My beautiful lady, Mary Lou was here as a guest during her transition from being at home. Unfortunately, she's at the moment now where she needs assisted living and that extra care. It's been a long and stressful time. Rough couple of months. Finally, while this was not permanent the staff and facilities here truly were a "god send!" This is a place where your loved ones are in great hands. The rooms primarily are not private but accommodate two individuals. They are co- ed. You never know how long your neighbor in the next bed will be here. The telephone and television are on both sides of the bed, but the loudness can interfere with the patients comfort level. The doctor and nurses here are so overwhelming great the way that they handle the patients care and pain levels. The patient, even Mary Lou, my angel, does not want to be here. She wants to go home . There is no more home for her. It's assisted living. They make this transition easier for the patient. The visiting hours are longer than normal and all an individual needs to do is sign it. You're allowed to bring outside food and drinks for patients . You can decorate and adorn their space with flowers and other trinkets to make them feel a little piece of home. Love is the focus and makes the world turn. Sometimes we cannot decide where we will be. When it comes to the end we will go in and out of who and where we are ?! My Miss Marylou, my angel you are loved and will be continued to be loved as long as you are here. We will go beyond to make you happy.

This facility is nice but the care provided is very inconsistent and lacking at best. My mom has…read morebeen here for several weeks and was transferred from an acute rehab for continued nursing and physical therapy. It seems as though the care provided has been no different than had we put her in hospice... mind you, we are nowhere near that point in her life. Her weakness has only gotten worse, physical therapy has not been done as we were advised it would be, and she continues to lose weight, her muscle strength, and has been laid up in her bed for more than three weeks now with very little movement. She has become stiff and rigid in the position that she's in and is very painful to even attempt to move her into a sitting position. Communication from the staff and her case manager has been extremely poor. I was notified last week on Wednesday that Medicare was only providing one more week of treatment for her and have yet to be provided a discharge date. I have left several messages for the staff with no response and still don't know when I need to set up transfer for her to depart that facility. Based on the timeframe provided, it would appear that I have 24 hours or so from now, yet no information has been provided to our family on when she will be discharged. Her time at this facility has been a complete waste of time and only further set back any progress she had made prior to admission. While Mom says "most of the people are nice," she also said that they very rarely even check on her and never move her. Please do your research to find the best fit for your loved one. We thought we had, and while we understand that skilled nursing is a lower level of care, we feel that there is zero excuse for the lack of treatment she has been provided. It is also important to note that many times when we have been there visiting, there have been other patients crying out for help from their rooms that has gone on for more than 10 minutes before anyone responds to their request or urgency. This is uncalled for! The staff is there to care for these patients and they hardly acknowledge the calls for assistance... definitely not in a timely manner. We are extremely disappointed with our experience and how our loved one has been cared for, or lack there of.

In 6 days of being admitted to EncompassRehab, his BP wasn't checked timely, his oxygen wasn't addressed properly, his workout too vigorous and now I have the worst decision of my entire life of either ending his life support by hospice or watch him suffer, while hoping he can recover. My father's last phone call to me was of him screaming in pain and his last text I'll ever read was in the form of the word "HELP!" Neglect at its finest and I WILL know what occurred no matter what the cost... Once a brilliant intelligent resilient kind man, who as of now, has no memory of his existence and little ability to speak, think, see, move or function. Was going to this location for a while for physical therapy. Great staff and very helpful…read more However, this is where the issue comes in where I do not recommend going not just here, but STP in general. I was told I had 1 session left, when I had at least 9 more left per my doctor and insurance company. They told me there was an issue with scheduling for me and they "will call me" when they have it resolved. Well shame on me for waiting for that, cause when I called them finally, they told me my remaining approved sessions were declined. Again, doctor and insurance both said I still had some left, and even went out of my way to get a new referral sent. I even called scheduling myself and they said I was good to go. The location said otherwise. They told me I was discharged, and I found out WEEKS after the supposed discharged date. No email discharge info, no mail discharge info, and even my doctor and insurance got nothing. And they wouldn't even say why. Next time I have an injury where I need physical therapy, even tho I really enjoyed the people helping me, I will never go to STP again. All the progress I had is gone and gotta start all over now.

I was always greeted on time, they were diligent to prepare me with self help exercises, and they were always calibrating with me regarding how I was feeling. They had encouraged us to discharge home, but my dad was still unable to walk, get out of bed, roll over or another kind of independent action at that time. We talked to Tammy at Sage Creek. She was so calming and reassured us that my dad would have the care he needed, they could provide transport for the office visits he needed for his heart surgery and would have the PT, OT and SLP that he needed. We were told at the rehab that a skilled nursing facility was going to just leave him in bed and he would lose any progress he had made with them. This is absolutely untrue for this facility. They got my dad up and dressed every day. He had therapy. The therapists were so patient and kind (Alexa, Vicky, Willy and all the others I'm so bad with names) Mo the nursing supervisor went above and beyond to take care of any concerns that we had. Judy, Tyler, TJ, Brandi, Lindsay, CC, Rosann, Malik and other nursing staff were all amazing. The facility doctors were wonderful with communication and the care they gave. The facility was clean and the food was good with lots of options. Danielle and Nicole in transportation were amazing taking my dad to his many doctors appointments. Paige, our case manager, worked so hard for us. Anything we need to get my dad in the VA Home she came through for me. Absolute godsend! We adopted the staff here as family and we already miss them. Their kindness and thoughtful caring was beyond any of our expectations. I 100% recommend this facility

