What most people need to realize, is the importance of building a relationship with the staff caring for your loved one. It is a team effort. This is a care facility. They cannot and should not be expected to give private duty nursing services. It is unrealistic for any facility to be expected to do this. You have to be willing to reach out to the staff and work with them. Come to them with concerns, but also realize a thank you goes a long way when you do this very hard work. I can't begin to thank the staff for the care dad received especially during his final days. I stayed with my dad to care for him and make sure he knew he was not alone. I believe that was my honor and duty, but this staff cared for me as well as my dad. The concern for my well being touched me to my core. I will not forget the compassion my father and I received by all the staff. The flower arrangement that was sent to the funeral home was so beautiful and very appreciated. Staff who became friends were there at the funeral home to support me and my family. Their concern did not stop as dad was taken from the facility. If you are looking for a facility to "team care" for your loved one, stop looking, the Lakes of Sylvania should be strongly considered. I will only refer folks to this facility.

The director, the nurses, the physical therapists, the chef, the cleaning department and various departments ALL provided the best of care possible. The communication which existed between Lakes of Sylvania and her family was also tremendous. Each day an updated Covid report was to be found in my email and a weekly video was posted to inform all families members of the events of the week. I was able the connect in person with the staff whenever I so desired. My dear one got the BEST of care and compassion from everyone within the walls of Lakes of Sylvania. I would give this campus my highest recomendation. I owe them my deepest gratitude.

But I unfortunately cannot say the same for their administrative and billing. Disorganized and sloppy, and worst of all, cost me money. I would advise everyone to keep their own records of all visits and conversations and discharge dates. Much better than spending hours on the phone afterwards, trying to sort things out with your insurance company.

This is the best format for senior living. Each house holds just 10 residents in 10 bedrooms, split between 2 hallways on either side of a central common area with dining space, living area, and an enormous kitchen where staff prepares meals and residents may use space, also. Each bedroom is private with en suite bathroom equipped with a walk in shower. Facility provides twin bed, sitting chair, side table, and dresser. There is space to personalize as well. We plan to add 2 sitting chairs for guests and a larger dresser. There is a large closet, between 5-6' wide. Laundry is done on a schedule and not mixed so chance if items missing is very low. Nurse is available, aids are assigned on shifts and are very capable and friendly. The center publishes activity calendar including social outings and shopping trips. There is a hair salon and manicures and pedicures can be scheduled.

I also think it would be helpful to have a list of phone numbers with heads of departments so I could get in touch with the appropriate person and know their names I also believe the activities seem to be improving as you now have a person in charge of that. I know the facility is very old but it sure could use some TLC. The other day the ceiling was dripping a puddle on the floor in the hallway. Bedside stands are missing doors. My father was at Foundation Park for 2-1/2 years, suffering from Dementia and Parkinsons - among…read moreother issues. When he was first admitted, the doctor that sent him had recommended hospice care immediately - but because of the care that was given to him at the Foundation Park facility - his health actually initially improved - and then remained steady for a long time before he ultimately declined. Hospice was not needed again until his medical issues finally took his life. Having said that - a facility like this can be difficult to visit. There is a lot of people staying there that suffer from various levels of mental decline, a lot of them wearing diapers - which can lead to an unpleasant smell throughout the facility. It can also be awkward as you go to visit your relative/loved one because some of the patients wander around the halls kind of aimlessly, and it is apparent they are not mentally correct....my father lost some of his personal items, and it is not clear if he lost them himself, or if they were picked up by other patients that wandered around the facility. I see other reviews where people are complaining because their loved one "had to wear a diaper", or "severely declined"....I understand that it is difficult to see a family member or loved one in this state - but to blame the facility for this seems a stretch. Unfortunately, people with these types of medical issues are going to have trouble using the restrooms, and they are going to decline - it's unpleasant to watch, but it is inevitable. Seems pretty easy to blame others for something that is occurring naturally. My experience with the staff, from the front office, to the nurses, to the administrators, and social service personnel - ALL OF THEM were caring, good people that were compassionate and helpful whenever we visited, or spoke with them on the phone. A place like this is never going to be fun to take a family member to - and it can be unpleasant to visit - but ultimately, the folks at Foundation Park do a good job and seem to care deeply about the people they are caring for, and their families.
