 8 years. He was started to have falls and more…read moredifficulty with everyday living activities. I had to make the hard decision that is time to place him in a 24 hour nursing facility. We have been working with a great team of hospice care givers at home. They helped me find a place that had a opening and was able to take him. He entered into Homes Lake Rehabilitation and care facility on September 18th 2019. He did have enough clarity to understand that a change was happening and handled very bravely. That night he was very upset and agitated which is very common for Alzheimer's patients. I helped get him calmed down and asked the aide if she could get me his dinner tray that I would help him with his dinner. She was a very sweet girl and did get his dinner. I already knew that was going to be an ongoing problem in the evening and asked if they could give him his medication early to help prevent this from happening. The order was at bedtime which I was told was between 7:00 and 8:00. The very next morning I called his hospice team to see if they could get his medication orders changed and they were. Now was able give as needed and his dr also added another anxiety medication to keep him calm. I left on Thursday about 3:00 to go home to try and get some sleep feeling better that he would have a better night. I arrived on Friday morning and talked to his nurse how his night was. I was told he had a good night and was only up once and he did need some convincing to return to bed. So was happy to hear it went better. I then went to his room and took him outside for some fresh air. Shortly after I received a call from the hospice social worker that she received a call from social worker at Holmes Lake stated that he would have to leave their facility today. That gave us no time on a Friday to find a better fit for him. I had to call our son to come immediately to help me get him home. We packed up his items and I went to the front desk tell them I had my husbands items packed and we were ready so if I needed paperwork to sign if they could please get them for me. His nurse and several other employees standing there told me I was not allowed to just take him out. I told them he has been kicked out so I could leave. The looked shocked and was told they knew nothing about this. The nurse left to go find the social worker I guess to find out what was going on. When the nurse came back I as told I would have to wait until they received their paper from his dr office. I replied I was not waiting so the social worker returned right away with the paperwork and we left. As of Monday September 23rd my portion that I would have to pay would been right around $10,000.00 a month. This price was just for room and board insurance was paying for medical costs. Very heartbreaking this is the care he was receiving for that price. Holmes Lake should take a better look at the care they are giving their patients. Very traumatic experience for my husband and our whole family. I would not recommend Holmes Lake care facility to anyone's loved ones. Pretty obvious they are in this for the money and not the care to their residents. On the positive side I enjoyed the nurse's and aides. They should maybe take another look at the empathy and compassion they are giving. I have worked in nursing homes and saw first hand how they are operated and it was very disturbing and finally left that field and hoped that things have changed but not the case. The social worker claimed he needed to much assistance and that's why they came to this conclusion. I thought that is why residents are there. I don't think people entering into a care facility because they can take care of themselves. I would also suggest they look into how they are treating their nursing staff. I have a suggestion how about spending some of your profits and get your employees more help so they can care for their residents that they Linda Clark

I worked for Tabitha as a Home Health RN until recently. The other nurses and staff are amazing,…read morebut management is abusive and toxic, and human resources as a whole does nothing but cover themselves legally. Although Tabitha advertises itself as an equal opportunity employer, I was refused reasonable accommodations and discriminated against and harassed because of my disability to the point where I was forced to resign. I would never want someone who is taking care of my loved ones to be subjected to that. I have ADHD and PTSD; I disclosed this to my manager upon hire. 2-3 months into training, my manager and, seemingly randomly, the QA manager began weekly meetings where my performance was critiqued. At this point, I was 1 of 3 new RN hires and the only new RN hire required to learn the admission process (a very detailed 4-6 hour long process.) I was given minimal training on this process, and when I asked for a checklist (a reasonable accommodation for my ADHD) I was refused. I was required to complete this process again and again, without accommodations, at least twice weekly. Management began telling me that all of my documentation needed to be completed by the end of the day, when all of the other nurses were given up to 5 days, even if this meant I was forced to work 12 hours with no notice. None of the other nurses were forced to do this. During weekly meetings, management would spend the entire time gaslighting me, stating that the other new nurses were being required to do the same things yet I was the only one failing. This is a complete lie, as the other new RNs did not learn admissions yet and were not independently case managing. The point of those meetings was to make me feel as inadequate as possible, and leave a paper trail of me "failing," even though I was further along than the other new RNs. Meetings were as degrading as possible, and often left me in tears for my following patient visits. At one meeting, it was determined by management that I was not excited enough to go over my schedule for the next week. After asking for several different reasonable accommodations and being subsequently refused, I replied "I guess" when asked if we should look at the following week's schedule. I got written up for saying "I guess" and was told that it the tone of my voice constituted "unbecoming conduct." This write up then gave management enough to place me on a formal improvement plan which was never lifted, even when I met all the goals set forth. It is quite clear to me that I was treated differently because of my disabilities, and was refused the opportunity to be successful with this company because of those. Tabitha could easily have granted any of the reasonable accommodations I requested: a detailed checklist, adequate training, safe caseload, reduced work hours to 36 hours/week, and several others. These accommodations were not an undue hardship, and I have to wonder why Tabitha refused them as an organization supposedly dedicated to vulnerable individuals.
