I have to say that this faculty was on top of their game never had a problem even they accommodated for food I didn't like or allergic too. There was a lot of people that helped me to my recovery and I'm very grateful cause I was very scared and ready to give up but an RN named Giselle talked to me which got me on track.. I have names i like to mention that helped me on my recovery and if I misspelled some my apologies. This names range from RN,LVN,CNA, physical therapy and other office staff, Vicente, Giselle, Aileen, Lorie,Patrick, Matthew, Don, Deja, Rowana, Rayna, Thad, and Alex. I have to give my appreciation to these to everyone helped but these names that I put stood out more because they went over and beyond what their job ask them to do. I have much love for these people. Thanks everyone at Diablo Valley Post Acute for everything.

    Update again: Had…read moreto move my grandma out of here and it was the best decision we made. The team's care here is inconsistent and with the exception of a very few people, most people here don't care about their jobs nor do they have the sympathy or mercy for the patients here. After a week or so of caring for my grandma, we saw drift where my grandma went back to being soiled for hours on end and being severely neglected. She developed bed sores (which are a med mal liability), developed diarrhea, and was sitting in her own feces for hours on end. So glad we're out of here and in a facility that actually cares for her.

The only "bed" Kaiser could find at that time was at San Miguel Villa in far-off Concord, a 90-120 minute round trip from our home. This is a very large nursing facility, very institutional in appearance, nothing like a nursing home. We were not prepared for it, but my husband immediately accepted its hospital-like environment and felt safe. While it does not look like a luxury accommodation, it is always clean and very well maintained and work on improvements is ongoing. SMV turned out to be a Godsend for us. The staff there worked diligently to accommodate my special requests, and they took excellent care of my husband given the limitations inherent in such a large facility. Though most were pleasant, efficient and caring, certain staff deserve special mention: Nursing supervisor Alison, maintenance supervisor Robin, LVNs Brenda, Maikapoe and Earl at station 3 and Steven and Abel at station 4 and Jennifer of social services, along with CNAs Brenna, LeShea, Valerie and Romulo. It took 8 months of weekly phone calls to closer nursing homes to finally find a new place for my husband. We are very grateful for this move, which makes it so much easier to enjoy more frequent visits with my husband, but we will never forget the kindness of all the staff at San Miguel Villa!

TERRIBLE care, disgusting food, inhumane conditions and treatment. The nurses are incompetent, the CNA's are violent towards patients, yes violent. My daughter saw so many illegal things happening yet when she tried to say something the administration team told her they'll handle it and nothing seemed to be done... don't bring anyone you love in here my daughter came home in tears in disbelief of how bad this place was treating their patients.

Not only is the building new and well-maintained, with private rooms and bathrooms for each patient, but the quality of care truly stands out. The Director of Nursing makes a point to round on patients regularly, remaining visible and available for questions, concerns, and feedback from both patients and staff. That leadership presence carries over to the frontline nurses, who consistently treat patients with empathy and compassion--qualities that can be lacking in many other SNFs. The food is also impressive. It may not be a five-star restaurant, but even as a family member, I found the meals enjoyable and something I wouldn't turn away. Like any facility, there are opportunities for improvement. Therapy services at times seemed to be provided at a minimal level, likely reflecting staffing and workload challenges--an issue I recognize is not unique to this SNF. That said, I'm grateful my family member was able to maximize her rehabilitation. Her extended stay following an acute stay helped build her endurance, and she received excellent nursing care before being discharged home. As a family, we felt more prepared and better equipped to care for her. Now, she's slowly but surely thriving again. Thank you, Shadelands team, for being part of our journey!

Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but the following are a few. I'd like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients' well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient's needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfasts always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs.