This facility is only for short term stay but if you have other options to chose from, I wouldn't…read morerecommend this place. It is a bit deceiving though cause when you initially walk in, it looks like a nice place, the rooms are updated but and yes there's a but.. the nurses aren't attentive to your loved ones in the room. My grandpa is currently in here trying to get rehabilitated to get moved to an assisted living or back home and he even hates being there. He presses the call button and they rarely checkin on him. He had to use the restroom as he's still able to do on his own, with assistance and they told him to use the diaper he's in cause they were short staffed and couldn't take him. I'm sorry but no, that's not right to let your patient "sh*t the bed" cause you can't grab another nurse or CNA for a few moments to help someone use the restroom. I've also noticed in order to get any sort of nurse in the room you have to walk up to the nurses station for them to actual come in and help, since they ignore the call button. I will say though the nurse he has right now has been very helpful and polite with him since I've been coming out to see him. As well as another nurse who was super helpful this weekend, but again they were only helpful when we called upon them at the station. However, the other nurses don't seem to really care much, they just sit around and socialize and they seem to have a little attitude when you ask questions, as we are just trying to get answers when his actual nurse isn't right there. The last few times I was in there the room was a mess. The bathroom was worse, the pee bottle he had on the toilet still had pee in it. He was laying with a dirty stained blanket all day before they came in and changed it. If you're the nurse or the cna look around your patients rooms and see if anything needs to be picked up. Make your patients feel comfortable even though they don't want to be there.

My mother in law is currently at this facility for rehab after knee replacement. The experience…read morehas not been very favorable. I will say that the speech therapist and physical therapist seem to be very kind and thorough, and one of the nurses/CNAs was very kind as well. They do have a large gym area for physical therapy. The facility, however, is depressing. When you enter, the lobby looks lovely with wood floors and a piano. The front desk staff range from mildly friendly to apathetic. On a recent visit a staff member was slurping soda through a straw from a can and just glared at us as we signed in without saying a word. As you move further from the lobby, however, the wood flooring is replaced with linoleum and the place looks increasingly dilapidated. The first room mom was placed in had a leaking HVAC unit with tons of towels piled under it apparently to soak up the leak. Gross. Eventually she was moved to a better room. And the food - yikes! Do they even have a dietician at this place? One dinner was a cheese enchilada - is that even a balanced meal? Last night it was a sad looking sausage sandwich with the driest and most over cooked french fries that you can possibly imagine. Anway, I guess she is getting the physical therapy which is the reason for being at TLC. But don't expect much more in terms of comforts...

People at nursing station are either sleeping or chitchating instead of doing their jobs. My personal opinion is: these folks chose this line of work. They sought an education in Healthcare. They applied for this job...but the attitudes are hateful. Shut down half of the facility. Fix what is broken. And the day shift management needs to rotate working night shifts. Not only to help but to see what improvements can be made. You are ALL aware of what a terrible facility this is. You avoid the upset Patients family, refuse to discuss negative situations and put folks in positions where they have to dial 911, from their rooms, to get proper attention. It's all about the $$ for each occupied bed...period. I feel terrible for those patients that have Noone to check in on them. Damn shame...

From the first interaction with Rest At Home, I knew something was different about this company. I…read morereceived three referrals from a skilled nursing facility where my father is housed. I called each referral and from the first two minutes, I knew this was the company that would best serve the needs of my father. I initially spoke to Chris who was very nice and knowledgeable about the services typically needed by the elderly community. He also was very helpful in getting me to understand 'why' my father acts a certain way and how we as a family can help him - to help himself. Chris carried on a conversation that was genuine and honest. Never once did I feel he was putting the companies interest above those of my family and my father. I had the opportunity to meet the caregiver before any decision was made. Chloe was an absolute sweetheart. She asked just the right questions and then described to me and my family what it would be like to have her care for my father. She explained her style, how she deals with difficult clients, and what she expects from the family. This was very refreshing because she actually set expectations for everyone. This made all of us siblings comfortable and we knew my father would like her. It's been a couple of months now and my father thinks he has a new 'girlfriend.' He acts like a kid now and has changed his behaviors for the better. Chloe has the patience of a Saint because my father can be difficult, but she never lost that smile - something our family had a hard time doing for the last couple of years. Taking care of a family member is very difficult. I am so glad I put my trust into Chloe and Chris. I want to thank both of them for changing my family's life and most importantly my father's life.