I stayed there for three months, and during that time I experienced nothing but care and support from the staff. The nurses and CNAs are attentive, friendly, and truly caring. I especially want to thank Marth and Arryan--they are so sweet and genuinely care about their patients. The night nurses were excellent as well. I'd also like to thank the CNAs Melissa, Beth, and of course Dalia. They were helpful, kind, and attentive. They took such great care of me and made me feel safe during my time away from home. Physical therapy and occupational therapy were also great throughout my recovery. They helped me from the very beginning to regain my mobility and strength. The therapists Jordan and Justin were fun, funny, and kind throughout my sessions. They made each visit engaging and always took the time to listen to my needs. They consistently found ways to challenge me while keeping the sessions enjoyable, and they helped me grow stronger every day. I truly appreciated their support and encouragement. My case manager, Bianca, was amazing throughout the whole process. She was always available whenever I had questions or needed someone to talk to. We definitely had our moments--lol--but she was truly supportive and helpful every step of the way. I also want to thank Claire, the activity director. She was so lovely and caring, and she truly tried to make every day fun and different for all the patients. She recognized each patient's unique qualities and worked hard to make their time at the facility as enjoyable as possible. I googled Hospice and came across MD Choice that had some good reviews. I had no idea who I was calling but there had to have been some divine intervention pushing me to this call and this company. After a few questions I received a call from Dr De Leon who was the most compassionate and caring doctor I have come across. He sensed my immediate need, came straight out to the house, assessed my mother and went himself to the pharmacy to pick up the needed medications. He knew the urgency and took it upon himself to do all the footwork. He obtained the needed comfort medication, stabilized my mother and allowed my mother to pass peacefully all within a matter of hours from making that initial call. When she did pass that evening, he again, arranged for everything from the pronouncing to the calls according to the pre-arrangements. Without MD Choice and the connection with Dr De Leon, I don't know how I would have been able to handle the situation. His immediate response on a Sunday, his Taking control of the situation and quick actions made such a difference in the final few hours my mother had. If you find yourself in a situation like this and needing someone to help in this time, I highly recommend MD Choice and Dr De Leon. I am forever grateful to MD Choice and Dr De Leon for acting so quickly and with such caring and love in this terrible time of need. Thank you from the bottom of my heart and the hearts of my family.

Not only were they able to provide significantly more daily visits from their team of nurses and aids than the other options we considered, but they were incredibly responsive, efficient, and helpful with every question or request we had--and that includes their 24/7 on-call team. Cheryl, the RN Case Manager, in particular, was a godsend. It was a huge relief to be able to rely on them so thoroughly from day one. They delivered everything we needed within hours of getting the paperwork done; and both Cheryl and an aide were there for the last few hours my brother was alive--providing support and care. We really can't recommend them highly enough!

We were happy with the care she received, that's why we brought her back again. First of all, it's a very clean facility, small and personal, but most of all great staff. The nurses my Mom had were professional & helpful. The CNA's were amazing. The ones I will mention were the very best. Mario, Smile, Rain, and one special CNA on swing shift named Michelle. Jessica on the weekends was also an excellent help with my Mom, Nursing director Teri was extremely helpful, always available to help. Vanessa social worker was also fantastic at assisting with so much. The physical therapists & occupational therapists were also great. I hope my Mom never has to go back, but if she ever did, Legacy would surely be our number 1 pick. Much love & thanks to the staff. I would…read morecome to see my mother after lunch to find that she didn't have her hearing aid in her ear and her dentures were still in the case. which had dirty water in it. I had to complain to severl people before anyone would do anything about the moldy water they were storing her dentures in. Very few people here care. Good luck trying to get someone on the phone "They're in a meeting and will call you back as soon as it's over" was the standare lie I was told. There was no set level of care at which my mother would be released so as long as they could bill for her she was stuck in a hostage situation. My mother's health declined horribly while here, she lost a lot of weight and she died a month after they FINALLY agreed to release her.

I filled out an online form with my insurance information to get a quote and more information, and received a call no more than 10 minutes later. I did not expect that at all! Alexa was on the other end of the line and was so caring, thoughtful, and informative not only during that first call, but throughout the week as the situation with my husband escalated. She and New Start were so helpful in helping me figure out the best way to get a referral from my insurance, and even gave some advice and comfort as I contemplated taking my husband to the ER (with the explicit caveat that they are not medical professionals). Alexa checked in regularly to see if we needed any help in getting my husband to the facility and had provided lots of helpful information about the program beforehand so my husband could make an informed choice about which program he wanted to go to. This was about a week ago. Fast forward to today. My husband has been in the program for about a week. Even after he entered the program, Alexa has been really helpful and responsive with some follow up questions I had. As for the program itself, it seems to be wonderful. Great structure, great staff, great facilities (my husband is at the house in Concord, CA). Of course, any program is only as good as someone's commitment to recovery and of course recovery is a lifelong journey. However, it does seem (and I am hopeful) that New Start is giving him a solid foundation to support this journey throughout his life. Thank you so much to Alexa and the New Start team!

    Absolute sham of a care center. Sexual assault swept under the rug by staff, counselors and…read moreso-called on-site medical personnel. Given the wrong medicines, double-dose administered on record, male staff not announcing that they were barging into female quarters, individuals from off the streets just able to walk into unsecured buildings, where residents sleep, nothing was done about these problems while I was there as a veteran, their veteran liason was overwhelmed and would not address me, acting like if they did not acknowledge me and the constant trauma incurred by their failure to execute ANY appropriate protocol, even for suicide watch, then it must not have happened. When interviewed by State investigator, I wanted a veteran counsel in the room with me, he shut down the meeting and claimed "it just must be too soon and you are too messed up to talk". Wowie-WOWWOW. I urge ANYONE CONSIDERING NSRS TO FIND A SAFER FACILITY FOR INPATIENT CARE, ANYWHERE ELSE BUT THERE.
